Effect of Oxygen Therapy by Venturi Mask versus Non Invasive Ventillation on the Outcome of Patients Who Devolope Hypoxia after Open Heart Surgery

Background: Cardiothoracic surgery is a common cause of acute respiratory failure and is associated with increased morbidity and mortality. We aimed to compare the outcomes of open heart surgery patients treated using oxygen delivery devices with patients who receive NIV as a first-line therapy for hypoxemic respiratory failure. Materials and Methods: 40 patients who developed acute hypoxemic respiratory failure after open heart surgery and admitted to cardiothoracic ICU 20 patients received NIV and 20 patient received oxygen by venture mask. For all patients the following measurements were performed before and after CPAP AND Venture use: CBC, blood urea, serum creatinine body temperature, chest X-ray, Arterial blood gases (arterial pH, sodium bicarbonate, pcO2, SpO2 and PaO2-to-FiO2 ratio). Results: Mean PO2 and SO2 have increased after using of both venture and Cpap, increase in both PCO2 and HCO3 levels after using Venturi mask, CPAP mask was superior to venturi mask in avoiding the need of intubation, decreasing The ICU stay median length and also median length of hospitalization, all were lower in CPAP group than venture group. Also the mortality rate was lower in CPAP group than the venturi group. Conclusion: Using CPAP mask in severe AHRF following open heart surgery can avoid intubation, decreases the levels of tachypnea and arterial hypoxemia, decreases ICU stay, the length of hospitalization and also decreases the mortality rate compared with patients receiving high-concentration oxygen therapy with venture mask.

Increasing global water shortages are accelerating the pace of membrane manufacturing,which generates many environmentally harmful *** challenges need a radical rethink of developing innovative membranes that can address freshwater production without generating environmentally harmful *** work utilized the synthesized ultra-long hydroxyapatite(UHA)by the solvothermal method using the green solvent oleic acid in preparing UHA-based forward osmosis *** membranes were developed using different loading ratios of graphene oxide(GO)by vacuum-assisted filtration *** prepared GO/UHA membranes were identified using X-ray diffraction,scanning electron microscope,Fourier-transform infrared spectroscopy,and X-ray photoelectron *** contact angle and pore size distribution were determined for the obtained GO/UHA *** obtained hierarchical porous structure in the prepared membranes with interconnected channels results in a stable water flux with reverse salt *** best water flux rate of 42±2 L·m^(–2)·h^(–1)was achieved using the 50 mg GO/UHA membrane,which is 3.3 times higher than the pristine membrane,and a reverse salt flux of 67 g·m^(-2)·h^(–1).The obtained results showed a promising capability of a new generation of sustainable inorganic-based membranes that can be utilized in freshwater generation by energy-efficient techniques such as forward osmosis.

Objective:To evaluate the combination therapy of pyronaridine tetraphosphate and diminazene aceturate against Babesia in vitro and in ***:Bioinformatic analysis was performed using atom pair *** in vitro combination test was performed against Babesia bovis and Theileria ***,the in vivo chemotherapeutic efficacy of pyronaridine tetraphosphate in combination with diminazene aceturate was investigated against the growth of Babesia microti in mice using a fluorescence inhibitory ***:Pyronaridine tetraphosphate and diminazene aceturate exhibited nearly similar molecular *** in vitro combination of pyronaridine tetraphosphate and diminazene aceturate was synergistic on Babesia bovis and additive on Theileria *** addition,5 mg/kg pyronaridine tetraphosphate combined with 10 mg/kg diminazene aceturate inhibited Babesia microti growth significantly compared with those observed after treatment with 25 mg/kg diminazene aceturate alone from day 6 post treatment to day 12 post *** combination therapy also normalized the hematological parameters of infected ***:An oral dose of pyronaridine tetraphosphate combined with a subcutaneous dose of diminazene aceturate inhibits Babesia in vitro and in mice,suggesting it might be a new paradigm for the treatment of babesiosis.

Herein,iron oxide/hydroxides deposits(gossans)were utilized,for the first time,in the fabrication of magnetite nanoparticles(MNPs)to load modified coal(MC).The as-synthesized MNPs@MC composite was characterized via different techniques and utilized for the Cr(Ⅵ)*** studies supported by theoretical treatment were applied to offer a new overview of the Cr(Ⅵ)adsorption geometry and mechanism at 25-45℃.Experimental results suggested that the Cr(Ⅵ)uptake was mainly governed by adsorption-reduction coupled *** Langmuir model fitted well the Cr(Ⅵ)adsorption data with maximum adsorption capacities extended from 115.24 to 129.63 mg·g^(-1).Theoretical calculations indicated that Cr(Ⅵ)ions were adsorbed on the MNPs@MC following the theory of the advanced monolayer statistical *** number of ions removed per site ranged from 1.88 to1.23 suggesting the involvement of vertical geometry and multi-ionic mechanism at all *** increment of the active sites density and the adsorption capacity at saturation with improving temperature reflected an endothermic ***,the Cr(Ⅵ)adsorption was controlled by physical forces as the adsorption energies were less than 40 kJ·mol^(-1).The calculated free enthalpy,*** internal energy explained the spontaneous nature and the viability of Cr(Ⅵ)adsorption on the MNPs@MC *** results offer a new approach in utilizing the iron-rich deposits as gossans in the preparation of magnetic and low-cost adsorbents for wastewater remediation.

Comprehensive evaluation and sensitivity analysis of regeneration energy for acid gas removal plant using single and activated-methyl diethanolamine solvents

The absorption of acid gas using reactive amines is among the most widely used types of capturing ***,the absorption process requires intensive energy expenditure majorly in the solvent regeneration *** study simultaneously evaluated the regeneration energy of MDEA and PZ/MDEA solvents in terms of heat of absorption,sensible heat,and vaporization *** Hysys version 8.8 simulation tool is applied to model the full acid gas removal plant for the chemical absorption *** new energy balance technique presents around the absorption and desorption columns to bring a new perspective of energy distribution in the capturing of acid gas *** analysis of regeneration energy and its three contributors is performed at several operation parameters such as absorber and stripper pressures,lean amine circulation rate,solvent concentration,reflux ratio,and CO2 and H2 S *** results show that the heat of absorption of PZ/MDEA system is higher than that for MDEA system for the same operating *** sensible heat is the main contributor in the required regeneration energy of MDEA solvent *** simulation results have been validated against data taken from real plant and *** product specifications of our simulation corroborate with real plant data in an excellent approach;additionally,the profile temperature of the absorber and the stripper columns are in good agreement with *** overall results highlight the direction of the effects of each parameter on the heat of absorption,sensible heat,and vaporization heat.

A Comparative Study of Intrathecal Injection of Bupivacaine Alone or with Fentanyl, Clonidine, and Neostigmine in Lower abdominal Surgeries

Background: Anesthesiologists are responsible for the development of pain services in the current era. Hence ideal adjuvants that can be used with bupivacaine for stable intraoperative conditions and prolonging the postoperative analgesia with fewer side effects are being investigated. Opioids, despite useful as adjuvants, are associated with undesirable side effects. Aim of the work: The study was done to compare analgesic efficacy and hemodynamic of intrathecal injection of bupivacaine alone or with fentanyl, clonidine, and neostigmine in lower abdominal surgeries, over the first 24 postoperative hours, in a randomized, double-blind, and clinical trial. Methods: 100 Patients were randomized into four equal groups, 25 patients in each group;Group B patients received 2.5 ml of 0.5% hyperbaric bupivacaine and 0.5 ml of normal saline. Group BF patients received 2.5 ml of 0.5% hyperbaric bupivacaine with (25 mics) of fentanyl. Group BC patients received 2.5 ml of 0.5% hyperbaric bupivacaine with 0.5 ml (75 mics) of clonidine. Group BN patients received 2.5 ml of 0.5% hyperbaric bupivacaine with 0.1 ml of neostigmine (50 mics) and 0.4 ml of normal saline. Intrathecal anesthesia was done with a recording of parameters intraoperative and the post-operative period. Each patient was assessed for hemodynamic parameters and effective analgesia in operation, and presence of complications (nausea, vomiting, sedation and pruritus) visual analogue pain score (VAS) postoperatively by a blinded investigator in the post-anesthesia care unit (PACU) and at 1, 2, 3, 4, 8 12, 18 and 24 h postoperatively. Results: The postoperative analgesia is more effective with group BC (the gold standard) than group B, group BF, and group BN. As regard complications during the study in all groups, complications as nausea, and vomiting were mainly with group BN;hypotension was primarily in group BC. Response of seed yield and fatty acid compositions for some sunflower genotypes to plant spacing and nitrogen fertilization

A field experiment was conducted at the experiment Farm of Kafr-El-Hamam Research Station,Zagazig,Sharkia Governorate,Agricultural Research Center,Egypt during the two successive summer seasons of 2015 and 2016 to achieve the highest yield and good oil quality of three tested sunflower *** both seasons,the experiment was conducted using the split split plot design in randomized complete block design with three replicates arrangement keeping plant spaces(15,20 and 25 cm apart between hills)in main plots,nitrogen fertilization levels(15,30 and 45 N fad.^-1)in sub plots and sunflower genotypes(Giza 102,Sakha 53 and promising line of L120)in sub sub *** and quality traits were significantly influenced by plant spaces,nitrogen fertilization levels and genotypes as well as interactions in both seasons and their combined *** wider plant spacing of 25 cm seems to be a good compromise between the highest seed yield fad.^-1 and good acid composition of *** increasing of nitrogen fertilization level had a positive reflected on yield and desirable acid composition of *** 53 was ranked in the first order in stem diameter,head diameter,100-seed weight,seed weight plant^-1,flowered late and hence seed yield fad.^-1 as well as seed oil content whereas,Giza 102 characterized with its contained the highest proportion of oleic and linoleic unsaturated fatty *** highest values of head diameter,100-seed weight,seed weight plant^-1 and hence seed yield fad.^-1 as well as the highest proportion of oleic and linoleic unsaturated fatty acids composition were obtained by grown sunflower *** 53 at wider spacing of 25 cm with application of nitrogen fertilization levels of 45 N fad.^-*** and path analyses revealed that 100-seed weight and head diameter had the highest direct and indirect influence on seed weight plant1,at the same time also,oleic acid content and linoleic acid content had the highest direct and indirect effect inf

A large subset of corneal pathologies involves the formation of new vessels(neovascularization), leading to compromised visual acuity. This article aims to review the clinical causes and presentations of corneal neovascularization(CNV) by examining the mechanisms behind common CNV-related corneal pathologies, with a particular focus on herpes simplex stromal keratitis,contact lenses-induced keratitis and CNV secondary to keratoplasty. Moreover, we reviewed CNV in the context of different types of corneal transplantation and keratoprosthesis, and summarized the most relevant treatment available so far.

Efficacy of octreotide in the prevention of complications after pancreaticoduodenectomy in patients with soft pancreas and non-dilated pancreatic duct: A prospective randomized trial

Background: The efficacy of octreotide to prevent postoperative pancreatic fistula(POPF) of pancreaticoduodenectomy(PD) is still controversial. This study aimed to evaluate the effect of postoperative use of octreotide on the outcomes after ***: This is a prospective randomized controlled trial for postoperative use of octreotide in patients undergoing PD. Patients with soft pancreas and pancreatic duct ality(P = 0.727).Conclusions: Octreotide did not reduce postoperative morbidities, reoperation and mortality rate. Also, it did not affect the incidence of POPF and its clinically relevant variants.
