该研究首次在一项降压临床试验中同时测量24h动态肱动脉压和中心动脉压的昼夜节律变化情况[通过安装在类似手表装置带子上的血压计(a tonometer mounted into the articulating strap of a wristwatch-like device,BPro)]。入选"阿利吉仑降压疗效研究"(ASSERTIVE)的亚组研究"动态中心动脉压研究"(AmcAP)中高血压患者171例,随机分为阿利吉伦组(300mg/d)或替米沙坦组(80mg/d)进行12周的降压治疗,记录基线和治疗12周后动态肱动脉压和动态中心动脉压。结果显示,肱动脉压和中心动脉压有典型的昼夜节律模式。与夜间肱动脉压比较,夜间中心动脉压降低幅度较小,脉压增大降低(均P<0.01)。降压治疗后,昼夜节律没有发生变化。结论:肱动脉压和中心动脉压的昼夜节律模式有所不同,都不受降压治疗的影响,但夜间中心动脉压相对较高,表明夜间中心动脉压对疾病预后具有重要临床诊断意义,值得进一步观察研究。

AIM： To assess the effects of Sb on fecal flora and shortchain fatty acids （ScFA） in patients on long-term TEN. METHODS： Ten patients （3 females, 7 males, 59±5.5 years）, on TEN for a median of 13 mo （1-125）, and 15 healthy volunteers （4 females, 11 males, 32±2.0 years） received Sb （0.5 g bid PO） for 6 d. Two stool samples were taken before, on the last 2 d and 9-10 d after treatment, for ScFA measurement and for culture and bacterial identification. Values （mean4-SE） were compared using sign tests and ANOVA. RESULTS： Fecal butyrate levels were lower in patients （10.1±2.9 mmol/kg） than in controls （19.2±3.9, P= 0.02）. Treatment with Sb increased total fecal ScFA levels in patients （150.2＋27.2 vs 107.5±18.2 mmol/kg, P= 0.02） but not in controls （129.0±28.6 vs 113.0±15.2 mmol/kg, NS）. At the end of treatment with Sb, patients had higher fecal butyrate（16.0±4.4 vs 10.1 [2.9] mmol/kg, P= 0.004）. Total ScFAs remained high 9 d after treatment was discontinued. Before the treatment, the anaerobe to aerobe ratio was lower in patients compared to controls （2.4±2.3 vs 69.8±1.8, P= 0.003）. There were no significant changes in the fecal flora of TEN patients. cONcLUSION： Sb-induced increase of fecal ScFA concentrations （especially butyrate） may explain the preventive effects of this yeast on TEN-induced diarrhea.

Standard parenteral nutrition solutions are mixtures comprising interacting components that may degrade themselves over time. The objective of this study was to investigate the physicochemical and microbiological stability of a hospital preparation for parenteral nutrition in neonatology. The analyses were performed throughout the storage of the preparations at 2–8 °c(up to 4 months). The extent of stability was based on the determination of amino acids dosage, visual and physicochemical properties(glucose and electrolytes concentrations, pH and osmolality measurements, particle counting) and microbiological analysis(sterility test). A thermal degradation of ascorbic acid was conducted to evaluate the antioxidant properties of the parenteral mixture. Physicochemical and microbiological controls were found to comply with the specifications. Amino acids showed a good stability throughout the 4 months storage except for cysteine, which was progressively degraded to cystine, conferring a yellow coloration to parenteral solutions. Parenteral nutrition standards solutions remain stable for 4 months at 2–8 °c,ensuring safe administration in preterm infants.

The role of geomorphic habitat type, drift cell scale, and geographic scale in defining fish use of nearshore habitats is poorly known, particularly for Pacific salmon and their prey. In this study, key areas of nearshore habitat in central and western Strait of juan de Fuca were categorized by geomorphic habitat type and assessed for fish use within a degraded （Elwha） and intact comparative drift cells over a one year period. juvenile chinook and coho salmon were also sampled for genetic analysis to define regional dispersal patterns. Key findings are：（1） Ecological function of the area＇s nearshore is complex, with very strong seasonal variation in fish use both within and across GMHT （geomorphic habitat type）; （2） GMHT link to nearshore function for fish use differs depending on the fish species and time of year. Surf smelt and sand lance were the most abundant. And they were seasonally used embayed, spit, and bluff shorelines more than lower rivers. juvenile chinook, coho, and chum salmon occurred in much lower density than forage fish species, and used lower rivers more than other GMHTs; （3） When GMHTs were combined and analyzed at the drift cell scale, the degraded drift cell had different ecological patterns than the intact drift cell; （4） cross regional juvenile fish use of nearshore is an important component of habitat use： juvenile chinook and coho from as far away as the columbia River Oregon and Klamath River california utilize central Strait of juan de Fuca shorelines. Forage fish species may do so as well. Drift cell and cross regional scales are therefore most important for accurately defining nearshore ecological function, management, and restoration actions.

背景病态肥胖症（morbidobesity，MO）、阻塞性睡眠呼吸暂停综合征（obstructivesleepapnea，OSA）、颈围（neckcircumference，Nc）被广泛认为是困难气管插管的独立危险因素。在本研究中，我们试图确定这些因素是否和减肥手术患者困难气管插管风险增加相关。研究的预测性因素是OSA及其严重程度；决定严重程度的指标有呼吸暂停低通气指数（apnea—hypopneaindex，AHI）、性别、颈围和体重指数（bodymassindex，BMI）。方法所有入选的MO患者均进行了术前多导联睡眠监测。OSA的严重程度评估使用AHI和美国麻醉医师学会（AmericanSocietyofAnesthesiologists，ASA）OSA严重度分级。患者均采用标准麻醉操作，包括置于“倾斜位”直接喉镜检查。结果连续入选180例患者，包括140例女性和40例男性。OSA的发病率是68％。平均BMI为49，4kg／m2。平均AHI为31．3（范围，0～135）。所有患者均由麻醉住院医师在没有气道辅助设备帮助下成功插管。6例患者需要3次甚至3次以上插管尝试，困难气管插管比例占3．3％。困难喉镜镜检（定义为***分级3—4级）发生率是8．3％。困难插管与Nc之间（优势率1．02，95％可信区间0．93—1．1），与诊断为OSA之间（P=0．09），或与BMI之间（优势率0．99，95％可信区间0．92—1．06，P=0．8）均无关联。插管尝试的次数和BMI（P=0．8）、AHI（P=0．82）或Nc（P=0．02）也无相关性。Mallampati分级Ⅲ级以上（P=0．02）和男性患者（P=0．02）预示困难插管。cormack—Lehane分级和BMI（P=0．88）、AHI（P=0．93）或OSA（P=0．6）之间也无相关性。颈围增加与喉镜镜检困难相关，但与困难气管插管无关（P=0．02）。结论MO患者进行减肥手术置于“倾斜位”时，是否存在OSA及其严重程度、BMI、Nc与气管插管或喉镜镜检的困难度之间没有相关性。仅MaUampati分级3—4级或男性患者可预测困难气管插管。
