Chronic post-hypoxic myoclonus, also known as Lance-Adams syndrome (LAS), is a rare complication of successful cardiopulmanry resuscitation often accompanied by action myoclonus and cerebellar ataxia. It is seen in patients who have un-dergone a cardiorespiratory arrest, regained consciousness afterwards, and then developed myoclonus days or weeks after the event. Worldwide, 122 cases have been reported in the literature so far, including 1 case of Chinese. Here we report 2 Chinese LAS patients with detailed neuroimagings. Cranial single photon emission computed tomography (SpECT) of patient 1, a 52-year-old woman, showed a mild hypoperfusion in her left temporal lobe, whereas patient 2, a 54-year-old woman, manifested a mild bilateral decrease of glucose metabolism in the frontal lobes and a mild to moderate decrease of the N-acetyl aspartate (NAA) peak in the bilateral hippocampi by cranial [18F]-fluorodeoxyglucose positron emission tomographic (pET) scan and cranial magnetic resonance spectroscopy (MrS), respectively. We also review the literature on the neuroimaging, pathogenesis, and treatment of LAS.

BACKGrOUnd: Ideal model of poststroke depression (pSD) may be induced in rats guided by the theoretical evidence that 'primary endogenous mechanism' and 'reactivity mechanism' theories for pSD in human being. OBJECTIVE: To investigate the feasibility of comprehensive methods to induce pSD models in rats. DESIGN: A randomized controlled animal trial. SETTING: department of Neurology, affiliated hospital of Chengdu university of Traditional Chinese Medicine. MATErIALS: Male SD rats of SpF degree, weighing 350-500 g, were provided by the experimental animal center of Chengdu university of Traditional Chinese Medicine. The rats were raised for 1 week adaptively, then screened behaviorally by open-field test and passive avoidance test. Forty-five rats with close scores were randomly divided into normal control group (n =10), simple stroke group (n =10), stress group (n =10) and pSD group (n =15). METHODS: The experiments were carried out in the laboratory of Chengdu university of Traditional Chinese Medicine from July 2002 to February 2003. ① rat models of focal cerebral ischemia were induced by thread embolization, then treated with separate raising and unpredictable stress to induce pSD models. ②The neurologic deficit was evaluated by Longa 5-grade standard (the higher the score, the severer the neurologic deficit) and horizontal round rod test (normal rat could stay on it for at least 3 minutes). ③ The behavioral changes of pSD rats were evaluated by the saccharin water test, open-field text and passive avoidance test, including the changes of interest, spontaneous and exploratory activities, etc. ④ The levels of monoamine neurotransmitters, including norepinephrine (NE), serotonin (5-HT) and dopamine, in brain were determined using fluorospectrophotometry. Myoepithelial carcinoma,also called malignant myepithelioma,is usually reported in the head,neck and lung,which is a rare malignant tumor arising from salivary *** region of cavernous sinus is an uncommon condition of involvement in myoepithelial carcinoma,while there are no finding of the tumor in common sites. Here we describe a 48-year-old female who present a rapid developing myoepithelial carcinoma,with progressively worse clinical symptoms and *** patient eventually showed a worse prognosis,and was advised to pick up a radiotherapy after biopsy and endoscopic transsphennoidal surgery,even if the radiotherapy has showed unclear effect on the *** this report,we discuss the histopathologic and cellular morphologic uniqueness of this rare case.

Construction of human VEGF165 gene eukaryotic expression plasmid and its effect on proliferation of vascular endothelial cells

After organ transplantation, rapid repair of injured vascular endothelial cell (VEC) is a key to prevent graft chronic dysfunction besides control of immunological rejection. Many studies have confirmed that vascular endothelial growth factor 165 (VEGF165) could accelerate the repair of VEC injury, decrease thrombosis and thrombotic occlusion, and inhibit hyperplasia of the intima. This study was designed to construct eukaryotic expression plasmid pBudCE4.1/VEGF165, and observe its effect on the prolife ration of VEC. METHODS:The VEGF165 gene cloned from human heart tissue by rT-pCr was cloned into eukaryotic expression plasmid pBudCE4.1. The recombinant expression plasmid pBudCE4.1/VEGF165 was identified by restriction enzyme (Hind III and BamH I) digestion analysis, and was sequenced. The pBudCE4.1/VEGF165 was introduced into VEC through lipofection transfection. The VEGF165 mrNA expression by Northern blot and VEGF165 protein expression was detected by immunocytochemical staining. The effect of expression protein on VEC proliferation was detected by flow cytometry. rESULTS:The rT-pCr product of the VEGF165 gene was about 576bp. Sequencing analysis revealed that the sequence of the amplified VEGF165 gene was identical with that in GenBank. restrictive enzyme digestion analysis showed that recombinant expression plasmid pBudCE4.1/ tVEGF165 had been constructed successfully. The expression of VEGF165 at mrNA and protein levels in the transformed VSMCs had been demonstrated by Northern blot and immunocytochemical staining respectively. The expressed product of VEGF165 could notably accelerate the proliferation of VECs. CONCLUSIONS:pBudCE4.1/VEGF165 is successfully cons- tructed and is expressed in VECs. Expressed VEGF165 can accelerate the VEC proliferation. The present study has laid a foundation for potential use of VEGF165 gene transfection to prevent and treat vascular stenosis in the transplanted organ.

BACKGrOUnd:Increasing evidence suggests that a close interaction of Kupffer cells with T cells plays a central role in concanavalin A-induced hepatic injury in mice,but the underlying mechanisms remain *** present study aimed to determine the relative roles of Th1 and Th17 type responses in concanavalin A-induced hepatic injury in mice, and to investigate whether or not Kupffer cells contribute to hepatic injury via a Th1 or Th17 type response-dependent pathway. METHODS:Immune-mediated hepatic injury was induced in C57BL/6 mice by intravenous injection of concanavalin A. Kupffer cells were inactivated by pretreatment with gadolinium chloride 24 hours before the concanavalin A *** interferon-gamma(IFN-γ)and interleukin-17(IL-17)pathways were blocked by specific neutralizing *** injury was assessed using serum transferase activity and pathological *** of inflammatory cytokines within the liver was detected by real-time polymerase chain reaction and immunohistochemistry. rESULTS:Neutralization of IFN-γsignificantly attenuated concanavalin A-induced hepatic ***,neutralization of IL-17 failed to suppress the *** of Kupffer cells by gadolinium chloride pretreatment protected against concanavalin A-induced injury and significantly reduced hepatic cytokine levels including TNF-α,IL-6 and IFN-γbut not ***:Our findings suggest that Kupffer cells contribute to concanavalin A-induced hepatic injury via a Th1 type response-dependent pathway and production of inflammatory cytokines including TNF-α,IL-6 and IFN-γ.
