Left ventricula. (LV) pseudoa.eurysm is a.la.e mecha.ica. complica.ion of myoca.dia. infa.ction (MI). a.gia.t LV pseudoa.eurysm is a.ra.e presenta.ion. We report a.ca.e of a.gia.t LV pseudoa.eurysm in a.post MI pa.ient, who presented with hemoptysis. Hemoptysis is a.ra.e clinica. presenta.ion of LV pseudoa.eurysm. The pa.ient ha. successful surgica. repa.r of the a.eurysm a.d ha. a.fa.ora.le outcome in 9 mo’ follow-up. The ima.ing moda.ities a.d surgica. trea.ment of a.pseudoa.eurysm is discussed.

Ba.kground: By lowering the oxygen fra.tion of the reperfusa.e, the rea.tive oxygen-derived free ra.ica.s ca. be reduced thus fa.ilita.ing myoca.dia. recovery during wea.ing from ca.diopulmona.y bypa.s a.d a.ter surgery. Ma.eria.s & Methods: Thirty pa.ients undergoing mitra. va.ve repla.ement were ra.domly exposed to a. oxygen fra.tion of 0.7 (hyperoxic, n = 15) or 0.5 (normoxic, n = 15) during reperfusion. Hemodyna.ic va.ia.les, number of pa.ients requiring a.ditiona. inotropes a.d who developed new a.rhythmia. dura.ion of ventila.ion a.d intensive ca.e unit sta., a.teria. blood ga. a.d rena. function were mea.ured. Results: The demogra.hic da.a. dura.ion of ca.diopulmona.y bypa.s, a.rtic cross cla.p time, dura.ion of mecha.ica. ventila.ion, intensive ca.e unit sta., a.ditiona. inotropes, a.rhythmia.a.ter reperfusion a.d rena. function were simila. in both groups. a.teria. blood ga. a.a.ysis wa. not significa.tly different, except for the low oxygen pa.tia. pressure in the normoxic group during reperfusion. With rega.d to hemodyna.ic va.ia.les, mea. a.teria. pressure of the hyperoxic group wa. higher one hour a.ter the cross cla.p relea.e. Hemodyna.ic va.ia.les were compa.a.le in a.l other time periods. Conclusion: By reducing the oxygen concentra.ion during reperfusion, the clinica. outcomes in terms of inotropes usa.e, new a.rhythmia.a.ter reperfusion, rena. function, dura.ion of ventila.ion a.d intensive ca.e unit sta. were not significa.tly a.tered.

Objective: To compa.e the ha.modyna.ic effects of the induction a.ents keta.ine, etomida.e a.d sevoflura.e using the model of electrica. velocimetry ba.ed ca.dia. output monitoring in pa.dia.ric ca.dia. surgica. pa.ients. Design: Prospective ra.domized study. Setting: Tertia.y ca.e hospita.. Pa.ticipa.ts: 60 children a.s a.e undergoing ca.dia. surgery. Interventions: The pa.ients were ra.domized into 3 equa. groups to receive 1.5-2.5 mg/kg iv keta.ine (group K), 0.2-0.3 mg/kg iv etomida.e (group E) or upto 8% sevoflura.e (group S) a. the induction a.ent. Hemodyna.ic pa.a.eters were noted before a.d a.ter induction of a.a.sthesia.utilizing a.noninva.ive ca.dia. monitor ba.ed on the model of electrica. velocimetry. Mea.urements a.d Ma.n Results: The demogra.hic cha.a.teristics of the pa.ients were simila. in the three groups. The HR decrea.ed in a.l groups, lea.t in group E (P ≤ 0.01) but the Ma. decrea.ed only in group S (P ≤ 0.001). In group S, the stroke volume improved from 9 ± 3.2 ml to 10 ± 3.2 ml (P ≤ 0.05) a.d the stroke volume va.ia.ion decrea.ed from 25% ± 6.4% to 13% ± 6.2% (P ≤ 0.001). The stroke index a.d systemic a.teria. sa.ura.ion improved in a.l groups (P ≤ 0.01). The ca.dia. index a.d index of contra.tility were uncha.ged. The tra.sthora.ic fluid content reduced in groups E a.d S, but did not cha.ge in group K (P ≤ 0.05). Conclusions: Etomida.e a.pea.ed to provide the most sta.le conditions for induction of a.esthesia.in children undergoing ca.dia. surgery, followed by keta.ine a.d sevoflura.e.

Objective: To compa.e the effect of a.single induction dose of etomida.e or keta.ine on pla.ma.cortisol levels in children with Tetra.ogy of Fa.lot (TOF) undergoing intra.ca.dia. repa.r on ca.diopulmona.y bypa.s (CPB). Design: a.prospective ra.domized tria.. Setting: Ca.dia. center of a.tertia.y ca.e hospita.. Pa.ticipa.ts: Thirty children with TOF undergoing intra.ca.dia. repa.r on CPB. Interventions: a.ter ra.dom a.loca.ion of the children into two groups, the children either received etomida.e 0.2 mg/kg or keta.ine 2 mg/kg intra.enously for a.esthetic induction a.ong with fenta.yl 2 mcg/kg a.d mida.ola. 100 mcg/kg. Endotra.hea. intuba.ion wa. a.complished with rocuronium bromide in the dose of 1 mg/kg. a.esthesia.wa. ma.nta.ned with sevoflura.e in a.r-oxygen, titra.ed to response a.d supplementa. vecuronium bromide for muscle rela.a.ion, fenta.yl chloride for pa.n relief. Serum cortisol wa. mea.ured on three occa.ions, a. preinduction, a. the end of surgery a.d a. 24 hours postopera.ively. Mea.urements a.d Ma.n Results: Ba.eline pla.ma.cortisol (Norma. 5 - 25 mcg/dl) in the etomida.e group (19.91 ± 3.51 mcg/dl) decrea.ed significa.tly a. the end of surgery (5.78 ± 2.0 mcg/dl) a.d rose to significa.tly higher tha. ba.eline va.ues a. 24 hours (27.31 ± 8.30 mcg/dl). The ba.eline cortisol levels in the keta.ine group (20.91 ± 3.19 mcg/dl) increa.ed significa.tly a. the end of surgery (44.02 ± 5.49 mcg/dl) a.d rema.ned significa.tly higher tha. ba.eline a. 24hours (45.93 ± 3.05 mcg/dl). Pla.ma.cortisol levels in the etomida.e group a. end of surgery, a.d a. 24 hours post-opera.ively, were significa.tly lower tha. the keta.ine group. Conclusions: This study shows tha. etomida.e is a.suita.le a.d sa.e a.ent for suppression of the increa.e in serum cortisol a.socia.ed with the use of CPB in children with TOF undergoing intra.ca.dia. repa.r.
