Visual field defects and retinal nerve fiber layer damage in buried optic disc drusen: a new insight

AIm: To assess the association between buried optic disc drusen(BODD) location using spectral-domain optical coherence tomography(sD-OCT) and the location of associated visual field defects(VFD) using the Garway-Heath ***: This monocentric retrospective crosssectional study was led at the authors’ institution. Adult patients diagnosed with BODD who had complete records with a reliable Humphrey? 24-2 visual field, macular, and papillary OCT were enrolled. Fisher’s exact test was used to measure the association between BODD location and VFD distribution according to Garway-Heath’s ***: Totally 20 eyes of 15 patients were included(60% females). The median age(interquartile range) was 63(43)y and the median best corrected visual acuity(BCVA) was-0.08(0.08) log mAR. BODD were mostly located in zones A, E, and F. The minimal rim width(mRW) was globally preserved. The retinal nerve fiber layer(RNFL) was predominantly altered in zones D, E, and F. There was a significant correlation between BODD location and that of RNFL alterations in zones D(P=0.03) and E(P=0.025);moreover, the presence of BODD in the E zone was significantly related to damaged RNFL in the neighbouring sectors D and F(P=0.012;P=0.02 respectively). sixty-three percent(12/19) of visual fields were abnormal and there was a significant match(Phi=0.7, P=0.009) between drusen location and VFD only in zone ***: BODD do not only affect young patients and can be more harmful than usually expected, as we found VFD in 63% of cases. There is a correspondence between BODD location, RNFL damage, and VFD distributions. The presence of BODD induces the overestimation of mRW,thereby disrupting its sensitivity as an early indicator of ganglion fibers damage.

Let u be a sequence of positive integers which grows essentially as a geometric progression. We give a criterion on u in terms of its distribution modulo d, d = 1, 2,..., under which the set of positive integers expressible by the sum of a prime number and an element of u has a positive lower density. This criterion is then checked for some second order linear recurrence sequences. It follows, for instance, that the set of positive integers of the form p ＋ [（2 ＋ √3）n], where p is a prime number and n is a positive integer, has a positive lower density. This generalizes a recent result of Enoch Lee. In passing, we show that the periods of linear recurrence sequences of order m modulo a prime number p cannot be ＂too small＂ for most prime numbers p.

The increasing trend for integrating renewable energy sources into the grid to achieve a cleaner energy system is one of the main reasons for the development of sustainable microgrid(mG)*** typical power-electronized power systems,mGs make extensive use of power electronics converters,which are highly controllable and flexible but lead to a profound impact on the dynamic performance of the whole *** with traditional large-capacity power systems,mGs are less resistant to perturbations,and various dynamic variables are coupled with each other on multiple timescales,resulting in a more complex system instability *** meet the technical and economic challenges,such as active and reactive power-sharing,voltage,and frequency deviations,and imbalances between power supply and demand,the concept of hierarchical control has been introduced into mGs,allowing systems to control and manage the high capacity of renewable energy sources and ***,as the capacity and scale of the mG system increase,along with a multi-timescale control loop design,the multi-timescale interactions in the system may become more significant,posing a serious threat to its safe and stable *** investigate the multi-timescale behaviors and instability mechanisms under dynamic inter-actions for AC mGs,existing coordinated control strategies are discussed,and the dynamic stability of the system is defined and classified in this ***,the modeling and assessment methods for the stability analysis of multi-timescale systems are also ***,an outlook and discussion of future research directions for AC mGs are also presented.

Characteristics of Infertile Clientele Attending a Public in Vitro Fertilization Clinic: Appraising Priorities in a Low-Income Country

Introduction: Infertilityaffects one in six couples, and it is an important public health issue largely due to thepervasive effects on the emotional and psychological wellbeing of affected couples. In many developing nations emphasis is placed on childbirth and inability to fulfill this role can be very distressing. There is an unmet need for assisted reproductive technology (art) in many developing countries and where facilities exist, they are mostly privately owned, expensive and concentrated in urban areas. To bridge this gap, public fertility clinics have been established to provide subsidized care. Evaluating the characteristics and peculiarities of clientele presenting at these public facilities will aid planning and prioritization of care. methodology: A descriptive retrospective study of 116 infertile patients presenting to the fertility clinic of the University College Hospital, Ibadan, Nigeria from inception on the 14sup>thsup> of February 2019 and 31sup>stsup> of December *** was analyzed using the statical Package for social sciences (IBm, sPss, New York) version 23. Descriptive statistics were used to summarize the results which were presented with the aid of bar charts and frequency tables. Result: The mean age of the patients was 40.70 ± 6.62 years. Post-menopausal patients accounted for about one-fifth of the study population while 80.2% (93 women) were older than 35 years. The mean duration of infertility was 9.39 ± 6.11years and nine patients (7.8%) had a duration greater than 2 decades. secondary infertility occurred in 67.2% of the women. Twenty-nine women (25%) had undergone myomectomy prior to presentation. Hypertension (11.2%) was the most prevalent comorbidity. Nineteen patients (16.4%) had used contraceptives in the past with the male condom (36.8%)being the most preponderant. sixty-seven patients had experienced pregnancy losses before 28 weeks of gestation while just 16 patients (13.8%) had undergone art, and none was successf

Utilizing supported single atoms as catalysts presents an opportunity to reduce the usage of critical raw materials such as platinum,which are essential for electrochemical reactions such as hydrogen oxidation reaction(HOR).Herein,we describe the synthesis of a Pt single electrocatalyst inside single-walled carbon nanotubes(sWCNTs)via a redox *** via electron microscopy,X-ray photoelectron microscopy,and X-ray absorption spectroscopy show the single-atom nature of the *** electrochemical behavior of the sample to hydrogen and oxygen was investigated using the advanced floating electrode technique,which minimizes mass transport limitations and gives a thorough insight into the activity of the *** single-atom samples showed higher HOR activity than state-of-the-art 30%Pt/C while almost no oxygen reduction reaction activity in the proton exchange membrane fuel cell operating *** selective activity toward HOR arose as the main fingerprint of the catalyst confinement in the sWCNTs.

Prediction of vertical displacement for a buried pipeline subjected to normal fault using a hybrid FEm-ANN approach

Fault movement during earthquakes is a geotechnical phenomenon threatening buried pipelines and with the potential to cause severe damage to critical ***,effective prediction of pipe displacement is crucial for preventive management *** study aims to develop a fast,hybrid model for predicting vertical displacement of pipe networks when they experience *** this study,the complex behavior of soil and a buried pipeline system subjected to a normal fault is analyzed by using an artificial neural network(ANN)to generate predictions the behavior of the soil when different parameters of it are *** this purpose,a finite element model is developed for a pipeline subjected to normal fault *** data bank used for training the ANN includes all the critical soil parameters(cohesion,internal friction angle,Young’s modulus,and faulting).Furthermore,a mathematical formula is presented,based on biases and weights of the ANN *** results show that the maximum error of the presented formula is 2.03%,which makes the proposed technique efficiently predict the vertical displacement of buried pipelines and hence,helps to optimize the upcoming pipeline projects.

EventLFm:event camera integrated Fourier light field microscopy for ultrafast 3D imaging

Ultrafast 3D imaging is indispensable for visualizing complex and dynamic biological *** scanning-based techniques necessitate an inherent trade-off between acquisition speed and space-bandwidth product(sBP).Emerging single-shot 3D wide-field techniques offer a promising alternative but are bottlenecked by the synchronous readout constraints of conventional CmOs systems,thus restricting data throughput to maintain high sBP at limited frame *** address this,we introduce EventLFm,a straightforward and cost-effective system that overcomes these challenges by integrating an event camera with Fourier light field microscopy(LFm),a state-of-theart single-shot 3D wide-field imaging *** event camera operates on a novel asynchronous readout architecture,thereby bypassing the frame rate limitations inherent to conventional CmOs *** further develop a simple and robust event-driven LFm reconstruction algorithm that can reliably reconstruct 3D dynamics from the unique spatiotemporal measurements captured by *** results demonstrate that EventLFm can robustly reconstruct fast-moving and rapidly blinking 3D fluorescent samples at kHz frame ***,we highlight EventLFm’s capability for imaging of blinking neuronal signals in scattering mouse brain tissues and 3D tracking of GFP-labeled neurons in freely moving *** believe that the combined ultrafast speed and large 3D sBP offered by EventLFm may open up new possibilities across many biomedical applications.

研究目的:疼痛为运动损伤中骨骼肌肉系统损伤的常见症状,疼痛的性质与程度可影响局部组织结构与功能、远端组织结构与功能及其他的系统性症状。当出现局部疼痛时,患者的整体姿势、稳定性、呼吸模式及运动表现都会受到影响,会出现局部不稳定和其他部位代偿的情况,从而进一步增加运动损伤风险,延迟康复进度。因此对疼痛症状的关注和管理在运动损伤的骨骼肌肉系统疾病中尤为重要。疼痛阈(Pain Threshold, PT)为疼痛管理中一重要概念,当前国内外许多研究皆通过局部疼痛强度和疼痛阈的变化作为某治疗方法对疼痛管理的评判标准。当前研究指出,指压手法可通过调节下行疼痛通路起到缓解症状的效果,另外,拮抗肌等长收缩可以通过交互抑制原理对肌肉产生放松作用,或可抑制疼痛。本研究旨在探讨两者结合的综合治疗方法对疼痛管理的效果。探讨指压结合拮抗肌等长收缩对腓肠肌疼痛阈的影响效果,探究骨骼肌肉疼痛调节的更有效干预手段。研究方法:18名健康大学生随机分为对照组和实验组。对照组方案为进使用指压手法按压腓肠肌外侧头肌腹进行治疗,实验组方案为使用指压手法按压腓肠肌外侧头肌腹同时进行50%最大背屈肌力的足背屈。实验在Biodex仪器上进行,起始姿势为受试者右下肢固定于髋关节屈曲90°,膝关节屈曲120°,踝关节屈曲90°位。受试者利用Biodex system 3进行最大等长跖屈力量(maximal Plantarflexion Force, mPF)测试,随后利用压力测试仪进行疼痛阈(Pain Threshold,PT)测试,疼痛阈定义为受试者主观疼痛在数字疼痛量表(Numeric Pain Rating scale, NRs)上达到5级(5/10NRs)。随后进行实验干预。对照组:实验者向受试者右下肢腓肠肌外侧头施加指压治疗手法,起始力量为疼痛阈(5/10NRs)力度,随后保持指压力量两分钟。受试者在实验30s,1min,1min30s,2min时报告NRs疼痛值。实验组:在对照组的基础上,受试者行10s踝关节50%1Rm等长背屈,随后休息10s,等长收缩与休息交替进行直到指压治疗结束。试验后再次进行最大等长跖屈力量测试和疼痛阈测试。实验前后疼痛阈测试分别施行三次,结果取平均值;最大等长跖屈力量测试分别测试三次,结果取最大值。利用协方差分析进行干预前后疼痛阈组间结果分析,利用配对样本t检验进行干预前后组内疼痛阈组间结果分析,利用重复测量的方差分析对实验中NRs值进行数据分析,数据统计结果标准为:p0.50。研究结果:(1)与实验前相比,实验组与对照组的疼痛阈无显著变化(p>0.05,r=0.026),最大等长跖屈力量未见显著变化(p>0.05,r=0.296);(2)在实验中,对照组与实验组的NRs值明显下降(pmin时达到最大(实验组NRs2min=1.722±0.364,95%CI:0.951-0.493;对照组NRs2min=2.667±0.364,95%CI:1.896-3.438),分组对NRs结果存在主效应影响(pmatter,PVG)、水管周围灰质(PeriaueductalGraymatter,PAG)、蓝斑(LocusCoeruleus)、网状结构(ReticularFormation)及大脊核(RapheNucleusmagnus)的下行抑制通路信号,使以上中枢系统产生更多5-羟色胺、去甲肾上腺素至脊髓后角,降低疼痛感受器信号、抑制信号传递,并激活脊髓后角的中间神经元,从而进一步抑制疼痛。拮抗肌(足背屈方向)的等长收缩可以通过γ系统调控(GammaLoopsystem)引起交互抑制,从而引起腓肠肌肌张力的下降。拮抗肌的等长收缩可降低H反射(H-reflex),证明交互抑制可引起肌张力和肌肉刚度(musclestiffness)的降低,因此,拮抗肌的等长收缩引起的放松作用或为在治疗中疼痛程度的原因。另外,疼痛阈结
