Patterns of brain injuries associated with maxillofacial fractures and its fate in emergency egyptian polytrauma patients

Purpose： Maxillofacial injuries are commonly encountered in the practice of emergency medicine. More than 50% of patients with these injuries have multisystem trauma that requires coordinated manage- ment between emergency physicians and surgical specialists in oral and maxillofacial surgery, otolar- yngology, plastic surgery, ophthalmology, and trauma surgery. The aim of this study is to identify the patterns of brain injuries associated with maxillofacial trauma and its outcome. Methods： This descriptive study （cross-sectional） was carried out among 90 polytrauma patients with maxillofacial fractures attending the emergency Department at Suez Canal university Hospital and fulfilling our inclusion and exclusion criteria. Results： This study demonstrates the relation between type of maxillofacial fracture and type of trau- matic brain injuries in which the majority of patients with epidural hemorrhage presented with mid face fractures （60%）, while the minority of them presented with upper and lower face fractures （20% for each of them）. The majority of patients with subdural hemorrhage were associated with mid face fractures （75%）, the majority of patients with brain contusions associated with mid face fractures （75%）, and all of the patients presented by pneumocephalus were associated with mid face fractures （100%）. Conclusion： The results of this study confirm the value of quick diagnosis and early intervention, which is fundamental to prevent morbidity as well as mortality especially with regards to prevention of traumatic brain injury as even a short duration of hvpoxia and edema will lead to significant permanent neurological deficits.

Despite the extensive studies conducted on the effectiveness of microwave treatment as a novel rock preconditioning method,there is yet to find reliable data on the rock failure mechanisms due to microwave *** addition,there is no significant discussion on the energy efficiency of the method as one of the important factors among the mining and geotechnical engineers in the *** study presents a novel experimental method to evaluate two main rock failure mechanisms due to microwave treatment without applying any mechanical forces,*** and concentrated *** result shows that the existence of a small and concentrated fraction of a strong microwave absorbing mineral will change the failure mechanism from the distributed heating to the concentrated heating,which can increase the weakening over microwave efficiency(WOMe)by more than 10 *** observation is further investigated using the developed coupled numerical *** is shown that at the same input energy,the existence of microwave absorbing minerals can cause major heat concentration inside the rock and increase the maximum temperature by up to three times.

Percutaneous Mesh expansion and Fixation at the Retro-Rectus Plane without Stabs by Using Redirecting Suture Hook in Midline Hernias Repair

Background: Mesh expansion and fixation at retro-rectus plane through multiples stabs produces good results. But these stabs cause cosmetic disorders for the patients and doctors. So, we find some modification to do this procedure without these stabbing wounds in midline hernial repair. Patients and methods: This technique was used to fix the mesh at retro-rectus plane in 50 patients suffering from midline hernias, from January 2008 through January 2010 at Zagazig university Hospital, egypt. Laparotomy incision was done over the hernial sac or at old incision;the contents were then released and reduced into peritoneal cavity without much subcutaneous dissection. The suitable sheet of polypropylene mesh to cover the hernial defect and any weak area was prepared and fixed at retro-rectus plane percutaneously without stabbing wounds by using redirecting suture hook. The mean period of follow up was 26 months. Results: There was no recurrence during the period of follow up. Five patients developed subcutaneous bluish discoloration at the site of some stitches, which disappear within two weeks with conservative treatment. Conclusion: Percutaneous mesh expansion and fixation at retro-rectus plane by using redirecting suture hook procedure has good results in recurrence rate and cosmetic appearance.

4-Aminobutyrate accumulates in plants under abiotic ***,targeted quantitative profiling of metabolites and transcripts was conducted to monitor glutamate-and polyamine-derived 4-aminobutyrate production and its subsequent catabolism to succinate or 4-hydroxybutyrate in apple(Malus x domestica Borkh.)fruit stored at 0°C with 2.5 kPa O2 and 0.03 or 5 kPa CO2 for 16 ***-temperature-induced protein hydrolysis appeared to be responsible for the enhanced availability of amino acids during early storage,and the resulting higher glutamate level stimulated 4-aminobutyrate levels more than *** CO2 increased the levels of polyamines,as well as succinate and 4-hydroxybutyrate,during early storage,and 4-aminobutyrate and 4-hydroxybutyrate over the longer *** of all of the genes likely involved in 4-aminobutyrate metabolism from glutamate/polyamines to succinate/4-hydroxybutyrate was induced in a co-ordinated ***2-regulated expression of apple gLUTAMATe DeCARBOXYLASe 2,AMINe OXIDASe 1,ALDeHYDe DeHYDROgeNASe 10A8 and POLYAMINe OXIDASe 2 was evident with longer term *** suggested that respiratory activities were restricted by the elevated CO2/O2 environment,and that decreasing NAD+availability and increasing NADPH and NADPH/NADP+,respectively,played key roles in the regulation of succinate and 4-hydroxybutyate ***,these findings suggest that both transcriptional and biochemical mechanisms are associated with 4-aminobutyrate and 4-hydroxybutyrate metabolism in apple fruit stored under multiple abiotic stresses.

Laparoscopic Inguinal Hernia Repair with Closure of Hernial Defect and Central Mesh Fixation Using glubran 2

Background: Laparoscopic tension-free repair of inguinal hernia was presented in 1990s, promising less pain and short recovery period, but carrying the risk mesh bulging and migration. Objective: We have presented our technique in which central zone of mesh is fixed only after closure of hernial defect. Patients and Methods: This study included 27 males patients (14 indirect inguinal hernias, 9 direct inguinal hernias, 4 both direct and indirect inguinal hernias on the same side). These cases are undergoing tension-free mesh repair after closure of hernial defect, and the mesh is fixed only at its central zone using gulbran 2, between April 2011 and March 2013. The follow-up period ranged from 6 to 30 months. The intra and postoperative complications were recorded. Results: Mean hospital stay was 1 day. The age of this group of patients ranged from 23 to 63 years (mean, 47 years). The operative time ranged from 30 to 100 minutes (mean, 45 minutes). The intraoperative complications were in form of mild bleeding in 7 patients (25.9%) during hernial sac dissection. Postoperative complications were mild inguinal pain in 4 patients (14.8%) for three weeks. Mild hydrocele in 3 patients (11%) was recorded. No recurrence or bulging at hernia site was noticed during the period of follow-up. Conclusion: Laparoscopic inguinal hernia repair with central mesh fixation after closure of hernial defect is effective, easy and free of complications.

Map Aided Pedestrian Dead Reckoning Using Buildings Information for Indoor Navigation Applications

Navigation systems play an important role in many vital disciplines. Determining the location of a user relative to its physical environment is an important part of many indoor-based navigation services such as user navigation, enhanced 911 (e911), law enforcement, location-based and marketing services. Indoor navigation applications require a reliable, trustful and continuous navigation solution that overcomes the challenge of global Navigation Satellite System (gNSS) signal unavailability. To compensate for this issue, other navigation systems such as Inertial Navigation System (INS) are introduced, however, over time there is a significant amount of drift especially in common with low-cost commercial sensors. In this paper, a map aided navigation solution is developed. This research develops an aiding system that utilizes geospatial data to assist the navigation solution by providing virtual boundaries for the navigation trajectories and limits its possibilities only when it is logical to locate the user on a map. The algorithm develops a Pedestrian Dead Reckoning (PDR) based on smart-phone accelerometer and magnetometer sensors to provide the navigation solution. geospatial model for two indoor environments with a developed map matching algorithm was used to match and project navigation position estimates on the geospatial map. The developed algorithms were field tested in indoor environments and yielded accurate matching results as well as a significant enhancement to positional accuracy. The achieved results demonstrate that the contribution of the developed map aided system enhances the reliability, usability, and accuracy of navigation trajectories in indoor environments.

A multi-band metamaterial antenna is proposed to operate at the terahertz(THz)band for medical *** proposed structure is designed on a polyimide as a support layer,and its radiating elements are made of ***,the design is started with a conventional shape showing a single operating frequency at 1.1 *** achieve a multi-band operating frequency,the conventional shape was replaced with the proposed metamaterial as a radiating patch that has properties not exist in *** multi-band frequencies are obtained without compromising the overall size of the *** overall size is 600×600×25μm^(3).The operating frequencies are 0.36,0.49,0.69,0.87,and 1.04 THz.A full ground plane is used to behave as isolation between the design and the human body *** proposed design is investigated on free space and on the human body model,showing excellent performance in both *** achieved gains for the following frequencies 0.36,0.49,0.69,0.87,and 1.04 THz are 4.81,6.5,8.41,6.02,and 7.96 dB,respectively,while the efficiencies are 83.91%,96.28%,90.80%,91.71%,and 92.99%,*** conventional design was modified to have a partial ground to show the benefit of using the full *** design is loaded on the human body model and its performance is *** efficiency and gain are 6.61 dB and 95.58.7%for the case of no human body model,and 4.26 dB and 40.30%for the case of using a human body ***,the proposed metamaterial antenna will be useful for future medical applications in the THz band.

Characterisation of Class 1 Integron among <i>escherichia coli</i>Isolated from Mansoura University Hospitals in egypt

extensive use of antibacterials in clinical practice has been associated with increasing frequency of multi-resistant e. coli strains. genetic elements such as Class 1 integrons have an important role in resistance development. In the current study, a total of 84 e. coli clinical isolates from Mansoura hospitals patients in egypt were screened for antibacterial susceptibility against 12 different antibacterials. High resistance rates were identified for Ampicillin (92.9%) and sulfamethoxazole/ trimethoprim (84.5%). Class 1 integron was investigated in e. coli isolates by PCR. As a result, Class 1 integron was identified in 51.2% of these isolates. The contents of amplified integron varriable regions, were digested by Alu I restriction endonuclease. Cluster analysis of Class 1 integron digested varriable regions revealed that RFLP digested fragments generated could be classified into 9 different patterns, namely A, B, C, D, e, F, g, H and I. The most prevalent genotype was identifed in group D. PCR and sequencing were used for detection of antimicrobial resistance genes harbored in integrons of group D. As a result, main phylogenetic group identified harbored integron cassette carrying resistance gene for two antimicrobial groups namely aminoglycoside and trimethoprim. Multiresistance profiles in group D exhibited association between antimicrobial resistance and integron presence. These findings suggest that the strategy for treatment of patients with e. coli infections needs to be revised. Furthermore, the high prevalence of Class 1 integron carrying gene confirms the importance of integron-mediated antimicrobial gene cassettes.

Palmitic Acid – Lauric Acid/expanded graphite as Form-Stable Composite Phase Change Material for Low-Temperature energy Storage Applications

Low thermal conductivity of binary fatty acid mixture of palmitic and lauric acids （PA-LA） within the value range of 0.15 – 0.17 W/（m·K） restricts its wide utilization as thermal energy storage material in the active regime of solar heating applications at low operating temperatures. Nevertheless， this mixture as phase change material （PCM） has a suitable phase-change temperature and heat of 36 ℃ and 176.3 J/g， respectively. Hence， the objective of this study is to formulate a novel form-stable composite PCM with the PA-LA mixture and expanded graphite （eg） as a thermal enhancer. PA-LA eutectic mixture with varied concentrations of eg was prepared and characterized. The thermal conductivity of PA-LA/eg increased gradually with the mass of eg. Optimum thermal properties were observed in PA-LA/（5% eg） composite， where its melting (Tm)， freezing temperature (Tf)， latent melting heat and thermal conductivity was 35.53 ℃， 34.84 ℃， 174 J/g， and 1.19 W/（m·K）， respectively. Also， the composite PCM is characterized by good chemical-thermal stability and thermal reliability for long-term usage. In conclusion， it can be utilized as a prospective form-stable PCM for thermal energy storage in solar heating systems， overheat treatment systems， and other thermal storage applications at low operating temperatures.
