Evaluation the Possibility of Using of miRNA-21 as a Prognostic and Predictive Factor in Colorectal Cancer (south Egypt Experience)

Background: Micro RNA 21?(miRNA-21) is overexpressed in numerous solid types of tumors, indicating that its biology is relevant to numerous types of cancer. However,?in Colorectal Cancer (CRC)?there is a lot of studies?confirmed its use as diagnostic marker, other studies suggested it could be used as a predictive marker for 5 Fu sensitivity so this study was conducted to verified the significance of using miRNA-21 as a prognostic and predictive factor in CRC. Patients and Methods: It is a prospective study that included 45 patients with stage I-IV CRC who attended at south Egypt Cancer Institute from April 2017 to October 2018.?miRNA-21 was measured by (q PCR) from peripheral blood.?Response to 5-FU containing regimens was assessed using RECIsT?criteria and its levels of expression were correlated to the clincopathological data,?response and survival of the patients.?Results:?High expression of the miRNA21 was significantly associated with advanced stage of CRC p ?0.001,?Lymph Node (LN)?invasion p ?0.001,distant metastasis p??0.01 lymphovascular (LVI) invasion p ?0.01 and pre-treatment level of Carcino?Embryonic Antigen (CEA) p ?0.031. also we found that high expression level significantly badly affected the response to 5-Fu containing regimen p ?0.001, DFs p ?0.01.?Conclusion:?miRNA-21 could be used as non-invasive prognostic and predictive marker in CRC.

Biological Effect of sucralose in Diabetic Rats

Among people that might take a large amount of sucralose, are diabetic people who are attempting to modify their carbohydrate intake. The objective of this study is to evaluate the impact of sucralose;an artificial sweetener derived from sucrose, at a dose approximately twice the ADI on hyperglycemia, hyperlipidemia and oxidative stress in diabetic rats. Diabetes was induced in male albino rats after an intraperitoneal streptozotocin injection (65 mg/kg body weight). Animals with fasting blood glucose levels ≥250 mg/dl were considered diabetics. sucralose was dissolved in water and administered to rats daily by oral gavages during a period of 6 weeks at a dose of 11 mg/kg body weight. Animals were divided into 4 groups and treated in parallel for 6 weeks. Control: rats received distilled water, sucralose: rats received sucralose, Diabetic: diabetic rats received distilled water, Diabeticrats + sucralose: diabetic rats received sucralose. The administration of sucralose to diabetic rats provoked a significant decrease (P P P > 0.05) on insulin, compared to their respective values in diabetic rats receiving distilled water. Biochemical analysis in brain and testis tissues showed that sucralose has no effect (P > 0.05) on superoxide dismutase (sOD), catalase, glutathione peroxidase (GsH-Px), and glucose-6-phosphate dehydrogenase (G-6-PDH) activities, and glutathione content (GsH), while reduced thiobarbituric acid reactive substances level (TBARs) (P spective values in diabetic rats receiving distilled water. It could be concluded that consumption of sucralose didn’t induce oxidative stress, has no effect on insulin, reduce glucose absorption and intensify hypercholesterolemia in sTZ-induced diabetic rats. Accordingly it is advised that diabetic people consuming high amount of sucralose must check their lipid profile to avoid diabetic complications.

Chronic ischemic mitral valve regurgitation is a result of disturbed left ventricular geometry secondary to myocardial ischemia in the absence of intrinsic mitral valve pathology. It is a common complication after myocardial infarction, and patients who have ischemic mitral regurgitation(IMR) have a worse prognosis compared to patients who have ischemic heart disease alone, and this is directly related to the severity of IMR. Medical therapy has limited efficacy, and surgical options including various repair techniques and valve replacement had been tried with variable success. still there is intense debate among surgeons whether to interfere with moderate degree IMR at the time of coronary artery revascularization.

The Inhibitory Effect of Curcumin on Ornithine Decarboxylase against Hepatic Carcinoma

Curcumin the active component of turmeric is widely used as an anticancer agent for treating many human cancers. This study aimed at the extraction of curcumin from Curcuma Longa and investigates its therapeutic effect as ornithine decarboxylase (ODC) inhibitor in HepG2 cells. The proliferation of HepG2 cells was carried out by using the MTT assay. In addition, cell cycle analysis was evaluated by using the flow-cytometric technique. Our results showed that curcumin has the ability to inhibit the proliferation of HepG2 cells with IC50 of 24.79 μg/ml and induced G2/M cell cycle arrest. Moreover, it caused an elevation in the intracellular concentration of Ca2+. Moreover, in the curcumin administration the downregulation expression level of ODC and Bcl-2 genes (p ≤ 0.05) was significant found. On the other hand, upregulation in the expression level of P53, Bax, and caspase-3 genes (p ≤ 0.05). This study concluded that curcumin may be considered as a new saving candidate for the future progress of antitumor agents.

Lamellar Bodies Count (LBC) as a Predictor of Fetal Lung Maturity in Preterm Premature Rupture of Membranes Compared to Neonatal Assessment

Background: Respiratory distress syndrome (RDs) is a major cause of neonatal morbidity and mortality, affecting approximately 1% of all live births and 10% of all preterm infants. Lamellar bodies represent a storage form of pulmonary surfactant within Type II pneumocytes, secretion of which increases with advancing gestational age, thus enabling prediction of the degree of FLM. Preterm premature rupture of membranes (PPROM) complicates approximately 1/3 of all preterm births. Birth within 1 week is the most likely outcome for any patient with PPROM in the absence of adjunctive treatments. Respiratory distress has been reported to be the most common complication of preterm birth. sepsis, intraventricular haemorrhage, and necrotizing enterocolitis.also are associated with prematurity, but these are less common near to term. Objective: To assess the efficacy of the amniotic fluid lamellar body counting from a vaginal pool in predicting fetal lung maturity in women with preterm premature rupture of membranes. Methods: This study was conducted at Ain shams University Maternity Hospital in the emergency ward from January 2019 to september 2019. It included 106 women with singleton pregnancies, gestational age from 28 - 36 weeks with preterm premature rupture of membranes. This study is designed to assess the efficacy of the amniotic fluid lamellar body counting (LBC) from a vaginal pool in predicting fetal lung maturity in women with preterm premature rupture of membranes. Results: The current study revealed a highly significant increase in the lamellar body count in cases giving birth to neonates without RDs compared to that cases giving birth to neonates with RDs. also, no statistically significant difference between LBC and age, parity and number of previous miscarriages in the mother was found. Impact of Thyroglobulin on survival and Prognosis of Differentiated Thyroid Cancer

Proper assessment of risk factors contributes to the principle management of differentiated thyroid carcinoma post operatively. Aim of the study: to investigate the effect of Thyroglobulin (Tg) levels on prognosis together with other risk factors for Differentiated Thyroid Cancer (DTC). Patients and methods: Medical records of all patients with DTC presented to Clinical Oncology and Nuclear Medicine Department referred from Diabetes & Endocrine unit (Internal Medicine Hospital) and surgery Department Mansoura University from 2011-2016 were retrospectively reviewed. Patients with distant metastasis or who lost follow-up were excluded. so data of 220 patients were analyzed. Data collected included pre-surgical assessment, also surgical interference either total or near total thyroidectomy with or without lymph node neck dissection were reviewed. Different prognostic factors that affect progression free survival (PFs) include age, umorsize, ymph node status, ex, multifocality, capsular infiltration, vascular invasion and Tg level were evaluated through multivariate analysis. Results: Most of the patients included were s (69.1%). Incidence of the disease was higher in female (80%) with papillary type predominance (80.9%). About 59.5% of cases presented with tumor size ≤ 2 cm and multifocality was reported in 13.6%. While 30% had lymph node metastasis, 11% had vascular invasion. Capsular infiltration was observed in 15% and most of them showed Tg level ≤ 10 ng/ml (68.2%). About 70% received ablative radioiodine. The 5-year Progression Free survival (PFs) was 85%. On multivariate analysis of variable prognostic factors on PFs, we found that tumor size, age, lymph node status, capsular infiltration, Tg level and vascular invasion significantly affected PFs (P = 0.01, 0.005, 0.004, 0.005, 0.02, 0.003) respectively. 作为文明古国,埃及在旅游资源方面具有天然的优势,同时埃及存在大量的沙漠区域,也限制了埃及经济的发展。旅游业作为埃及典型的服务行业,且全球都处于“体验经济时代”中,服务质量对旅游业具有极为重要的意义,是提升景区及旅游目的地形象和品牌的关键因素。虽然埃及旅游业是埃及的支柱行业,但是服务质量水平却相对落后,因此提高服务质量一直是埃及旅游业发展过程中强调的重点。景区是旅游业的心脏,是旅游业发展的必要物质基础。而目前埃及大部分景区认为拥有得天独厚的旅游资源,就可以进行垄断经营,因此形成“门票经济”,而忽视对景区服务质量的改进。因此“一流的景观、三流的服务”、服务供给与服务需求关系失衡、业务流程响应游客需求不够、员工价值发挥不充分等问题比比皆是,游客体验满意度普遍下降,游客抱怨投诉增加。长久下去,必然会影响埃及旅游业的发展。因此如何提升埃及旅游业的游客满意度具有实践及理论意义。本次研究即以服务质量为切入点,探讨埃及旅游景区顾客感知服务质量对顾客满意度的影响。论文在文献研究和定性分析研究的基础上,梳理了景区服务质量、顾客感知服务质量、期望服务质量以及顾客满意度的概念和内涵。结合埃及旅游景区的特点,参考格罗鲁斯的顾客感知服务质量模型、PZB的差距模型及ACsI模型,提出了本次研究的游客感知景区服务质量与顾客满意度关系模型及研究假设,通过实证方式对假设进行验证。研究认为,游客的景区感知服务质量对顾客满意度产生正向影响,影响显著,即游客感知到的服务质量越好,顾客满意度也就越高;游客的景区预期服务质量对顾客满意度产生反向影响,影响显著,也即游客对景区服务质量的期望越高,最终的顾客满意度也越低;游客的景区预期服务质量对游客的景区感知服务质量产生反向影响,影响显著,也就是说游客对景区服务质量的预期越高,实际体验到的服务质量会降低。根据研究结论,论文从提升游客感知质量、维持游客对服务质量预期在合适范围内两大方面对埃及旅游景区提出了一些管理建议。论文研究成果希望能对埃及旅游景区游客满意度的提升具有一定的理论指导意义。

strong>Background:strong> Hepatitis C Virus (HCV) infection is a progressive disease that may result in chronic hepatitis, fibrosis and cirrhosis. Assessment of liver fibrosis is an essential factor in the management of chronic HCV. strong>Objective:strong> To evaluate plasma soluble Urokinase Plasminogen Activator Receptor (sUPAR) and interleukin-34 (IL-34) as serological markers of liver fibrosis in patients with chronic HCV. strong>Methods:strong> This case-control study enrolled 60 chronic HCV patients who were subdivided into three groups of mild, moderate and severe hepatic fibrosis depending on Fibrosis-4 score (FIB-4). Patients were compared with 20 age and sex-matched controls. Plasma sUPAR and IL-34 levels were measured by Enzyme Linked Immunosorbent Assay (ELIsA). strong>Results:strong> Plasma sUPAR and IL-34 were significantly increased in HCV patients when compared with controls, and their increase was positively correlated with the progression of hepatic fibrosis. Plasma sUPAR and IL-34 positively correlated with Aspartate Aminotransferase (AsT) and alanine Aminotransferase (alT), and negatively correlated with hemoglobin concentration and platelet count. The output data of Receiver Operating Characteristic (ROC) curve to differentiate patients from controls revealed that sUPAR at cut-off > 186.2 ng/L and Area Under Curve (AUC) of 0.944 had (85%) sensitivity and (100%) specificity, and IL-34 at cut off > 16.4 ng/L and AUC of 0.942 had (75%) sensitivity and (100%) specificity. The output data of ROC curve to differentiate severe from mild to moderate hepatic fibrosis patients revealed that sUPAR at cut-off > 510 ng/L and AUC of 0.837 had (80%) sensitivity and (90%) specificity. While IL-34 at cut off > 55.3 ng/L and AUC of 0.844 had (85%) sensitivity and (80%) specificity. strong>Conclusions:strong> Increased plasma levels of sUPAR and IL-34 in chronic HCV patients with liver fibrosis and their increase was parallel to the degree of liver
