Background:Acute calculous cholecystitis(ACC)is frequently seen in cirrhotics,with some being poor candidates for initial ***,these patients may undergo percutaneous cholecystostomy tube(PCT)*** studied the healthcare utilization and predictors of cholecystectomy and PCT in patients with ***:The National Database was queried to study all cirrhotics and non-cirrhotics with ACC between 2010-2014 who underwent initial PCT(with or without follow-up cholecystectomy)or *** patients were divided into compensated and decompensated *** predictors and outcomes of initial PCT and failure to undergo subsequent cholecystectomy were ***:Out of 919189 patients with ACC,13283(1.4%)had *** cirrhotics,cholecystec-tomy was performed in 12790(96.3%)and PCT in the remaining 493(3.7%).PCT was more frequent in cirrhotics(3.7%)than in non-cirrhotics(1.4%).Multivariate analyses showed increased early readmis-sions[odds ratio(OR)=2.12,95%confidence interval(CI):1.43-3.13,P<0.001],length of stay(effect ratio=1.39,95%CI:1.20-1.61,P<0.001),calendar-year hospital cost(effect ratio=1.34,95%CI:1.28-1.39,P<0.001)and calendar-year mortality(hazard ratio=1.89,95%CI:1.07-3.29,P=0.030)in cir-rhotics undergoing initial PCT compared to *** cirrhosis(OR=2.25,95%CI:1.67-3.03,P<0.001)had the highest odds of getting initial ***,regardless of compensated(OR=0.56,95%CI:0.34-0.90,P=0.020)or decompensated(OR=0.28,95%CI:0.14-0.59,P<0.001),reduced the chances of getting a subsequent ***:Cirrhotic patients undergo fewer cholecystectomy incurring initial PCT ***,the rates of follow-up cholecystectomy are lower in *** healthcare utilization is seen with initial PCT amongst cirrhotic *** situation reflects suboptimal management of ACC in cirrhotics and a call for action.

The elaborate redox network of the cell,comprising of events like turnover of reactive oxygen species(ROS),redox sensing,signaling,expression of redox-sensitive genes,etc.,often orchestrates with other bonafide hormonal signaling pathways through their synergistic or antagonistic action in the plant *** redox cue generated in plant cells under fluctuating environmental conditions can significantly influence other hormonal biosynthetic or signaling mechanisms,thereby modulating physiology towards stress acclimation and *** is also strong evidence of the recruitment of ROS as a‘second messenger’in different hormonal signaling pathways under ***,the retrograde signaling initiated by ROS also found to strongly influence hormonal homeostasis and *** present review,in this aspect,is an effort towards understanding the regulatory roles of ROS in integrating and orchestrating other hormonal signaling pathways or vice versa so as to unfold the relationship between these two signaling episodes of plant cells under environmental *** also accentuate the significance of understanding the utterly complex interactions,which occur both at metabolic and genetic levels between ROS and phytohormones during stress ***,the significant and decisive role of ROS turnover,particularly the contribution of RBOH(respiratory burst oxidase homologs)in the synergism of redox and hormone signaling during systemic acquired acclimation under stress is also discussed.

BACKGROUND The management of high-grade pancreatic trauma is *** To review our single-institution experience on the surgical management of blunt and penetrating pancreatic *** A retrospective review of records was performed on all patients undergoing surgical intervention for high-grade pancreatic injuries [American Association for the Surgery of Trauma(AAST) Grade Ⅲor greater] at the Royal North Shore Hospital in Sydney between January 2001 and December 2022. Morbidity and mortality outcomes were reviewed, and major diagnostic and operative challenges were *** Over a twenty-year period, 14 patients underwent pancreatic resection for highgrade injuries. Seven patients sustained AAST Grade Ⅲinjuries and 7 were classified as Grades Ⅳ or Ⅴ. Nine underwent distal pancreatectomy and 5 underwent pancreaticoduodenectomy(PD). Overall, there was a predominance of blunt aetiologies(11/14). Concomitant intra-abdominal injuries were observed in 11 patients and traumatic haemorrhage in 6 patients. Three patients developed clinically relevant pancreatic fistulas and there was one in-hospital mortality secondary to multi-organ failure. Among stable presentations, pancreatic ductal injuries were missed in two-thirds of cases(7/12) on initial computed tomography imaging and subsequently diagnosed on repeat imaging or endoscopic retrograde cholangiopancreatography. All patients who sustained complex pancreaticoduodenal trauma underwent PD without mortality. The management of pancreatic trauma is evolving. Our experience provides valuable and locally relevant insights into future management *** We advocate that high-grade pancreatic trauma should be managed in high-volume hepatopancreato-biliary specialty surgical units. Pancreatic resections including PD may be indicated and safely performed with appropriate specialist surgical, gastroenterology, and interventional radiology support in tertiary centres.

Background:Classic dichotic listening tests using speech stimuli result in right ear advantage,due to the dominant crossed pathway for speech and *** is presumed that similar crossed dominance could exist for non-speech stimuli ***,this is an attempt to develop and validate the dichotic nonspeech test using environmental stimuli and explore the effect of focused attention on this *** and method:Three lists of dichotic stimuli were created using these sounds with fifteen tokens in each *** professionals and non-professionals validated these *** estimation was obtained by administering the newly developed test on 70 adults and 70 children using a free-recall and forced-recall ***:The results showed a significant difference between the left ear and right scores where the left ear score was better than the right,depicting left ear advantage(LEA)for free recall condition in both *** the forced recall condition,LEA was not seen;rather the mean score was significantly higher in the attended ear,irrespective of the stimuli presented to the right or left *** test-retest reliability in free recall was good in both the ears and moderate for forced right ear ***:The novel test consistently showed LEA with good reliability and can be used to assess the hemispheric asymmetry in normal subjects and also in test batteries for the clinical population.

Endoscopic ultrasound guided liver biopsy(EUS-LB)has emerged as a minimally-invasive alternative to the traditional(percutaneous or transjugular)liver biopsy techniques for the diagnosis of liver parenchymal ***,EUS-LB combines the advantages of percutaneous and transjugular liver biopsy in addressing focused sampling in addition to measuring portal ***,EUS-LB facilitates access to both the lobes of the liver which is not considered with the traditional percutaneous liver *** studies have compared EUS-LB with conventional liver biopsy and reported comparable diagnostic yield,increased acquisition of complete portal tracts,and longer specimen length as compared to the traditional ***-LB is associated with lesser post-procedural pain and shorter recovery time,while providing lower risk of complications when compared to traditional liver *** in needle types,needle sizes and suction techniques have aimed at further optimizing the EUS-LB *** review article updates current literature with focus on the variations in the technique and equipment used for EUS-LB,and compares EUS-LB with traditional methods of liver biopsy.
