A Real Life Study on Treatment of Egyptian Patients with HCV Genotype IV with Simeprevir and Sofosbuvir

Background and Aims: Treatment with a combination of the nucleotide polymerase inhibitor sofosbuvir and NS3A (non-structural protein 3A) protease inhibitor simeprevir resulted in high rates of sustained virological response in chronic hepatitis C Genotype 4. Methods: We conducted a real life study on Egyptian patients coming to tropical medicine department clinic at El Mery main university hospital from February 2015 to February 2016 for treatment naive and treatment experienced patients with chronic HCV genotype 4, including cirrhotics and non cirrhotics. Naive (cir-rhotics and non cirrhotics) and relapsers (non cirrhotics) received nucleotide polymerase inhibitor sofosbuvir and NS3A inhibitor simeprevir once daily for 12 weeks and 24 weeks for relapser cirrhotic patients. The primary end point was a sustained virologic response at 12 weeks after end of treatment. An informed consent was obtained from each patient at the beginning of the study (Real life study: a study on Egyptian patients when the drug was available in the market). Results: 30 naive patients with HCV genotype 4 and 20 relapsers (10 non cirrhotic and 10 cirrhotic patients) were enrolled. Patient inclusion criteria: Naive patients are those who tested positive for HCV RNA by PCR and had no experience to HCV treatment;Relapsers are those who tested positive for HCV RNA by PCR and had a previous treatment for HCV. Cirrhosis was diagnosed on ultrasound basis. Mean age was 53.57 ± 10.682 years old in naive patients and 48.30 ± 5.100 years old in relapsers. Median baseline HCV RNA was 360,069 IU/mL for naive patients and 1,245,000 IU/mL for relapsers;using Fib4 20% of naive patients were F3-F4, while 40% of relapsers were F3-F4. degree of fibrosis was confirmed by fibrotest in relapsers. Upon treatment of patients with sofosbuvir and semiprevir once daily for 12 weeks and 24 weeks only to cirrhotic relapsers, end of treatment PCR was negative in 100% in all groups including cirrhotics and non cirrhotics.

The frequency and antimicrobial resistance patterns of nosocomial pathogens recovered from cancer patients and hospital environments

Objective:To determine the prevalence and antimicrobial resistance rates of nosocomial pathogens isolated from cancer patients and hospital ***:A descriptive cross-sectional study was conducted between december 2010 to May 2013 at Radiation and Isotopes Centre of Khartoum,Sudan.A total of 1 503 samples(505 clinical and 998 environmental)were *** were identified,and their antimicrobial susceptibility was determined using standard laboratory ***:Out of 505 clinical samples,nosocomial pathogens were found as 48.1%.Among hospital environment samples,bacterial contaminants were detected in 29.7%of *** main microorganisms recovered from cancer patients were Proteus spp.(23.5%),Escherichia coli(22.2%),Pseudomonas aeruginosa(***)(21.0%)and Staphylococcus aureus(20.2%).The most frequent isolates from hospital environments were Bacillus spp.(50.0%),Staphylococcus aureus(14.2%)and ***(11.5%).The proportions of resistance among Gram-negative pathogens from cancer patients were high for ampicillin,cefotaxime,ceftazidime and *** resistance rates were recorded to ciprofloxacin,such as 51.0%for ***,21.7%for Klebsiella pneumoniae and 55.5%for Escherichia *** Klebsiella,there were no significant differences(P0.05)of resistance rates between Gram-negative isolates from cancer patients to those from the hospital *** proportions of extended-spectrum b-lactamase producing isolates from cancer patients were not differ significantly(P=0.763)from those collected from the hospital environments(49.2%;91/185 vs.47%;32/68).Conclusions:The prevalence of nosocomial infection among cancer patients was high(48.1%)with the increasing of antimicrobial resistance *** environments are potential reservoirs for nosocomial infections,which calls for intervention program to reduce environmental transmission of pathogens.

dyslipidemia is a disorder where abnormally lipid concentrations circulate in the bloodstream. The disorder is common in type 2 diabetics (T2d) and is linked with T2d comorbidities, particularly cardiovascular disease. dyslipidemia in T2d is typically characterized by elevated plasma triglyceride and low high-density lipoprotein cholesterol (HdL-C) levels. There is a significant gap in the literature regarding dyslipidemia in rural parts of Africa, where lipid profiles may not be captured through routine surveillance. This study aimed to characterize the prevalence and demo-graphic profile of dyslipidemia in T2d in the rural community of Ganadougou, Mali. We performed a cross-sectional study of 104 subjects with T2d in Ganadougou between November 2021 and March 2022. demographic and lipid profiles were collected through cross-sectional surveys and serological analyses. The overall prevalence of dyslipidemia in T2d patients was 87.5% (91/104), which did not differ by sex (P = .368). High low-density lipoprotein cholesterol (LdL-C) was the most common lipid abnormality (78.9%, [82/104]). dyslipidemia was associated with age and hypertension status (P = .013 and.036, respectively). High total and high LdL-C parameters were significantly associated with hypertension (P = .029 and .006, respectively). In low-resource settings such as rural Mali, there is a critical need to improve infrastructure for routine dyslipidemia screening to guide its prevention and intervention approaches. The high rates of dyslipidemia observed in Gandadougou, consistent with concomitant increases in cardiovascular diseases in Africa suggest that lipid profile assessments should be incorporated into routine medical care for T2d patients in African rural settings.

In this article,we consider the Bagley-Torvik type fractional differential equation;d;l（t）-a;d;l（t） = g(t, l（t）) and differential inclusion;d;l（t）-a;d;l（t） ∈ G(t, l（t）),t ∈（0, 1） subjecting to l（0） = l;,and■ ds where 1 0, a, λ′are given constants. By using Leray-Schauder degree theory and fixed point theorems, we prove the existence of solutions. Our results extend the existence theorems for the classical Bagley-Torvik equation and some related models.

Microstructure evolution and mechanical properties of heat treated LCB titanium alloy

The influence of ageing time on microstructure and mechanical properties of low-cost beta （LCB） titanium alloy with a chemical composition of Ti-6.6Mo-4.5Fe-1.5Al was investigated. The correlation between microstructure and fatigue crack initiation and growth was also studied. Increasing ageing time tended to increase the volume fraction of the secondary α-precipitates, β-grain size and partial spheroidization of primary α-phase. The maximum tensile strength （1565 MPa） and fatigue limit （750 MPa） were obtained for the samples aged at 500 °C for 0.5 h, while the minimum ones of 1515 MPa and 625 MPa, respectively, were reported for the samples aged at 500 °C for 4 h. The samples aged at 500 °C for 4 h showed a transgranular fracture mode. However, the samples aged at 500 °C for 0.5 h revealed a mixture fracture mode of transgranular and intergranular. The formed cracks on the outer surface of the fatigue samples were found to propagate through the β-grains connecting the primary α-particles existing at the β-grain boundaries.

AIM:To identify whether there could have been changes in survival if lymph node ratio (N ratio) had been ***:We assessed 334 gastric adenocarcinoma cases retrospectively between 2001 and *** hundred and sixteen patients out of 334 were included in the *** were grouped according to disection1 (d1) or dissection 2 (d2) *** compared the estimated survival and actual survival determined by Pathologic nodes (pN) class and N ratio,and SPSS 15.0 software was used for statistical ***:Ninety-six (44.4%) patients underwent d1 dissection and 120 (55.6%) had d2 *** groups were evaluated,23 (24.0%) patients in d1 and 21 (17.5%) in d2 had stage migration (P=0.001).When both d1 and d2 groups were evaluated for number of pathological lymph nodes,despite the fact that there was no difference in N ratio between d1 and d2 groups,a statistically significant difference was found between them with regard to pN1 and pN2 groups (P=0.047,P=0.044 respectively).In d1,pN0 had the longest survival while pN3 had the *** d2,pN0 had the longest survival whereas pN3 had the shortest ***:N ratio is an accurate staging system for defining prognosis and treatment plan,thus decreasing methodological errors in gastric cancer staging.

Physical Measurements for the Accuracy of Cone-Beam CT in dental Radiography

The introduction of cone-beam computed tomography (CBCT) for the maxillofacial imaging provided opportunities for dental practitioners to request multiplanar imaging. Currently CBCT is used during implant treatment-planning to measure accurately the height of bone available for implant placement to avoid serious problems e.g. compromising vital structure such as the inferior alveolar nerve during placement of implants. We present a physical method to assess the accuracy of measurements obtained from an i-CAT CBCT dental radiography unit used at king abdulaziz university hospital and to appraise the effect of voxel size on the accuracy of measurement by an i-CAT CBCT unit. Not significant difference was found between the physical and CBCT measurements for all the samples. The maximum measurements variation was + or -0.15 mm. Therefore, the linear measurements from i-CAT CBCT unit are accurate and the adjustment of the scan parameters like the FOV and voxel size will not significantly affect this accuracy.

Background:Extra genetic material in patients with down syndrome(dS)may affect the function of any organ *** evaluated cardiac functions using conventional tissue doppler and two-dimensional speckle tracking echocardiography in patients with dS in the absence of congenital and acquired heart disease in ***:A total of 115 patients with dS between 6 and 13 years of age with clinically and anatomically normal heart and 55 healthy children were included in this cross-sectional *** was diagnosed by a karyotype *** with mosaic type were not included in this *** and diastolic functions were evaluated by ***:Pulsed waved doppler transmitral early/late inflow velocity(E/A),tissue doppler mitral annular early/late diastolic peak velocity(Ea/Aa),transtricuspid E/A and tricuspid valve annulus Ea/Aa,pulmonary venous doppler systolic/diastolic(S/d)wave ratio were lower in patients with down syndrome than in the control group(P=0.04,P=0.001,Pd tricuspid annular Ea were lower in patients with dS(Pd left ventricular myocardial performance indexes were higher in patients with dS than in the controls(Pd significantly higher left ventricular mass,ejection fraction,the mitral annular plane systolic excursion ***,the down syndrome group compared with the controls had a lower strain values examined by two-dimensional longitudinal speckle-tracking strain ***:These findings suggest conventional tissue doppler and two-dimensional longitudinal speckletracking strain echocardiography were useful methods of investigating ventricular function and identifying a higher incidence of biventricular dysfunction in patients with down syndrome compared with the healthy controls.

First-Principles Calculations of the Structural, Mechanical and Thermodynamics Properties of Cubic Zirconia

The structural, mechanical and thermodynamics properties of cubic zirconium oxide (cZrO2) were investigated in this study using ab initio or first-principles calculations. density functional theory was used to optimize the crystal structure of cZrO2 and thereafter, simulations were conducted to predict the lattice parameters and elastic constants. The Zr-O bond distance was calculated as 2.1763 Å with unit cell density of 6.4179 g/cm3. The data obtained were used to determine Young’s modulus, bulk modulus, Poisson’s ratio and hardness of cZrO2 as 545.12 GPa, 136.464 GPa, 0.1898 and 12.663(Hv) respectively. The result indicates that cZrO2 is mechanically stable with thermodynamics properties of a refractory material having potential for structural and catalytic applications in various forms as a nanomaterial.

Cortisol and uncomplicated Plasmodium falciparum malaria in an area of unstable malaria transmission in eastern Sudan

Objective:To investigate the levels of serum Cortisol in patients with uncomplicated Plasmodium falciparum(***) malaria in an area of unstable malaria transmission in eastern Sudan. Methods:The concentrations of Cortisol were measured in sera of 25 patients with uncomplicated *** malaria(at presentation,24 h and 7 d later) and 25 healthy volunteers using radioimmunoassay gamma ***:There was no significant difference in mean(Sd) of total Cortisol levels in patients with malaria in comparison with the control group;602.2(369.6) vs. 449.2(311.7) ng/mL,P=*** patients with uncomplicated *** malaria,the mean(Sd) presenting Cortisol levels were significantly higher in comparison to the levels on day 7;602.2 (369.6) vs.373.6(139.1) ng/mL,P=*** the patients with uncomplicated *** malaria (on presentation) Cortisol levels were not correlated with initial temperature or the presenting ***:Thus,Cortisol levels were not significantly different between the patients and the controls.
