Introduction: Inflammatory pseudopolyps (IPs) are a well-recognized entity in patients with inflammatory bowel disease (IBd), most likely developing from long-standing chronic inflammation. Similarly, IPs have been associated with ischemic and infectious colitis, intestinal ulcers, and mucosal anastomoses. This study aimed to analyze inflammatory pseudopolyps without a history of these known associated pathologies. materials and methods: A database search was conducted for patients who underwent biopsies at thomas jefferson University Hospital from 2003-2013 for the presence of colorectal IPs. Exclusion criteria consisted of patients with a history of IBd, mucosal anastomoses, ischemic and infectious colitis. Spatial and temporal associations between colonic pathologies and IPs were assessed via Fisher’s exact and chi-square test, respectively. Results: Seventy-five polyps from 70 patients fulfilled the database search criteria. Forty-one pseudopolyps (55%) arose from the rectosigmoid region. Twenty-two patients had no associated colon pathology (31%);35 patients had epithelial polyps (50%), such as tubular adenomas, serrated adenomas, and hyperplastic polyps;10 patients had colonic adenocarcinoma (16%), and 18 patients had diverticulosis (26%). Epithelial polyps were significantly associated with IPs in the same region. However, diverticulosis was independent of IPs in regard to space and time. Conclusion: Colorectal inflammatory pseudopolyps may develop sporadically in up to one third of the cases while others frequently arise in the background of non-IBd colonic pathology. The increased presence of these polyps in the left colon raises the possibility that a subset of them may arise in predisposed mucosa. These polyps need to be differentiated from other morphologically similar colonic polyps.

17-β-estradiol (estrogen) is a steroid hormone important to human development;however, high levels of this molecule are associated with increased risk of breast cancer primarily due to estrogen’s ability to bind and activate the estrogen receptor (ER) and initiate gene transcription. Currently, estrogen mechanisms of action are classified as genomic and non-genomic and occur in an ER-dependent and ER-independent manner. In this study, we examine estrogen signaling pathways, by measuring changes in protein expression as a function of time of exposure to estrogen in both ER-positive (mCF-7) and ER-negative (mdA-mB-231) cell lines. Using a robust experimental design utilizing isotopic labeling, two-dimensional LC-mS, and bioinformatics analysis, we report genomic and non-genomic ER regulated estrogen responsive proteins. We find a little over 200 proteins differentially expressed after estrogen treatment. Cell proliferation, transcription, actin filament capping and cell to cell signaling are significantly enriched in the mCF-7 cell line alone. Translational elongation and proteolysis are enriched in both cell lines. Subsets of the proteins presented in this study are for the first time directly associated with estrogen signaling in mammary carcinoma cells. We find that estrogen affected the expression of proteins involved in numerous processes that are related to tumorigenesis such as increased cellular division and invasion in an ER-dependent manner. moreover, we identified negative regulation of apoptosis as a non-genomic process of estrogen. This study complements gene expression studies and highlights the need for both genomic and proteomic analyses in unraveling the complex mechanisms by which estrogen affects progression of breast cancer.

Background:Netherton syndrome (NTS) is a rare autosomal recessive multisystem disorder characterized by congenital erythroderma and ichthyosis, hair shaft abnormalities and immune dysregulation. The disorder is caused by deleterious mutations in the SPINK5 gene, encoding the serine protease inhibitor LEKTI. Objective:Our objective was to investigate if the erythrodermic variant of peeling skin syndrome is also caused by SPINK5 mutations and to study the consequences of the disease on infantile brain development. methods:In an infant with extensive erythroderma, peeling skin and failure to thrive, we analyzed the SPINK5 gene for pathogenic mutations by direct dNA sequencing and performed repeated brain mRI studies with diffusion-weighted imaging. Results:We identified a homozygous 4-base-pair insertion in exon 5 of SPINK5, which introduces a premature termination codon and appears to be a common mutation among West Indies islanders. mRI analyses revealed a persistent diffuse volume loss. Conclusion:Our results confirm that early truncation mutations of the coding sequence of SPINK5 produce a severe phenotype and that generalized peeling skin is one of the manifestations of NTS. We further demonstrate for the first time that NTS may be associated with mRI abnormalities indicative of a permanent tissue injury of the brain.

背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TdS）联合静脉注射昂丹司琼（ONd）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的PONV高危女性患者，进入手术室前2小时分别给患者贴上TdS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射ONd4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生PONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TdS＋ONd组与ONd组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TdS＋ONd组没有呕吐、干呕且未追加过止吐药的患者占48％，而ONd组占39％（P〈0．02）。TdS＋ONd组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于ONd组（分别为35％和25％，P〈0．01）。TdS＋ONd组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于ONd组（P〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，P〈0．01）。结论与单独应用ONd相比。TdS＋ONd能更大程度地减少PONV，主要是通过减少不良反应而获得。
