Open versus laparoscopic right hemicolectomy in the elderly population

AIM:To compare short term outcomes of elective laparoscopic and open right hemicolectomy(RH) in an elderly population.METHODS:All patients over the age of 70 undergoing elective RH at Ninewells Hospital and Perth Royal Infirmary between January 2006 and May 2011 were included in our analysis.Operative details,hospital length of stay,morbidity and mortality was collected by way of proforma from a dedicated prospective database.An extracorporeal anastomosis was performed routinely in the laparoscopic group.The primary endpoints for analysis were morbidity and mortality.Our secondary endpoints were operative duration,length of hospital stay and discharge destination.RESULTS:Two hundred and six patients were included in our analysis.One hundred and twenty-five patients underwent an open resection and 81 patients had a laparoscopic resection.The mean operating time was significantly longer in the laparoscopic group(139 ± 36 min vs 197 ± 53 min,P = 0.001).The mean length of hospital stay was similar in both groups(11.2 ± 7.8 d vs 9.6 ± 10.7 d,P = 0.28).The incidence of postoperative morbidities was 27% in the open group and 38% in the laparoscopic group(P = 0.12).Overall inhospital mortality was 0.8% in open procedures vs 1% in laparoscopic.CONCLUSION:Laparoscopic RH was associated with a significantly longer operative time compared to open RH.In our study,laparoscopic RH was not associated with reduced post-operative morbidity or significantly shorter length of hospital stay.

Malignant melanoma in the pediatric population

Controversial pigmented lesions in children are a problem for pathologist, clinicians and families that are confronted with this dilemma. Some skin lesions in this population defy diagnosis with pathologists split between a benign diagnosis and a cancer diagnosis. Three cases of controversial pigmented lesions in the pediatric population are presented. Three patients underwent radical resection of the controversial pigmented lesion, intra-operative lymphatic mapping and sentinel lymph node(SLN) biopsy. Due to the low morbidity of the SLN procedure a case is made to perform lymphatic mapping in this clinical scenario. If the SLNs are negative, not much is lost except for the scar and this becomes another line of evidence that perhaps the original lesion was benign. If the SLN shows metastatic cells, then the original skin lesion must be malignant and the patient is offered stage Ⅲ recommendations that would include complete node dissections and adjuvant Interferon therapy. This strategy provides for adequate treatment of the worse-case scenario, that the skin lesion is malignant. The cost to the patient is a low morbidity procedure, the SLN biopsy.

Numerical and Physical Modeling of Turbulence in a Peirce-smith Copper Converter

In this work, a numerical and experimental analysis of the very early stages of biphasic flow and the onset of turbulence in Peirce-smith copper converters with submerged lateral air injection is presented. Dimensions and operating conditions corresponding to industrial size copper converters are employed in 2D transient two-phase numerical simulations. A submergence depth for tuyeres of 0.5 m and 1.0 m of molten bath depth were assumed in the numerical simulations. Air velocities at the nozzle inlet were varied from 5 to 50 m/s, and integration times up to 10 s were considered. A transition from bubbling to jetting regime is observed as injection velocity is increased. Computer results show that the metallostatic pressure exerted by the molten copper pushes the injecting jet towards the nearest converter wall. Results from a set of independent tests conducted in a water model validate the numerical data obtained.

本文描述军用计算机系统寿命周期成本的计算方法,计算机系统作为三种竞争军用计算机系列体系结构(IBM S/370,DEC PDP-11,Interdata 8/32)的函数;介绍此方法用于两种不同的军用计算机资源要求的模型(名曰“自顶向下”和“自底向上”)所得的结果。体系结构的比较这样进行:设计,计算,并使硬件与软件成本的估算跟依赖于体系结构的因素相结合以求得寿命周期(研制周期加维护周期)成本与成本比。结果表明,对于这两种模型,三种体系结构的寿命周期成本大致可相比较。然而,如果忽略假设及输入数据的不定性,那末对于自底向上模型来说,DEC PDP-11多数情况下优于IBM S/370和Interdata 8/32。另一方面,对于自顶向下模型,当软件与硬件之成本比大于1时,S/370为优;此比值小于1/4时,Interdata 8/32稍佳;比值大于1/4而小于1时,PDP-11最好。

Damage mechanism identification has scientific and practical ramifications for the structural health monitoring,design,and application of composite systems.Recent advances in machine learning uncover pathways to identify the waveform-damage mechanism relationship in higher-dimensional spaces for a comprehensive understanding of damage evolution.This review evaluates the state of the field,beginning with a physics-based understanding of acoustic emission waveform feature extraction,followed by a detailed overview of waveform clustering,labeling,and error analysis strategies.Fundamental requirements for damage mechanism identification in any machine learning framework,including those currently in use,under development,and yet to be explored,are discussed.

洪涝、干旱及水体污染等迫切问题暴露了印度尼西亚雅加达市的治理危机。尽管洪涝问题已经上升到政治层面,当地政府也正在推动河岸景观的改善,但雅加达的公共政策仍未厘清供水不足与洪涝问题之间的复杂联系。早在1872年,雅加达的洪涝问题就已非常显著。随后出现了多次重大洪涝灾害,最近一次发生于2015年。当地政府制定了多项政策来解决这一难题,新近提出的"河流正常化"政策即为其中一项。其旨在提高河流泄洪能力,避免暴雨期间河水泛滥,具体措施包括取缔河岸地区的非正规居住点、拓宽河道和运河宽度及疏浚河床。但不少学者对这项政策持批判态度,技术官僚性过强是弊病之一,以开发流域基础设施为名借机引资为其二。此外,该政策也未顾及非正规居住点居民的利益,难以促进建立兼具包容性与韧性的社会-水文系统。本研究借助系统动力学模型,针对雅加达的洪涝问题,剖析了社会和水文-地貌因素间的相互作用,对当前用以改善洪涝问题、预防潜在风险的"河流正常化"政策进行了评估,并进一步对两类不同的洪水治理政策--提升疏浚成效和综合水景政策进行了检验和对比,结果表明:尽管前者可在短期之内实现较高的效益且实施难度较小,后者却能够更加系统地应对雅加达的洪涝问题以及未来的气候风险,为城市带来更加持久的韧性。但综合水景政策在实施过程中既需要以多中心治理制度为保障,也需要依靠边界组织来推动不同治理层面各行为主体的积极参与。

我们先前研究表明胶质细胞源性神经营养因子(GDNF)联合施万细胞移植能促进脊髓损伤后轴突再生和髓鞘形成。然而,GDNF介导这一过程的细胞靶点尚不清楚。在此,我们报道了GDNF可增加在体再生轴突的数目和直径,并促进体外背根神经节神经元的轴突向外生长,提示GDNF对神经元有直接作用。在施万细胞-背根神经节神经元共培养下,GDNF显著增加施万细胞生成的髓鞘数目;GDNF处理对孤立培养的施万细胞增殖无作用,但可促进已与神经轴突有突触联系的施万细胞增殖;GDNF可增加孤立施万细胞中分子量为140kDa的神经细胞黏附分子(NCAM)的表达,但对黏附分子L1表达或神经营养因子NGF、NT3及BDNF分泌没有影响。总之,这些结果支持假设:GDNF提高轴突再生和施万细胞髓鞘形成主要是通过GDNF对神经元的直接作用介导的,并且提示GDNF联合施万细胞移植可能是促进脊髓损伤后轴突再生和髓鞘形成的有效策略之一。
