Sponta.eous ba.teria. peritonitis is a.complica.ion of a.citic pa.ients with end-sta.e liver disea.e(ESLD); sponta.eous funga. peritonitis(SFP) is a.complica.ion of ESLD less known a.d described. ESLD is a.socia.ed to immunodepression a.d the resulting increa.ed susceptibility to infections. Recent perspectives of the ma.a.ement of the critica.ly ill pa.ient with ESLD do not specify the ra.e of isola.ion of fungi in critica.ly ill pa.ients,not even the a.tifunga.s used for the prophyla.is,neither optima. trea.ment. We reviewed,in order to focus the epidemiology,cha.a.teristics,a.d,considering the high morta.ity ra.e of SFP,the use of optima. empirica. a.tifunga. thera.y the current litera.ure.

In a.previous pa.er "to retrofit or not to retrofit?"(Nuti a.d Va.zi, 2003) a.stra.ghtforwa.d procedure a.le to foreca.t the economic return of seismic structura. upgra.ing wa. presented. More recently, the a.thors rea.ized tha. the fina. ma.hema.ica. results ca. be much simplified so a. to a.low ba.k-of-a.-envelope computa.ion. The title of this pa.er tries to highlight precisely this a.pect, na.ely tha. for ma.y a.regula. seismic structura. upgra.ing ca.es, nea.ly no computa.ion is needed(a.a.t from one subtra.tion a.d one multiplica.ion) to a.sess their economic convenience. These findings a.e presented a.d discussed in this pa.er, together with a.sta.e of the a.t on the cost-studies a.a.la.le in litera.ure a.d technica. codes. Methods We enrolled 74 infa.ts with TGa.consecutively into this study. CI a.d dp/dt(ma.) were mea.ured with PRa. a.d echoca.diogra.hy a. 0, 4, 8, 12, 24 a.d 48 h postopera.ively. Blood bra.n na.riuretic peptide (BNP) a.d blood la.ta.e (La.) were mea.ured a. ba.eline a.d a.ter opera.ion. Results The media. a.e a. surgery wa. 13 da.s (ra.ge 1–25 da.s) with a. a.era.e weight of 3.24 kg (ra.ge 2.31–4.17 kg). CI esti-ma.ed by PRa. wa. 1.11 ± 0.12 L/min/m2 (ra.ge 0.69–1.36) a.d by Doppler echoca.diogra.hy wa. 1.13 ± 0.13 L/min/m2 (ra.ge 0.76–1.40). dp/dt(ma.) estima.ed by PRa. wa. 1.31 ± 0.03 mmHg/s (ra.ge 1.23–1.43) a.d by Doppler echoca.diogra.hy wa. 1.31 ± 0.04 L/min/m2 (ra.ge 1.25–1.47). CI (r = 0.817,Pa.d dp/dt(ma.) (r = 0.794,Pa.ured by two meth-ods were highly correla.ed with a.linea. rela.ion. Blood BNP a.d la.ta.e increa.ed to the highest level a. 8–12 h post-opera.ively. Conclusions In the ea.ly post-opera.ive period, PRa. provides relia.le estima.es of ca.dia. index a.d dp/dt(ma.) va.ue compa.ed with echoca.diogra.hic mea.urements. PRa. through mostca.e? is a.relia.le continuous monitoring method for peri-opera.ive ma.a.ement in children with congenita. hea.t disea.e.
