Background:Anomalous aortic origin of a coronary artery(AAOCA)is a congenital heart disease with a 0.3%−0.5%*** is challenging due to nonspecific clinical *** stratification and treatment are currently based on expert consensus and single-center case ***:demographical and clinical data of AAOCA patients from 17 tertiary-care centers were *** imaging studies(Bidimensional echocardiography,coronary computed tomography angiography[CCTA]were *** correlations with anomalous coronary course and origin were ***:Data from 239 patients(42%males,mean age 15 y)affected by AAOCA were collected;154 had AAOCA involving the right coronary artery(AAORCA),62 the left(AAOLCA),23 other anomalies.211(88%)presented with an inter-arterial *** electrocardiogram(ECG)was abnormal in 37(16%).AAOCA was detected by transthoracic echocardiography and CCTA in 53%and 92%of patients,*** of the patients reported cardiac symptoms(119/239;50%),mostly during exercise in 121/178(68%).An ischemic response was demonstrated in 37/106(35%)and 16/31(52%)of patients undergoing ECG stress test and stress-rest single positron emission cardiac *** with AAORCA,patients with AAOLCA presented more frequently with syncope(18%vs.5%,P=0.002),in particular when associated with inter-arterial course(22%vs.5%,Presentations and low sensitivity of first-line cardiac screening *** seems to be strictly correlated to AAOLCA with inter-arterial course.

Sarcomatoid carcinomas or carcinosarcoma are rare tumors composed of mixed carcinoma cells and mesenchymal *** cases with colorectal involvement have been published to date. We report a case of sarcomatoid carcinoma of the colon in a 67-year-old woman hospitalized with a history of anemia and bloody stools. The patient underwent a left *** revealed two cell components (undifferentiated carcinomatous and sarcomatous; components). The patient died of her tumor 2 months after the operation. Our review of the literature stresses the poor prognosis associated with colonic sarcomatoid carcinoma.

Background: When the ductus arteriosus (DA) is patent, the ductal shunt is proportional to the ratio of left ventricular output (LVO) to systemic blood flow. Systemic blood flow can be estimated by measuring flow in the superior vena cava (SVC). Objective: To re-evaluate the accuracy of standard echocardiographic markers of patent ductus arteriosus (PDA) using LVO/SVC flow ratio. Methods: Prospective study. Preterm infants of 24-30 weeks gestational age and postnatal age less than 48 hours. The following echocardiographic criteria were measured: left atrial to aortic root ratio (la/Ao); DA diameter by B mode and colour Doppler; mean and end diastolic flow velocity of the left pulmonary artery (LPA); LVO; SVC flow. results: Twenty three preterm infants were enrolled (median gestational age 28 weeks (range 24-30), median birth weight 840 g (500-1440)). The DA was closed in eight (mean (SD)-LVO/SVC 2.4 (0.3)) and open in 15 (mean (SD) LVO/SVC 4.5 (0.6)). An la/Ao ratio ≥ 1.4, a DA diameter ≥ 1.4 mm/kg, and a mean and end diastolic flow velocity of LPA respectively ≥ 0.42 and ≥ 0.20 m/s identified an LVO/SVC ≥ 4 with a sensitivity and a specificity above 90% . Conclusion: This study indicates that la/Ao ratio, DA diameter, and mean and end diastolic flow velocity of the LPA are accurate markers of PDA. These standard echocardiographic variables are easy to measure and need less skill and resources than direct measurements of ductal shunt.

. The aim of this study was to evaluate the impact of the surgical approach on the management and outcomes of patients with early borderline ovarian tumors (BOTs). Material and methods. We retrospectively reviewed the medical charts of patients with stage Ia to Ic BOT treated surgically between January 1, 1985, and december 31, 2001. We compared patients initially managed by laparoscopy vs laparotomy in terms of potentially harmful procedures and quality of staging. results. Of the 118 included patients, 48 (41% ) had laparoscopy for initial surgery, 54 (45% ) had laparotomy, and 16 (14% ) had conversion from laparoscopy to laparotomy. Conservative treatment (57% of patients) was more common with laparoscopy (vs laparotomy, P der than 44 years (vs younger than 44 years, P re occurred in 9% of patients and was not associated with the surgical approach (P = 0.1). Bag extraction was used in 19 (40% ) of the 48 laparoscopy patients. Staging was incomplete in 73% of patients overall. By univariate analysis, better quality of staging was associated with bilateral adnexectomy, age >44 years, laparotomy, hysterectomy, and treatment after 1995. By multivariate analysis, bilateral adnexectomy or hysterectomy was associated with better staging. Mean follow-up was 40 months, during which recurrence and survival rates were similar in the laparoscopy and laparotomy groups. Conclusion. Staging of macroscopic early stage BOTs was better in patients requiring radical surgery. After adjustment on disease severity, type of surgical access was not related to staging quality.

Aortic size index(ASI) has been proposed as a reliable criterion to predict risk for aortic dissection in Turner syndrome with significant thresholds of 20-25 mm/m2. We report a case of aortic arch dissection in a patient with Turner syndrome who, from the ASI thresholds proposed, was deemed to be at low risk of aortic dis-section or rupture and was not eligible for prophylactic surgery. This case report strongly supports careful monitoring and surgical evaluation even when the ASI is re present.
