Upper GIS Endoscopy Indications of Patients Consulted at Internal Medicine Outpatient Clinics and Data Obtained According to These Indications

Background and aim: Esophagogastroduodenoscopy is an innovative method used in order to diagnose esophagus, stomach, and duodenum diseases. Esophagogastroduodenoscopy is fundamental for the prognosis of various benign and malign upper gastrointestinal diseases, as well as for therapy or disease follow-up. The aim of the present study is to classify endoscopy results according to indications and to reveal which indications and results are most commonly seen. Materials and methods: The Esophagogastroduodenoscopy results of 6243 patients were evaluated retrospectively;all patients had applied to the Gaziosmanpa?a Taksim Education and Research Hospital Department of Internal Medicine from 2010 to 2015 on either an outpatient or inpatient basis. Results: In our study, 2548 of the patients were male and 3695 were female. The mean age of the patients was 49.37 ± 16.90 years. The indications for Esophagogastroduodenoscopy were dyspeptic symptoms for 72.8% of females and 70.4% of males. Anemia was the indication for 12.1% of females and 11.8% of males. Other indications included gastrointestinal bleeding, dysphagia, nausea and vomiting, gastroesophageal reflux disease symptoms and weight loss. Peptic ulcer disease was the result of Esophagogastroduodenoscopy for 55.7% of females and 50.6% of males. Gastric ulcer (15.2% of females and 16% of males), reflux esophagitis (8.1% of females and 10.1% of males), and duodenal ulcer (6.8% of females and 10.1% of males) were the other results. Malignancy was mostly observed in patients whose indications were anemia. Conclusion: The upper gastrointestinal system endoscopy continues to be an up-to-date method of displaying the effectiveness of diseases to assist in the diagnosis and treatment of the symptoms and complaints of the gastrointestinal system, particularly in the evaluation of patients having persistent symptoms.

A Study on Biosorption Potential of <i>Aspergillus</i>sp. of Tannery Effluent

Chromium toxicity is one of the major environmental pollutants. As conventional methods for heavy metal removal are very expensive, bioremediation using fungi is extensively explored. Therefore in the present investigation, out of 23 fungi isolated from tannery effluent, 4 Aspergillus species namely A. terreus, A. tamarii, A. flavus and A. niger were selected for evaluating chromium tolerance and biosorption potential. Growth of the fungi was tested using various concentrations of Potassium dichromate (Cr VI) viz., 1 mM, 3 mM and 5 mM amended in potato dextrose agar. For all the fungi, growth was observed only in 1 mM and 3 mM. No growth was observed in 5 mM. Biosorption efficiency of both live and alkali pretreated Aspergillus species was compared. In the present study, alkali pretreated form exhibited highest biosorption efficiency than the live biomass in which A. terreus showed highest chromium biosorption potential compared to the other fungi.

Development of diabetic cataract is mainly associated with the accumulation of sorbitol via the polyol pathway through the action of Aldose reductase(AR).Hence,AR inhibitors are considered as potential agents in the management of diabetic *** study explored the AR inhibition potential of Hemidesmus indicus *** root extract by in silico and ex vivo *** docking studies(Auto Dock tool)betweenβ-sitosterol,hemidesminine,hemidesmin-1,hemidesmin-2,and AR showed thatβ-sitosterol(−10.2 kcal/mol)and hemidesmin-2(−8.07 kcal/mol)had the strongest affinity to AR *** vivo studies were performed by incubating isolated goat lenses in artificial aqueous humor using galactose(55 mM)as cataract inducing agent at room temperature(pH 7.8)for 72 *** treatment with Vitamin E acetate−100μg/mL(standard)and test extract(500 and 1000μg/mL)separately,the estimation of biochemical markers showed inhibition of lens AR activity and decreased sorbitol ***,extract also normalized the levels of antioxidant markers like SOD,CAT,*** results showed evidence that *** *** root was able to prevent cataract by prevention of opacification and formation of polyols that underlines its potential as a possible therapeutic agent against diabetic complications.

Assessment on self-care, mobility and social function of children with spina bifida in Turkey

The aim of the study was to investigate the functional performance in children with spina bifida, using the Pediatric Evaluation of Disability Inventory （PEDI） to look into capacity of twen- ty-eight children with spina bifida with lesions at different levels in different dimensions of self- care, mobility and social function. Mean age of the patients was 3.5 ± 2.3 （1-10） years. In the muscle test carried out, 13 patients （44.8%） had no movements including pelvic elevation in lower extremity muscles and they were at level 5. Sixteen patients （54%） were non-ambulatory according to the Hoofer ambulation classification. Raw and scale scores in the self-care, mobil- ity and social function domains both in the functional skill scale and in the caregiver scale were found to be lower compared to the data of the normal population. A statistically significant correlation was observed in the self-care values of the Functional Skills Scales and the Caregiver Assistance Scale measurements, which was positive for age and negative for Functional Ambu- lation Scale and muscle test （P 〈 0.05）. A positive relation was found between the Functional Skills Scales-mobility area and age while a negative relation was observed between Functional Ambulation Scale and muscle test （P 〈 0.005）. A negative relation was also found between Care- giver Assistance Scale-mobility and Functional Ambulation Scale and muscle test （P 〈 0.005）. In our study, the functional performance of the children was found to be low. Low-level lesions, encouraging muscular strength and independence in mobility are all very important factors for functional independence.

背景:粘液表皮样癌是发生在大小涎腺的最常见的恶性肿瘤，我们研究的目的是要确定显着影响生存率的粘液表皮样癌的临床特点。设计:回顾性研究方法:对2003年至2011年在华中科技大学同济医学院口腔医学中心颌面外科治疗的40例患者进行回顾性临床分析。其临床特征、病理分级和生存率之间的关系所用到的统计方法是Kaplan-Meier生存分析和log-rank检验。结果:5年存活率和无瘤生存率分别是80.8%和60.7%。在单变量统计学分析时，影响生存率的显著性因素是是患者的年龄、T地位、N现状、临床分期和病理分级;影响无瘤生存率的显着因素是患者的年龄和组织学分级。结论:在我们的研究中，我们发现患者的年龄和组织学分级对生存率有重要影响。手术的目标是完整的切除病区，高级别的粘液表皮样癌应该辅助放疗，同时可以降低中分化的和低分化的粘液表皮样癌的复发。

Malnutrition associated factors on children under 5 years old in Lhaviyani Atoll,Maldives

In this report,we aimed to analyze the prevalence of undernutrition and associated factors among children under 5 years of age in Lhaviyani Atoll,Maldives.A total of 800 children(under 5 years old) and their mothers were selected for this *** was collected by using a pretested questionnaire and anthropometric measurements were taken from the hospital record ***-square tests and multivariate logistic regression were used to find the association between nutritional status and *** distribution of height for age and weight for age in surveyed children in Maldives was skewed to the left compared with the WHO *** prevalence of undernutrition based on underweight(10.75%),stunting(13.5%),and wasting(9.60%) was estimated to be 23.85% among *** age,gender and mother’s education were significantly associated with undernutrition(P<0.05).Our survey highlighted that better nutritional interventions are needed to improve child health in this region.

A State Space Modeling of Non-Isolated Bidirectional DC-DC Converter with Active Switch

In this paper, analysis, design and implementation of non-isolated soft-switching bidirectional DC-DC converter with an active switch are described. The proposed topology gives the output voltage as twice as the input voltage and enhances the efficiency up to 94.5% and 92.9% for boost and buck mode operation by proper selection of the duty cycle. Soft switching can be achieved at both steps up and step down operating modes. Small signal analysis based on state space averaging and transfer functions have been presented in detail for the proposed converter. Finally, the feasibility of the desired converter is confirmed to mat lab simulation and investigational results.

Reno-protective effect of garlic extract against immobilization stress induced changes in rats

Objective: To examine immobilization stress-induced antioxidant defense alterations in rat kidney and the antioxidant effects of aqueous garlic extract in pre and post stress extract treatments. Methods: Albino rats were treated with aqueous extract of garlic both before and 6 h of immobilization stress. Pro-oxidant eminence of rat kidney was assessed by determining the levels of glutathione, thiobarbituric acid reactive substances, aspartate aminotransferase, alanine aminotransferase, glucose, uric acid, alkaline phosphatase and antioxidant enzymes activities. Results: In response to 6 h of immobilization stress, a significant rise in the level of kidney enzymes was recorded. However, antioxidant enzyme activities showed a sharp decline. Conclusions: The extract treatment before and after the stress reverted the activities of above mentioned enzymes towards their control values. Hence, garlic extract can be given as nutritional supplement for scavenging the free radicals generated in rat kidney.

Comparison of Numerical Approximations of One-Dimensional Space Fractional Diffusion Equation Using Different Types of Collocation Points in Spectral Method Based on Lagrange’s Basis Polynomials

Recently many research works have been conducted and published regarding fractional order differential equations. There are several approaches available for numerical approximations of the solution of fractional order diffusion equations. Spectral collocation method based on Lagrange’s basis polynomials to approximate numerical solutions of one-dimensional (1D) space fractional diffusion equations are introduced in this research paper. The proposed form of approximate solution satisfies non-zero Dirichlet’s boundary conditions on both boundaries. Collocation scheme produce a system of first order Ordinary Differential Equations (ODE) from the fractional diffusion equation. We applied this method with four different sets of collocation points to compare their performance.
