Recent years have witnessed a dynamic develop of Peking Union medical College(PUMC),which was founded in 1921,and chinese academy of medical sciences(CAMS),which was founded in 1956.In the new era of building a"Healthy China",General Secretary Xi Jinping has emphasized the importance of a profound understanding of the chinese civilization to let the world better know and understand China,and urged efforts to combine theories and practices to expound how to better uphold the chinese path,promote the chinese spirit,and pool together the chinese strength[1].As an active response,PUMC organized the first Conference of History of Medicine on May 20-21,2023[2],Based on the rich history of PUMC and with an attempt to explore history,inherit the traditional chinese culture,and promote the spirit of modern medicine,we issued the following call at the conference。

chinese medicine （CM） pattern diagnosis （Bianzheng or syndrome differentiation）, as another patient classification approach, has been incorporated with biomedicine diagnosis in clinical practice in China, and the clinical experience has proven that the integration of biomedicine and CM is better in the treatment of many diseases. CM pattern diagnosis is completely different from the diagnosis in biomedicine, and it must take an active role in innovation for medical sciences. This paper is trying to discuss how CM pattern diagnosis lead to innovations in basic research, clinical research and new drug discovery. Scientific basis of CM pattern in innovation of basic research, clinical efficacy and safety, CM pattern based clinical trial design, CM pattern and clinical practice guideline in innovation of clinical study, CM pattern based indication finding, and CM pattern based network pharmacology in innovation of new drug discovery have been discussed.

Immune cells play key roles in cancer and chronic inflammatory disease. A better understanding of the mechanisms and risks will help develop novel target therapies. At the 2017 International Workshop of the Chinese Academy of Medical Sciences Initiative for Innovative Medicine on Tumor Immunology held in Beijing, China, on May 12, 2017, a number of speakers reported new findings and ongoing studies on immune-related diseases such as cancer, fibrotic disease, diabetes, and others. A considerably insightful overview was provided on cancer immunity, tumor microenvironments,and new immunotherapy for cancer. In addition, chronic inflammatory diseases were discussed. These findings may offer new insights into targeted immunotherapy.

Medication Rule Analysis of the Diagnosis and Treatment Programs of chinese Medicine for the Prevention and Treatment of COVID-19 in China

Objectives:To summarize the measures and rules of chinese medicine(CM)and provide reference for clinical application in the prevention and treatment of coronavirus disease(COVID-19).Methods:The data source was from CM COVID-19 prevention and treatment programs on government websites and official media websites of the different provinces and cities.The search lasted from December 8,2019 to March 10,2020.Main variables were medication frequency and combinations of medicines.Cluster analysis and complex network analysis were used by prevention and treatment stage and by area.Results:Among 27 CM diagnosis and treatment plans,203 therapeutic prescriptions were enrolled,of which the top 4 herbs were:Radix glycyrrhizae,Semen armeniacae amarum,Herba ephedrae,and Herba agastachis,respectively.The core combinations were Herba ephedrae and Semen armeniacae amarum.Forty-eight preventive formulae were identified.Ten herbs,including Radix Astragali seu hedysari,Radix glycyrrhizae,Radix saposhnikoviae,Flos lonicerae,etc.were most frequently used.The core prescription of CM compatibility was Radix astragali seu hedysari,Radix glycyrrhizae,and Radix saposhnikoviae,which is the main component of Yu Ping Feng San.There were 45 prevention and treatment prescriptions in East China;the most used CM was Radix glycyrrhizae,Herba agastachis,Pericarpium citri reticulatae,and Gypsum fibrosum.Fifty prescriptions were identified in North China.According to CM analyses,Herba agastachis,Semen armeniacae amarum,Herba ephedrae,and Poria were most frequently used.Conclusions:CM for COVID-19 prevention mainly focuses on improving human immunity;for treatment,prescription focuses on clearing the lungs and removing dampness.Prescriptions vary with regions,perhaps due to climatic and environmental differences,which help clinicians to quickly make CM plans and treat patients according to clinical status,further minimizing resource wastage.

Renal interstitial fibrosis(RIF)is the crucial pathway in chronic kidney disease(CKD)leading to the end-stage renal failure.However,the underlying mechanism of Shen Qi Wan(SQW)on RIF is not fully understood.In the current study,we investigated the role of Aquaporin 1(AQP1)in SQW on tubular epithelial-to-mesenchymal transition(EMT).A RIF mouse model induced by adenine and a TGF-β1-stimulated HK-2 cell model were etablished to explore the involvement of AQP 1 in the protective effect of SQW on EMT in vitro and in vivo.Subsequently,the molecular mechanism of SQW on EMT was explored in HK-2 cells with AQP1 knockdown.The results indicated that SQW alleviated kidney injury and renal collagen deposition in the kidneys of mice induced by adenine,increased the protein expression of E-cadherin and AQP1 expression,and decreased the expression of vimentin andα-smooth muscle actin(α-SMA).Similarly,treatmement with SQW-containing serum significantly halted EMT process in TGF-β1 stimulated HK-2 cells.The expression of snail and slug was significantly upregulated in HK-2 cells after knockdown of AQP1.AQP1 knockdown also increased the mRNA expression of vimentin andα-SMA,and decreased the expression of E-cadherin.The protein expression of vimentin increased,while the expression of E-cadherin and CK-18 significantly decreased after AQP1 knockdown in HK-2 cells.These results revealed that AQP1 knockdown promoted EMT.Furthermore,AQP1 knockdown abolished the protective effect of SQW-containing serum on EMT in HK-2 cells.In sum,SQW attentuates EMT process in RIF through upregulation of the expression of AQP1.

Objective: To investigate the effects of salvianolic acid A(SAA) on cardiomyocyte apoptosis and mitochondrial dysfunction in response to hypoxia/reoxygenation(H/R) injury and to determine whether the Akt signaling pathway might play a role. Methods: An in vitro model of H/R injury was used to study outcomes on primary cultured neonatal rat cardiomyocytes. The cardiomyocytes were treated with 12.5, 25, 50 μg/m L SAA at the beginning of hypoxia and reoxygenation, respectively. Adenosine triphospate(ATP) and reactive oxygen species(ROS) levels were assayed. Cell apoptosis was evaluated by flow cytometry and the expression of cleavedcaspase 3, Bax and Bcl-2 were detected by Western blotting. The effects of SAA on mitochondrial dysfunction were examined by determining the mitochondrial membrane potential(△Ψm) and mitochondrial permeability transition pore(m PTP), followed by the phosphorylation of Akt(p-Akt) and GSK-3β(p-GSK-3β), which were measured by Western blotting. Results: SAA significantly preserved ATP levels and reduced ROS production. Importantly, SAA markedly reduced the number of apoptotic cel s and decreased cleaved-caspase 3 expression levels, while also reducing the ratio of Bax/Bcl-2. Furthermore, SAA prevented the loss of △Ψm and inhibited the activation of m PTP. Western blotting experiments further revealed that SAA significantly increased the expression of p-Akt and p-GSK-3β, and the increase in p-GSK-3β expression was attenuated after inhibition of the Akt signaling pathway with LY294002. Conclusion: SAA has a protective effect on cardiomyocyte H/R injury; the underlying mechanism may be related to the preservation of mitochondrial function and the activation of the Akt/GSK-3β signaling pathway.

Tyrosine-decahydrofluorene derivatives are a class of hybrid compounds that integrate the properties of polyketides and nonribosomal peptides.These compounds feature a[6.5.6]tricarbocyclic core and a para-cyclophane ether moiety in their structures and exhibit anti-tumor and anti-microbial activities.In this study,we constructed the biosynthetic pathway of xenoacremones from Xenoacremonium sinensis ML-31 in the Aspergillus nidulans host,resulting in the identification of four novel tyrosine-decahydrofluorene analogs,xenoacremones I–L(1-4),along with two known analogs,xenoacremones A and B.Remarkably,compounds 3 and 4 contained a 12-membered para-cyclophane ring system,which is unprecedented among tyrosine-decahydrofluorene analogs in X.sinensis.The successful reconstruction of the biosynthetic pathway and the discovery of novel analogs demonstrate the utility of heterologous expression strategy for the generation of structurally diverse natural products with potential biological activities.

How to Estimate the Mortality Risk of COVID-19:A New Approach with a Three-Factor Decomposition

Statistical analysis of COVID-19 mortality is challenging due to its non-stationarity and cross-sectional instability.In this paper,the authors introduce a unified method to evaluate the fatality rate of COVID-19 across countries,whose method provides more reliable information for cross-country comparison than the traditional case-fatality rate(CFR).It emerges that the new method,the blockwise case-fatality rate(BCFR),varies for different countries and in different periods.The authors also decompose the COVID-19 fatality data by three factors:1)The virus infection dynamics over population in different countries,2)pure distribution and evolution of instantaneous death rate attributed to different individual’s physical characteristics such as age and health,and 3)individual countries’variations affecting interactions between the virus infection and the instantaneous mortality due to individual’s physical characteristics.Based on the new three-factor model,the authors obtain six key findings of the COVID-19 fatality rate.Our study suggests that,on average,the estimated instantaneous fatality rate contributes about 57.0%to the global BCFR while the time-varying weight contributes about 41.5%in December 2020.The country-specific contribution of instantaneous fatality rate is significantly higher than that of the time-varying weight.Besides,the country-specific characteristics in demographical,social,and economic aspects would affect the relative severity of the disease.

As pancreatic cancer(PC)is highly malignant,its patients tend to develop metastasis at an early stage and show a poor response to conventional chemotherapies.First-line chemotherapies for PC,according to current guidelines,include fluoropyrimidine-and gemcitabine-based regimens.Accumulating research on drug resistance has shown that biochemical metabolic aberrations in PC,especially those involving glycolysis and glutamine metabolism,are highly associated with chemoresistance.Additionally,lipid metabolism is a major factor in chemoresistance.However,emerging compounds that target these key metabolic pathways have the potential to overcome chemoresistance.This review summarizes how PC develops chemoresistance through aberrations in biochemical metabolism and discusses novel critical targets and pathways within cancer metabolism for new drug research.
