purpose: During catheter angiogram (CA) there is momentary increase in intravascular volume and pressure due to intra-arterial injection that can potentially cause vascular distention at the stenotic site, whereas on CT angiogram (CTA) is unlikely due to intravenous administration. Methods: CA and CTA of the carotid artery from 29 patients were retrospectively studied. CA and CTA were obtained for each patient. Curved sagittal MpRs mirroring the carotid artery on CA were used to measure the diameter at stenosis and at the distal lumen. Mural plaque calcium content was graded on axial CTAs. Results: Accounting for repeated measurements, the likelihood that the lumen diameter from CA will be larger than CTA was higher at stenosis than distal to it but the difference in lumen diameters at stenosis was similar to CTA. There is insufficient evidence that intra-arterial hand-injection during CA leads to underestimation of the degree of stenosis. percentage stenosis using the NASCET criteria differed between the 3 measurements, post hoc analysis showed significant difference between CA and axial CTA (p p > 0.99). The difference in lumen diameter did not depend on our calcium grading (p = 0.484). Conclusions: There is insufficient evidence to suggest that intra-arterial hand-injection contributes to vessel distention and underestimation of percent stenosis during CA in this study. Mural plaque calcium does not affect the degree of stenosis on CTA.

polypyrrole (ppy) was synthesized using Ammonium peroxy Disulfate (ApS) as oxidant in a standard ratio of monomer to oxidants at 5℃. Attempts were made to increase the electrical conductivity by using various dopants viz. Lithium per Chlorate (LiClO4), para-Toluene Sulfonate (p-TS) and Napthalene Sulfonic acid (NSA). The materials were characterized using FTIR, X Ray diffraction and SEM. The electrical conductivity was measured by two probe method and was found to be in the range of 10-3 S/cm. Thin films of these preparations were casted on the interdigited electrodes to study the detection of gas such as ammonia. It was found that for the pure ppy when ammonia gas was allowed to flow in, there was a sudden increase in the current, which decreased rapidly when gas was stopped. However when ppy doped with p-TS, NSA and LiClO4, the trend was reversed.

Incidentally discovered hepatocellular carcinomas (iHCC) are tumors which are discovered on the explanted liver which were not present on imaging prior to transplant. The natural history, histopathologic characteristics and prognosis of iHCC are not clearly defined. Methods: A retrospective analysis was performed to compare the characteristics of iHCC and established HCC within Milan criteria (eHCC) in patients who underwent liver transplantation at our center between 2000 and 2010. Results: During the study period a total of 975 adult patients were transplanted in our center;124 (12.7%) patients had eHCC and 26 (2.6%) patients had iHCC. A larger number of patients with iHCC (73.1%) had ascites when compared to eHCC (41.3%) (p = 0.035). patients with iHCC had a higher mean bilirubin (p 0.001) and mean INR (p = 0.05) than patients with eHCC. Around 70% of patients with iHCC had a Model for End Stage Liver Disease (MELD) score greater than 15 at the time of listing while only 25% of patients with eHCC had a MELD greater than 15 at listing (p 0.001).The mean alphafetoprotein (AFp) in patients with iHCC was significantly lower (11.6 ± 16.5) than the patients with eHCC (564.9 ± 2180;p = 0.024). In patients with iHCC, 30.8% had a multiple tumors and 23% had bilobar involvement. The average number of tumors was 1.6 and the cumulative tumor size was 2.1 cm (SD 1.4). The cumulative tumor size in iHCC was significantly smaller than in eHCC (mean 3.9 cm) (p = 0.035). American Joint Committee on Cancer (AJCC) T1 tumor stage was found in 58% of patients with iHCC and 48.4% of patients with eHCC (p = 0.829).The median survival was 9.47 years for iHCC (95% CI 7.0 - 11.9) and 8.7 years (95% CI 6.1 - 11.4) for eHCC (p = 0.328). While none of the patients with iHCC had recurrence of HCC, the incidence of recurrence in patients with eHCC was 6.4%. 伴皮层下囊肿的巨脑性白质脑病(Megalencephalic leukoencephalopathy with subcorticalcysts,MLC)是一种常染色体隐性遗传的单基因遗传病,典型表现为出生后头围大与缓慢进展性运动功能障碍如走路不稳、共济失调、肢体痉挛和瘫痪。头颅MRI特征性改变为弥漫性大脑白质信号异常及肿胀,伴有双侧颞叶、额叶前部和额顶交界区特征性的皮层下囊肿。其致病基因于2001年定位并命名为MLC1。MLC1基因包含12个外显子,编码377个氨基酸的MLC1蛋白,包含8个跨膜功能域,其功能尚未明确。到目前为止,人类基因突变数据库(Human GeneMutation Database)中报道的与MLC相关的MLC1基因突变有100余种,尚未见中国人MLC1基因突变的报道。目的对13例临床诊断MLC患儿进行MLC1基因分析及突变功能研究。方法收集并分析13例MLC患儿的临床资料;DNA直接测序进行MLC1基因突变检测;构建MLC1质粒突变体,应用Wester Blot及免疫细胞化学方法进行突变功能研究。结果13例患儿均于生后发现头大和典型的MRI表现,伴或不伴运动与智力异常,临床符合MLC诊断。13例患儿中9例在分子水平明确诊断,共检测到11种MLC1基因的改变,包括:5种新的错义突变(c.65G>A,p.R22Q;c.2 1 8G>A,p.G73E;c.823G>A,p.A275T;c.832T>C,p.C278R;c.95C>T,p.A32V)、1种新的剪接突变IVS9-1G>C、已知报道的1种无义突变(c.594delCTCA,p.Y198X)与2种错义突变(c.206C>T,p.S69L;c.353C>T,p.T118M)。Western blot结果显示:与野生型MLC1质粒相比较,突变体S69L、G73E、Y198X、A275T、C278R MLC1蛋白表达量均有减少,突变体R22Q、A32V、T118M MLC1蛋白表达与野生型相似。应用激光共聚焦显微镜技术结果表明:与野生型MLC1质粒相比较,突变体G73E与Y198X聚集于内质网内,但突变体A275T与野生型MLC1质粒表现相似。结论本研究首次在国内从分子水平明确了9例MLC患儿诊断,发现了6种国际上尚未报道的与MLC相关的MLC1基因新突变;G73E与Y198X突变可以导致MLC1蛋白表达与细胞中内质网潴留,最终影响MLC1蛋白的功能。这是中国MLC患儿进行MLC1基因突变分析与突变功能研究的首次报道。
