Rare presentation of post-transplant lymphoproliferative disorder isolated to gastroesophageal junction

Post transplant lymphoproliferative disorder (PTLD) represents a life threatening disorder occurring after transplantation, ranging from a polyclonal mononucleo- sis like illness to a monomorphic high grade neoplasm with cytologic and histopathologic evidence indica- tive of transformation to lymphoma. PTLD of diffuse large B-cell lymphoma (DLBCL) subtype, isolated to the esophagus is a rare diagnosis. We describe the first case of an immunocompromised adult patient diagnosed with DLBCL-PTLD limited to his esophagus without an associated mass or locoregional lymphade-nopathy on imaging since the institution of the revised Cheson criteria, which includes positron emission tomography-computed tomography as the standard staging modality. Even more unique to our case was the suggestion of underlying cytomegalovirus (CmV) gastritis leading to a hypothesis about a less well understood relationship between CmV and Epstein Barr virus (EBV). In the post transplant setting, im- munocompromised state, or EBV positive state, upper gastrointestinal symptoms should prompt investigation with an upper endoscopy (EGD). Additionally, specific to our case, the fact that the patients' presentation was suspicious for CmV gastritis raises the possibility that the CmV infection predated his PTLD increasing his risk of acquiring PTLD. This reemphasizes the im- portance and diagnostic utility of early screening with EGD in patients after transplantation.

The dietary phytochemicals curcumin(CUR) and sulforaphane(SFN) have shown remarkable cancer chemopreventive effects in many model systems. This study was designed to investigate the induction of Nrf2-mediated antioxidant enzymes by combining doses of CUR and SFN and the effect of their combination on the Nrf2-ARE(antioxidant response element) response in Hep G2-C8 cells. We hypothesized that the combination of the polyphenol CUR and the isothiocyanate SFN could enhance the induction of AREs and Nrf2-target enzymes. Hep G2-C8 cells were treated with a combination of low doses of CUR, SFN or both. The induction of Nrf2-mediated antioxidant and phase II detoxifying enzymes–heme oxygenase-1(HO-1) and UDP-glucuronosyltransferase-1A(UGT1A)–was measured by real-time RT-PCR and western blotting. ARE-luciferase activity was also quantified. Low doses of CUR(10 μm) and SFN(12.5 μm) significantly induced the expression of HO-1 and UGT1A1 proteins. Through the use of chemical inhibitors of mR NA and protein synthesis, the combination of CUR and SFN was shown to affect the transcriptional regulation of both HO-1 and UGT1A1. Additionally, the combination of CUR and SFN synergistically induced the expression of Nrf2- and ARE-luciferase activity in Hep G2-C8 cells. Thus, CUR and SFN at low concentrations augment therapeutic effects in Hep G2-C8 cells. The enhanced ARE-luciferase activity of combined CUR and SFN treatment could partly explain the significant induction of the Nrf2-target enzymes HO-1 and UGT1A1. Taken together, our results suggest that combining low doses of CUR and SNF could be a promising strategy for cancer chemoprevention in humans.

Ensemble Based Learning with Accurate motion Contrast Detection

Recent developments in computer vision applications have enabled detection of signiﬁcant visual objects in video *** quoted in literature have detected objects from video streams using Spatiotemporal Particle Swarm Optimization(SPSOm)and Incremental Deep Convolution Neural Networks(IDCNN)for detecting multiple ***,the study considered opticalﬂows resulting in assessing motion *** methods have issue with accuracy and error rates in motion contrast ***,the overall object detection performance is reduced signiﬁ***,consideration of object motions in videos efﬁciently is a critical issue to be *** overcome the above mentioned problems,this research work proposes a method involving ensemble approaches to and detect objects efﬁciently from video *** work uses a system modeled on swarm optimization and ensemble learning called Spatiotemporal Glowworm Swarm Optimization model(SGSOm)for detecting multiple signiﬁcant objects.A steady quality in motion contrasts is maintained in this work by using Chebyshev distance *** proposed system achieves global optimization in its multiple object detection by exploiting spatial/temporal cues and local *** experimental results show that the proposed system scores 4.8%in mean Absolute Error(mAE)while achieving 86%in accuracy,81.5%in precision,85%in recall and 81.6%in F-measure and thus proving its utility in detecting multiple objects.

Improved Network Validity Using Various Soft Computing Techniques

An Efficient Allocation for Lung Transplantation Using Ant Colony Optimization

A relationship between lung transplant success and many features of recipients’/donors has long been ***,modeling a robust model of a potential impact on organ transplant success has proved *** this study,a hybrid feature selection model was developed based on ant colony opti-mization(ACO)and k-nearest neighbor(kNN)classiﬁer to investigate the rela-tionship between the most deﬁning features of recipients/donors and lung transplant success using data from the United Network of Organ Sharing(UNOS).The proposed ACO-kNN approach explores the features space to identify the representative attributes and classify patients’functional status(i.e.,quality of life)after lung *** efﬁcacy of the proposed model was veriﬁed using 3,684 records and 118 input features from the *** developed approach examined the reliability and validity of the lung allocation *** results are promising regarding accuracy prediction to be 91.3%and low computational time,along with better decision capabilities,emphasizing the potential for automatic classiﬁcation of the lung and other organs allocation *** addition,the proposed model recommends a new perspective on how medical experts and clinicians respond to uncertain and challenging lung alloca-tion *** such ACO-kNN model,a medical professional can sum-marize information through the proposed method and make decisions for the upcoming transplants to allocate the donor organ.

Automated Teller machine Authentication Using Biometric

This paper presents a novel method of a secured card-less AutomatedTeller machine (ATm) authentication based on the three bio-metrics measures. Itwould help in the identification and authorization of individuals and would provide robust security enhancement. moreover, it would assist in providing identi-fication in ways that cannot be impersonated. To the best of our knowledge, thismethod of Biometric_ fusion way is the first ATm security algorithm that utilizesa fusion of three biometric features of an individual such as Fingerprint, Face, andRetina simultaneously for recognition and authentication. These biometric imageshave been collected as input data for each module in this system, like a fingerprint, a face, and a retina module. A database is created by converting theseimages to YIQ color space, which is helpful in normalizing the brightness levelsof the image hence mainly (Y component’s) luminance. Then, it attempt toenhance Cellular Automata Segmentation has been carried out to segment the particular regions of interest from these database images. After obtaining segmentation results, the featured extraction method is carried out from these criticalsegments of biometric photos. The Enhanced Discrete Wavelet Transform technique (DWT mexican Hat Wavelet) was used to extract the features. Fusion ofextracted features of all three biometrics features have been used to bring in themultimodal classification approach to get fusion vectors. Once fusion vectorsware formulated, the feature level fusion technique is incorporated based on theextracted feature vectors. These features have been applied to the machine learning algorithm to identify and authorization of multimodal biometrics for ATmsecurity. In the proposed approach, we attempt at useing an enhanced Deep Convolutional Neural Network (DCNN). A hybrid optimization algorithm has beenselected based on the effectiveness of the features.
