Antihyperglycaemic activity of ethylacetate extract of Chlorophytum alismifolium in type 2 diabetes:The involvement of peroxisome proliferator-activated receptor-γand dipeptidyl peptidase-4

Objective:This research is to investigate the antihyperglycaemic activity and the underlying mechanisms of action of the ethylacetate extract of Chlorophytum alismifolium(EACA)tubers in a type 2 diabetes ***:The tubers were processed and sequentially extracted in hexane followed by ethylacetate,using a Soxhlet apparatus,and subjected to gas chromatography-mass spectrometry(GC–MS).The acute toxicity of EACA was investigated in albino Wistar *** antihyperglycaemic study was carried out using high-fat diet(pelletized diet and margarine in the ratio of 10:1 and 20%fructose solution)and streptozotocin-induced hyperglycaemic Wistar *** effects of the extract(150,300 and 600 mg/kg)on blood glucose level,insulin,peroxisome proliferator-activated receptor-c(PPAR-c)and dipeptidyl peptidase-4(DPP-4)were evaluated using enzyme-linked immunosorbent ***:The oral median lethal dose in Wistar rats was estimated to be>5000 mg/*** with EACA at all doses significantly reduced the fasting blood glucose levels,compared to the hyperglycaemic control,and over *** of EACA increased the serum insulin and PPAR-c levels while decreasing DPP-4 ***–MS analysis revealed the presence of 13 compounds,with isothiazole and isoxazolidine covering total area of 24.6%and 22.69%,***:The findings from this study showed that EACA has important compounds with beneficial effect in type 2 diabetes and acts by increasing insulin secretion and PPAR-c level and decreasing DPP-4 activity.

Introduction: Automated blood culture systems for incubation and growth monitoring have become the standard in high-income countries (HICs), but are still relatively expensive and not universally available for implementation in most low- and middle-income countries (LMIC). We aimed to report blood culture isolates using Automated technique in children and adults admitted into the Federal Teaching Hospital Gombe from 2016 to 2020. Materials and Methods: Blood Culture Isolates in children (0 - 18 years) and adults (>19 yrs) by Bactec 9050 Automated culture system from 2016-2020 were retrieved from the medical and laboratory register. Information analyzed included, age, sex, month, and year and culture growth and reported antibiotic sensitivity. A Bactec Blood culture tests is $20 in this facility. In Nigeria, the minimum monthly wage is $70 (Official currency exchange rate is N423/US Dollar). Results: Of the 1713 blood cultures performed, children 0 - 18 years were 1322 (77.2%) and adult (19 years above) (22.8%). Overall positivity was 733 (42.2%) with males 385 (52.5%). Of the 1322 Blood cultures (BC) in children 615 (46.5%) were positive for isolates and adults 118 (30.2)%. Blood culture positivity decreased with increasing age with newborns 251 (34.5%) and adults > 65 years 18 (2.5%). Staphylococcus aureus constituted 61.3% of all isolates and was the leading isolates in all age groups;Alkaligenes (9.1%);Citrobacter 8.1%, Klebsiella 6.7%;Pseudomonas 6.1%;E. coli 2.7%;Enterococcus 2%;Proteus 1%. Of the Antimicrobial resistance priority isolates E. coli susceptibility ranged from 71% to Gentamycin and 100% to Cefixime;Klebsiella from 25% sensitivity to Amikacin to 78% each to chloramphenicol and ciprofloxacin;Salmonella was 100% sensitive to chloramphenicol, ciprofloxacin and cefuroxime. Klebsiella was 100% sensitive to Cefoxitin;Proteus sensitivity ranged from 35% to ampicillin and 100% to ciprofloxacin and cefuroxime. Staph aureus sensitivity was 35% to cefoxitin,

Introduction: Antimicrobial Resistance surveillance is predicated on blood culture as a priority clinical specimen in especially resource limited settings. Establishing trends in blood stream infections and resistance patterns can inform institutional and national policy on antimicrobial stewardship, surveillance, infection prevention and control. Methodology: Blood Culture isolates in children (0 - 18 years) by conventional method from 2008-2012 and Bactec Automated culture system from 2015-2020 were retrieved. Information analyzed included age, sex, month, and year and culture growth/identity of microorganisms and their sensitivity/resistance patterns. Clinical and Laboratory Standards Institute (CLSI) guideline for antibiotic susceptibility testing was used. Results: 20,540 children were admitted: 8964 (44.6%) and 11,630 (55.4%) in the Manual and Bactec blood culture era respectively. Blood cultures were done in 5271 in the manual culture era and 1077 in the Bactec culture era;of these cultures, 514 (9.7%) and 461 (42.8%) were positive for isolates in the respective era (p = 0.01). There were no statistically significant differences in trend between positive and negative blood cultures in males and females. Newborns, followed by children 1 - 5 years had more blood culture performed on them than other age categories. In general, there is no significant relationship in blood culture outcomes between the age categories and sex of the patients. The isolation of Staph aureus, Citrobacter and Alkaligenes increased two-fold with Bactec automated system. Resistance to the quinolones and the penicillin was high. Resistance trend to Genticin, an aminoglycoside was less than 40%. Resistance to Ceftazidime was high. Conclusion: Antimicrobial resistance surveillance is critical to reduce AMR related morbidity and mortality.

Introduction: Transitioning to more efficacious Antiretrovirals for HIV infected pregnant women and infant prophylaxis has reduced Mother to child transmission of HIV significantly. This study aimed to determine HIV infection status in HIVexposed infants who had their first DNA polymerase chain reaction test in our molecular Laboratory. Subjects, Materials and Methods: Dried Blood Spots for HIV DNA results from 5 states between 2009 and 2020 were analyzed in the PCR laboratory of the Federal Teaching Hospital, Gombe. Results: Nine thousand eight hundred and twenty-three Human Immunodeficiency Virus Deoxyribonucleic acid polymerase Chain Reaction results were analysed;4937 (50.2%) were males. During the study period, there was an overall declining trend in the mother-to-child transmission rate from 3.8% in 2009 to 1.0% in 2020. 6120 (62.3%) of HIV + mothers received Highly active antiretroviral therapy HAART before pregnancy. 7845 (76.2%) of the infants received Nevirapine prophylaxis. Dried blood spot samples were collected from 4077 (41.5%) at 6 - 8 weeks. 8438 (85.9%) received cotrimoxazole. 9469 (96.4%) were ever breastfed. Of the 9823 HIV DNA PCR results, 255 (2.6%) were positive while 69/4077 (1.7%) and 109/2662 (4.1%) were positive for HIV DNA at 6 - 8 weeks and > 12 weeks respectively. (p = 0.001). 86/747 (11.5%) of infants whose HIV-positive mothers received no ARVS were HIV DNA positive. (p = 0.001). 106/884 (12.0%) of infants who had no Antiretroviral prophylaxis had positive HIV DNA results;7/413 (1.7%) with Zidovudine/Nevirapine prophylaxis had positive results. (p = 0.001). 246/9469 (2.6%) of infants that were ever breastfed were positive for HIV DNA;11/354 (3.0%) that never breastfed had positive HIV DNA. Conclusion: Lack of maternal/infant ARVs and prolonged breastfeeding increased the risk of infant HIV infection.

Background:Malaria during pregnancy remains a serious public health problem,with substantial risks for the mother,her foetus and the *** aim of this study was to determine the prevalence of malaria and possible risk factors for malaria infection among pregnant women in a semi-urban area in north-western *** women are among the most susceptible to malaria *** of their malaria infection status is an important yardstick to measure the effectiveness of any malaria control ***:We conducted a cross sectional study in the semi-urban area of Argungu,Kebbi State *** hundred and fifty five pregnant women were included in the study after informed verbal consent was *** each participant,the socio-demographic profile,stage of pregnancy and attitude to the use of insecticide-treated nets(ITNs)were investigated using a *** blood samples were collected and thick blood smears were prepared and stained with Giemsa stains to check for malaria *** associations between age,education level and use of ITNs with occurrence of malaria infection during pregnancy were analysed using the chi-square ***:One hundred and six(41.6%)out of 255 pregnant women were infected with malaria parasites,with a mean parasite density of 800 parasitesμl−*** was found that prevalence and parasite density decreased as age *** chi-square test indicated that a lack of education and non-usage of ITNs were significantly associated with malaria ***:Malaria is still a major public health issue among pregnant women mainly due to illiteracy and non-compliance to using *** awareness about malaria preventive measures and early attendance of antenatal care services will help to reduce malaria and,consequently,its associated morbidities and mortalities.

Geology of the Structurally Controlled Quartz Vein Gold Mineralization in the Dikko Area, Part of Zungeru—Minna—Birnin Gwari Schist Belt, NW Nigeria

Dikko area, Niger State, North Central Nigeria falls within the Pan-African thermo-tectonic event which occurred in Nigeria as in other parts of West Africa with widespread effect on the mineralization of rejuvenated basement and geosynclinal trough deposits. The Pan-African Belt in Nigeria forms part of the Benin-Nigeria (Dahomeyan) shield which is regarded as the southern prolongation of the circumcratonic Pan-African belt of the West African Craton. Structurally controlled, gold-bearing quartz veins were measured and studied in the area from an extensive geological ground investigation. Field and petrographical studies indicate that the Neoproterozoic rocks constitute gneisses, amphibolite schist, amphibolites and migmatites. The amphibolites are intruded by pegmatites. The associated quartz veins are generally concordant with the main N-S regional structural trend. The lithological and structural studies carried out in these areas have demonstrated the relationship between structures and mineralization and/or lithology. The main characteristics of the gold mineralization in the area appear to indicate hydrothermal mineralization in connection with shear zones. This study allows us to predict that quartz veins in the amphibolite can be mined at a profit and a detailed geochemical and resource valuation needs to be carried out for the Au and associated Galena and Ag mineralization to fully exploit their economic potential.

Utilization of Oil Properties to Develop a Spreading Rate Regression Model for Nigerian Crude Oil

The target of this study is to develop a spreading rate regression model capable of predicting rate of spread of Nigerian crude oil spills on water. The major factors responsible for spreading rate of crude oil on water were considered, namely surface tension, viscosity, and specific gravity/American Petroleum Institute degree (0API), all at specified temperature values. The surface tension, viscosity and density parameters were interactively measured under controlled factorial analysis. The spreading rate of each crude oil was determined by artificially spilling them on laboratory calm/stagnant water in a rectangular tank and their averages were also computed. These averages were used to develop a regression model equation for spreading rate. The model developed indicated that an average spreading rate was 3.3528 cm/s at 37.5°C and the predictive regression model is evaluated with the interactions of specific gravity, viscosity and surface tension. It is convenient to state that the model will predict the spread rate of crude oils which possess imputed physicochemical properties having pour point averaged 15.5°C on calm seawater.

Importance of Sand Grading on the Compressive Strength and Stiffness of Lime Mortar in Small Scale Model Studies

Mortars provide the continuity required for the stability and exclusion of weather elements in masonry assemblies. But because of the heterogeneity of the mortar, its mechanism of behaviour under different load effects is dependent on the properties of the constituents of the mortar. The aim of paper is to determine the effect sand grading for various cement-sand-lime mortar designations (BS) and strength classes (EC) on the compressive strength and stiffness of mortar. Two silica sands;HST 95 and HST60 were used to make mortars in three strength classes: M2, M4 and M6, corresponding to mortar designations iv, iii and ii respectively. The results show that mortar made with the HST60 sand (coarser grading) usually resulted in mortar with a higher compressive strength and stiffness. The One Way ANOVA analysis of both compressive strength and stiffness at a significance level of 5% on the effect of sand grading on the two parameters also shows that they are both significant. There is also strong evidence of a linear correlation between the stiffness and compressive strength. The results indicate that in order to replicate full scale behaviour of masonry at model scales, the grading of fine aggregate in the models should be similar so as to properly model full scale behavior.

Epispadias in a Child with Intact Prepuce: A Rare Congenital Abnormality

Epispadias is a rare genitourinary malformation, more so is epispadias with intact prepuce. Incidence of epispadias with intact prepuce is not known however, there are few case reports and a case series. We present a 5-year-old boy brought to our facility by his parent for circumcision. Further evaluation revealed a short penis with broad base and intact prepuce. The preputial slit was dorsally oriented. On retracting the prepuce, there was corona epispadias with wide and healthy urethral plate. No meatal stenosis or chordee noticed. Other findings were normal. He had epispadias repair. The postoperative period was uneventful. Three months after operation, the prepuce was easily retractable over the penis and the penis was cosmetically acceptable.
