BACKGROUND The results obtained to date concerning food groups,diet quality and colorectal cancer(CRC)risk vary according to criteria used and the study *** To study the relationships between food groups,diet quality and CRC risk,in an adult population of the Basque Country(North of Spain).METHODS This observational study included 308 patients diagnosed with CRC and 308 ageand sex-matched subjects as *** recruitment,dietary,anthropometric,lifestyle,socioeconomic,demographic and health status information was *** to the dietary recommendations was evaluated utilizing the Healthy Eating Index for the Spanish Diet and the *** logistic regressions were used to evaluate the associations of food group intakes,diet quality scores,categorized in tertiles,with CRC *** The adjusted models for potential confounding factors showed a direct association between milk and dairy products consumption,in particular high-fat cheeses[odds ratio(OR)third tertile vs first tertile=1.87,95%confidence intervals(CI):1.11-3.16],and CRC *** the consumption of fiber-containing foods,especially whole grains(OR third tertile vs first tertile=0.62,95%CI:0.39-0.98),and fatty fish(OR third tertile vs first tertile=0.53,95%CI:0.27-0.99)was associated with a lower risk for ***,higher md.adherence was associated with a reduced CRC risk in adjusted models(OR third tertile vs first tertile=0.40,95%CI:0.20-0.80).CONCLUSION Direct associations were found for high-fat cheese,whereas an inverse relation was reported for fiber-containing foods and fatty fish,as well as adherence to a Mediterranean dietary pattern.

TiNiHf/SiO_(2)/Si shape memory film composites for bidirectional micro actuation

The martensitic phase transformation in Ti_(40.4)Ni_(48)Hf_(11.6) shape memory alloys is leveraged for bi-directional actuation with TiNiHf/SiO_(2)/Si *** shape memory properties of magnetron sputtered Ti_(40.4)Ni_(48)Hf_(11.6) films annealed at 635℃-5 min are influenced by film thickness and the underlying *** TiNiHf film thick-ness from 21μm to 110 nm results in the reduction of all characteristic transformation temperatures until a critical thickness is ***,Ti_(40.4)Ni_(48)Hf_(11.6) thin films as low as 220 nm show transfor-mations above room temperature when deposited on SiO_(2) buffer layer,which is of great interest in *** comparison,220 nm films on Si substrates are austenitic at room temperature,and thus not suitable for *** fatigue tests on TiNiHf/SiO_(2)/Si bimorphs demonstrate better functional fatigue characteristics than freestanding films,with an average reduction of 15℃ after 125 cycles,with tempera-ture stabilization *** bi-directional actuation results are promising in the development of bistable actuators within a PMMA/TiNiHf/Si trimorph composite,whereby the additional PMMA layer undergoes a glass transition at 105℃.With the aid of constitutive modeling,a route is elaborated on how bistable actuation can be achieved at micro-to nanoscales by showing favorable thickness combinations of PMMA/TiNiHf/Si composite.

Multimodal Pain Management in the Perioperative Setting

In the context of the current U.S. opioid crisis, there is renewed interest and heightened relevance in the practice of multimodal analgesia for postoperative surgical patients. The goal of this review article is to present an accounting of the available opioid and non-opioid pharmacologic agents, regional analgesia techniques, and complementary modalities that may be considered for optimal pain relief in the perioperative setting. In adhering to the practice of multimodal pain therapy, clinicians improve the quality of pain control and reduce reliance on opioid use. There is strong evidence to support the benefits of multimodal analgesia.

背景:精神分裂症患者的代谢综合征是一个重要的健康问题。瑞舒伐他汀对血脂异常的辅助性治疗的有效性和安全性存在争议。目的:评价瑞舒伐他汀对精神分裂症患者血脂异常的辅助性治疗的有效性和安全性。方法:我们从以下数据库中系统地检索了2017年9月28日以前相关的临床对照试验:Pub Med、Psyc INFO、Cochrane图书馆、中国知网、万方数据库、中国生物医学文献数据库。我们计算了标准平均差(Smd.、风险比(RR)及其95%的可信区间(CIs)。使用偏移评估工具中的Cochrane风险评估来评价所纳入研究的质量。并采用GRADE系统推荐的等级方法(推荐、评估、发展、评价的等级)作为参照标准。结果:确认和分析了4项比较瑞舒伐他汀组(n=138)和对照组(n=136)的研究(n=274)。瑞舒伐他汀辅助治疗显示对低密度脂蛋白胆固醇(LDL-C)[4项试验,n=272,Smd.-1.31(95%CI:-1.93,-0.70),I^2=81%]、总胆固醇(2项试验,n=164,Smd.-2.00(95%CI:-2.79,-1.21);I^2=76%)、和甘油三酯(2项试验,n=164,Smd.-1.05(95%CI:-1.38,-0.72);I^2=0%)的疗效比对照组更有效,但对高密度脂蛋白胆固醇(2项试验,n=164,Smd.0.14(95%CI:-0.16,0.45);I^2=0%)的疗效没有显著差异。去除一项没有随机试验的LDL-C研究之后,显著差异仍然存在[3项试验,n=172,Smd.-1.07(95%CI:-1.60,-0.53);I^2=63%]。体重(3项试验,n=208,Smd.-0.40(95%CI:-1.29,0.49);I^2=89%)、身体质量指数(2项试验,n=164,Smd.-0.34(95%CI:-1.23,0.56);I^2=87%)、腰围(3项试验,n=208,Smd.-0.43(95%CI:-1.31,0.46);I^2=89%)、空腹血糖(4项试验,n=272,Smd.-0.25(95%CI:-0.65,0.15);I^2=62%)方面组间没有显著的差异。两组之间的不良反应及停药率相近。根据GRADE分级方法,主要结果的证据水平低被评为"非常低"(35.3%)到"低"(64.7%)。其中,主要结果(LDL-C)被评为"非常低"。结论:对精神分裂症患者血脂异常的瑞舒伐他汀辅助性治疗的现有数据尚不足以对其有效性和安全性做出明确的解释。需进一步针对高质量的延长治疗时间的随机对照试验来验证该结论。

目的：观察青光眼手术患者虹膜切除标本的组织病理学特征。比较拉坦前列腺素治疗组与非拉坦前列腺素治疗组（对照组）组织学差异。方法：虹膜切除标本及患者病史资料呈送拉坦前列腺素病理学中心。标本由3位眼科病理学家以盲法分别观察。评估内容包括：癌变。癌前改变及其他病变：色素沉着水平差异，细胞构成差异。炎症，血管异常情况及虹膜色素上皮改变。结果：所收集标本中，449例曾接受拉坦前列腺素治疗，142例无拉坦前列腺素或其衍生物治疗史。所有标本均未发现癌或癌前病变。拉坦前列腺素治疗组，虹膜痣发生率显著高于对照组（P=0．001）。基质成纤维细胞和黑色素细胞数为对照组2倍（P〈0．001）。采用免疫组化染色技术分析2002年6月收集的各亚组标本，结果显示：拉坦前列腺素治疗组与非拉坦前列腺素治疗组黑色素细胞均数无显著差异（P=0．35）。结论：本研究结果支持已有观点，即：拉坦前列腺素诱发的眼部颜色的改变是由于虹膜基质黑色素细胞内黑色素数量增多而引起，虹膜痣数量增加可能是此效应的局部表现。
