Cranial neuropathies, confusion, and ataxia— Challenges for diagnosis and therapy

Introduction: The differential diagnosis of patients presenting with multiple cranial neuropathies, ataxia, and altered mentation is broad and includes immunologic, infectious, vasculitic and metabolic conditions. Primary considerations are Bickerstaff’s brainstem encephalitis (BBE), the Miller Fisher syndrome (MFS), Wernicke’s encephalopathy and botulism. The initial workup may be unrevealing. Timely treatment is imperative and unnecessary treatment can be associated with serious adverse reactions. Sensitivity to the decisions needed in such patients is therefore important. Case report: A 58-year-old male presented with symptoms of altered mental status, blurred vision, dysphagia and dysarthria, impaired pupillary responses to light, facial diplegia, ataxia, and decreased tendon reflexes after an episode of a self resolving diarrheal disease. Primary initial diagnostic concerns were Bickerstaff’s brainstem encephalitis (BBE), Miller Fisher syndrome (MFS), Wernicke’s encephalopathy and botulism. Initial work-up including cerebrospinal fluid analyses, imaging studies, and an electrodiagnostic examination did not provide information helpful for narrowing this differential. The patient was treated with botulinum antitoxin, thiamine and intravenous immune globulin (IVIG) before the results of specialized tests were available. The patient’s clinical condition improved. Retrospectively, the patient was diagnosed as BBE. Conclusion: This case emphasizes the difficulties in distinguishing between BBE, MFS, and botulism as well as demonstrating the complexities of treating such patients.

The excitation function of the 58Ni(n,p)58Co reaction was measured using the well-established neutron activation technique andγ-ray *** in the energy range of 1.7 to 2.7 MeV were generated using the 7Li(p,n)*** neutron flux was measured using the standard 115In(n,n’)115mIn monitor *** results of the neutron spectrum averaged cross-section of 58Ni(n,p)58Co reactions were compared with existing cross-section data available in the EXFOR data library as well as with various evaluated data libraries such as ENDF/B-VIII.0,JEFF-3.3,JENDL-4.0,and *** calculations were performed using the nuclear reaction code *** nuclear level density(NLD)models were tested,and their results were compared with the present *** NLDs were also obtained through the spectral distribution method(SDM).The cross-section results,along with the absolute errors,were obtained by investigating the uncertainty propagation and using the covariance *** forγ-ray true coincidence summing,low-energy background neutrons,andγ-ray self attenuation were *** experimental cross-section obtained in the present study is consistent with previously published experimental data,evaluated libraries,and theoretical calculations carried out using the TALYS code.

Is CT scan a predictor of instability in recurrent dislocation shoulder?

Purpose: Glenoid bone defect and the defect on the posterior-superior surface of the humerus “Hill- Sachs lesion” are the commonly seen bony lesions in patients with recurrent dislocation shoulder. Computed tomography (CT) scan is considered as the best option in assessing the bony defects in the recurrent dislocation shoulder. The aim of this study was to assess the clinical and radiological corelation in the patients with recurrent dislocation shoulder. Methods: Forty-four patients of recurrent dislocation shoulder who were evaluated between January 2015 and December 2017 at a tertiary care center, clinically and radiologically using CT scan and meeting the inclusion criteria, were included. The correlation between the clinical history of the number of dislocations and the bone loss using CT scan was evaluated. Two sided statistical tests were performed at a significance level of α=0.05. The analysis was conducted using IBM SPSS STATISTICS (version 22.0). Results: All the patients were male with mean age of 25.95 (SD ± 4.2) years were evaluated. Twenty-four patients sustained injury in sporting activities while 20 patients sustained injury in training. There were an average of 4.68 (SD ± 3.1, range 2e15, median 3) episodes of dislocation. Forty-one patients had the glenoid bone loss while 40 had the Hill-Sachs lesions. The mean glenoid width defect was 10.80%(range 0e27%) while the mean Hill-Sachs defect was 14.27 mm (range 0e26.6 mm). The mean area of bone loss of the glenoid surface was 10.81%(range 0e22.4%). The lesions were on track in 34 patients and off track in 10 patients. Conclusions: CT scan of the shoulder joint is an effective method for assessing the amount of bone loss. The number of dislocations are correlated significantly with off-track lesions and the amount of bone loss on the glenoid and Hill-Sachs lesion. The glenoid width bone loss of more than 9.80% or Hill-Sachs defect of more than 14.80 mm are the critical defects after which the frequency

Background and Aims:The anticipated fear of serious out-comes in coronavirus infected liver transplant recipients led to disruption of transplant services *** aim of our study was to analyze COVID-19 severity in transplant recipients and to compare the difference of COVID-19 clinical outcomes in early(1 year)post-transplant ***:41 post-living donor liver transplant recipients with COVID-19 infection were studied retrospectively from 1st April 2020 to 28th February ***:The median age was 49.00 years with a male preponderance(80.49%).Fif-teen patients had infection within 1 year of transplant and 26 were infected after 1 year of *** overall median interval between transplantation and COVID-19 diagnosis was 816.00 *** and malaise were the common presenting *** most common associated comorbidities were diabetes mellitus(65.85%)and hypertension(46.34%).The severity of illness was mild in 28(68.29%),moderate in 4(9.76%),severe in 6(14.63%)and critical in 3(7.32%).To identify associated risk factors,we divided our patients into less severe and more severe *** for lymphopenia,there was no worsening of total bilirubin,transaminases,al-kaline phosphatase,and gamma-glutamyl transferase in the more severe ***(19.51%)patients required inten-sive care unit admission and three(7.32%)died,while none suffered graft *** recipients with early *** post-transplant COVID-19 infection,there were similar outcomes in terms of severity of COVID-19 illness,intensive care unit care need,requirement of respiratory support,and ***:Living donor liver transplantation can be per-formed during the COVID-19 pandemic without the fear of poor recipient outcome in cases of unfortunate contraction of severe acute respiratory syndrome coronavirus-2.

On Channel Estimation of OFDM-BPSK and -QPSK over Generalized Alpha-Mu Fading Distribution

This paper evaluates the performance of OFDM-BPSK and -QPSK system in α-μ distribution. A fading model which is based on the non-linearity present in the propagation medium is utilized here for generation of α-μ variants. Different combinations of α and μ provides various fading distributions, one of which is Weibull fading. Investigations of channel estimation schemes gave an idea of further reducing the BER as to improve the performance of OFDM based systems. In flat fading environment, channel estimation is done using phase estimation of the transmitted signal with the help of trained symbols. Final results show the improvement in BER. However, the amount of results improved depends upon the amount of trained symbols. The more trained symbols will result into more improved BER.
