Undifferentiated high-grade pleomorphic sarcoma of the common bile duct:A case report and review of literature

BACKgROUND Undifferentiated pleomorphic sarcoma(UPS)is a rare malignant mesenchymal tumor with a poor *** mainly occurs in the extremities,trunk,head and neck,and retroperitoneum *** to the lack of specific clinical manifestations and imaging features,UPS diagnosis mainly depends on pathological and immunohistochemical examinations for exclusive *** we report an extremely rare case of high-grade UPS in the common bile duct(CBD).There are limited available data on such *** SUMMARY A 70-year-old woman was admitted to our department with yellow eyes and urine accompanied by upper abdominal distending pain for 2 *** laboratory data suggested significantly elevated hepatorenal function *** imaging data revealed calculous cholecystitis,intrahepatic and extrahepatic bile duct dilation with extrahepatic bile duct calculi,and a space-occupying lesion at the distal *** endoscopic biliary stenting and symptomatic support therapy,CBD exploration and biopsy were *** frozen section indicated malignant spindle cell tumor of the CBD mass,and further radical pancreaticoduodenectomy was ***,the neoplasm was diagnosed as a high-grade UPS combined with the light-microscopic morphology and immunohistochemical *** This extremely rare case highlighted the need for increasing physicians'vigilance,reducing the odds of misdiagnosis,and providing appropriate treatment strategies.

greater participation in global value chains(gVCs)has highlighted the impact of exchange rate shocks on international *** paper examines how such participation influences firms'responses to exchange rate movements,focusing on the relationship between firms'pricing and value-added in *** study,using detailed Chinese firm-level data,demonstrates that firms with a high degree of gVC participation reacted to currency appreciation by lowering their export prices more substantially and reducing their export volumes *** is mainly attributable to the"cost-hedging effect"within the marginal cost channel and the"pricing inhibition effect"within the markup *** categorizing export firms by trade models and product types,this study further demonstrates that processing trade firms at the low end of the value chain and those with low product differentiation were more inclined to absorb exchange rate *** study adds to the existing theoretical framework and provides strong evidence for China in deepening gVC integration and supporting the development of high-quality export firms.

Clinical study on microscopic syndrome differentiation and traditional Chinese medicine treatment for liver stomach disharmony in chronic gastritis

BACKgROUND Chronic gastritis(Cg)is a common gastrointestinal disorder characterized by inflammation of the stomach ***-stomach disharmony(LSD)syndrome is believed to contribute to Cg *** To evaluate the efficacy and safety of microcosmic syndrome differentiation and Chinese herbal medicine(CHM)treatment in patients with Cg and LSD *** Sixty-four patients with Cg and LSD syndrome were randomly divided into two groups:The treatment group received CHM based on microcosmic syndrome differentiation and the control group received conventional Western *** treatment course lasted 12 *** primary outcome was improvement in dyspeptic symptoms,measured using the Nepean Dyspepsia *** secondary outcomes included the improvement rate of endoscopic findings,histopathological findings,and microcosmic syndrome scores and the incidence of adverse *** After 12 wk of treatment,the treatment group showed significantly greater improvement in dyspeptic symptoms than the control group(93.75%vs 65.63%,Pgroup also showed a significantly higher improvement rate in endoscopic findings than the control group(81.25%vs 53.13%,Pgical findings and microcosmic syndrome scores were not significantly different between the two groups(P>0.05).No serious adverse events were observed in either *** Microcosmic syndrome differentiation and CHM treatment can effectively improve dyspeptic symptoms and endoscopic findings in patients with Cg and LSD syndrome and have a good safety *** studies with larger sample sizes and longer follow-up periods are required to confirm the long-term efficacy and mechanism of action of this treatment.

ground losses due to tunneling would induce settlement of nearby raft *** study the change in behavior of the raft foundations over time due to tunnel excavation in soft clay,a series of centrifuge model tests were *** results reveal that the raft stiffness has a significant influence on the development of the gap between the raft and the *** width of the gap beneath the flexible foundation would increase over time,leading to a further increase in tensile strain after excavation,whereas the gap for raft foundations with a large stiffness would reduce with time,causing a gradual decrease in tensile *** modification factor(MF)design approach is also evaluated with the test results and demonstrates that the MF design approach would underestimate the tensile strain of the flexible raft and provide a relatively conservative prediction for larger stiffnesses.

Parkinson’s disease(PD)is a complicated neurodegenerative disease,characterized by the accumulation ofα-synuclein(α-syn)in Lewy bodies and neurites,and massive loss of midbrain dopamine *** evidence suggests that gut microbiota and microbial metabolites are involved in the development of *** these,short-chain fatty acids(SCFAs),the most abundant microbial metabolites,have been proven to play a key role in brain-gut *** this review,we analyze the role of SCFAs in the pathology of PD from multiple dimensions and summarize the alterations of SCFAs in PD patients as well as their correlation with motor and non-motor *** research should focus on further elucidating the role of SCFAs in neuroinflammation,as well as developing novel strategies employing SCFAs and their derivatives to treat PD.

A method to interpret fracture aperture of rock slope using adaptive shape and unmanned aerial vehicle multi-angle nap-of-the-object photogrammetry

The aperture of natural rock fractures significantly affects the deformation and strength properties of rock masses,as well as the hydrodynamic properties of fractured rock *** conventional measurement methods are inadequate for collecting data on high-steep rock slopes in complex mountainous *** study establishes a high-resolution three-dimensional model of a rock slope using unmanned aerial vehicle(UAV)multi-angle nap-of-the-object photogrammetry to obtain edge feature points of *** opening morphology is characterized using coordinate projection and *** central axis is determined using vertical measuring lines,allowing for the interpretation of aperture of adaptive fracture *** feasibility and reliability of the new method are verified at a construction site of a railway in southeast Tibet,*** study shows that the fracture aperture has a significant interval effect and size *** optimal sampling length for fractures is approximately 0.5e1 m,and the optimal aperture interpretation results can be achieved when the measuring line spacing is 1%of the sampling *** fractures in the study area generally have larger apertures than shear fractures,and their tendency to increase with slope height is also greater than that of shear *** aperture of tensile fractures is generally positively correlated with their trace length,while the correlation between the aperture of shear fractures and their trace length appears to be *** of different orientations exhibit certain differences in their distribution of aperture,but generally follow the forms of normal,log-normal,and gamma *** study provides essential data support for rock and slope stability evaluation,which is of significant practical importance.

To better understand the failure behaviours and strength of bolt-reinforced blocky rocks,large scale extensive laboratory experiments are carried out on blocky rock-like specimens with and without rockbolt *** results show that both shear failure and tensile failure along joint surfaces are observed but the shear failure is a main controlling factor for the peak strength of the rock mass with and without *** rockbolts are necked and shear deformation simultaneously happens in bolt reinforced rock *** the joint dip angle increases,the joint shear failure becomes more *** number of rockbolts has a significant impact on the peak strain and uniaxial compressive strength(UCS),but little influence on the deformation modulus of the rock *** the Winkler beam model to represent the rockbolt behaviours,an analytical model for the prediction of the strength of boltreinforced blocky rocks is *** agreement between the UCS values predicted by proposed model and obtained from experiments suggest an encouraging performance of the proposed *** addition,the performance of the proposed model is further assessed using published results in the literature,indicating the proposed model can be used effectively in the prediction of UCS of bolt-reinforced blocky rocks.

Numerical Study on the Effect of gap Diffraction on the Hydrodynamic Performance of A Floating Breakwater

Two-dimensional(2D)flume experiments are useful in investigating the performances of floating breakwaters(FBs),including hydrodynamic performances,motion responses,and mooring *** a reasonable gap between the flume wall and the FBs is a critical step in 2D flume ***,research on the effect of the gap on the accuracy of 2D FB experimental results is *** address this issue,a numerical wave tank is developed using CFD to estimate the wave-FB interaction of a moored dual-cylindrical FB,and the results are compared to experimental data from a previously published *** is good agreement between them,indicating that the numerical model is sufficiently *** numerical model is then applied to explore the effect of gap diffraction on the performance of FBs in2D *** was discovered that the nondimensional gap length L_(gap)/W_(Pool)should be smaller than 7.5%to ensure that the relative error of the transmission coefficient is smaller than 3%.The influence of the gap is also related to the entering wave properties,such as the wave height and period.
