Angiographic and clinical outcomes in elderly subjects treated with percutaneous coronary intervention following fibrinolytic administration for ST-elevation myocardial infarction

Background Prior studies have demonstrated that the achievement of faster coronary artery flow following reperfusion therapies is associated with improved outcomes among ST-elevation myocardial infarction (STEMI) patients. The association of patient age with angiographic characteristics of flow and perfusion after rescue/adjunctive percutaneous coronary intervention (PCI) following the administration of fibrinolytic therapy has not been previously investigated. Objectives and Methods We examined the association between age (≥ 70 years or ＜ 70years)and clinical and angiographic outcomes in 1472 STEMI patients who underwent rescue/adjunctive PCI following fibrinolytic therapy in 7 TIMI trials. We hypothesized that elderly patients would have slower post-PCI epicardial flow and worsened outcomes compared to younger patients. Results The 218 patients aged ≥ 70 years (14.8%) had more comorbidities than younger patients. Although these patients had significant angiographic improvement in TIMI frame counts and rates of TIMI Grade 3 flow following rescue/adjunctive PCI, elderly patients had higher (slower)post-PCI TIMI frame counts compared to the younger cohort (25 vs 22 frames, P = 0.039), and less often achieved post-PCI TIMI Grade 3 flow (80.1 vs 86.4%, P = 0.017). The association between age ( ≥70 years) and slower post-PCI flow was independent of gender, time to treatment, left anterior descending (LAD) lesion location, and pulse and blood pressure on admission. Elderly patients also had 4-fold higher mortality at 30 days (12.0 vs 2.7%,P = 0. 001 ). Conclusions This study suggests one possible mechanism underlying worsened outcomes among elderly STEMI patients insofar as advanced chronological age was associated with higher TIMI frame counts and less frequent TIMI Grade 3 flow after rescue/adjunctive PCI.

Retinitis pigmentosa is a hereditary retinal disease that affects rod and cone photoreceptors,leading to progressive photoreceptor loss.Previous research supports the beneficial effect of electrical stimulation on photoreceptor survival.This study aims to identify the most effective electrical stimulation parameters and functional advantages of transcorneal electrical stimulation(tcES)in mice affected by inherited retinal degeneration.Additionally,the study seeked to analyze the electric field that reaches the retina in both eyes in mice and post-mortem humans.In this study,we recorded waveforms and voltages directed to the retina during transcorneal electrical stimulation in C57BL/6j mice using an intraocular needle probe with rectangular,sine,and ramp waveforms.To investigate the functional effects of electrical stimulation on photoreceptors,we used human retinal explant cultures and rhodopsin knockout(Rho^(-/-))mice,demonstrating progressive photoreceptor degeneration with age.Human retinal explants isolated from the donors’eyes were then subjected to electrical stimulation and cultured for 48 hours to simulate the neurodegenerative environment in vitro.Photoreceptor density was evaluated by rhodopsin immunolabeling.In vivo Rho^(-/-)mice were subjected to two 5-day series of daily transcorneal electrical stimulation using rectangular and ramp waveforms.Retinal function and visual perception of mice were evaluated by electroretinography and optomotor response(OMR),respectively.Immunolabeling was used to assess the morphological and biochemical changes of the photoreceptor and bipolar cells in mouse retinas.Oscilloscope recordings indicated effective delivery of rectangular,sine,and ramp waveforms to the retina by transcorneal electrical stimulation,of which the ramp waveform required the lowest voltage.Evaluation of the total conductive resistance of the post-mortem human compared to the mouse eyes indicated higher cornea-to-retina resistance in human eyes.The temperature recordings during and after electrical

Wild relatives of crop are key genetic resources serving as diversity reservoirs for crop improvement under changing environments.Rice(Oryza sativa)is one of the most important crops in the world,providing staple food for half of the world's population.Wild rice is thus a critical germplasm resource for sustained global food security,ensuring high production yields,improved quality,and stress resistance in the face of climate change.Wild rice is closely related to domesticated rice and has a rich genetic diversity and exceptional adaptability to extreme environments.It has played a pivotal role in the history of rice hybridization and has become a key resource for rice breeding programs.The identification of wild-type cytoplasmic male sterility resources paved the way for the achievement of the“three lines”goal in hybrid rice,leading to a significant increase in rice yields.In addition,the use of resistance alleles found in wild rice is making rice production more resilient to losses caused by environmental stresses.However,wild rice germplasm resources are threatened due to habitat destruction and other anthropogenic factors.At the same time,the lack of centralized distribution of wild rice has hampered the sharing of basic information on wild rice resources and the utilization and conservation of wild rice in each country,as well as collaboration among scientists.

Magma produced by melting of continental crust and mantle at the Archean-Proterozoic boundary are compositionally variable and chemical compositions provide evidence for the mixing of two sources. Understanding the composition of hybrid magma is essential for determining the comparative infl uence of crust and mantle sources during orogenesis. The hybrid granites are less documented in Indian cratons, especially less in Dharwar Craton. Here we present petrographic and whole-rock geochemical data of Madgulapalli granitic rocks situated in the NE part of the Eastern Dharwar Craton(EDC), to elucidate their petrogenesis and role in crust formation. The Madugulapalli granites(MPG) are composed chiefl y of plagioclase, quartz, and alkali feldspar with associated biotite showing alteration and inter-granular textures. Geochemically, they are metaluminous to peraluminous in nature with calc-alkaline hybrid granite. The hybrid granites exhibit both negative and positive europium anomalies;the lower Rb/Sr, Rb, Sr, and higher Sr/Y,(Dy/Yb)N ratios suggest that the interaction of older rocks with residual garnet source melted at high pressures. We hypothesize that hybrid granites are formed by interaction(e.g., metasomatism, mingling, or mixing) between parental magmas and pre-existing rocks with the infl uence of sanukitoid melts(heat source) in a subduction environment. The genesis of the hybrid granites demonstrates the mixing coupled with diff erentiation in the petrogeny’s residue system in a syn-collision setting followed by continental crust stability in EDC during the Neoarchean period.

In this paper,sine trigonometry operational laws(ST-OLs)have been extended to neutrosophic sets(NSs)and the operations and functionality of these laws are studied.Then,extending these ST-OLs to complex neutrosophic sets(CNSs)forms the core of thiswork.Some of themathematical properties are proved based on ST-OLs.Fundamental operations and the distance measures between complex neutrosophic numbers(CNNs)based on the ST-OLs are discussed with numerical illustrations.Further the arithmetic and geometric aggregation operators are established and their properties are verified with numerical data.The general properties of the developed sine trigonometry weighted averaging/geometric aggregation operators for CNNs(ST-WAAO-CNN&ST-WGAO-CNN)are proved.A decision making technique based on these operators has been developed with the help of unsupervised criteria weighting approach called Entropy-ST-OLs-CNDM(complex neutrosophic decision making)method.A case study for material selection has been chosen to demonstrate the ST-OLs of CNDM method.To check the validity of the proposed method,entropy based complex neutrosophic CODAS approach with ST-OLs has been executed numerically and a comparative analysis with the discussion of their outcomes has been conducted.The proposed approach proves to be salient and effective for decision making with complex information.

Resistances of grain junctions of bulk polycrystalline YBa_(2)Cu_(3)O_(7−δ)(YBCO) and DyBa_(2)Cu_(3)O_(7−δ)(DyBCO) superconductors have been extracted following(i)Ambegaokar‐Baratoff(AB)and(ii)de Gennes(dG)equations.Current–voltage(IV)below the critical temperature(T_(c))has been used to extract transport critical current density(j_(c)).The variations of the junction resistances,(RN)with temperature(T)exhibit that below a critical value of the normalised superfluid density(NSD),junctions become very low resistive and exhibit metallicity.Dependence of this feature of RN on the energy gaps has also been explored.Weak scattering limit is found to be compatible with the maximum of RN(T)as is observed from the corresponding NSD.

Impact of depressed left ventricular function on outcomes in patients with three-vessel coronary disease undergoing percutaneous coronary intervention

Background Patients with multivessel coronary artery disease and depressed left ventricular ejection fraction （LVEF） represent a high risk group of patients for coronary revascularization. There are limited data on percutaneous coronary intervention treatment in this population. Methods Among a cohort of 4335 patients with three-vessel disease with or without left main disease undergoing percutaneous coronary intervention, 191 patients had LVEF 〈40% （low ejection fraction （EF）） and 4144 patients had LVEF _〉40%. In-hospital and long-term outcomes were examined according to LVEF. Results The estimated two-year rates of major adverse cardiac events, cardiac death, and myocardial infarction were significantly higher in the low EF group （19.64% vs. 8.73%, Log-rank test： P 〈0.01; 10.30% vs. 1.33%, Log-rank test： P 〈0.01, and 10.32% vs. 2.28%, Log-rank test： P 〈0.01 respectively）, but there was no difference in the rates of target vessel revascularization （6.18% vs. 6.11%, Log-rank test： P=0.96）. Using the Cox proportional hazard models, LVEF 〈40% was a significant risk factor for cardiac death, myocardial infarction, and major adverse cardiac events （OR （95% CI）： 4.779 （2.369-9.637）, 2.673 （1.353-5.282）, and 1.827 （1.187-2.813） respectively）, but was not a statistically significant risk factor for target vessel revascularization （OR （95% CI）： 1.094 （0.558-2.147））. Conclusion Among patients undergoing percutaneous coronary intervention for multivessel coronary artery disease, left ventricular dysfunction remains associated with further risk of cardiac death in-hospital and during long-term follow-up.

Nanostructured porous polyaniline(PANI)has been synthesized and coated simultaneously on a highly flexible and conductive carbon cloth(CC)substrate using a simple in-situ chemical oxidative polymerization technique.PANI coated CC(PANI-CC)based flexible electrodes were further used for the fabrication of flexible supercapacitor devices.For the comparison purpose,pure PANI has also been synthesized and tested for its electrochemical performance.The energy storage capacity of PANI and PANI–CC composite was investigated using electrochemical techniques like CV,GCD,and EIS in a potential range from 0 to 0.8 V in 1 M H_(2)SO_(4)electrolyte.PANI-CC flexible electrodes exhibited the highest specific capacitance of 691 F/g;whereas,pure PANI could only achieve 575 F/g of specific capacitance at 1 A/g.Composite also exhibited outstanding cyclic stability by recollecting 94%of its initial capacitance after 2000 GCD cycles.For actual implementation,a flexible supercapacitor device has been fabricated using stainless steel sheets and PANI-CC flexile electrodes.The energy storage performance of the PANI-CC flexible supercapacitor device was tested at several bending angles,which resulted in 72%of capacitance retention at a maximum bending angle of 140°compared to the capacitance obtained at an angle 0°(flat state).PANI-CC exhibited improved electrochemical performance than pure PANI due to the synergistic effect between PANI and CC.Here,CC helped in enhancing the conductivity and stability;whereas,PANI boosted the capacitance owing to its excellent porosity and fast pseudocapacitive charge storage response.

Epiphyseal injuries of distal femur are rare with an incidence of 1%—6% among all physeal injuries. Prompt diagnosis and appropriate surgical treatment is crucial to achieve satisfactory functional outcomes. A conjoint bicondylar coronal split (Hoffa) fracture with complete transaction of ipsilateral patellar tendon has been reported in a 12 year old child. The injury was managed by open reduction and internal fixation and bone to ten don repair. This case emphasizes the need of accurate intraepiphyseal fixation for the management of these fractures in skeletally immature patients.
