Purpose: Although patient-related factors affect surgical outcomes, preoperative functional status is not measured by any cardiac risk score. Functional status can, however, be objectively measured using validated outcome tools such as the Late-Life Function and Disability Instrument (LLFDI). The purpose of this study was to determine 1) if there was a change over time in functional status, as measured by the LLFDI, in patients who underwent elective cardiac surgery, and if so, 2) what specific aspect(s) of functional status changed. Methods: A prospective longitudinal study of one year was conducted on elective cardiac surgery patients (n = 43) using the self-reported LLFDI, which measures Disability Frequency (frequency of participation in social tasks), Disability Limitation (ability to participate in social tasks) and Function Total (ease in performing routine activities). Higher scores indicate increased function and decreased disability. LLFDI scores were compared at three times (preoperative, six-week and one-year postoperative) using repeated measures ANOVA. Post hoc pairwise comparison was conducted for specific interactions. Results: Both Function Total and Disability Frequency significantly changed over time (p = 0.047 and p = 0.013, respectively). Specifically, patients’ function level was significantly higher one-year postoperative compared to preoperative (M difference = +3.48, SE = 1.48, p = 0.026). Likewise, Disability Frequency scores were significantly higher (i.e. more active) at one-year postoperative versus preoperative (M difference= +5.98, SE = 2.19, p = 0.033). Disability Limitation scores were not significantly different between any time points (p > 0.05). Conclusion: By one-year postoperative, patients demonstrated increased ease in their routine physical activities and were more participatory in social life tasks. The synapse is an incredibly specialized structure that allows for the coordinated communication of information from one neuron to another. When assembled into circuits, steady streams of excitatory and inhibitory synaptic activity shape neural outputs. At the organismal level, ensembles of neural networks underlie behavior, emotion and memory. Disorder or dysfunctions of synapses, a synaptopathy, may underlie a host of developmental and degenerative neurological conditions. There is a possibility that amyotrophic lateral sclerosis may be a result of a synaptopathy within the neuromotor system. To this end, particular attention has been trained on the excitatory glutamatergic synapses and their morphological proxy, the dendritic spine. The extensive detailing of these dysfunctions in vulnerable neuronal populations, including corticospinal neurons and motor neurons, has recently been the subject of original research in rodents and humans. If amyotrophic lateral sclerosis is indeed a synaptopathy, it is entirely consistent with other proposed pathogenic mechanisms – including glutamate excitotoxicity, accumulation of misfolded proteins and mitochondrial dysfunction at distal axon terminals(cortico-motor neuron and neuromuscular). Further, although the exact mechanism of disease spread from region to region is unknown, the synaptopathy hypothesis is consistent with emerging die-forward evidence and the prion-like propagation of misfolded protein aggregates to distant neuronal populations. Here in this mini-review, we focus on the timeline of synaptic observations in both cortical and spinal neurons from different rodent models, and provide a conceptual framework for assessing the synaptopathy hypothesis in amyotrophic lateral sclerosis.

Characterization of growth and survival of mice displaying early onset hypertonic symptoms is critical as these animals are important for research investigating mechanisms and treatments of pediatric conditions associated with hypertonia,such as cerebral ***,most animal models of cerebral palsy reproduce risk factors for developing this condition,with most failing to develop the physical symptoms or failing to survive in the postnatal *** ***-Glrb spa/J(Gly receptor mutation)transgenic mouse(spa mouse),displays symptoms of early onset hypertonia,though little has been reported on growth and survival,with no reports of growth and survival since genotyping became *** found that the majority of spa mice display symptoms by *** mice surviving to weaning,only^9%were spa *** weaning age,spa mice had significantly lower weights than their heterozygote and wild-type *** mice that died after weaning and prior to use in experiments or being culled,48%were spa *** poor growth and decreased survival of spa mice across multiple developmental and adult ages resembled the varied survival rates observed in humans with mild or severe cerebral *** understanding of the expected survival of these mice is helpful for planning breeding and animal numbers for *** to the symptoms and timing of symptom onset,spa mice will be valuable in uncovering mechanisms and long-term effects of early onset hypertonia in order to move toward interventions for these conditions.

This review,which was adapted from a Tenth International Workshop on Tropical Cyclones(IWTC-10)report,discusses research findings and operational practices relevant to cyclone types and phase transitions(extratropical,subtropical,and tropical).The cyclone phase space(CPS)method is widely used in both historical investigations and real-time evaluation of cyclone type and transition;however,CPS parameter values depend on input data resolution,and universal thresholds do not currently exist to delineate when a cyclone transitions from one type to *** of phase transitions in a changing climate highlight potential latitude shifts in extratropical transition and increased potential for tropical transition,but realistic projections of future trends likely require high-resolution simulations that can capture the cyclone warm *** meteorological centers apply varied approaches to cyclone classification via CPS parameters and other criteria,some of which depend on the tropical basin,yet these approaches cannot fully address challenges in operational classification and subsequently in communicating risks associated with these phase *** recommend a multivariate historical assessment of tropical and subtropical cyclones across all basins in which they occur,including the South Atlantic Ocean and the Mediterranean Sea,to identify the potential for a more universal cyclone classification approach that meets operational needs.

This article describes recent advances in the capability of new satellite sensors for observing Tropical Cyclones(TC)fine structure,wind field,and temporal *** article is based on a World Meteorological Organization(WMO)report prepared for the 10th International Workshop on Tropical Cyclones(IWTC),held in Bali in December 2022,and its objective is to present updates in TC research and operation every four *** we focus on updates regarding the most recent space-based TC observations,and we cover new methodologies and techniques using polar orbiting sensors,such as C-band synthetic aperture radars(SARs),L-band and combined C/X-band radiometers,scatterometers,and microwave imagers/*** additionally address progress made with the new generation of geostationary and small satellites,and discuss future sensors planned to be launched in the next *** then briefly describe some examples on how the newest sensors are used in operations and data assimilation for TC forecasting and research,and conclude the article with a discussion on the remaining challenges of TC space-based observations and possible ways to address them in the near future.

An estimated 4 million Americans currently have atrial fibrillation (AF), the most common form of cardiac arrhythmia. The annual growth rate of AF is approximately 9.2% and advanced statistical analyses project that the prevalence of AF will be 12.1 million in the United States by 2030. The current therapeutic options to treat AF put certain patient groups at a high risk for a serious bleeding event, stroke or thromboembolism, myocardial infarction, and death. The most effective treatment for AF—catheter ablation—has been widely reported to require repeat operations and only leaves 59.4% of patients AF-free after five years. A majority of the reported complications and increased risks for catheter ablation occur because the procedure is invasive. Thus, there is a need for a novel, non-invasive therapeutic option to treat AF that can potentially expand the treatable patient population, increase patient safety, and improve clinical outcomes. CyberHeart Inc. has been exploring the efficacy and potential of cardiac radiosurgery as an option to treat heart conditions such as AF. With its proprietary CardioPlanTM software, the CyberHeart System utilizes the Cyberknife stereotactic radiosurgery platform to non-invasively ablate the heart. Early studies have displayed that the CyberHeart System is highly accurate at delivering radiation to cardiac targets and effective at minimizing radiation exposure to healthy tissue that is in close proximity to the targeted anatomic region. The focus of this review is to present the current standard of treatment for AF, identify the specific patient population that is at a high risk for the current treatment modalities, and address the potential for the CyberHeart Stereotactic Radiosurgery System to noninvasively ablate cardiac tissue and return sinus rhythm to patients with AF.
