Treatment of early stage(T1) esophageal adenocarcinoma:Personalizing the best therapy choice

Esophagectomy is considered the primary form of management for esophageal adenocarcinoma(EAC);however,the surgery is associated with high rates of morbidity and *** patients with early-stage EAC,endoscopic resection(ER)presents a potential curative treatment option that is less invasive and carries fewer risks procedure related risks,but it is associated with higher rates of cancer recurrence following the *** some patients,age and comorbidities may prevent them from having esophagectomy as a treatment option,while other patients may be operative candidates but do not wish to undergo esophagectomy for a variety of reasons related to their values and ***,while anxiety of cancer recurrence following ER may significantly diminish a patient’s quality of life(QOL),so might the morbidity surrounding *** addition to considering health status,patient preferences,and impacts on QOL,physicians and patients must also consider what treatments would be both beneficial and available to the patient,considering esophagectomy methods-minimally invasive vs open-or the use of chemoradiotherapy in addition to *** article reviews and summarizes available treatment options for patients with early EAC and their potential effects on the health and wellbeing of patients based on the current *** conclude with a request for more research of available options for early EAC patients,the conditions that determine when each option should be employed,and their effects not only on patient health but also QOL.

目的：调查降胆固醇治疗与自杀、意外事故或外伤致死（非疾病死亡率）危险性之间的关系。设计：降胆固醇治疗的大型随机对照试验中非疾病死亡率结果的汇总分析。研究回顾：21组入选试验中19组提供了非疾病死亡率的可用数据。干预回顾：过去1~10年间采取饮食调节、药物治疗或行部分回肠旁路手术。主要结果测定指标：治疗组与对照组间自杀、意外事故和暴力致死的比较。结果：汇集所有试验，治疗组非疾病死亡率与对照组的比数比为1.18（95%可信区间0.91~1.52； P=0.20）。初级预防试验的比数比为1.28（0.94~1.74 P = 0.12），二级预防试验为1.0（0.65~1.55 P = 0.98）。使用他汀类药物的随机对照试验表明，治疗与非疾病死亡率升高无关（0.84，0.50~1.41： P = 0.50）。相反，试验显示，饮食干预及非他汀类药物有增加自杀及暴力致死的趋势（1.32，0.98~1.77； P =0.06）。已发现胆固醇降低数值与非疾病死亡率之间并无关系（ P = 0.23）。结论目前尚无证据表明降胆固醇治疗使非疾病死亡率明显上升，而饮食调节及非他汀类药物可使非疾病死亡率轻度增加。

目的探讨肺表面活性物质相关基因功能性变异在中国人群中的携带情况。方法以广西壮族自治区南宁市汉族和壮族杂交足月新生儿258例为研究对象，男146例，女112例，提取基因组DNA并按组别混合DNA，以表面活性蛋白b（SFTPb）、表面活性蛋白C（SFTPC）、ATP连接盒转运子A3（AbCA3）、磷脂酰转移酶（LPCAT1）、胆碱磷酸转移酶（CHPTl）、磷酸胆碱胞苷酰转移酶（PCYTIb）为候选基因，应用Ⅲumina第二代测序平台对候选基因进行全外显子深度测序，并应用合并变异的统计分析方法，获取功能性变异的等位基因频率。结果（1）两组共发现变异128种，其中同义变异44种，非同义变异66种；碱基插入／缺失18种；在这些变异中，有28种为功能性变异，其中有2种变异出现2次，其他变异各出现一次，共有30个功能性变异的等位基因；（2）男女两组均以AbCA3基因出现功能性变异的频率最高，其等位基因频率分别为0．014（1．4％）和0．04（4％）；6个候选基因功能性变异的总等位基因频率在男女组分别为0．041（4．1％）和0．08（8％），P=0．06。结论（1）汉族和壮族杂交人群携带有肺表面活性物质相关基因的功能性变异；（2）出现变异频率最高的基因为AbCA3，提示该基因变异在新生儿呼吸窘迫综合征及其他肺疾患的发病中发挥着重要的作用。

Aim:To develop a comprehensive item library of patient-reported,immunotherapy-related adverse events(irAEs)that draws from and expands on the Functional Assessment of Chronic Illness Therapy(FACIT)Measurement ***:Literature review and iterative expert *** on a literature review of irAEs,we developed a framework of immunotherapy classes and their associated *** experts then reviewed iterations of symptom summaries and item maps linked to the immunotherapy *** provided content review and feedback was shared across experts until consensus was *** iterative process facilitated creation of a Primary Symptom List associated with immune checkpoint modulators(ICMs),drawn from the larger set of *** FACIT items were mapped to the symptom list,and new items were written as needed to create the item ***:The full item library of irAEs is comprised of 239 items,covering 142 unique symptoms across 75 inflammatory reactions/immune conditions.A subset of 66 items comprises a Primary Symptom List considered most common/relevant to ICM *** includes gastrointestinal,skin,pulmonary,neurologic,musculoskeletal,and multiple miscellaneous and constitutional ***:The FACIT Immunotherapy Item Library is a compilation of 239 self-report items that capture the wide range of AEs experienced by people receiving immune treatments.A subset of 66 items comprises a Primary Symptom List meant for ICM *** of items selected from this library is encouraged in clinical research and clinical practice evaluation.
