摘 要：Background It was difficult to predict the operative effect and outcome for severe traumatic brain injury(TBI) before *** present of active contrast extravasation at computed tomography angiography(CTA) can predicts in-hospital mortality in patients with nontraumatic intracerebral *** study aims to approve an evaluated method of CTA to predict the effect of operation and outcome in severe *** From 8760 patients with a TBI treated between January 2010 and January 2016,we retrospectively reviewed 358 severe TBI patients who presented to our emergency department and underwent CTA at admission and a follow-up CTA *** experienced readers,blinded to clinical data,reviewed the CTAs to assess for the presence of cerebrovascular changes included cerebrovascular shift(CS),cerebral vasospasm(CVS),large artery occlusion(LAO) and deep venous system occlusion(DVSO).Medical records were reviewed for baseline clinical characteristics and the relationship between cerebrovascular changes and operation effect and 6 months *** Cerebrovascular changes was identified in 197(55.0%) of 358 severe *** hundred seventeen(32.7%) had CS,and just 9(7.7%) of them died,63(53.8%) patients had good ***-four(21.0%) patients had CVS,31(41.9%) had a good outcome,25(33.8%) had severe disability and 18(24.3%) *** were twenty-five(7.0%) patients had LAO and thirty(8.4%) patients had DVSO,all these 55(100%) patients were died regardless whether they had *** Cerebrovascular injuries and changes are frequent after severe TBI,and correlates with prognosis *** is an important tool in evaluating the severity,predicting operation effect and prognosis,and guiding therapy of severe ***,this is a single-hospital retrospectively ***,well-designed,multicentre,randomized controlled trials needed to evaluate the value of the CTA for severe TBI.
