Pulmonary diseases are common throughout the world,especially in developing *** diseases include chronic obstructive pulmonary diseases,pneumonia,asthma,tuberculosis,fibrosis,and recently *** general,pulmonary diseases have a similar footprint on chest radiographs which makes them difficult to discriminate even for expert *** recent years,many image processing techniques and artificial intelligence models have been developed to quickly and accurately diagnose lung *** this paper,the performance of four popular pretrained models(namely VGG16,DenseNet201,DarkNet19,and XceptionNet)in distinguishing between different pulmonary diseases was *** the best of our knowledge,this is the first published study to ever attempt to distinguish all four cases normal,pneumonia,COVID-19 and lung opacity from ChestX-Ray(CXR)*** models were trained using Chest-X-Ray(CXR)images,and statistically tested using 5-fold cross *** individual models,XceptionNet outperformed all other models with a 94.775%accuracy and Area Under the Curve(AUC)of Receiver Operating Characteristic(ROC)of 99.84%.On the other hand,DarkNet19 represents a good compromise between accuracy,fast convergence,resource utilization,and near real time detection(0.33 s).Using a collection of models,the 97.79%accuracy achieved by Ensemble Features was the highest among all surveyed methods,but it takes the longest time to predict an image(5.68 s).An efficient effective decision support system can be developed using one of those approaches to assist radiologists in the field make the right assessment in terms of accuracy and prediction time,such a dependable system can be used in rural areas and various healthcare sectors.

Geochemical characteristics of natural gases and source rocks in Obayied sub-basin,north Western Desert,Egypt:implications for gas-source correlation

Western Desert represents a major oil and gas province in Egypt producing more than 50%of the country’s oil *** and gas blend occurs in most producing fields,however,the genetic link between gas and liquid hydrocarbon phases are not *** sub-basin in the Western Desert where oil and gas phases coexist in the Middle Jurassic sandstones of the Khatatba Formation provides an ideal place to investigate the link between oil and gas *** analyses on rock samples(Rock–Eval pyrolysis,vitrinite reflectance,R_(o))and gases(molecular and isotopic composition)were conducted in order to identify the genetic characteristics of the hydrocarbon phases ***-relevant parameters(Rock–Eval T_(max),vitrinite reflectance R_(o))elucidate that only Middle Jurassic Khatatba organofacies capable of generating wet and dry hydrocarbon ***,the enrichment of C_(7)normal alkanes,mono-branched alkanes relative to polybranched components in the Obayied gases reflect their generation via cracking of *** modelling results confirm gas generation through both primary and secondary ***,secondary cracking of liquid hydrocarbon phases is volumetrically more *** cracking of the Khatatba organofacies likely predate and catalyze the secondary cracking of the liquid phases and therefore the volume of generated gas increases incrementally eastward where both processes *** present study highlights the significant role of secondary cracking in the generation and accumulation of huge gas accumulations in the basins containing oil-prone source intervals.

Role of ERCP in the era of laparoscopic cholecystectomy for the evaluation of choledocholithiasis in sickle cell anemia

AIM:To evaluate the role of endoscopic retrograde cholangiopancreatography (ERCP) for choledocholithiasis in patients with sickle cell anemia (SCA) in the era of laparoscopic cholecystectomy (LC). METHODS:Two hundred and twenty four patients (144 male,80 female; mean age,22.4 years; range,5-70 years) with SCA underwent ERCP as part of their evaluation for cholestatic jaundice (CJ). The indications for ERCP were:CJ only in 97,CJ and dilated bile ducts on ultrasound in 103,and CJ and common bile duct (CBD) stones on ultrasound in 42. RESULTS:In total,CBD stones were found in 88 (39.3%) patients and there was evidence of recent stone passage in 16. Fifteen were post-LC patients. These had endoscopic sphincterotomy and stone extraction. The remaining 73 had endoscopic sphincterotomy and stone extraction followed by LC without an intraoperative ***:In patients with SCA and cholelithiasis,ERCP is valuable whether preoperative or postoperative,and in none was there a need to perform intraoperative cholangiography. Sequential endoscopic sphincterotomy and stone extraction followed by LC is beneficial in these patients. Endoscopic sphincterotomy may also prove to be useful in these patients as it may prevent the future development of biliary sludge and bile duct stones.

Hydrocarbon-source correlation in the Obayied sub-basin， north Western Desert， Egypt: controls on generation of natural gas and light crude hydrocarbon blend

Obayied sub-basin provides one-third of the annual natural gas production in the Egyptian Western Desert. The origin of the Obayied hydrocarbons are however poorly constrained. In this study， the molecular biomarkers of the Obayied hydrocarbon blend were studied to infer on their origin and generation mechanism. API values are in the range of (41.3°-53.7°) reflecting post-mature hydrocarbons. The molecular biomarkers suggest a generation of Obayied crude from clay-rich fluvio-deltaic source rocks. Age- and maturity-relevant biomarkers (e.g. Ts/Tm trisnorhopanes and methylphenanthrene indices) reflect a successive expulsion of the Obayied crudes from mature Jurassic rocks (> 1%Ro). Biological markers correlate perfectly with those of the Jurassic Khatatba shale and coal extracts attesting. Additionally， the Obayied gases are wet， thermogenic and have been derived from a mature (1.3%-2%Ro) type III kerogen. The studied gases display compositional characteristics of mixed coal- and oil-type gases， and were therefore derived via primary cracking of the Khatatba coal as well as secondary cracking of the light liquid crudes. The present study clarifies the controls on the geochemical processes responsible for the accumulation of liquid and gas hydrocarbon mix in the deep as well as shallow inverted rift basins of the north Western Desert， Egypt.

BACKGROUND Nonalcoholic steatohepatitis-related cirrhosis is one of the liver complications in type 2 diabetes mellitus(T2DM)and reported to be a risk factor for developing hepatocellular carcinoma(HCC).A reliable screening biomarker of liver cirrhosis(LC)and HCC among T2DM patients is important to reduce the morbidity and mortality of this ***(miRNA)is considered a key player in HCC and T2DM,and it might be a hidden culprit in diabetes-associated HCC,making it a promising reliable prognostic *** To investigate the signature of serum miRNAs as early biomarkers for the screening of HCC among diabetic *** Expression profiles of miRNAs in serum samples of diabetic LC and diabetic HCC patients were assessed using Illumina sequencing;then,RT-qPCR was used to validate significantly altered miRNAs between the two *** miRNAs were tested in serum samples of 200 T2DM patients,270 LC patients,200 HCC patients,and 225 healthy control ***,receiver operating characteristic(ROC)analysis,with area under the curve(AUC),was performed to assess the diagnostic performance of the screened miRNAs for discriminating HCC from LC and nonmalignant patients(LC+T2DM).RESULTS Expression of the sequenced miRNAs in serum was different in HCC vs LCpositive T2DM *** miRNAs(miR-34a,miR-221)were significantly upregulated and five miRNAs(miR-16,miR-23-3p,miR-122-5p,miR-198,miR-199a-3p)were significantly down-regulated in HCC compared to LC *** of ROC curve demonstrated that the combination of these seven miRNAs can be used as a reliable biomarker for detection of HCC in diabetic patients,as it could identify HCC with high diagnostic accuracy in diabetic LC patients(AUC=0.993)and in diabetic nonmalignant patients(AUC=0.961).CONCLUSION This study validates a panel of serum miRNAs that can be used as a reliable noninvasive screening biomarker of HCC among T2DM cirrhotic and noncirrhotic *** study recommends furt

目的确定接受不同治疗方式的急性心肌梗死患者的疾病认知及二级预防结局的差异。方法采用重复测量设计,对接受3种不同治疗方式的心肌梗死患者进行比较。3种治疗方式包括:对ST段抬高心肌梗死患者进行经皮冠状动脉介入治疗;对ST段抬高心肌梗死患者进行溶栓治疗;对非ST段抬高心肌梗死患者进行药物治疗。采用便利抽样法在安曼国际心脏研究所选取206例急性心肌梗死患者,他们均同意参加此次研究。在患者入院及出院6个月时收集其疾病认知、体力活动、人口统计学及临床资料。结果共有186例患者完成了此项研究。经皮冠状动脉介入治疗组与溶栓治疗组、非ST段抬高心肌梗死药物治疗组患者比较,具有较高的疾病敏感性(P=0.034),较低的自我管理(P=0.032),较高的治疗管理(P=0.025),较高的对疾病认知的一致性(P=0.022)。同时,经皮冠状动脉介入治疗组对血压控制较差(P=0.013),体力活动较少(P=0.001)。结论与溶栓治疗组及非ST段抬高心肌梗死药物治疗组患者比较,经皮冠状动脉介入治疗组患者出院6个月后具有消极的疾病认知及有限的行为改变。应进行更深入的研究了解延长随访时间和不同文化背景下治疗方式与心肌梗死患者疾病认知和二级预防的关系。

Diffuse Cervico-Facial Cellulitis: Epidemiological and Clinical Aspects of 297 Cases

Introduction: Cervicofacial cellulitis is a medical and surgical emergency, it is an extensive infection of the aponeurotic spaces of the face and neck. although frequent in black Africa rather than in developed countries, they remain associated with a poor prognosis and few studies have been done on this subject. Their management must be early, probabilistic and then adapted to the bacteriological results of the pus taken. As Senegal is one of the many sub-Saharan African countries without recent data on cervicofacial cellulitis, we felt it was important to review the situation. The aim of our study was to describe the epidemiological and clinical aspects of cervicofacial cellulitis in the ENT department of Lamine Sine DIOP of the Fann National University Hospital of Dakar. Materials and Method: This was a retrospective study conducted over 5 years from January 2017 to December 2021 in the ENT-CCF LAMINE SINE DIOP Department of the CHNU FANN in Dakar. Our data sources were hospitalization registers and medical records of patients hospitalized for cervicofacial cellulitis. Our data were analyzed using the spss 12 for windows software. Results: During this 5-year period, we collected 297 cases of cervicofacial cellulitis, i.e. 19% of the hospitalizations. The average age was 35 years with a sex ratio of 2.23. The notion of taking NSAIDs was found in 90.57% of the cases, the immunocompromised background was present in 18 patients, i.e. 6.06% of the cases. The site of the swelling was sub-mento-maxillary in 49% of the cases and anterior cervical in 5% of cases. The portal of entry was dental in 189 patients, i.e. 64% of cases, and unknown in 3%. 4 deaths were recorded, i.e. 1.35% of patients hospitalized for cervicofacial cellulitis, and 1 case of mediastinitis with polyseritis was transferred to the Department of Thoracic and Cardiovascular Surgery, which had a favourable outcome. Conclusion: Cervicofacial cellulitis is a fairly common infection with a poor prognosis
