Effects of HBV gene variations on disease development and antiviral therapy for patients with chronic hepatitis B

Viral variation may change pathogenicity, escape immunity, lead to persistence infection, and cause drug resistance against antiviral therapy. This study was undertaken to investigate the effects of HBV gene variation on the progression of disease and on the efficacy of antiviral therapy for patients with chronic hepatitis B(CHB). METHODS:Hepatitis B virus (HBV) gene mutational sites were detected using gene chip in selected hepatitis B patients. RESULTS:In the patients HBeAg did not show serologic conversion or HBeAg(-)/anti-HBe(+), but their HBV DNA remained positive 2. weeks after α-interferon therapy, which was associated with mutations of nt1896, nt1814, nt1762.and nt1764. In the patients, that HBV DNA levels decreased or were undetectable, but rebounded later after antiviral therapy by lamivudine was associated with mutations of aa52. and(or) aa552.*** mutation), which resulted in lamivudine-resistance. YMDD mutation was prone to occur 52.weeks after lamivudine therapy in some chronic hepatitis B patients (2..4%). Nt1896 mutation was common in most chronic hepatitis B patients (68.5%). Chronic severe hepatitis, cirrhosis, and primary liver carcinoma were related to the mutations of nt1896, nt1762.and nt1764. CONCLUSIONS:HBV gene mutations could aggravate patient's condition and affect the efficacy of antiviral therapy. The regular detection of HBV gene mutation is helpful for identification of disease prognosis and adjustment of therapeutic strategy.

BACKGROUND:12.lipoxygenase(12.LOX) has been reported to be an important gene in cancer cell proliferation and survival,and tumor ***,its role in hepatocellular carcinoma(HCC) cells remains ***:Expression of 12.LOX was assessed in a diethylnitrosamine-induced rat HCC model,and in SMMC-772.,HepG2.and L-02.cells using immunohistochemical staining and reverse transcriptase-polymerase chain reaction(RT-PCR).GST-π and Ki-67 were determined in vivo by immunohistochemical *** was evaluated by TUNEL *** viability and apoptosis were determined by MTT assay and flow cytometry,***-related proteins in SMMC-772. and HepG2.cells were detected by Western ***:Immunohistochemical staining and RT-PCR showed that 12.LOX was over-expressed in rat HCC and two HCC cell lines,while the expression was inhibited by baicalein,a specific inhibitor of *** inhibited cell proliferation and induced apoptosis in rat HCC and both cell lines in a dose-and time-dependent *** in vivo study demonstrated that baicalein also reduced neoplastic ***,baicalein reduced Bcl-2.protein expression coupled with a slight increase of the expression of Bax and activation of ***,baicalein inhibited the activation of ERK-1/2.phosphorylated).Interestingly,the effects of baicalein were reversed by 12.S)-HETE,a metabolite of ***:Inhibition of 12.LOX leads to reduced numbers of HCC cells,partially caused by increased apoptosis.12.LOX may be a potential molecular target for HCC prevention and treatment.

Treatment of Hemangioma by Transfection of Antisense VEGF Gene

The effect of transfection of antisense vascular endothelial growth factor （VEGF） gene on the growth of hemangioma was studied. A total of 49 cases of capillary hemangiomas of the skin were collected. Immunohistochemical method was used to detect the expression of PCNA in hemangioma tissues. According to the finding, 49 cases of hemangiomas fell into proliferating phase （2. cases） and involuting phase （2. cases） respectively. Another 5 cases of normal skin tissues adjacent to the tumor tissues served as control. Immunohistochemical staining was performed to detect the expression of VEGF in the tumor tissues and the normal tissues. The average absorbance （A） values and the average positive area rate of VEGF were measured by image analysis system （HPIAS-2.00）. Endothelial cells from the tumor tissues in proliferating phase were cultured. Eukaryotic expression vector was constructed by sub-cloning, and transfected into human hemangioma endothelial cells by using cation liposome as vector. The expression of VEGF mRNA and protein was detected by RT-PCR and indirect immunofluorescence assay （IFA）, respectively, and the biological characteristics of the transfected endothelial cells were examined by MTT assay and flow cytometry （FCM） after transfection. Immunohistochemical results showed that the expression of VEGF in proliferating endothelial cells was remarkably higher than those in involuting endothelial cells and normal endothelial cells （P〈0.01）, but there was no significant difference in the expression of VEGF between involuting endothelial cells and normal ones （P〉0.01）. Electrophoresis and sequencing indicated that the eukaryotic expression vector containing antisense VEGF gene, i.e. pcDNA3.1-VEGF, was success- fully constructed. Effects of irbesartan on atrial cell electrophysiology

Atrial fibrillation (AF) is the most common sustained arrhythmia encountered in clinical practice.~1 Its incidence increases with age and the presence of structural heart disease. It is a major cause of stroke, especially in the elderly. It has been shown that angiotensin converting enzyme inhibitor (ACEI) can reduce the incidence of AF after acute myocardial infarction.~2.Several studies have shown that activation of the rennin-angiotensin system is associated with the mechanisms of AF. Irbesartan is a long-acting angiotensin Ⅱ type 1 receptor antagonist used widely in the treatment of hypertension.~3 In recent years, it has been demonstrated that patients treated with amiodarone plus irbesartan had a lower rate of recurrence of atrial fibrillation than did patients treated with amiodarone alone.~4 These findings suggest that the inhibition of angiotensin Ⅱ may prevent AF, but its underlying electrophysiological mechanisms are obscure. The purpose of this study is to investigate the effects of irbesartan on atrial cell electrophysiology.

Neodymium:Yttrium Aluminum Garnet Laser for Mini-Percutaneous Treatment of Upper Urinary Tract Stones

To observe the efficacy and safety of mini-percutaneous nephrolithotomy with Neodymium：Yttrium Aluminum Garnet （Nd-YAG） laser in the treatment for upper urinary tract stones, from December 2.05 to September 2.06, 31 patients with renal stones, 15 patients with ureteral stones and 7 patients with renal and ureteral stones underwent mini-percutaneous nephrolithotomy with Nd-YAG laser by combination of rigid ureteroscope and flexible ureteroscope under B-ultrasound guidance. Clinical data including operation time, lithotripsy time, complications and stone-free rate were analyzed retrospectively. Our study showed that the percutaneous renal access （F14-F18） was successfully established under B-ultrasound guidance in all cases. Immediate phase Ⅰ lithotripsy was performed in 47 cases through single tract, and in one case through two tracts. Delayed phase Ⅱ lithotripsy was done in 5 cases of renal stones. Operation time ranged from 55 to 180 rain with an average time of 100±15 min. Lithotripsy time was from 2. to 135 min and the average lithotripsy time was 65±11 min. No severe complications occurred in our series. Complex renal stones were cleared in 34 of 38 cases （89.5 %）. All ureteral stones were completely removed in 15 cases （100 %）. It was concluded that mini-percutaneous nephrolithotomy with Nd：YAG laser for the treatment of upper urinary tract stones by combination of rigid ureteroscope and flexible ureteroscope has the advantages of effectiveness, mini-invasion, shorter operative time and safety.

Objective To investigate whether laparoscopic surgery is valuable during early pregnancy and its influence on delivery outcome of the *** A retrospective analysis was undertaken of 14 heterotopic pregnancies conceived by IVF/ICSI-ET and underwent laparoscopic salpingectomy from December 2.01 to September *** The mean age of 14 patients was 2. ± 5.3 years with range of 2.-39 *** ectopic pregnancy was located in the ampulla（11 patients,78.57%） and isthmus（3 patients,2..43%）.All 14 patients underwent successful laparoscopic salpingectomy without ***（78.57%） patients delivered live singleton infants at full term via caesarean section while 3（2..43%） had spontaneous *** birth weight of viable newborns ranged from 2.600 g to 3 72. g and had a high Apgar *** During early pregnancy,the heterotopic pregnancy should be diagnosed and treated early and laparoscopic salpingectomy can be a safe and effective *** well experienced surgeons and accessibility of the modern equipments are mandatory.

Expression of Matrix Metalloproteinase and Its Tissue Inhibitor in Haemangioma

The action mechanism of matrix metalloproteinases-2.（MMP-2. and tissue inhibitor of metalloproteinases-2.（TIMP-2. in the genesis, development and degeneration of haemangioma was investigated by detecting their expression in the tissue of haemangioma in different phases by using the immunohistochemistry. Fifty paraffin-embedded specimens of skin capillary haemangioma were collected, which were documented in the Department of Pathology, renmin hospital of{2. from 2.00 to 2.06. All samples were stained by regular HE method, and proliferative cell nuclear antigen （PCNA） was tested by immunohistochemical S-P method. The samples were classified according to the Mulliken criteria and the expression pattern of PCNA. Immunohistochemical S-P method was ap- plied to detect the expression of MMP-2.and TIMP-2.in proliferative and degenerative phases of cutaneous capillary haemangioma, and in normal skin tissues. In combination with the detection of the expression of factor Ⅷ-related antigen, it was verified that in haemangioma tissues, the cells expressing MMP-2.and TIMP-2.were vascular endothelial cells. The MMP-2.and TIMP-2.expression was quantitatively analyzed by image analysis system （HPIAS-1000）, and one-way ANOVA（107） and SNK（q） test were done to analyze average absorbance （A） and positive area rate of immunohistochemically positive particles by using SPSS11.5. The results showed：（1） Among 50 samples of haemangioma, there were 2. proliferative haemangiomas, and 2. degenerative haemangiomas, respectively; （2. The expression of MMP-2.was weak in normal vascular endothelial cells, cytoplasm of connective tissues and extracellular matrix around blood vessels. The expression of MMP-2.in proliferative group was significantly higher than in degenerative group and control group （normal skin）（P〈0.05）, but there was no statistically significant difference between the latter two groups; （3） TIMP-2.was highly expressed in normal tissues, degenerative vascular endothel

Clinical Application of Percutaneous Nephrostomy in Some Urologic Diseases

Percutaneous nephrostomy was applied in some other urologic diseases and the efficacy was evaluated. Percutaneous nephrostomy for percutaneous nephrolithotomy （PNL） was performed in patients with various renal, perinephric and bladder diseases （n=79）. The tract establishment, operation duration and complications were observed and the efficacy was assessed. The results showed that the tracts were successfully established in 79 cases. The operation lasted 4–2. min. 12.–16F single tract was established in nephrohydrop patients and 16F–2.F single or multiple tracts were established in patients with pyonephrosis, renal cortical abscess, renal cyst and perinephric abscess. During dilation, no leakage of liquor puris was noted. Establishment of 18F single tract was achieved in one urinoma patient. In two patients with foreign body in kidney, the foreign bodies were removed via established 14F single tract. 18F tracts were established in 2.patients with bladder contracture, which was followed by the placement of 16F balloon urethral catheter for drainage. No complications, such as massive bleeding, intestinal injury and spreading of infection took place in our series. All the patients were followed up for 2.12.months. No long-term complications such as dropping of drainage tube occurred. It is concluded that as a minimally invasive technique, percutaneous nephrostomy has the advantages of convenience, simplicity and causing less complications and can be used for various urologic diseases.
