Modified Appleby operation in treatment of distal pancreatic cancer

OBJECTIVE: To introduce our experience with modified Appleby operation in the treatment of a patient with advanced carcinoma of the body and tail of the pancreas, which invaded the common hepatic, splenic and celiac arteries. METHODS: The celiac artery, was ligated at its start point, and distal pancreas and involved arteries were resected en bloc. Pyloric and right gastroepiploic arteries should be preserved to keep blood supply to the stomach and pulsation of the proper hepatic artery be felt after occlusion of the common hepatic artery. RESULTS: The patient was free from epigastric and back pain after the operation, although she died of liver metastasis after seven months. CT scan did not show local reccurrence before her death. CONCLUSION: This procedure can increase the resectability and radical extent of locally advanced cancer of the body and tail of the pancreas and offer patients a better quality of life.

Background: Neural invasion is a special metastatic route in pancreatic cancer and responsible for the high recurrence in curatively resected cases. Objective: To summarize the characteristics and mechanisms of neural invasion in pancreatic carcino- ma for the better treatment of this disease. Data sources: The international literatures were re- viewed about the definition, incidence and mecha- nisms of neural invasion and its clinicopathology, di- agnosis and treatment. Data synthesis: Neural invasion is defined when the medial perineurium is involved by cancer cells, ac- counting for 45 %--100 % of all cases. It can be divid- ed into different kinds or stages according to its loca- tions and the number of nerve fascicles involved. In- vasion along vascularity, lymphatic vessels, perineu- ral space and neurotropism is considered as its pri- mary mechanisms. No clinicopathologic factors are correlated with neural invasion. Intravascular ultra- sound, CT scan and immunostaining K-ras gene a- nalysis can be used to diagnose neural invasion pre-, intra- or postoperatively. Conclusion: Neural invasion is an important prognos- tic factor for the recurrence of pancreatic carcinoma after pancreatectomy. Because of its high incidence, pancreatectomy with extended radical retroperitoneal dissection should be considered as a basic procedure in the treatment of pancreatic carcinoma.

Effect of puerarin on myocardial perfusion and ventricular wall motion in patients with acute coronary syndrome

Objective To investigate the effects of puerarin (Pur) on myocardial perfusion and ventricular wall motion in patients with acute coronary syndrome (ACS).Methods Thirty-seven patients with ACS were randomly divided into two groups:conventional treatment group (n= 17,11 males,range of age:32-80 years,average age:60.9±4.9 years) and Pur treatment group (n=20,12 males,range of age:40-76 years,average age:62.7±3.5 years).Patients in the conventional treatment group received standard treatment according to the current guidelines,while patients in the Pur treatment group received intravenous administration of Pur (500 mg/day) for 10 days plus conventional ***-time myocardial contrast echocardiography (RT-MCE) was performed to evaluate the change in myocardial perfusion index (MPI) and ventricular wall motion index (VWMI) at admission and 10 days after *** At 10 days after treatment,MPI was significantly higher (P<0.01) and VWMI significantly lower (P<0.01) in the Pur group comparing with those in the conventional *** Puerarin might improve myocardial microcirculation perfusion and ventricular wall motion in patients with ACS.

Effects of allicin on both telomerase activity and apoptosis in gastric cancer SGC-7901 cells

AIM: To investigate the effects of allicin on both telomeraseactivity and apoptosis in gastric cancer SGC-7901 ***: The gastric cancer SGC-7901 adenocarcinomacells were treated with allicin and the cell cycle, inhibitoryrate, apoptosis, telomerase activity and morphologic changeswere studied by MTT assay, flow cytometry (FCM), TRAP-PCR-ELISA assay, light microscope, electron microscoperespectively. Results were compared with that of AZT (3′-Azido-3′-deoxythymidine).RESULTS: SGC-7901 cells were suppressed after exposureto allicin of 0.016 mg/ml, 0.05 mg/ml, and 0.1 mg/mi for48 h. Compared with the control, the difference wassignificant (P＜0.05). Allicin could induce apoptosis of thecells in a dose-dependent and non-linear manner andincrease the proportion of cells in the G2/M phase. Comparedwith the control, the difference was significant in terms ofthe percentage of cells in the G2/M phase (P＜0.05). Allicincould inhibit telomerase activity in a time-dependent anddose-dependent pattern. After exposure to allicin at 0.016mg/ml for 24 hours, SGC-7901 cells showed typicalmorphologic ***: Allicin can inhibit telomerase activity andinduce apoptosis of gastric cancer SGC-7901 cells. Allicinmay be more effective than AZT.

GABAA receptor partially mediated propofol-induced hyperalgesia at superspinal level and analgesia at spinal cord level in rats

AIM: To observe effects of propofol on nociceptive response at superspinal and spinal level in rats. METHODS: Two hundreds and fifty-eight Sprague-Dawley male rats were randomized into thirty-two groups. Propofol and bicuculline were microinjected into lateral ventricle (icv), ventrolateral periaqueductal gray (vlPAG), intrathecal (ith), and intraperitoneal (ip). The noxious responses were evaluated by hot plate and formalin test. RESULTS: In hot-plate test, systemic and superspinal administration of propofol (40 mg·kg-1 ip, 100 μg in 10 μL, icv, and 4 μg in 0.4 μL vlPAG microinjection) produced hyperalgesia (P30 min by microinjection of bicuculline (10 ng in 0.4 μL, vlPAG) (P3 %, 54.8 %, 80.8 %, and 97.4 % at 10, 20, 30 and 40 min by ith bicuculline (P30 min by ith bicuculline (P<0.05) after formalin injection. Hyperalgesia produced by ip propofol in both hot plate and formalin test could not be antagonized by vlPAG administration of bicuculline. CONCLUSION: GABAA receptor partly mediated propofol-induced hyperalgesia at superspinal and analgesia at spinal cord in rats.

Aim:The present study aimed to elucidate the role of T-subtype calcium channels（Cav3.1,Cav3.2,and Cav3.3）in the pathogenesis of neuropathic pain at spinal ***:The chronic compression of the dorsal root ganglion（CCD）rat modelwas *** antisense oligonucleotide of Cav3.1,Cav3.2,and Cav3.3 or nor-mal saline（NS）were intrathecally administered twice per day from the first day tothe fourth day after *** mechanical withdrawal threshold and pawthermal withdrawal latency were measured to evaluate the tactile allodynia andthermal hyperalgesia,***:CCD rats developed reliable tactileallodynia and thermal hyperalgesia after *** administration ofantisense oligonucleotide of Cav3.2 and Cav3.3 significantly relieved tactileallodynia and thermal hyperalgesia in CCD rats,but not ***:Cav3.2and Cav3.3 subtype calcium channels in the spinal cord may play an important rolein the pathogenesis of neuropathic pain,which may contribute to the manage-ment of the neuropathic pain.

Telomerase is an attractive molecular target for cancer therapy because it is present in mostmalignant cells but is undetectable in most normal somatic *** telomerase consists of two subunits,an RNA component(hTR)and a human telomerase reverse transcriptase component(hTERT).Smallinterfering RNA(siRNA),one kind of RNA interferences,has been demonstrated to be an effective methodto inhibit target gene expression in human *** investigated the effects of siRNA targeting at both hTRand hTERT mRNA on the inhibition of telomerase activity in human renal carcinoma cells(HRCCs).Theproliferation and apoptosis of HRCCs were *** treatment of HRCCs using hTR and hTERTsiRNAs resulted in significant decrease of hTR mRNA,hTERT mRNA and hTERT *** siRNA canalso inhibit the telomerase activity and the proliferation of ***,they can induce apoptotic celldeath in a dose-dependent *** these findings,we propose that the inhibition of telomerase activityusing siRNA targeting hTR and hTERT might be a rational approach in renal cancer therapy.
