Endoscopic management of biliary disorders during pregnancy

BACKGROUND:Biliary interventions during pregnancy are associated with risks to both the pregnancy and developing *** this report we summarize our experience with endoscopic interventions including endoscopic ultrasound (EUS)in the management of biliary disorders during pregnancy. METHODS:Endoscopic retrograde cholangiopancreatographies (ERCPs)performed between May 2003 through January 2010(n=607)were identified from our database,and cases of interventions during pregnancy were *** procedures were done using conscious sedation and lead shielding. RESULTS:Nine ERCPs(1.5%)were performed in 8 pregnant *** median gestational period was 22 weeks (range,<2-36 weeks).Two,5 and 2 patients were in their first,second and third trimester,*** for ERCP included obstructive jaundice(6 patients)cholangitis (2),and acute pancreatitis/obstructive jaundice(1).Two patients underwent EUS before *** was used in 5 ERCPs(median 12 seconds;range 2-20 seconds),and the overall time for a ERCP ranged from 5 to 25 minutes. During ERCP endoscopic sphincterotomy was performed in 5 patients,stenting in 6,and balloon clearance in *** procedure caused complication in induction of *** pregnancy,there were 4 non-procedure related complications including acute cholecystitis(1),HELLP syndrome resulting in spontaneous abortion(1)and stent migrations(2).Five pregnancies had uncomplicated term deliveries,whereas 2 required urgent caesarian sections(one for fetal distress and 1 for cholangitis secondary to stent migration).One patient was well in her second trimester during *** babies were well at birth with median APGAR scores of 9,and 10 at 5 and 10 minutes,*** baby died of sudden death syndrome at age of 40 days. The Solar Flare Index is regarded as one of the most important solar indices in the field of solarterrestrial research. It has the maximum effect on Earth of all other solar activity indices and is being considered for describing the short-lived dynamo action inside the Sun. This paper attempts to study the short as well as long-term temporal fluctuations in the chromosphere region of the Sun using the Solar Flare Index. The daily Solar Flare Index for Northern, Southern Hemisphere and Total Disk are considered for a period from January 1976 to December 2014(total 14 245 days) for chaotic as well as periodic *** 0–1 test has been employed to investigate the chaotic behavior associated with the Solar Flare *** test revealed that the time series data is non-linear and multi-periodic in nature with deterministic chaotic features. For periodic analysis, the Raleigh Power Spectrum algorithm has been used for identifying the predominant periods within the data along with their confidence score. The well-known fundamental period of 27 days and 11 years along with their harmonics are well affirmed in our investigation with a period of 28 days and 10.77 years. The presence of 14 days and 7 days periods in this investigation states the short-lived action inside the Sun. Our investigation also demonstrates the presence of other mid-range periods including the famous Rieger type period which are very much confirming the results obtained by other authors using various solar activity indicators.

Enhancement of Urinary Elimination of 3-Bromobenzanthrone Metabolites by Oral Supplementation of Ascorbic Acid in Guinea Pigs

Objective 3-Bromobenzanthrone (3-BBA), an anthraquinone intermediate dye, is extensively used in textile industry. Since, our prior studies have shown that 3-BBA caused significant depletion of ascorbic acid (AsA) levels, the effect of exogenous supplementation of AsA on the urinary elimination of 3-BBA metabolites was investigated. Method Guinea pigs were treated with single oral dose of 3-BBA (50 mg/kg b. wt.) in groundnut oil while another group was treated with single oral dose of 3-BBA (50 mg/kg b. wt.) along with 3 day prior and post oral supplementation of AsA. Control groups were either treated with groundnut oil or AsA alone. Urine from individual animals was collected, extracted and analysed on HPTLC. Results The highest elimination of 3-BBA (75 mg) was found to be in 0-24 h urine fraction which decreased to 18 mg and 5 mg in the two subsequent 24 hourly fractions of urine. Exogenous supplementation of AsA increased the total urinary elimination of 3-BBA by almost 77%. A total of 10 fluorescent metabolites excluding the parent compound were eliminated in the urine of guinea pigs treated with 3-BBA. Densitometric scanning of chromatogram showed different peaks at Rf 0.18, 0.22, 0.27, 0.34, 0.40, 0.48, 0.56, 0.66, 0.72, 0.80, and 0.95 which were eliminated and marked as urinary metabolite 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, and 11 respectively. AsA not only significantly enhanced the elimination of 3-BBA metabolites but also modified the pattern of metabolites drastically in 0-6 h, 6-24 h and 24-48 h urine fractions. Conclusion These results indicate that AsA may be useful in protecting the toxicity of 3-BBA by fascilitating the urinary metabolite(s) excretion of 3-BBA.

Objective: Non-hodgkin’s lymphoma (NHL) refers to a group of malignant neoplasms that arise from cellular elements of lymphoid or extranodal tissue. Lymphomas are a frequent cause of malignant lymphadenopathy in the head &neck. The head &neck is the second must frequent anatomical site of extranodal lymphomas after gastrointestinal tract. That’s why dental & maxillofacial surgeon should be aware of malignant lymphoma to diagnose properly & efficiently. This study was performed to evaluate oral & maxillofacial region manifestations, staging, grading and prognosis of NHL.Materials & Method: A retrospective analysis or review of 25patients with oral &maxillofacial region lymphoma was made in our hospital (Dalian Medical University, Stomatology Department of First Affiliated hospital) from 2002jan to 2006 august .Specifically, clinical presentation, staging, grading & therapy for NHL with oral &maxillofacial region involvement were sought. Staging was done according to the Ann Arbor staging system and diagnosis made by immunohistochemistry.Result: 25 patients of NHL were included in our study, among them 20(80%) patients with extra nodal (eNHL) &5(20%) patients with nodal (NHL) involvement. The incidence rate increases exponentially with age between 24 and 79 years. The median age at the diagnosis of the patients was *** above 50 years were 72%.Male (60%) is slightly more than female (40%). Among 20 patients, NHL of Waldeyer’s ring was the most frequent site occurring in 10 patients (50%) followed by NHL of oral cavity (20%), of salivary glands (30%). Constitutional B symptomatology was uncommon, occurring in 25% patients. The typical clinical appearance is a painless local mass lateral or bilateral (90%) and confused with squamous cell carcinoma. Most of the non-Hodgkin’s lymphoma was B-cell lineage (90%), & overall diffuse large B-cell lymphoma (76%) was the most common type. Mostly patients involving non-Hodgkin’s lymphoma in oral &

Effect of solar wind plasma parameters on space weather

Today’s challenge for space weather research is to quantitatively predict the dynamics of the magnetosphere from measured solar wind and interplanetary magnetic field（IMF） conditions. Correlative studies between geomagnetic storms（GMSs）and the various interplanetary（IP） field/plasma parameters have been performed to search for the causes of geomagnetic activity and develop models for predicting the occurrence of GMSs, which are important for space weather predictions. We find a possible relation between GMSs and solar wind and IMF parameters in three different situations and also derived the linear relation for all parameters in three *** the basis of the present statistical study, we develop an empirical model. With the help of this model, we can predict all categories of GMSs. This model is based on the following fact： the total IMF Btotalcan be used to trigger an alarm for GMSs, when sudden changes in total magnetic field Btotaloccur. This is the first alarm condition for a storm’s arrival. It is observed in the present study that the southward Bzcomponent of the IMF is an important factor for describing GMSs. A result of the paper is that the magnitude of Bzis maximum neither during the initial phase（at the instant of the IP shock） nor during the main phase（at the instant of Disturbance storm time（Dst） minimum）. It is seen in this study that there is a time delay between the maximum value of southward Bzand the Dst minimum, and this time delay can be used in the prediction of the intensity of a magnetic storm two-three hours before the main phase of a GMS. A linear relation has been derived between the maximum value of the southward component of Bzand the Dst, which is Dst =（-0.06）＋（7.65）Bz＋ *** auxiliary conditions should be fulfilled with this, for example the speed of the solar wind should, on average, be 350 km s-1 to 750 km s-1, plasma β should be low and, most importantly, plasma temperature should be low for intense storms. Sliding mode controller design for frequency regulation in an interconnected power system

In this paper, a Sliding mode controller design method for frequency regulation in an interconnected power system is presented. A sliding surface having four parameters has been selected for the load frequency control (LFC) system model. In order to achieve an optimal result, the parameter of the controller is obtained by grey wolf optimization (GWO) and particle swarm optimization (PSO) techniques. The objective function for optimization has been considered as the integral of square of error of deviation in frequency and tie-line power exchange. The method has been validated through simulation of a single area as well as a multi-area power system. The performance of the Sliding mode controller has also been analyzed for parametric variation and random loading patterns. The performance of the proposed method is better than recently reported methods. The performance of the proposed Sliding mode controller via GWO has 88.91% improvement in peak value of frequency deviation over the method of Anwar and Pan in case study 1 and similar improvement has been observed over different case studies taken from the literature.
