Plastic surgeons accustomed to use autologous fat grafs for augmentation of soft tissues in the last two decades. The main drawback of fat grafting is unpredictable graft resorption. Several studies have searched for new ways of increasing the viability of the transplanted tissue to overcome this disadvantage. The most recent and promising approach is to mix the fat graft with Platelet-Rich Plasma (PRP) before transplantation. The purpose of this article is to present the initial experience using the autologous fat injection with platlet rich plasma as a method for fascial augmentation.

Introduction: Penile skin loss poses a particular challenge for reconstruction to the plastic surgeon. These defects, depending on their size, have been reconstructed using skin grafts or regional flap. Patients & methods: Ten patients with variable sized penile skin defects were included in this work. Scrotal fascio-myo-cutaneous (Dartos) flap was harvested and used for penile shaft resurfacing. Results: All harvested flaps were successful. None of them showed any ischemic manifestations. Discussion: Despite being a simple and robust flap, its use for resurfacing of moderate to extensive penile skin defects isn’t popular. We propose the use of Dartos flap as a good alternative for challenging moderate to extensive penile skin losses.

Low-Level Laser Therapy versus Four Layers Compression Technique in the Treatment of Venous Ulcers

Objective: The aim of the study is to evaluate the effect of low-level laser therapy in comparison to compression bandage therapy in the treatment of venous ulcers. Subjects & methods: This study was carried out in the period from march 2013 to march 2014. The study included 40 adult patients with a diagnosis of venous ulcer classified as C6 according to the clinical classification of CEAP classification. The patients were divided into 2 groups: the first one included 20 patients treated by low-level laser therapy at the Rheumatology and Rehabilitation department, and the second group included 20 patients in whom we use four layers compression bandaging at the vascular surgery department. Results: 32 ulcers were treated in group I and 35 ulcers were treated in group II. The patients of group I were 7 (35%) females and 13 (65%) males, their age range were (24 - 56) years. The patients of group II were 11 (55%) males and 9 (45%) females;their age ranges were (32 - 50) years. measurements of the ulcers size by (cm2) were taken at the beginning of therapy, at 1 month, at 2 months and at 3 months later on. The calculation of the area of the ulcer was done by using the graph papers to document the ulcer’s perpendicular linear dimensions. According to the size of the ulcer, some ulcers heal within 1 month (15.6%) ulcers in group I, and (28.5%) in group II. Some ulcers heal within 2 months (28%) in group I, and (37%) in group II. The remaining ulcers heal within 3 months or more which are (56%) ulcers in group I, and (34.2%) ulcers in group II. The recurrence rate of chronic venous leg ulcer in compression bandage technique used in group II was the least one as the total number of recurrent cases were 5 (25%), followed by laser therapy used in group I. Conclusion: There was no significant efficacy of the low-level laser therapy over the four layers compression technique in the management of chronic venous ulcers.

Laparoscopic Inguinal Hernia Repair with Closure of Hernial Defect and Central mesh Fixation Using Glubran 2

Background: Laparoscopic tension-free repair of inguinal hernia was presented in 1990s, promising less pain and short recovery period, but carrying the risk mesh bulging and migration. Objective: We have presented our technique in which central zone of mesh is fixed only after closure of hernial defect. Patients and methods: This study included 27 males patients (14 indirect inguinal hernias, 9 direct inguinal hernias, 4 both direct and indirect inguinal hernias on the same side). These cases are undergoing tension-free mesh repair after closure of hernial defect, and the mesh is fixed only at its central zone using Gulbran 2, between April 2011 and march 2013. The follow-up period ranged from 6 to 30 months. The intra and postoperative complications were recorded. Results: mean hospital stay was 1 day. The age of this group of patients ranged from 23 to 63 years (mean, 47 years). The operative time ranged from 30 to 100 minutes (mean, 45 minutes). The intraoperative complications were in form of mild bleeding in 7 patients (25.9%) during hernial sac dissection. Postoperative complications were mild inguinal pain in 4 patients (14.8%) for three weeks. mild hydrocele in 3 patients (11%) was recorded. No recurrence or bulging at hernia site was noticed during the period of follow-up. Conclusion: Laparoscopic inguinal hernia repair with central mesh fixation after closure of hernial defect is effective, easy and free of complications.

Background: mesh expansion and fixation at retro-rectus plane through multiples stabs produces good results. But these stabs cause cosmetic disorders for the patients and doctors. So, we find some modification to do this procedure without these stabbing wounds in midline hernial repair. Patients and methods: This technique was used to fix the mesh at retro-rectus plane in 50 patients suffering from midline hernias, from January 2008 through January 2010 at Zagazig university Hospital, Egypt. Laparotomy incision was done over the hernial sac or at old incision;the contents were then released and reduced into peritoneal cavity without much subcutaneous dissection. The suitable sheet of polypropylene mesh to cover the hernial defect and any weak area was prepared and fixed at retro-rectus plane percutaneously without stabbing wounds by using redirecting suture hook. The mean period of follow up was 26 months. Results: There was no recurrence during the period of follow up. Five patients developed subcutaneous bluish discoloration at the site of some stitches, which disappear within two weeks with conservative treatment. Conclusion: Percutaneous mesh expansion and fixation at retro-rectus plane by using redirecting suture hook procedure has good results in recurrence rate and cosmetic appearance.

A phenomenon known as trapped penis has numerous origins, including injudicious circumcision. materials and methods: Surgery for trapped penis with glanular amputation was performed for 5 children. The surgical policy in these cases included complete penile degloving, excising the scarring due to circumcission, fixing the penile skin at the penopubic and penoscrotal angles and reconstructing the penile skin with full thickness skin graft. Results: Improvement was noted in all cases. Conclusion: Surgery for hidden penis using full thickness skin graft achieves very satisfactory aesthetic and functional improvement.

Neoproterozoic island arc assemblage of the Arabian–Nubian Shield(ANS)in the Eastern Desert(ED)of Egypt comprises juvenile suites of metavolcanics(mV),large amounts of meta-sedimentary rocks(mS),and voluminous metagabbros-diorites(mGD)and syn-tectonic intrusions of older granitoids(OG).We report here the updates of these four rock units in terms of classification,distribution,chemical characteristics,geodynamic evolution,metamorphism,and *** addition,we discuss these integrated data to elucidate a reasonable and reliable model for crustal evolution in the *** main features of these rock units indicate their relation to each other and the geodynamic environment dominated by early immature oceanic island arcs to primitive continental *** information of the island arc metavolcanic and plutonic rocks(gabbros,diorites,tonalites,and granodiorites)furnish evidence of the genetic *** include proximity and a coeval nature in the field;all protolith magmas are subalkaline in nature following calc-alkaline series with minor tholeiitic affinities;common geochemical signature of the arc rocks and subduction-related magmatism;their similar enrichment in LREEs;and similar major element compositions with mafic melts derived from metasomatized mantle *** volcano-sedimentary and the OG rocks underwent multiphase deformation events whereas the mGD complexes deformed *** on the magmatic,sedimentological,and metamorphic evolutions constrained by geochronological data as well as the progressive evolutionary trend from extensional to compressional regimes,a possible gradual decrease in the subducted slab dip angle is the most infl uential in any geodynamic model for arc assemblage in the ED of Egypt.

Twenty-four rice genotypes were examined to assess genetic variability,heritability,and correlations for seven-grain quality traits,eight nutritional elements,and *** revealed signiﬁcant differences for the quality traits *** every trait under study,the phenotypic coefﬁcient of variation was higher than the correspon-dence genotypic coefﬁcient of *** in a broad sense varied from 29.75%for grain length to 98.31%for the elongation *** percentage recovery had a highly signiﬁcant positive correlation with milling and head rice ***,milling percentage had a highly positive correlation with head rice *** amylose percentage,all the genotypes belonged to low amylose except the Hassawi-1 variety,which had intermediate amylose *** nutrition contents of magnesium(mg),sodium(Na),potas-sium(K),calcium(Ca),copper(Cu),manganese(mn),zinc(Zn),iron(Fe),or protein percentage gave different variations for 24 rice genotypes under all the nutritional *** the 24 genotypes,ten rice genotypes–HighNutrient-1,HighNutrient-2,HighNutrient-9,HighNutrient-8,HighNutrient-3,Hassawi-2,HighNutrient-7,HighNutrient-6,Hassawi-1,and HighNutrient-4–had the highest heist value for all nutritional and protein con-tents,and could be used as a donor to improving new *** was a positive and signiﬁcant correlation between magnesium mg,K,Zn and ***,K had a positive correlation with zinc Zn,Fe,and protein *** analysis was divided into two groups:theﬁrst group included all genotypes rich in nutri-ents,while the remaining genotypes with low nutritional content were included in the second group.

Air pollution from shipping is becoming a critical issue,particularly in dense hub port *** proposed solution to minimize ship-based emissions at ports is the implementation of an Onshore Power Supply(OPS)*** allows ships to shut off their auxiliary engines and instead connect to the port *** there have been numerous studies conducted on ports in Europe and the United States,little research has been done on Egyptian ***,this paper aims to investigate the feasibility of implementing OPS at Port Said West Port in Egypt,aligning with Egypt Vision 2030’s goals for addressing climate *** research primarily focuses on analyzing data collected from calling ships to generate socio-economic and cost-effectiveness analyses of *** further enhance the environmental benefits of OPS,the paper proposes the use of solar energy as the OPS electricity *** findings of the study revealed that by relying on the national grid,emissions can be reduced by 28%.moreover,it is predicted that this reduction could reach 100%if electricity generation is solely based on solar ***,the economic analysis demonstrates promising profitability,with a payback period of approximately two years.
