Tamsulosin Monotherapy Is Effective in Reducing Ureteral Stentrelated Symptoms:A Meta-analysis of Randomized Controlled Studies

This study aimed to evaluate the effectiveness of tamsulosin monotherapy for the treatment of ureteral stent-related symptoms(SRSs)and compare it with that of solifenacin monotherapy and combined therapy of tamsulosin and *** controlled trials(RCTs),which evaluated the effectiveness of tamsulosin for the treatment of SRSs,were searched from the databases PubMed,EMBASE and the Cochrane Library published up to November *** RCTs involving 1087 participants were finally included in this *** results showed that tamsulosin monotherapy could significantly decrease the urinary symptoms[mean difference(MD)—7.56,95%confidence interval(CI)(-11.47,-3.65),P=0.0001]and body pain[MD-5.25,95%CI(-8.03,-2.46),P=0.0002],and improve the sexual performance[MD-1.06,95%CI(-1.89,-0.24),P=0.01]compared with the control ***,there was no significant difference between tamsulosin monotherapy and solifenacin monotherapy in all outcomes except for significantly better sexual performance in solifenacin group[MD 0.29,95%CI(0.06,0.51),P=0.01].In addition,the effectiveness of combined therapy of tamsulosin and solifenacin was not superior to that of tamsulosin *** study demonstrated that tamsulosin monotherapy was effective for the treatment of patients with SRSs;evident superiority could not be found for therapy of tamsulosin and solifenacin combined.

BACKGROUND Left-sided portal hypertension(LSPH),also known as sinistral portal hypertension or regional portal hypertension,refers to extrahepatic portal hypertension caused by splenic vein obstruction or stenosis.N-butyl-2-cyanoacrylate(NBC)has been widely used in the endoscopic hemostasis of portal hypertension,but adverse events including renal or pulmonary thromboembolism,mucosal necrosis and gastrointestinal(GI)bleeding may occur after ***,we report successfully managing gastric variceal(GV)hemorrhage secondary to LSPH using modified endoscopic ultrasound(EUS)-guided selective NBC *** SUMMARY A 35-year-old man was referred to our hospital due to an upper GI *** revealed GV hemorrhage and computed tomography venography(CTV)confirmed *** patient requested endoscopic procedures and rejected surgical therapies including ***-guided selective NBC injections were performed and confluences of gastric varices were selected as the injection sites to reduce the injection ***“sandwich”method using undiluted NBC and hypertonic glucose was *** complications *** patient was followed up regularly after *** months later,the follow-up gastroscopy revealed firm gastric submucosa with no sign of NBC expulsion and the follow-up CTV showed improvements in *** recurrent GI hemorrhage was reported during this follow-up *** EUS-guided selective NBC injection may represent an effective and economical treatment for GV hemorrhage in patients with LSPH.

BACKGROUND： Estrogen is neuroprotective effects such as breast carcinoma, endometria but long-term estrogen treatment can induce side cancer, and stroke. However, phytoestrogen is neuroprotective without these side effects. OBJECTIVE： To study the effects of Ginsenoside Rgl on facial neurons and brain-derived neurotrophic factor （BDNF） expression in the facial nucleus in ovariectomized rats. DESIGN, TIME AND SETTING： The randomized, controlled animal experiments were performed at the Ultrasonic Institute, second affiliated hospital, chongqing medical university, China, from September 2007 to September 2008. MATERIALS： Ginsenoside Rgl （Sigma, USA）, rabbit anti-rat BDNF, Bcl-2, Bax antibodies, biotin-labeled goat anti-rabbit IgG （Boster, China）, and a TUNEL kit （Roche, Germany） were used in this study. METHODS： A total of 48 adult Sprague Dawley rats undergoing ovariectomy were randomly assigned into sham operation （n = 8）, model （n = 20）, and Ginsenoside Rgl （n = 20） groups. Facial nerve damage was induced by bilateral clamping of the facial nerve trunk. The bilateral facial nerve trunk was exposed in the sham operation group, with no clamping. Rats in the Ginsenoside Rgl group were intraperitoneally injected with 10 mg/kg per day Ginsenoside Rgl; other groups received 2 mL saline, once a day, for 14 days. MAIN OUTCOME MEASURES： Morphologic changes in neurons of the facial nucleus were observed following hematoxylin-eosin staining. Neuronal apoptosis was detected by TUNEL. Changes in ultrastructure of the facial nerve fibers were observed with a transmission electron microscope. Expression of BDNF, Bcl-2, and Bax protein was quantified by semiquantitative immunohistochemistry. RESULTS： At 3-14 days following facial nerve damage, Ginsenoside Rgl increased BDNF expression and the number of regenerated nerve fibers, and produced thicker myelin sheaths （P 〈 0.05）. BACKGROUND Multiple primary malignancies(MPMs)refer to more than one primary malignancy in the same or separate organs of the same patient,and MPMs are considered when different histological characteristics are detected in epidemiological ***,we report a case presumed to be primary pancreatic cancer with multiple liver metastases by positron-emission tomography/computed tomography(PET/CT)and confirmed to be synchronous liver and pancreatic MPMs by endoscopic ultrasound-guided fine-needle aspiration(EUSFNA).CASE SUMMARY A 50-year-old man was referred to our hospital due to abdominal discomfort for 2 *** CT at a local hospital revealed a pancreatic mass with multiple liver *** being transferred to our hospital,PET/CT confirmed all these lesions to have elevated metabolic activity,and therefore primary pancreatic cancer with multiple liver metastases was ***-guided liver aspiration unexpectedly found signet-ring cells with a high Ki-67 positive rate(20%),while EUS-guided pancreatic aspiration detected pancreatic neuroendocrine cells with a relatively low Ki-67 positive rate(1%).The final diagnosis from the multidisciplinary team was simultaneous liver and pancreatic *** patient returned to his local hospital for neoadjuvant chemotherapy and surgery,and he is still alive during the 6-mo postoperative *** Although rare,MPMs should be considered when treating pancreatic mass with suspected metastatic lesions,and EUS-FNA has proved minimally invasive and accurate.

BACKGROUND Hepatectomy is the main treatment for patients with hepatocellular carcinoma(HCC)and it has a high possibility for long-term cure *** the postoperative mortality and recurrence rates remain *** the long-term prognosis of HCC patients is strongly linked to liver function,preoperative assessment of liver function is very important for HCC *** To compare the predictive power of the modified Child-Pugh(MCP)and albumin-bilirubin(ALBI)grades for the long-term outcome of *** From January 2010 to June 2017,a total of 204 patients with HCC who underwent surgery at the second affiliated hospital of chongqing medical university were enrolled in this retrospective *** Cox regression analysis was used to determine the independent predictive factors of survival and *** area under the curve(AUC)was used to evaluate the discriminative performance of the MCP grade and ALBI grade to predict the postoperative overall survival(OS)time and recurrence-free survival(RFS)*** The median OS and RFS times were 44.0 mo(range:22.0-74.0 mo)and 22.0 mo(range:5.0-45.0 mo),*** median OS and RFS times of MCP grades 1,2,and 3 patients were 60.0,39.0,and 18.0 mo(P<0.001)and 36.0,15.0,and 7.0 mo(P<0.001),*** median OS and RFS times of ALBI grades 1,2,and 3 patients were 56.0,26.0,and 6.0 mo(P<0.001)and 25.0,10.0,and 3.0 mo(P=0.003),*** the MCP and ALBI grades were more accurate than the Child-Pugh grade for predicting long-term *** analysis demonstrated that for both predicting OS and RFS,the MCP grade performed better than the ALBI grade(AUC:0.642 vs 0.605 for OS;0.659 vs 0.594 for RFS).CONCLUSION The MCP grade is more accurate than the ALBI grade for predicting long-term outcome of patients with HCC.

AIM: To investigate the inhibitive effect and its possible mechanism of Cordyceps Sinensis (CS) on CCl4-plus ethanolinduced hepatic fibrogenesis in experimental ***: Rats were randomly allocated into a normal control group, a model control group and a CS group. The latter two groups were administered with CCl4 and ethanol solution at the beginning of the experiment to induce hepatic fibrosis. The CS group was also treated with CS 10 days after the beginning of CCl4 and ethanol administration. All control groups were given corresponding placebo at the same time. At the end of the 9th week, rats in each group were humanely sacrificed. Blood and tissue specimens were ***, radioimmunological, immunohistochemical and molecular biological examinations were used to determine the level change of ALT, AST, HA, LN content in serum and TGFβ1, PDGF, collagen Ⅰ and Ⅲ expression in tissue at either protein or mRNA level or both of ***: As compared with the model control group,serum ALr, AST, HA, and LN content levels were markedly dropped in CS group (86.0±34.4 vs224.3±178.9, 146.7±60.2vs272.6±130.1, 202.0±79.3 vs316.5±94.1 and 50.4±3.0vs 59.7±9.8, respectively, P＜0.05). Tissue expression of TGFβ1 and its mRNA, collagen I mRNA were also markedly decreased (0.2±0.14 vs1.73±1.40, 1.68±0.47 vs3.17±1.17,1.10±0.84 vs 2.64±1.40, respectively, P＜0.05). More dramatical drop could be seen in PDGF expression (0.87±0.43vs1.91±0.74, P＜0.01). Although there was no statistical significance, it was still strongly suggested that collagen Ⅲ mRNA expression was also decreased in CS group as compared with model control group (0.36±0.27 vs0.95±0.65,P=0.0615). In this experiment, no significant change could be found in PDGF mRNA expression between two groups (0.35±0.34 vs 0.70±0.46, P＞0.05).CONCLUSION: Cordyceps sinensis could inhibit hepatic fibrogenesis derived from chronic liver injury, retard the development of cirrhosis, and notably ameliorate the liver function.
