OBJECTIVE: To introduce our experience with modified Appleby operation in the treatment of a patient with advanced carcinoma of the body and tail of the pancreas, which invaded the common hepatic, splenic and celiac arteries. METHODS: The celiac artery, was ligated at its start point, and distal pancreas and involved arteries were resected en bloc. Pyloric and right gastroepiploic arteries should be preserved to keep blood supply to the stomach and pulsation of the proper hepatic artery be felt after occlusion of the common hepatic artery. RESULTS: The patient was free from epigastric and back pain after the operation, although she died of liver metastasis after seven months. CT scan did not show local reccurrence before her death. CONCLUSION: This procedure can increase the resectability and radical extent of locally advanced cancer of the body and tail of the pancreas and offer patients a better quality of life.

Background: Neural invasion is a special metastatic route in pancreatic cancer and responsible for the high recurrence in curatively resected cases. Objective: To summarize the characteristics and mechanisms of neural invasion in pancreatic carcino- ma for the better treatment of this disease. Data sources: The international literatures were re- viewed about the definition, incidence and mecha- nisms of neural invasion and its clinicopathology, di- agnosis and treatment. Data synthesis: Neural invasion is defined when the medial perineurium is involved by cancer cells, ac- counting for 45 %--100 % of all cases. It can be divid- ed into different kinds or stages according to its loca- tions and the number of nerve fascicles involved. In- vasion along vascularity, lymphatic vessels, perineu- ral space and neurotropism is considered as its pri- mary mechanisms. No clinicopathologic factors are correlated with neural invasion. Intravascular ultra- sound, CT scan and immunostaining K-ras gene a- nalysis can be used to diagnose neural invasion pre-, intra- or postoperatively. Conclusion: Neural invasion is an important prognos- tic factor for the recurrence of pancreatic carcinoma after pancreatectomy. Because of its high incidence, pancreatectomy with extended radical retroperitoneal dissection should be considered as a basic procedure in the treatment of pancreatic carcinoma.

AIM: To investigate whether brain ischemia induces serine phosphorylation of neuronal nitric oxide synthase(nNOS) by Ca2+/calmodulin-dependent protein kinase II (CaMKII) and the interaction between CaMKIIa and nNOSin rat hippocampus. METHODS: Brain ischemia was induced by bilateral carotid artery occlusion *** and the interaction of proteins were studied by immunoprecipitation and immunoblotting. Weinvestigated during brain ischemia serine phosphorylation and amount of nNOS in crude membranes fraction (P)and cytosolic fraction (S), interaction between CaMKIIa and nNOS, and the effects of 1-[N,O-bis-(5-isoquinolinesulfonyl)-N-methyl-L-tyrosyl]-4-phenylpiperazine (KN-62, a selective inhibitor of CaMKII) on phos-phorylation and the interaction of proteins in P. RESULTS: Serine phosphorylation of nNOS in P increased persis-tently during brain ischemia, and 15 min ischemia-induced serine phosphorylation of nNOS was attenuated signifi-cantly by KN-62. But there was no serine phosphorylation of nNOS in S. The distributions of nNOS were notaffected by ischemia and KN-62. However, the binding levels of both CaMKIIa with nNOS and Thr286autophosphorylated CaMKIIa with nNOS increased after ischemia, and were diminished by KN-62. CONCLUSION:CaMKII interacted with nNOS and regulated serine phosphorylation of nNOS during brain ischemia.

AIM: To investigate the role of c-Jun N-terminal protein kinase 1 and 2 (JNK1/2) and the main signal pathway forits activation in hydrogen peroxide (H2O2) induced apoptotic-like cortical cell death. METHODS: Using the modelof oxidative stress induced by H2O2, the expression and diphosphorylation of JNK1/2 was examined by immunoblottinganalysis, and neuronal apoptotic like cell death was determined by 4’,6-diamidino-2-phenylindole (DAPI) ***: The elevation in diphosphorylation level of JNK1/2 (4.40-/5.61-fold vs sham control) was associatedwith the concentration of H2O2 (0-100 mol/L) and the development of apoptotic-like cell death (11.04 %-81.01 %).There was no alteration of JNK1/2 protein expression following H2O2 treatment and recovery at different timepoints. Administration with JNK1/2 antisense oligonucleotides not only significantly decreased JNK1/2 proteinexpression and activation level, but also significantly reduced cortical cell death induced by H2O2 ***, both JNK1/2 diphosphorylation and apoptotic-like cell death were largely prevented by pretreatmentwith (5S,10R)-()-5-methyl-10,11-dihydro-5H-dibenzo[a,d]cyclohepten-5,10-imine hydrogen maleate (MK-801)or omission of Ca2+ in incubation medium with ethylene glycol-bis(2-aminoethylether)-N,N,N’,N’-tetraacetic acid(EGTA). CONCLUSION: JNK1/2 is activated and participates in H2O2-induced apoptotic-like death in cultured ratcortical neurons mainly via N-methyl-D-aspartate (NMDA) receptor-mediated influx of extracellular Ca2+.

Aim:To explore the intracellular mechanisms underlying the survival/differentia-tion effect of the glial cell line-derived neurotrophic factor(GDNF)on dopamine(DA)***:Midbrain slice culture and primary cell culture wereestablished,and the cultures were divided into 3 groups:control group,GDNFgroup,and the phosphatidylinositol 3-kinase/Akt(PI3-K/Akt)*** the expression of tyrosine hydroxylase(TH)was detected byimmunostaining as well as Western ***:GDNF treatment inducedan increase in the number of TH-immunoreactive(ir)cells and the neurite numberof TH-ir cells,as well as in the level of TH expression in cultures(Number of TH-ir cells in the slice culture:control group,8.76±0.75;GDNF group,18.63±*** of TH-ir cells and neurite number of TH-ir cells in cell culture:controlgroup,3.65±0.88 and 2.49±0.42;GDNF group,6.01±0.43 and 4.89±0.46).Meanwhile,the stimulation of cultured cells with GDNF increased the phosphorylation of Akt,which is a downstream effector of PI3-K/*** effects of GDNF were specifi-cally blocked by the inhibitor of the PI3-K/Akt pathway,wortmannin(Number ofTH-ir cells in slice culture:PI3-K/Akt pathway-inhibited group,6.98±***-ber of TH-ir cells and neurite number of TH-ir cells in cell culture:PI3-K/Aktpathway-inhibited group,3.79±0.62 and 2.50±0.25,respectively).Conclusion:The PI3-K/Akt pathway mediates the survival/differentiation effect of GDNF onDA cells.

Objective To investigate the effects of puerarin (Pur) on myocardial perfusion and ventricular wall motion in patients with acute coronary syndrome (ACS).Methods Thirty-seven patients with ACS were randomly divided into two groups:conventional treatment group (n= 17,11 males,range of age:32-80 years,average age:60.9±4.9 years) and Pur treatment group (n=20,12 males,range of age:40-76 years,average age:62.7±3.5 years).Patients in the conventional treatment group received standard treatment according to the current guidelines,while patients in the Pur treatment group received intravenous administration of Pur (500 mg/day) for 10 days plus conventional ***-time myocardial contrast echocardiography (RT-MCE) was performed to evaluate the change in myocardial perfusion index (MPI) and ventricular wall motion index (VWMI) at admission and 10 days after *** At 10 days after treatment,MPI was significantly higher (P<0.01) and VWMI significantly lower (P<0.01) in the Pur group comparing with those in the conventional *** Puerarin might improve myocardial microcirculation perfusion and ventricular wall motion in patients with ACS.

AIM: To investigate the effects of allicin on both telomeraseactivity and apoptosis in gastric cancer SGC-7901 ***: The gastric cancer SGC-7901 adenocarcinomacells were treated with allicin and the cell cycle, inhibitoryrate, apoptosis, telomerase activity and morphologic changeswere studied by MTT assay, flow cytometry (FCM), TRAP-PCR-ELISA assay, light microscope, electron microscoperespectively. Results were compared with that of AZT (3′-Azido-3′-deoxythymidine).RESULTS: SGC-7901 cells were suppressed after exposureto allicin of 0.016 mg/ml, 0.05 mg/ml, and 0.1 mg/mi for48 h. Compared with the control, the difference wassignificant (P＜0.05). Allicin could induce apoptosis of thecells in a dose-dependent and non-linear manner andincrease the proportion of cells in the G2/M phase. Comparedwith the control, the difference was significant in terms ofthe percentage of cells in the G2/M phase (P＜0.05). Allicincould inhibit telomerase activity in a time-dependent anddose-dependent pattern. After exposure to allicin at 0.016mg/ml for 24 hours, SGC-7901 cells showed typicalmorphologic ***: Allicin can inhibit telomerase activity andinduce apoptosis of gastric cancer SGC-7901 cells. Allicinmay be more effective than AZT.
