Introduction: Noma is a severe and mutilating gangrenous stomatitis of the orofacial sphere, starting on periodontal side. It is rare and occurs mainly in children that land is weakened by malnutrition. We report a case of mandibular sequela noma requiring plastic surgery. Clinical case: This was a 7-year-old patient with moderate acute malnutrition. she was referred from a peripheral care facility for gangrene of the left cheek, communicating the oral cavity with the outside, through an orostoma. This lesion had been evolving for two months, with a tendency towards progressive destruction of the soft tissues of the cheek and the mandibular bone. The neck and facial CT-scan demonstrated destruction of the mandibular bone exposed on the left side, without signs of tumor invasion. After correction of nutritional status and local infection, the patient was treated with a metal plate recovered by sub-mental flap. Conclusion: Noma is a severe gangrenous stomatitis that transforms patients’ faces into ghosts by destroying soft tissues and underlying bone planes. The management of his sequelae is surgical, sometimes using complex techniques of plastic surgery.

A New X-Ray Tidal Disruption Event Candidate with fast Variability

高等工程教育在培养工程科技人才,提高中国在工程科技领域的竞争力中发挥着重要的作用。在中国建设创新型国家和走新型工业化道路的背景下,培养大量具有创新能力、多层次和专业化的工程科技人才具有重大的战略意义。本文分析了专业人才的定义、特点和不足,总结出工程科技人才成长的多层次、专业化、社会性、长期性和流动性等培养规律,并以此为理论框架,结合美国麻省理工学院CDIO工程教育模式在知识、技能和实践方面的人才理念,提出了中国在工程科技人才的培养上需要关注的问题和相关建议。我们认为,工程科技人才的培养需要学校、政府和企业全面合作,通过丰富教育资源、优化教育环节、完善教育环境,切实提高高等工程教育的质量和水平,实现人才培养目标。

In this study, a laminated woven bamboo/woven E glass/unsaturated polyester composite is developed to combat a ballistic impact from bullet under shooting test. The aim of this study is to understand the fundamental effects of the woven bamboo arrangement towards increasing ballistic resistance properties. The work focusses on the ballistic limit test known as NIJ V50, which qualifies materials to be registered for use in combat armor panels. The results show that the composites withstood 482.5 m/s ± 5 limit of bullet velocity, satisfying the NIJ test at level II. The findings give a strong sound basis decision to engineers whether or not green composites are qualified to replace synthetic composites in certain engineering applications.

Phytoremediation is a promising approach for reclamation of salt-affected *** is the most commonly used process,which exploits plants to absorb,immobilize,and accumulate salt in their *** this study,halotolerant plant growth-promoting rhizobacteria(PGPR)were isolated from the rhizosphere of wild grasses growing naturally in salt-affected areas of Lucknow,Uttar Pradesh(India)and were tested for their efficacies of salt-tolerance and plant growth-promoting(PGP)*** on 16s rRNA sequences,the most efficient halotolerant isolates possessing PGP traits were identified as Pseudomonas plecoglossicida(KM 233646),Acinetobacter calcoaceticus(KM 233647),Bacillus flexus(KM 233648),and Bacillus safensis(KM 233652).Application of these isolates as bio-inoculants significantly(Psed the growth and bacoside A yield of a medicinal plant,Bacopa monnieri(L.)Nash,grown on natural salt-affected *** phytoremediation of salt-affected soil was evident by the substantial increase in shoot Na^+:K^+ratio of bio-inoculant-treated *** compared to un-inoculated control plants,the soil physico-chemical properties of bio-inoculant-treated plants were *** shoot and root biomass(fresh and dry weights),soil enzymes,and soil nutrient parameters showed significant positive correlations with the shoot Na+:K+***,the halotolerant PGPR screened in this study could be useful for the reclamation of saline soils concomitant with improved plant growth and bacoside A yield.

Rhizopus is fungus of the order Mucorales, responsible for invasive diseases called mucormycosis. The correct identification of these fungi is one of the main questions raised in the literature since the diagnosis is not simple, being the necessary detection in biopsies of infected tissues and isolation of the microorganism for identification by morphological and molecular methods. In vitro susceptibility testing results are not fully clear since there are reports showing that in vitro resistance does not always mean therapeutic failure. This paper describes a case of rhino-orbito-cerebral mucormycosis in a patient with alcoholic chronic pancreatitis and secondary mellitus diabetes involving microbiological diagnostic, antifungal susceptibility tests for conidia and hypha and clinical correlation. The causative agent was identified by morphological characteristics and DNA sequencing. Minimal Inhibitory Concentration (MIC) for amphotericin B (AMB), itraconazole (ITZ) and terbinafine (TERB) was determined by broth microdilution method. standardization of the dynamic growth evaluation patterns using the automated system BCT (BioCell-Tracer?) was obtained allowing to determining hyphae growth control and evaluating the effect of different concentrations of amphotericin B, itraconazole and terbinafine directly on growing hyphae. The fractional inhibitory concentration (fIC) for the combination of the two antifungals that the patient received at the same time, terbinafine and itraconazole, was also determined trying to correlate with clinical outcome. strains isolated of retro-orbital abscess were identified as Rhizopus oryzae. MIC results (MIC) testing conidia showed high sensibility to amphotericin B and itraconazole in comparison to the obtained for terbinafine. The tests in the automated system BCT using hyphae showed sensibility to all tested antifungal agents and the fIC results for combination in vitro between terbinafine and itraconazole showed synergism consisten

Diversity of Human Immunodeficiency Virus Type-1 subtypes in Western Kenya

Background: HIV/AIDs is the principal pandemic in the world today.?Two viral types (HIV-1 and HIV-2), with numerous groups (M, N and O for HIV-1 and A through H for HIV-2) have emerged. These have further proliferated into numerous subtypes, sub-subtypes and circulating recombinant forms (CRf) over the last 30 years.?HIV-1 variants circulate together within a geographical region providing an opportunity for recombination of viral strains within infected individuals.?In Kenya, at least nine different genetic HIV-1 subtypes and several recombinant forms have been defined within group M, which accounts for the majority of cases in the AIDs pandemic. Objective:?To determine the genetic diversity of HIV-1 in the western region of Kenya bordering Uganda. Methodology: A cross sectional study was carried out at Busia District Hospital between 2007 and 2009. A total of 75 patients were sampled randomly from a cohort of 1000 clients on antiretroviral therapy. Blood samples were analysed at the HIV Laboratory, Kenya Medical Research Institute, Nairobi, Kenya. PCR was carried out on the?Pol?region of HIV, sequenced and analysed by BLAsT for subtypes. Results: BLAsT analysis revealed the following circulating subtypes: 40/75 (53.30%) were HIV-1 group M subtype A1;21/75 (28.0%) were subtype D;5/75 (6.7%) were subtype G;4/75 (5.30%) were subtype C;and 2/75 (2.70%) were subtype A2. Only one isolate was identified for the other subtypes?viz: 1/75 (1.30%) resembled subtype B;1/75 (1.30%) was A1/C, and 1/75 (1.30%) was A1/D. Conclusion: The study showed increasing HIV-1 diversity along the Kenya-Uganda border with the emergence of A1/C and A1/D recombinants. such HIV-1?diversityvis a vis?the recent expanded access to antiretroviral therapy in resource limited settings calls for continuous evaluation of anti-HIV regimens. Photometric investigation of eight ultra-short period eclipsing binaries from OGLE

We performed a detailed photometric analysis of eight ultra-short period eclipsing binaries(UsPEBs) using the Wilson-Devinney method. We present the modeled light curves and derived photometric solutions. The UsPEBs with period(P)≤0.21 d considered in our study belong to W-subtype having shallow contact factor(f)ss ratio(q)>~0.7 and later spectral types. The absolute parameters for these short-period binaries were derived applying empirical relations. We discuss the evolutionary stage of these UsPEBs using the mass-radius, color-density and period-color diagrams. The objects showed poor metallicities, and some objects were even found to be existing around fully convective limits. The period distribution of UsPEBs exhibited a sharp cut-off at 0.22 d;however, we observed significant deficits for our objects in the literature. We examined the statistics of UsPEBs studied to date(in terms of the distribution of period, mass ratio and component temperatures of UsPEBs) and observed that a dominant distribution of component temperatures for these UsPEBs was towards lower temperatures.

Repression of oxidative stress/reactive metabolite regulated gene expression is associated with conversion of carbamazepine into a hepatotoxicant in LPs and Dss rat models

Idiosyncratic hepatotoxicity accounts for many drug failures in the clinic and is a leading cause for black-boxed and withdrawn drugs. This toxicity has proven difficult to predict preclinically, but correlates with oxidative stress/reactive metabolites (Os/RM). As noted previously for antiepileptic compounds, many drugs causing idiosyncratic adverse drug effects are detected by Os/RM gene expression responses in the rat. In the present study, two immune activation models, low dose lipopolysaccharide (1 mg/kg IV) and 5% dextran sulphate sodium (Dss) in drinking water, were examined to determine if either would convert the non-toxic idiosyncratic toxicant carbamazepine (225 mg/kg) into a rat hepatotoxicant at 24 hours. Using the low dose LPs model, about 1/3 of the carbamazepine-treated rats either showed robust ALT and AsT elevations with histopathological evidence of hepatotoxicity, or died. Rats in this LPs/carbamazepine group were subdivided based on ALT values into non-responders, responders or robust responders. Whereas most carbamazepine-induced mRNAs were repressed by LPs across all rats in this group, the Os/ RM genes aflatoxin aldehyde reductase (Afar) and glutathione transferase Ya (Gstya) were repressed only in the robust responder subgroup;it is unclear whether repression of these genes contributes to or results from hepatotoxicity. The Os/RM gene microsomal epoxide hydrolase (mEphx) showed repression across all rats. NAD(P)H: menadione oxidoreductase (Nmor) is an Os/RM-responsive gene that is also induced by LPs, confounding interpretation of its changes. After pretreatment with 5% Dss at 24 hours or for 5 days, using a protocol that reportedly produces increased endotoxin absorption, carbamazepine was not converted to a hepatototoxicant in any rats. Instead, Dss produced a pronounced (2- to 6-fold) and selective potentiation of carbamazepine induction of Os/RM-responsive mRNAs.
