Dried-leaf Artemisia annua: A practical malaria therapeutic for developing countries?

Artemisinin from the plant Artemisia annua （A. annua） L., and used as artemisinin combination therapy （ACT）, is the current best therapeutic for treating malaria, a disease that hits children and adults especially in developing countries. Traditionally, A. annua was used by the Chinese as a tea to treat “fever”. More recently, investiga-tors have shown that tea infusions and oral consumption of the dried leaves of the plant have prophylactic and therapeutic effcacy. The presence of a complex matrix of chemicals within the leaves seems to enhance both the bioavailability and effcacy of artemisinin. Although about 1000-fold less potent than artemisinin in their antiplasmodial activity, these plant chemicals are mainly small molecules that include other artemisinic compounds, terpenes （mainly mono and sesqui）, favonoids, and polyphenolic acids. In addition, polysaccharide constituents of A. an-nua may enhance bioavailability of artemisinin. Rodent pharmacokinetics showed longer T？ and Tmax and greater Cmax and AUC in Plasmodium chabaudi -infected mice treated with A. annua dried leaves than in healthy mice. Pharmacokinetics of deoxyartemisinin, a liver metabolite of artemisinin, was more inhibited in infected than in healthy mice. In healthy mice, artemisinin serum levels were 〉 40-fold greater in dried leaf fed mice than those fed with pure artemisinin. Human trial data showed that when delivered as dried leaves, 40-fold less artemisinin was required to obtain a therapeutic response compared to pure artemisinin. ACTs are still unaffordable for many malaria patients, and cost estimates for A. annua dried leaf tablet production are orders of magnitude less than for ACT, despite improvements in the production capacity. Cartography of Air Pollution in an Industrial City in North-Eastern Algeria by Using Two Indexes： Poleotolerance Index and Atmospheric Purity Index

Of all the methods of studies on bio-estimation of air pollution by lichen flora, the authors cannot justify a choice of one method over another. Consultation of previous work by researchers has allowed us to compare these methods and to better understand their strengths and limitations. Under the terms of estimating the pollution, these methods are classified into three categories. Some are qualitative, quantitative and others are finally some indirect methods. The methods used to assess the overall air pollution relative value for each station studied. As part of our work, the authors have chosen a quantitative approach based on the combination of two methods. They are based on pollution indices obtained from mathematical formulas based on various parameters related to the lichen flora. They are represented by the I.P. （index poleotolerance） and the I.A.P. （index of atmospheric purity） and these two indices allowed us to map of global air pollution in the city of Skikda and petrochemical industry as well as peri-urban areas （Haddaiek, Hamadi Krouma, Hamrouche Hamoudi, Larbi Ben M＇Hidi）. The authors have identified areas of iso-pollution around different emission centers represented by the largest petrochemical area, traffic and households in urban areas of Skikda, Hamadi Krouma, Hamrouche Hamoudi and Larbi Ben M＇Hidi.

Clinical Profile of Lymphatic Malformation from One Oral and Maxillofacial Center in Sudan

Background: Lymphatic malformations are developmental defects of the lymphatic system. They are frequently present at birth and are most commonly found in the head and neck regions. The aim of the study was to investigate the clinical presentation of head and neck Lymphatic malformations in relation to, age, gender, color and site of distribution. Methods: This is a retrospective descriptive study, where the record of the patient diagnosed as lymphatic malformation at the Department of Oral and Maxillofacial Surgery at Khartoum Teaching Dental Hospital, during the period of 2005 to 2008 where retrieved and analysed. Result: A cervicofacial lymphatic malformation was found more common in females than in males with a ratio of 11:9. The mean age was 14 ± 10 years, with lip and tongue being the most common sites, followed by the buccal mucosa in the intraoral site and neck in the posterior triangle was the most common site for extraoral site (30%). It was found that the bluish color was the commonest presenting color in oral lymphangioma followed by red pink color. Conclusion: Females are more affected than males, while infants and children were more affected than other age groups. The bulks of the patients were presented with soft swelling, blue to normal colour in the lip, neck and in the tongue. Further prospective studies are needed to evaluate the clinic picture and treatment outcome.

<i>Salvadora persica</i>(Miswak): Antioxidant and Promising Antiangiogenic Insights

The use of plants to improve dental health and to promote oral hygiene has been practiced for centuries and persists in several communities throughout the world. Salvadora persica (Miswak) has a wide geographic distribution. The ancient Arabs had the habit of using it to whiten and polish the teeth. Here, we determined in vitro antioxidant activity, total phenols and flavonoids and evaluated antiproliferative activity of the extract of S. persica (Miswak). The MTT assay was used to estimate the antiproliferative activities of the extract against human hepatoma (Hepg2) cancer cell line. Inhibition percentage of DPPH scavenging activity was dose-dependent and ranged from (30.7% ± 0.62) to (5.89% ± 0.98). The phenolic content was (2.7 ± 0.11) mg gAE/g while the flavonoid content was (2.70 ± 0.45) mg QE/g. Antiproliferative results of the extracts were found to be consistent with their antioxidant activity. Our extract also exhibited clear antiangiogenic activity. These findings introduce S. persica as the useful and novel potential anti-tumor agent for hepatocellular carcinoma (HCC) treatment.

Targetoid Polycyclic Concentric Hair Regrowth Pattern Is a Sign of Immunological Recovery in Alopecia Areata

Background: Alopecia areata is a common autoimmune disease, where there are many topical and systemic therapies, during which hair growth appears in a uniform pattern. In certain cases, hair regrowth takes a polycyclic pattern simulating a target, so called targetoid phenomena. Objective: To screen all cases of treated alopecia areata for hair targetoid regrowth pattern. ?Methods: This is an observational study, was conducted in Dermatology Department of Baghdad Teaching Hospital in Iraq, during the period, August 2017 to October 2017. All cases treated for alopecia areata with IM triamcinolone 20 - 40 mg injection every 2 weeks with topical clobetasone ointment and zinc sulfate 2 mg/kg/day for 2 months, were examined for the targetoid concentric hair regrowth pattern. Result: Among all cases of alopecia areata, seven patients were recorded to have the targetoid concentric hair pattern, which seen 2 - 4 weeks after therapy, their ages ranged from 4 to 25 years old with a mean 14.5, all of them were males. All these patients returned back with hair growth in a form of rings of hair growth alternating with rings of little or no hair growth forming a typical picture of target. Conclusion: Targetoid polycyclic concentric hair regrowth pattern is an important phenomenon seen in patients receiving treatment for alopecia areata, which is a good sign of recovery where its etiopathogenesis could not well be elucidated but alternating immunological reactions could explain this targetoid pattern.

Prognostic Value of Serum Uric Acid Level in Patients with ST Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention

Background: Serum uric acid (SUA) has been correlated with cardiac morbidity and mortality. However, its prognostic value in acute ST-segment elevation myocardial infarction (STEMI) is still uncertain. The aim of this study was to evaluate the prognostic value of SUA on admission in patients with STEMI undergoing primary percutaneous coronary intervention (PPCI). Methods: We prospectively enrolled 150 STEMI patients underwent PPCI. The mean age of the studied population was 56.1 years, 78%?were males?while 22% were females. Patients were divided into tertiles based on the basal serum uric acid level. Patients with high SUA (n = 72) was defined as a value in the third tertile (>6.4 mg/dl), and a low SUA group (n = 78) was set as a value in the lower two tertiles ( Results: SUA level on admission carried prognostic value in patients with STEMI undergoing PPCI where the low uric acid group had better KILLIP class (P =0.001), better TIMI flow (P = 0.001), higher ejection fraction (49.53 ± 8.75 versus 39.95 ± 7.06;P =0.001), better survival and lower incidence of other major adverse cardiac events (MACE) (P = 0.01) during the hospital stay and three months follow up?than the high uric acid group. Age, SUA > 6 mg/dl, TIMI flow, Killip class and EF 40% were independent predictors for MACE in such patients. Conclusions: High SUA level on admission was associated with higher frequency of in-hospital and three months follow up MACE in patients with acute STEMI undergoing PPCI.

Physiological Races of Fusarium oxysporum f. sp. Ciceris in Iraq

Twenty isolates of Fusarium oxysporum f. sp. ciceris were isolated from wilted chickpea plants obtained from different districts of north part of Iraq to assess variability in pathogenicity of the populations. Each isolate was tested on 12 differential chickpea varieties. Isolates showed highly significant variation in wilt severity on the differential varieties. Based on the reaction types that induced on differential varieties, isolates were grouped into four groups, First group included isolates FocSl, FocQ7, FocQ 10, FocFI3, FocH 17 and FocHl8; the second group included isolates FocS2, FocS3, FocS4, FocQ5, FocQ8, FocQ9, FocF11, FocF12, FocFl4 and FocH19; the third group included isolates FocF15, FocHl6, FocH20; where the isolate FocQ6 was placed in the fourth group. Results showed that the percentage of genetic similarity was ranged 42% to 100% and was 42% between the first group and other groups and 72% between the three groups the rest and thus this indicate the presence of four races of the fungus which are O, 4, 5 and 1B/C, this represent the first record of these races in lraq.

Y-Shaped Intra-Scleral Fixated Lens versus Retro-Pupillary Iris Claw Lens in the Treatment of Aphakia

Background: Many reasons can lead to an aphakia without adequate capsular support for implantation of a posterior chamber intraocular lens (IOL), such as intraoperative unintentional rupture of posterior capsule during phacoemulsification, planned intracapsular cataract extraction, ocular trauma and lens dislocation due to congenital and acquired causes. Purpose: To compare Y-shaped intra-scleral fixation of a posterior chamber IOL with retro pupillary fixation of an iris-claw intraocular lens (IOL) for Aphakic eyes without sufficient capsular support as respects safety, visual recovery and complications of both methods. Patients and Methods: One hundred Aphakic eyes were arbitrarily distributed between two groups. group A included 50 eyes treated with retropupillary fixation of iris claw lens and group B included 50 eyes treated with Y-shaped intra-scleral fixation technique. Preoperative, intraoperative and postoperative data were analysed including best corrected visual acuity (BCVA), intraocular pressure (IOP), surgical time, intraoperative problems, IOL malposition and postoperative complications. Following up on patients was carried out for at least six months. Results: The mean duration of surgery was 21 ± 5.3 min in group A and was 53.4 ± 6.9 min in group B (p-value 0.05). IOL tilt was found in 0 (0%) eyes in group A and in 5 (10%) eyes in group B (p 0.05). Conclusion: The results of our study indicated that both methods are satisfactory in correcting aphakia without sufficient capsular support as regards postoperative best corrected visual acuity (BCVA);however the surgical technique of retropupillary iris claw lens is easier, shorter, with low intra- and postoperative complications and safer than those used for intra-scleral fixation of IOL. But for eyes which lack both iris and capsular support, a scleral fixation of a posterior chamber IOL remains the only option.
