Some pelvic pain syndromes are very resistant to medical treatment. Several studies have demonstrated that sacral neuromodulation, which has been successfully used for the treatment of bladder dysfunction, incontinence, urinary retention and urinary frequency [1]-[3], can be successfully used for the treatment of chronic pelvic pain [4]-[7]. Several studies have also demonstrated significant involvement of dorsal column pathways in the transmission of visceral pelvic pain [8] and the successful use of spinal cord stimulation for the treatment of chronic pelvic pain [9]. We report three cases of severe chronic pelvic pain that failed conservative treatment modalities. Placement of a combined sacral nerve roots stimulator and a low thoracic spinal cord stimulator resulted in a significant pain relief and improvement in daily life activities. We believe that this combination may help patients suffering from chronic pelvic pain resistant to medical management.

In this article we discuss the effect of posture on impedance during the placement of a spinal cord stimulator. Although, according to several studies, there appears not to be a specific and consistent change in impedance with change in posture, we noticed on multiple occasions that if the impedance remains very high during spinal cord stimulator (SCS) lead placement, asking the patient to sit up from the prone position may drop the impedance down. This could be explained by having air around the lead, from using loss of resistance to air technique in finding the epidural space, which moves up when sitting the patient up, or having the lead immersed in epidural fat. Sitting the patient up from the prone position should be tried if the impedance remains very high before aborting the procedure and taking the lead out.

A 40-years-old female patient with severe right facial pain with a throbbing component along the mandibular division of the trigeminal nerve resistant to medical management. Continuous inferior alveolar nerve block with local anesthetics using an indwelling catheter provided a complete pain resolution for the patient for 2 weeks, after which the catheter got infected and was removed. A trial of balloon occlusion of the right internal maxillary artery provided complete resolution of the throbbing component of the patient’s pain. This was followed by permanent embolization with multiple coils.

BACKGROUND Proton pump inhibitors are often used to prevent gastro-intestinal lesions induced by nonsteroidal anti-inflammatory drugs.However,they are not always effective against both gastric and duodenal lesions and their use is not devoid of side effects.AIM To explore the mechanisms mediating the clinical efficacy of STW 5 in gastroduodenal lesions induced by nonsteroidal anti-inflammatory drugs(NSAIDs),exemplified here by diclofenac,in a comparison to omeprazole.METHODS Gastro-duodenal lesions were induced in rats by oral administration of diclofenac(5 mg/kg)for 6 successive days.One group was given concurrently STW 5(5 mL/kg)while another was given omeprazole(20 mg/kg).A day later,animals were sacrificed,stomach and duodenum excised and divided into 2 segments:One for histological examination and one for measuring inflammatory mediators(tumor necrosis factorα,interleukins-1βand 10),oxidative stress enzyme(heme oxygenase-1)and apoptosis regulator(B-cell lymphoma 2).RESULTS Diclofenac caused overt histological damage in both tissues,associated with parallel changes in all parameters measured.STW 5 and omeprazole effectively prevented these changes,but STW 5 superseded omeprazole in protecting against histological damage,particularly in the duodenum.CONCLUSION The findings support the therapeutic usefulness of STW 5 and its superiority over omeprazole as adjuvant therapy to NSAIDs to protect against their possible gastro-duodenal side effects.

Objective:Placement of human placenta derived grafts during robotic-assisted radical prostatectomy(RARP)hastens the return of continence and potency.The long-term impact on the oncologic outcomes remains to be investigated.Our objective was to determine the oncologic outcomes of patients with dehydrated human amnion chorion membrane(dHACM)at RARP compared to a matched cohort.Methods:In a referral centre,from August 2013 to October 2019,599 patients used dHACM in bilateral nerve-sparing RARP.We excluded patients with less than 12 months follow-up,simple prostatectomy,and unilateral nerve-sparing.Patients with dHACM(amnio group)were 529,and were propensity score matched 1:1 to 2465 patients without dHACM(non-amnio group)and a minimum follow-up of 36 months.At the time of RARP,dHACM was placed around the neurovascular bundle in the amnio group.Continuous and categorical variables in matched groups was tested by two-sample Kolmogorov-Smirnov test and Fisher's exact test respectively.Outcomes measured were biochemical recurrence(BCR),adjuvant and salvage therapy rates.Results:Propensity score matching resulted in two groups of 444 patients.Cumulative incidence functions for BCR did not show a difference between the groups(p=0.3).Patients in the non-amnio group required salvage therapy more frequently than the amnio group,particularly after partial nerve-sparing RARP(6.3%vs.2.3%,p=0.001).Limitations are the absence of prospective randomization.Conclusion:The data suggest that using dHACM does not have a negative impact on BCR in patients.Outcomes of cancer specific and overall survival will require follow-up study to increase our understanding of these grafts’impact on prostate cancer biology.

Micro RNAs and clinical implications in hepatocellular carcinoma

AIM To assess the role of some circulating miRNAs(miR-23a, miR-203, miR338, miR-34, and miR-16) as tumor markers for diagnosis of hepatocellular carcinoma(HCC).METHODS One hundred and seventy-one subjects were enrolled, 57 patients with HCC, 57 patients with liver cirrhosis(LC) and 57 healthy subjects as control group. Severity of liver disease was assessed by Child Pugh score. Tumor staging was done using Okuda staging system. Quantification of Micro RNA(miR-23a, miR-203, miR338, miR-34, and miR-16) was performed.RESULTS All studied miRNA showed significant difference between HCC and cirrhotic patients in comparison tohealthy control. miR-23a showed statistically significant difference between HCC and cirrhotic patients being higher in HCC group than cirrhotic. miR-23a is significantly higher in HCC patients with focal lesion size equal or more than 5cm, patients with multiple focal lesions and Okuda stage Ⅲ. At cutoff value ≥ 210, miR-23a showed accuracy 79.3% to diagnose HCC patients with sensitivity 89.47% and specificity about 64.91%. At cut off level ≥ 200 ng/mL, serum alpha fetoprotein had 73.68% sensitivity, 52.63% specificity, 43.75% PPV, 80% NPV for diagnosis of HCC.CONCLUSION MicroR NA 23a can be used as a screening test for early detection of HCC. Also, it is related to larger size of tumour, late Okuda staging and multiple hepatic focal lesions, so it might be a prognostic biomarker.

Temperature Effects on the Electrical Performance of Large Area Multicrystalline Silicon Solar Cells Using the Current Shunt Measuring Technique

The temperature effects on the electrical performance of a large area multicrystalline silicon solar cell with back-contact technology have been studied in a desert area under ambient conditions using the current shunt measuring technique. Therefore, most of the problems encountered with traditional measuring techniques are avoided. The temperature dependency of the current shunt from 5oC up to 50oC has been investigated. Its temperature coefficient proves to be negligible which means that the temperature dependency of the solar cell is completely independent of the current shunt. The solar module installed in a tilted position at the optimum angle of the location, has been tested in two different seasons (winter and summer). The obtained solar cell short circuit current, open circuit voltage and output power are correlated with the measured incident radiation in both seasons and all results are discussed.

The Lie group method is applied to present an analysis of the magneto hydro-dynamics（MHD） steady laminar flow and the heat transfer from a warm laminar liquid flow to a melting moving surface in the presence of thermal radiation.By using the Lie group method,we have presented the transformation groups for the problem apart from the scaling group.The application of this method reduces the partial differential equations（PDEs） with their boundary conditions governing the flow and heat transfer to a system of nonlinear ordinary differential equations（ODEs） with appropriate boundary conditions.The resulting nonlinear system of ODEs is solved numerically using the implicit finite difference method（FDM）.The local skin-friction coefficients and the local Nusselt numbers for different physical parameters are presented in a table.

Background: Malignant pleural mesothelioma is a lethal disease and hence the strong need for identifying new prognostic factors. Methods: This is a retrospective study including all eligible patients with advanced malignant pleural mesothelioma (MPM) presenting to National Cancer Institute, Cairo University. Neutrophil lymphocyte (N/L) ratio was assessed before second line chemotherapy. 2.5 was used as the cutoff point. Endpoints were the assessment of correlation between N/L ratio and clinical response (CR), progression free survival (PFS) and overall survival (OS). Results: 52 patients (19 stage III and 33 stage IV) MPM were included and followed up during the period from July 2009 till November 2012 with a median follow-up period of 2.6 months. 87.5% of patients with N/L ratio > 2.5 showed progressive disease versus 91.7% in patients with N/L ratio 2.5 versus 14% for patients with N/L ratio 2.5 versus 66% for patients with N/L ratio < 2.5 (P-value = 0.4). Conclusion: N/L ratio is a potential prognostic marker for advanced MPM treated with second line chemotherapy.

Groundwater Conditions and the Geoenvironmental Impacts of the Recent Development in the South Eastern Part of the Western Desert of Egypt

The area to the southeast of the Western Desert of Egypt has been subjected to considerable development activities over the last few years. The development includes the cultivation of about 2260 km2 of the desert lands “the well-known Toshka Project”. The hydrogeological conditions of the area are subjected to detailed investigation based upon the construction of the water table maps, hydrologeologic cross-sections, pumping tests, aquifer geometry, and recharge-discharge relationship. The study revealed that the Quaternary and the Nubia sediments are the main water bearing layers in the area. The Quaternary aquifer is of limited potential and made of mixed sand with clay deposit ranges in thickness between 5 to 10 m. The Nubia aquifer is the oldest sedimentary formation and the main groundwater resources in the area. It is represented by multilayered of sand and silt exists generally under artesian conditions. It is composed of three water bearing horizons partially separated by two confining horizons and extends in thickness ranges between 70 and 230 meters. The thickness increases away from the high dam lake. The analysis of pumping tests of the aquifer indicated that its potentiality is increasing north of the High Dam Lake (HDL) whereas it decreases in the other direction. This is due to high hydraulic conductivity and aquifer thickness in the area northeast of Khor Toshka and at west of Garf Hussein. The hydraulic conductivity of the aquifer ranges between 12.73 and 0.9 m/day. The review of the changes in groundwater levels in the area showed that there is a drop in ranges between 1 and 14 meters in the last few years indicating that the extraction from the groundwater is much more higher that the replacement rate. Also, the analysis of the fluctuation of water levels of the HDL and the groundwater level indicated that the influence of water on groundwater level in the area is observed only at a distance less than 10 km from the lake shore line. Seepage from the HDL is estimated as 238.13 × 106 m3/year. The
