Effect of environmental endotoxin exposure on development of pediatric asthma among Egypytian school children

Objective: The aim of this study was to assess school indoor exposure to microbial products and prevalence of asthma and allergies in rural and urban ***: This study was carried on a rural and an urban *** endotoxin level was measured in multiple samples of the ambient indoor air dust collected on special aseptic filter papers from the two *** two hundred children history taking,clinical examination,allergen skin prick test and basic pulmonary function test were ***: Environmental endotoxin levels showed significantly higher mean values(P<0.01) in rural school(3 EU/mg) as compared to the urban school(0.1 EU/mg) with(OR=5.163;95% CI: 0.95-28).History of allergic symptoms was significantly more in urban than rural students(P=0.01).Mean values of pulmonary function parameters were significantly lower values in urban students compared to rural *** prick test results showed significant reactions to all tested allergens in urban children compared to rural children(P<0.05).Conclusion: There is an inverse association between environmental exposure to endotoxins and susceptibility for allergic manifestations in school children.

Electrical Parameters Investigation and Zero Flow Rate Effect of Nitrogen Atmospheric Nonthermal Plasma Jet

The construction and operation of atmospheric nonthermal plasma jet, ANPJ, are presented in this work as well as the experimental investigations of its electrical parameters, the configuration of plasma jet column and its temperature. The device is energized by a low-cost Neon power supply of (10 kV, 30 mA, and 20 kHz) and the discharge takes place by using N2 gas with different flow rates from 3 to 25 L/min and input voltage of 6 kV. Diagnostic techniques such as voltage divider, Lissajous figure, image processing and thermometer are used. The electrical characteristics of discharge at different flow rates of N2 gas such as discharge voltage, current, mean power, power efficiency, and mean energy have been studied. The experimental results show that the maximum plasma jet length of 14 mm is detected at flow rate of 12 L/min. The results of plasma jet (heavy particles) temperature along the jet length show that jet plasma has approximately a room temperature at the jet column end. The results of zero flow rate effect on the ANPJ operation show damage in the Teflon insulator and a corrosion in the Aluminum electrodes.

Incidence and Outcomes of Surgical Site Infections after Adult Cardiac Surgery: A Single-Center Experience

Background: Surgical site infections (SSIs) remain a challenging medical problem, especially in cardiac surgery patients. There is a lack of studies evaluating the rate of and outcomes of SSIs following cardiac surgeries in Saudi Arabia. Aims: This study aimed to determine the incidence of SSIs after adult cardiac surgeries that were done in Madinah Cardiac Center, Saudi Arabia. Further, to identify the outcomes and risk factors contributing to death among surgical site infection patients. Methods: This was a 6-year, single-center, retrospective cohort study that included 93 consecutive patients who underwent adult cardiac surgery between August 2016 and August 2022. All adult patients aged 18 years or older who had clinical evidence of postoperative surgical wound infection were included. Preoperative, operative, and postoperative data (early and late) were collected from medical records. Microbiological culture reports and clinical outcomes were also recorded. Results: The study revealed a 9.1% (93/1021) incidence rate of SSIs. Of the 93 patients with SSIs, 60 had superficial incisional infections and 33 had deep infections with incidence rates of 5.9% and 3.2%, respectively. In-hospital mortality due to SSI was recorded in 4 out of 93 patients with an incidence rate of 4.3%. There was a significant association between the in-hospital mortality and the type of SSIs (p = 0.014). All non-survivors had deep SSIs. The type of cardiac surgery also showed a significant association with the in-hospital mortality (p = 0.017). Furthermore, the median duration of antibiotic administration was significantly longer in the non-survivors than in the survivors (72.5 vs 17, respectively, p Conclusions: In conclusion, the incidence of surgical wound infections following cardiac surgery is not low (9.1%);of which 3.2% were deep infections. AIM:To assess the effectiveness of hepatitis B virus(HBV) vaccination program among fully vaccinated ***:A national community based crosssectional study was carried out in 6 governorates representing Egypt. A total of 3600 children aged from 9 mo to 16 years who were fully vaccinated with HBV vaccine during infancy were recruited. Face to face interviews were carried out and sera were evaluated for hepatitis B surface antigen(HBsA g),anti-HBV core antibodies(total) and quantitative detection of hepatitis B surface antibody using enzyme linked immunoassays techniques. Samples positive to HBs Ag/anti-HBV core antibodies were subjected to quantitative HBV-DNA detection by real time polymerase chain reaction with 3.8 IU/L detection limit. RESULTS:Sero-protection was detected among 2059 children(57.2%) with geometric mean titers 75.4 ± 3.6 IU/L compared to 3.1 ± 2.1 IU/L among nonseroprotected children. Multivariate logistic analysis revealed that older age and female gender were the significant predicting variables for having non seroprotective level,with adjusted odds ratio 3.3,9.1and 14.2 among children aged 5 to 0.05. C ON C LU S I ON :T he E gy pt ian c ompuls or y H B V vaccination program provides adequate protection. Occult HBV infection exists among apparently healthy vaccinated children. Adherence to infection control measures is mandatory.

目的探讨周期性机械拉伸对类风湿关节炎成纤维样滑膜细胞(RA-FLSs)增殖能力的影响。方法实验组细胞在周期性机械拉伸频率为1.0 Hz、拉伸幅度为3%、6%和9%的条件下,分别对RA-FLSs加载2、6和12 h。对照组细胞在保持与实验组培养条件一致的情况下不进行拉伸刺激。机械拉伸后,用流式细胞术和MTS检测细胞的增殖和活性。RT-PCR检测加载后细胞周期调控因子(CyclinD1、CyclinE1、CDK2、P27)在基因水平上的表达变化。结果 6%和9%的拉伸刺激持续作用6、12 h使RA-FLSs增殖和活性显著降低(P<0.05),同时CDK2和CyclinE1的mRNA表达降低,P27 mRNA的表达增高(P<0.05),周期性机械拉伸对CyclinD1表达的影响相对较小。结论周期性机械拉伸对RA-FLSs增殖能力的影响与拉伸强度以及持续的时间有关,6%和9%的机械拉伸刺激可以抑制RA-FLSs的增殖,而这种增殖抑制作用可能是通过调控CyclinE1,CDK2和P27的表达来实现的。本研究对于探讨力学刺激在类风湿性关节炎的发病机制以及临床防治中的作用具有一定意义。

本文评估了公共部门高级官员认为的新公共管理(NPM)式改革对欧洲国家的影响。通过使用在二十个欧洲国家/地区进行的行政调查数据,我们研究了新公共管理的五项关键改革(组织精简、机构化、外包、顾客导向和弹性化雇佣的实践)与公共部门绩效的四个维度(成本效率、服务质量、政策的连贯与协调和平等获得服务的机会)之间的关系。通过建立结构方程模型,我们发现,将服务使用者视为客户以及弹性化雇佣均与改善全部四个绩效维度之间呈正相关的关系。外包和机构精简,二者都和提高效率成正比关系,但是服务质量的下降也会伴随机构精简而来。创建自治机构和绩效之间并无关系。这表明,决策者在对公共部门进行现代化改革时,公共组织内部的管理性改革应优先于结构性改革。
