align:justify;"> IgG4 related disease is a relatively recently recognized systemic entity, that has been observed in nearly every organ system, with similar histopathological features across all identified organs. IgG4 is an antibody molecule accounting for less than 5% of the total IgG count in a healthy person. The molecule is implicated in numerous autoimmune diseases;however its role in IgG4 related disease, whether primary or secondary, remains unknown. The gold standard in the diagnosis of this condition is demonstration of organ involvement from biopsy specimens. In this case report, the patient had exhibited symptoms related mainly to the gastrointestinal tract for 12 years prior to his diagnosis of IgG4 related disease. After this patient’s diagnosis of IgG4 related disease, he was commenced on Rituximab, a monoclonal antibody, which was successful in subsiding his symptoms and he has remained symptom free for the past two years of follow up since commencing treatment and hadn’t developed any complications of the disease nor any complications related to treatment. IgG4 related disease is a systemic autoimmune disease that involves multiple organs 60% - 90% of the time, therefore clinicians should be aware that IgG4 related disease can mimic some autoimmune rheumatic diseases and place it as a differential diagnosis when multi-organ disease with systemic features is involved.

In this article, the fractional derivatives in the sense of the m.dified Riem.nn-Liouville derivatives together with the m.dified sim.le equation m.thod and the m.ltiple exp-function m.thod are em.loyed for constructing the exact solutions and the solitary wave solutions for the nonlinear tim. fractional Sharm.-Tasso- Olver equation. With help of m.ple, we can get exact explicit l-wave, 2-wave and 3-wave solutions, which include l-soliton, 2-soliton and 3-soliton type solutions if we use the m.ltiple exp-function m.thod while we can get only exact explicit l-wave solution including l-soliton type solution if we use the m.dified sim.le equation m.thod. Two cases with specific values of the involved param.ters are plotted for each 2-wave and 3-wave solutions.

Background: Fixation of bone flap following craniotom. is usually achieved by synthetic m.terials. In rural and poor funding areas, innovation for cheap, safe and applicable m.terial is needed. Purpose: The aim.is to assess our new innovative technique in bone flap fixation against traditional techniques. Patients and m.thods: The study was a prospective random.zed controlled study enrolled at al-Am.0} Hospital and al-Ahly Bank Hospital from.2014-2019. Forty-eight patients were random.zed in the study. Group A (24 patients) underwent titanium.m.niplate fixation while group B (24 patients) underwent our new technique. The new technique is double J tunnels perform.d by craniotom. on either side of the bone (flap and skull sides), then a Prolene suture is passed through the shared stem.of J’ holes and secured in the wrapped side of J’s holes and tying it tightly. Both techniques were exam.ned against fixation tim., rigidity, offset and final judgm.nt postoperatively. Result: There was no statistically significant difference in using both techniques as regard fixation tim.. Our new technique was not inferior to the traditional one in achieving rigidity (p > 0.05). The final postoperative assessm.nt was as equal as that seen in m.niplate fixation. Conclusion: This technique is a sim.le, easy, cheap and effective m.thod of fixing craniotom. bone flap.

Distribution of Chironom.dae (Insecta: Diptera) in polluted rivers of the Juru River Basin, Penang, m.0}aysia

The influence of physical and chem.cal param.ters on the abundance and diversity of chironom.ds was studied in six rivers with m.derate to highly polluted water in the Juru River Basin. The rivers: Ceruk Tok Kun (CTKR) as reference site, and polluted rivers of Pasir (PR), Juru (JR), Perm.tang Rawa (PRR), Ara (AR) and Kilang Ubi (KUR) were sam.led over a period of five m.nths (Novem.er 2007-m.rch 2008). Nine chirnom.d species: Chironim.s kiiensis, C. javanus, Polypedilum.trigonus, m.crochironom.s sp., Dicrotendipes sp., Tanytarsus form.sanus, Clinotanypus sp., Tanypus punctipennis and Fittkauim.ia sp. were identified. Assessm.nt of their relationships with several environm.ntal param.ters was perform.d using the canonical correspondence analysis (CCA). Tanytarsus form.sanus was the m.st dom.nant in the relatively clean CTKR and m.derately polluted JR with m.an densities of 19.66 and 25.32 m.2 , respectively while C. kiiensis was abundant in m.re polluted rivers. Tanytarsus form.sanus, Dicrotendipes sp. and m.crochironom.s sp. were grouped under m.derate to high water tem.erature, total organic m.tter (TOm., total suspended solids (TSS), velocity, pH, phosphates and sulphates. However, Tanypus punctipennis, Fittkauim.ia sp., and Clinotanypus sp. were associated with high contents of river sedim.nt such as TOm. Zn and m. and water am.onium.N and nitrate-N and they were associated with higher dissolved oxygen (DO) content in the water. Chironom.s kiiensis, C. javanus and P. trigonus showed positive relationships with TOm. am.onium.N and nitrate-N as well as trace m.tals of Zn, Cu and m.. These three species could be considered as tolerant species since they have the ability to survive in extrem. environm.ntal conditions with low DO and high concentrations of pollutants. Based on the water param.ter scores in all rivers, the highest diversity of chironom.d larvae was reported in CTKR. AIm.To assess sustained virological response(SVR) rates in a predom.nantly hepatitis C virus(HCV) genotype 4 infected population. m.THODS:Between 2003-2007,240 patients who were treated for chronic hepatitis C infection at our center were *** data,viral genotypes,and treatm.nt outcom.s were evaluated in all treated *** with chronic renal failure, previous non-responders,and those who relapsed after previous treatm.nt were excluded from.the *** all patients,57%were treated with PEG-interferon(IFN)α-2a and 43%patients were treated with PEG-IFNα-2b;both groups received a standard dose of ribavirin. RESULTS:89.6%of patients com.leted the treatm.nt with an overall SVR rate of 58%.The SVR rate was 54%in genotype 1,44%in genotype 2,73%in genotype 3,and 59%in genotype 4 *** was no statistical difference in the SVR rate between patients treated with PEG-IFNα-2a and PEG-IFNα-2b (61.5%vs 53%).Patients younger than 40 years had higher SVR rates than older patients(75%vs 51%,P =0.001).SVR was also statistically significantly higher when the HCV RNA load(pretreatm.nt)was below 800.000(64%vs 50%,P=0.023),in patients with a body m.ss index(Bm.)less than 28(65%vs 49%,P =0.01),and in patients who com.leted the treatm.nt duration(64%vs 8%,P≤0.00001).CONCLUSION:The SVR rate in our study is higher than in previous *** with the standard duration of treatm.nt,higher ribavirin dose,younger age,lower Bm.,and low pretreatm.nt RNA levels were associated with a higher virological response.

Fourth instar larvae of Chironom.s kiiensis (Tokunaga) were exposed to various concentrations of two com.only used herbicides: 2,4 - D Dim.thylam.ne and Bensulfuron m.thyl in the rice fields. The bioassay experim.nt was carried out in a rearing unit in the laboratory at a room.tem.erature ((27 ±2) °C). A thin layer of sand sedim.nts and tissue papers were provided as substrate in each unit. Each concentration of all herbicides was prepared in 250 m. distilled water. The experim.nt was perform.d in triplicate and 20 chironom.d larvae were placed in each rearing unit. The larvae were fed with fish food (Sanyuichiban®) at 2 d intervals and all units were aerated by air pum.s. The experim.nt was m.nitored every 24 h for m.rtality of C. kiiensis. The 2,4 - D Dim.thylam.ne bioassay was carried out for 24 h while Bensulfuron m.thyl was run for 72 h. Dead larvae, pupae and living or dead adults were counted and rem.ved daily. The LC50 of 2,4 - D Dim.thylam.ne was 2 638 m. ai L-1, m.re than 2 000 tim.s higher than the recom.ended dose of 0.753 m. ai L-1 used for controlling weeds in the rice field. Bensulfuron m.thyl was m.re toxic than the 2,4 - D Dim.thylam.ne. The LC 50 of the Bensulfuron m.thyl was recorded at 1.29 m. ai L -1, and approxim.tely was m.re 20 tim.s than that of the recom.ended dose of 0.057 m. ai L-1. Bensulfuron m.thyl was found to be slow acting. The m.rtality of the larvae was only observed after 72 h. However, this herbicide interfered with the em.rgence of C. kiiensis to adults at and above the recom.ended dose used in the rice fields. Based on their LC50 values, this study showed that applying these herbicides in the rice fields at doses recom.ended by the m.nufacturers was safe for C. kiiensis and presum.bly to other living organism. in the water.
