Two reports of trials investigating the potential effect of acupuncture in increasing live birth rate(LBR) in women with polycystic ovary syndrome(PcOS) or those undergoing in vitro fertilization(IVF) were published by Journal of American Medical Association in 2017 and 2018 respectively. The trial investigators did not recommend acupuncture for the women with PcOS or IVF based on their findings. This paper raises the concern that the findings that acupuncture did not increase LBR for women with PcOS or undergoing IVF may be the result of methodological flaws in the studies and unintended bias such as use of an invalid control intervention and underestimation of the true acupuncture effect. Therefore, their findings may not be a valid reflection of acupuncture’s effect in improving LBR in women with these clinical conditions.

Building and managing electronic journal collections in the digital environment poses many challenges for librarians, as electronic journals exist in a complex and constantly changing state involving different digital formats, new pricing structures, and various access routes. This paper reviews critical issues that North American libraries face in managing electronic journals, including a brief history, the selection criteria, consortia purchasing, licensing, operational costs, providing access, user support, usage statistics, canceling print subscriptions, and electronic journal archives. Future trends discussed include open access e-journals, and the continuing development of e-journal collections and best practices in managing this essential constituent of academic library collections.

AIM:To report the incidence of non-small-bowel bleeding pathologies encountered during double-balloon enteroscopy (DBE) procedures and to analyse their ***: A retrospective study of a prospective DBE database conducted in a tertiary-referral center was conducted. A total of 179 patients with obscure gastrointestinal bleeding (OGIB) referred for DBE from June 2004 to November 2008 were analysed looking for the incidence of non-small-bowel lesions (NSBLs; all and newly diagnosed) encountered during ***: There were 228 (150 antegrade and 78 retrograde) DBE procedures performed in 179 patients. The mean number of DBE procedures was 1.27 per patient. The mean age (SD) of the patients was 62 ± 16 years old. There were 94 females (52.5%). The positive yield for a bleeding lesion was 65.9%. Of the 179 patients, 44 (24.6%) had NSBLs (19 of them had dual pathology with small-bowel lesions and NSBLs); 27 (15.1%) had lesions not detected by previous endoscopies. The most common type of missed lesions were vascular ***: A significant proportion of patients (24.6%) had lesions within reach of conventional endoscopy. careful repeat examination with gastroscopy and colonoscopy might be required.

When United Nations Secretary-General Antonio Guterres diagnosed the world with a bad case of‘trust deficit disorder’in 2018,UN peacekeeping seemed to have been somewhat sheltered from an increasingly chaotic world *** September 2018,more than 150 member states signed a Declaration of Shared commitments on UN Peacekeeping Operations(A4P).But a year later,however,on the eve of the organization’s 75th anniversary,what remains the most visible conflict management tool at the disposal of the Security council is seeing fault lines *** traditional consensus over peacekeeping is ***-sharing between those countries who,respectively,mandate,finance,and provide peacekeepers has become ever more complex in a context of an increasingly divided council and new financial *** visions have grown further apart as some member states push for more militarized stabilization and protection of civilians approaches when others call for a return to the core principles of *** author concludes that despite these fault lines,peacekeeping remains a remarkable enterprise of multilateralism through which trust and solidarity between member states can be *** at the condition that greater emphasis be put when carrying out necessary reforms on a more just and equitable burden-sharing-among the P5,between the P5 and ten elected members of the council,and between the council and Troop contributing countries-and on rebuilding consensus and coherence both in New York and in field missions.

Nanoscale plasmonic systems combine the advantages of optical frequencies with those of small spatial scales, circumventing the limitations of conventional photonic systems by exploiting the strong field confinement of surface plasmons. As a result of this miniaturization to the nanoscale, electron microscopy techniques are the natural investigative methods of choice. Recent years have seen the development of a number of electron microscopy techniques that combine the use of electrons and photons to enable unprecedented views of surface plasmons in terms of combined spatial, energy, and time resolution. This review aims to provide a comparative survey of these different approaches from an experimental viewpoint by outlining their respective experimental domains of suitability and highlighting their complementary strengths and limitations as applied to plasmonics in particular.

Background: costal fracture surgical is still a debate, therefore we shall select between early and delay surgical management. case Report: We are reporting two cases of post road traffic clash delay ribs fractures osteosynthesis involving a 63-year-old man with multistage fractures on the left and pulmonary pinning of one of the costal arches, complicated by a homolateral haemothorax and a 41-year-old man with a bilateral flail chest. conclusion: The simple postoperative course and the immediate postoperative improvement in the patient’s clinical respiratory condition enabled us to discuss the time frame for management, in this case the indication for early or later surgery.

Erectile dysfunction （ED） commonly results from endothelial dysfunction of the systemic vasculature. Although phosphodiesterase type 5 （PDE-5） inhibitors are effective at treating most cases of ED, they must be taken routinely and are ineffectual for a meaningful number of men. In recent years gene and stem cell-based therapies targeted at the penile endothelium have been gaining momentum in preclinical studies. These early studies reveal that gene and stem cell-based therapies may be both enduring and efficacious, and may eventually lead to a cure for ED. The following review will highlight our current understanding of endothelial-specific gene and stem cell-based therapies performed to date in a number of experimental animal models.

Background and Objective: HIV, hepatitis B virus (HBV) and hepatitis c virus (HcV) are very widespread in the world, however, less than 20% of the people affected are diagnosed and treated. This study aimed to determine the prevalence of HIV, HcV and HBV co-infections in pregnant women at Bangui community University Hospital and the cost of screening. Methods: A cross-sectional study involving consenting pregnant women who came for antenatal care was performed. HIV, HcV antibodies and HBV antigens were detected using Exacto Triplex? HIV/HcV/HBsAg rapid test, cross-validated by ELISA tests. Sociodemographic and professional data, the modes of transmission and prevention of HIV and both hepatitis viruses were collected in a standard sheet and analyzed using the Epi-Info software version 7. Results: Pregnant women aged 15 to 24 were the most affected (45.3%);high school girls (46.0%), and pregnant women living in cohabitation (65.3%) were the most represented. Twenty-five (16.7%) worked in the formal sector, 12.7% were unemployed housewives and the remainder in the informal sector. The prevalence of HIV, HBV, and HcV viruses was 11.8%, 21.9% and 22.2%, respectively. The prevalence of co-infections was 8.6% for HIV-HBV, 10.2% for HIV-HcV, 14.7% for HBV-HcV and 6.5% for HIV-HBV-HcV. All positive results and 10% of negative results by the rapid test were confirmed by ELISA tests. The serology of the three viruses costs 39,000 FcFA (60 Euros) by ELISA compared to 10,000 FcFA (15.00 Euros) with Exacto Triplex? HIV/HcV/AgHBs (BioSynex, Strasbourg, France). conclusion: The low level of education and awareness of hepatitis are barriers to development and indicate the importance of improving the literacy rate of women in the central African Republic (cAR). Likewise, the high prevalence of the three viruses shows the need for the urgent establishment of a national program to combat viral hepatitis in the cAR.
