Background: Askin tumor is a primitive neuroectodermal tumor of the Ewing sarcoma family arising from soft tissues of the chest wall. It is the commonest chest wall malignancy in children and adolescents. Its diagnosis is complex and management requires a multidisciplinary work including chemotherapy for systemic disease and radiation therapy to assist local control which is achieved through surgery with or without reconstruction. Objective: To analyze report and understand the clinicopathological features, results and outcome of this tumor with assessment of early and late postoperative complications following resection and chest wall reconstruction. Materials and Methods: This is a retrospective analysis of 30 cases with chest wall ES/PNET presented to the National Cancer Institute;Cairo University between January 2011 and December 2015. All patients’ records were revised for age, sex, clinical presentation, imaging, pathology, operative notes, different treatment modalities given, early and late postoperative complications following surgical resection or reconstruction, and outcome including overall survival (OS) and disease free survival (DFS). Results: Our cohort included 30 patients with chest wall ES. The median follow up period (n = 30) was 33.7 months (ranging from 3.7 to 69.3 months). The median OS was 54.2 months with cumulative OS at 60 months which was 45.6%. The median DFS was 27.9 months with cumulative DFS at 60 months which was 40.6%. All cases were below 18 years with a male predominance (n = 19). The commonest affected sites were ribs (n = 22, 73.3%), the scapula (n = 5, 16.6%), the clavicle (n = 2, 6.66%) and the sternum (n = 1, 3.33%). 23 patients (76.6%) were presented with localized disease, and 7 patients (23.4%) were metastatic from the start. Objective:To investigate the effect of two extracts of Bougainvillea spectabilis(B. spectabilis) flowers with yellow and pink/purple on brain oxidative stress and neuronal damage caused in rats by systemic rotenone injection. Methods:Rotenone 1.5 mg/kg was given three times per week alone or in combination with B. spectabilis flowers extracts(25 mg or 50 mg) via the subcutaneous route for 2 weeks. Brain concentrations of the lipid peroxidation marker malondialdehyde(MDA),reduced glutathione,nitric oxide(nitrite),the pro-inflammatory cytokine interleukin-1beta(Il-1β) as well as butyrylcholinesterase,and paraoxonase-1(PON-1) activities,were determined. Histopathology and caspase-3 immunohistochemistry were also performed. Results:Rotenone resulted in significant increases of brain MDA(the product of lipid peroxidation),and nitric oxide content along with decreased brain reduced glutathione. There were also marked and significant inhibition of brain PON-1 and BCh E activities and increased Il-1β in brain of rotenone-treated rats. B. spectabilis flowers extract itself resulted in brain oxidative stress increasing both lipid peroxidation and nitrite content whilst inhibiting PON-1 activity. The yellow flowers extract inhibited BCh E activity and increased brain Il-1β. When given to rotenone-treated rats,B. spectabilis extracts,however,decreased lipid peroxidation while their low administered doses increased brain GSH. Brain nitrite decreased by the pink extract but showed further increase by the yellow extract. Either extract,however,caused further inhibition of PON-1 activity while the yellow extract resulted in further inhibition of BChE activity. Histopathological studies indicated that both extracts protected against brain,liver and kidney damage caused by the toxicant. Conclusions:These data indicate that B. spectabilis flowers extracts exert protective effect against the toxic effects of rotenone on brain,liver and kidney. A micro structure porous cored octagonal photonic crystal fiber （P-OPCF） has been proposed to sense aqueous analysts （alcohol series） over a wavelength range of 0.80 μm to 2.0 μm. By implementing a full vectorial finite element method （FEM）, the numerical simulation on the proposed O-PCF has been analyzed. Numerical investigation shows that high sensitivity can be gained by changing the structural parameters. The obtained result shows the sensitivities of 66.78%, 67.66%, 68.34%, 68.72%, and 69.09%, and the confinement losses of 2.42×10^-10 dB/m, 3.28x×10^-11 dB/m, 1.21 ×10^-6 dB/m, 4.79×10^-10 dB/m, and 4.99×10^-9 dB/m at the 1.33 ktm wavelength for methanol, ethanol, propanol, butanol, and pentanol, respectively can satisfy the condition of much legibility to install an optical system. The effects of the varying core and cladding diameters, pitch distance, operating wavelength, and effective refractive index are also reported here. It reflects that a significant sensitivity and low confinement loss can be achieved by the proposed P-OPCE The proposed P-OPCF also covers the wavelength band （O＋E＋S＋C＋L＋U）. The investigation also exhibits that the sensitivity increases when the wavelength increases like SO-band〈SE-band 〈SS-band 〈 SC-band 〈SL-band 〈SU-band. This research observation has much pellucidity which has remarkable impact on the field of optical fiber sensor.

Impact of replacing egg yolk with lecithin on quality of pre-freeze and post-thaw buffalo spermatozoa

Objective:To estimate the result of egg yolk replacement with alternative cryopreservatives such as plant-derived lecithin from soybean on sperm quality parameters pre and post freezing in buffalo ***: The control cryopreservation extender was tris-citric acid-fructose-egg yolk-glycerol (TCFYG) diluent. Semen samples were extended gradually 1:10 with TCFYG control extender and tris-citric acid-fructose-glycerol (TCFG) extender plus variable concentrations of soybean lecithin (0.5%, 1.0%, 1.5%, 2.0%, 2.5% and 3.0%) to ensure 60 million active spermatozoa/mL of the extended semen. The diluted semen samples were refrigerated slowly (roughly for 2 h) up to 5℃ and equilibrated for 2 h. Semen was filled into 0.25 mL polyvinyl French straws (IMV, France). After equilibration period, the straws were placed horizontally on a rack and frozen in a vapor 4 cm above liquid nitrogen for 10 min and were then dipped stored in liquid nitrogen at -196℃. Results:The respective overall percentages of forward motile spermatozoa, live spermatozoa, morphologically normal spermatozoa, acrosome integrity and hypo-osmotic swelling reactivity observed primarily in fresh semen, after equilibration (pre-freeze stage) and post freezing (post-thaw stage) in TCFYG (control) extended semen declined progressively and statically (P<0.01) during these periods of study. Pre-freezing stage: replacement of egg yolk into TCFG with soybean lecithin at concentrations of 1.0% and 1.5% significantly (P<0.01) ameliorated the maintenance of (motility, viability, acrosome and membrane integrity %), meanwhile it had significantly (P<0.01) reduced the abnormality % of spermatozoa to the lowest value compared to control TCFYG and to some other concentrations in use. Background Flexible ureterorenoscopy(fURS)and mini-percutaneous nephrolithotomy(mPCNL)have been increasingly used for the treatment of renal ***,current guidelines do not recommend one modality over the *** aim of this study is to compare the safety and efficacy of treatment with fURS versus mPCNL for renal stones sized 2 cm or *** and methods A prospective,randomized,comparative study was conducted between January 2019 and July 2021 at 3 tertiary care urology *** criteria were adult patients with renal stone(s)≤2 cm with inappropriateness or failure of extracorporeal shock-wave *** were assigned to 1 of 2 treatment groups,either mPCNL or *** primary outcomes were assessed:(1)initial success rate,defined as the absence of clinically significant residual fragments(>2 mm)on kidney ureter bladder X-ray and ultrasound on the first postoperative day;and(2)complications,which were reported according to the Modified Clavien-Dindo classification *** outcomes included final success rate,defined as the absence of clinically significant residual fragments on noncontrast computed tomography on the 90th postoperative day;operative time;auxiliary procedures and blood transfusion rates;hemoglobin drop;and length of hospital *** One hundred and eighteen procedures were analyzed(59 in each group).The initial success rate of the mPCNL group(93%)was significantly higher than that of the fURS group(70%).Complications occurred more frequently with mPCNL than fURS(44.1%vs.18.6%,respectively).Final success rate,operative time,and length of hospital stay were comparable between the 2 *** Mini-percutaneous nephrolithotomy is more effective than fURS as a single-step treatment for renal stones<2 cm because of its higher initial success rate and lower auxiliary procedure ***,mPCNL results in significantly higher complication rates than fURS.

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma (HCC) with a panel of serum proteins in combination with α-fetoprotein (AFP).METHODS: Serum levels of interleukin (IL)-8, soluble intercellular adhesion molecule-1 (sICAM-1), soluble tumor necrosis factor receptor II (sTNF-RII), proteasome, and β-catenin were measured in 479 subjects categorized into four groups: (1) HCC concurrent with hepatitis C virus (HCV) infection (n = 192); (2) HCV related liver cirrhosis (LC) (n = 96); (3) Chronic hepatitis C (CHC) (n = 96); and (4) Healthy controls (n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different ***: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model (-11.3 + 7.38 × Prot + 0.00108 × sICAM + 0.2574 × β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × sICAM-1 + (4.251 × 10-4) × sTNF + 0.02567 × β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. In the present work, lignin is isolated from three different agro-industrial waste, sweet sorghum, rice straw and sugarcane bagasse using in-situ sodium hydroxide-sodium bisulfate methodology. Characterization was performed using fourier transform infrared analysis (FTIR), scan electron microscopy (SEM), thermo gravimetric analysis (TGA). The SEM micrographs showed sponge-like structure except for sugarcane bagasse lignin reveals rock-like structure. The FTIR indicates the presence of hydroxyl, carbonyl and methoxyl groups in the lignin structure. TGA thermograms were relatively same and sugarcane bagasse lignin was found the most thermally stable up to 201˚C as compared to both of soda and kraft sugarcane bagasse lignin and its maximal temperature degradation rate DTGmax was found at 494˚C while 450˚C, 464˚C in addition to thermal stabilities up to 173˚C and 180˚C for sweet sorghum and rice straw lignins respectively.
