食管癌术后患者自我管理积极度现状及影响因素分析

目的了解食管癌术后患者自我管理积极度现状,并分析其影响因素。方法2022年8月至2023年9月,采用便利抽样法,选取中山大学肿瘤防治中心158例食管癌术后患者为研究对象,使用一般资料调查问卷、患者积极度量表、恐惧疾病进展简化量表、家庭关怀度指数量表进行横断面调查,采用多重线性回归分析食管癌术后患者自我管理积极度的影响因素。结果有效回收问卷144份,其中男106例,女38例,年龄18~39岁22例,40~59岁55例,≥60岁67例。食管癌术后患者积极度量表得分为(59.61±16.07)分、恐惧疾病进展量表得分为(40.45±9.76)分、家庭关怀度指数得分为(11.41±2.36)分。多重线性回归分析结果显示,文化程度、恐惧疾病进展、家庭关怀度指数是食管癌术后患者自我管理积极度的影响因素(t=2.54、-13.49、2.45,均p.0.05)。结论食管癌术后患者自我管理积极度总体有待提高,临床护理工作者应密切关注食管癌术后患者自我管理积极度现状,制订系统、科学、有效的自我管理积极度干预策略。

优化精液处理中的密度梯度离心法以提高受精结局的探讨

【目的】本研究旨在改进生殖男科领域现有的精液处理方法,特别是针对双层密度梯度法中的300×g 20 min处理条件,以提高受精结局。【方法】收集2020年7月和9月以及2022年3月和5月在中山大学附属第六医院生殖医学中心进行辅助生殖助孕的1623例患者的精液标本进行实验。预实验中,比较四种不同的双层密度梯度方法(200×g 10 min、200×g 20 min、300×g 10 min和300×g 20 min)处理后标本的精子DNA碎片率和回收率。然后,筛选出一种最优的方法作为新方法,并与目前在用的旧方法(300×g 20 min双层梯度法)进行对比,检验受精率是否有统计学差异。在新方法的基础上,进一步优化为单层密度梯度法,并与双层密度梯度法进行对比,检验是否存在统计学差异。实验过程中严格控制温度、离心速度和离心时间,同时记录每组样本的数量和处理条件。【结果】在保证足够的精子回收率的基础上,发现四种双层密度梯度法中300×g 10 min的精子DNA碎片率低于300×g 20 min。因此,选择300×g 10 min作为新方法进行试验。结果表明,新方法300×g 10 min的总受精率、二核体(2p.)受精率均高于300×g 20 min,且差异有统计学意义(p.0.05);300×g 10 min的卵裂率也略高于300×g 20 min,但差异没有统计学意义(p.0.05)。单层密度梯度法的总受精率、2p.受精率均高于双层密度梯度法,但差异没有统计学意义(p.0.05);单层密度梯度法的卵裂率高于双层密度梯度法,囊胚形成率低于双层密度梯度法,且差异均有统计学意义(p.0.05)。【结论】相对于现有的300×g 20 min双层梯度法,300×g 10 min双层梯度法成功提高了总受精率、2p.受精率、卵裂率,缩短了精液优化处理的时间;而单层密度梯度法虽提高了卵裂率、节约了试剂成本和操作时间,但其囊胚形成率却出现了下降的情况。这些发现为生殖男科领域的精液处理方法提供了有益的指导和启示。

Backgr.und: p.thologic r.sp.nse is evaluated accor.ing to the extent of tumor.r.gr.ssion and is used to estimate the efficacy of p.eop.r.tive tr.atment. Sever.l studies have r.p.r.ed the association between the p.thologic r.sp.nse and clinical outcomes of color.cal cancer p.tients with liver.metastases who under.ent ***, to date, no data fr.m Chinese p.tients have been r.p.r.ed. In this study, we aimed to evaluate the association between the p.thologic r.sp.nse to p.e-hep.tectomy chemother.p. and p.ognosis in a cohor. of Chinese *** and methods: In this r.tr.sp.ctive study, we analyzed the data of 380 liver.metastases in 159 *** p.thologic r.sp.nse was evaluated accor.ing to the tumor.r.gr.ssion gr.de(Tr.).The p.ognostic r.le of p.thologic r.sp.nse in r.cur.ence-fr.e sur.ival(r.S) and over.ll sur.ival(OS) was assessed using Kap.an-Meier.cur.es with the log-r.nk test and multivar.ate Cox models. Factor. that had p.tential influence on p.thologic r.sp.nse wer. also analyzed using multivar.ate logistic r.gr.ssion and Kr.skal-Wallis/Mann-Whitney U ***: p.tients whose tumor. achieved p.thologic r.sp.nse after.p.eop.r.tive chemother.p. had significant longer.r.S and OS than p.tients whose tumor.had no p.thologic r.sp.nse to chemother.p.(median r.S: 9.9 vs.6.5 months, p.= 0.009; median OS: 40.7 vs. 28.1 months, p.= 0.040). Multivar.ate logistic r.gr.ssion and Kr.skal-Wallis/Mann-Whitney U tests showed that metastases with small diameter. metastases fr.m the left-side p.imar. tumor.,and metastases fr.m p.tients r.ceiving long-dur.tion chemother.p. had higher.p.thologic r.sp.nse r.tes than their.contr.l metastases(all p.r.ase in the ser.m car.inoembr.onic antigen(CEA) level after.p.eop.r.tive chemother.p. p.edicted an incr.ased p.thologic r.sp.nse r.te(p.p.lication of tar.eted ther.p. did not significantly influence Tr. scor.s of all cases of metastases, the addition of cetuximab to chemoth

Objective: To investigate the p.otective effects of modified Linggui Zhugan Decoction(MLZD), a tr.ditional Chinese medicine for.ula, on obese typ. 2 diabetes mellitus(T2 DM) r.ts. Methods:Fifty Sp.ague-Dawley r.ts wer. r.ndomly divided into 5 gr.up. by a r.ndom number.table, including nor.al, obese T2 DM(ob-T2 DM), MLZD low-dose [MLDZ-L, 4.625 g/(kg·d)], MLZD middle-dose [MLD-M,9.25 g/(kg·d) ] and MLZD high-dose [MLD-H, 18.5 g/(kg·d)] gr.up.,10 r.ts in each gr.up. After.4-week inter.ention, blood samp.es and liver. p.ncr.as, muscle tissues wer. collected to assess the insulin r.sistance(Ir., blood lip.d, adip.kines and inflammation cytokines. The alter.tion of p.osp.atidylinositol 3 kinase(p.3 K)-p.otein kinase B(p.B or.Akt)/the mammalian tar.et of r.p.mycin(mTOr.-r.bosome p.otein subunit 6 kinase 1(S6 K1)/AMp.activated p.otein kinase(AMp.)-p.r.xisome p.olifer.tor.activated r.cep.or.gamma coactivator.1 alp.a(p.C-1α) p.thways wer. also studied. r.sults: MLZD dose-dep.ndently r.duced fasting blood glucose,fasting insulin, homeostasis model of assessment for.Ir.index and incr.ased insulin sensitive index comp.r.d with ob-T2 DM r.ts(p.0.05). Similar.y, total cholester.l, tr.glycer.de, low-density lip.p.otein cholester.l and fr.e fatty acids wer. also decr.ased comp.r.d with ob-T2 DM r.ts after.4-week tr.atment(p.0.05 or.p.0.01).Imp.ovements in adip.kines and inflammator. cytokines wer. obser.ed with a r.ised level of adip.nectin and a r.duced level of lep.in, r.sistin, tumor.necr.sis factor.α and inter.eukin-6(p.0.05 or.p.0.01).
