Introduction:Pharmacologic dilation of the pupil results in twice the sensitivity of detection of diabetic retinopathy compared with undilated retinal examination.The potential risk of acute angle-closure glaucoma after pupil dilation has been hypothesized to be higher in Asian patients with diabetes mellitus.Clinical case:A 61-year-old man with diabetes mellitus and hypertension was incidentally found to have elevated intraocular pressure(IOP)before routine retinal photography.He was asymptomatic and the visual acuity was 0.67 for both eyes.An ophthalmologist later found he had anatomical narrow-angle borderline glaucoma.Topical administration of pilocarpine and oral administration of acetazolamide were initiated,and laser iridotomy was later performed.IOP screening:Among 1736 diabetic and/or hypertensive patients who underwent IOP screen-ing,31 patients(1.8%)had IOP of any eye persistently higher than 21 mm Hg on at least two occa-sions.The mean(standard deviation)IOP of the right eye was 24.1(2.1)mm Hg,while that of the left eye was 24.6(2.5)mm Hg.Four patients(12.9%)were found to have glaucoma,and treatment was initiated by an ophthalmologist.Therefore further study should be conducted to evaluate the cost-effectiveness of IOP screening among this group of patients.

Introduction: Wise prescription of antibiotics is an ethical duty of physicians in view of rising antimicrobial resistance in the community, it should be balanced between the health requirements of the patients and resulting long-term antibiotics resistance. Overuse of antimicrobials is a major cause of emerging resistance to antimicrobials. There are multiple factors in the community that influence the physician’s antibiotic prescriptions. Methods: This is a systematic case-control study on antibiotics prescription for paediatric patients attending Latifa hospital for Women and Children (LWCH), Dubai Health authority, to know the effects of behavioral interventions on rates of inappropriate antimicrobials prescription by doctors in the Paediatric Emergency Department. Results: The results of our study showed the effectiveness of behavioral insights by peer comparison in antibiotic use among paediatricians in Latifa hospital had a statistical significance (P = 0.0038). The rate of the prescription decreased from 41% to 21%, a difference of 20%. Conclusion: The study concluded behavioural intervention is an effective measure in reducing the improper prescription of antibiotics in the hospital setting.

Objective:To explore the effect of bisoprolol in combined with trimetazidine on the cardiac function rehabilitation in patients with chronic heart failure (CHF).Methods: A total of 84 patients with CHF who were admitted in our hospital from November, 2015 to October, 2016 were included in the study and randomized into the observation group and the control group. The patients in the two groups were given oxygen inhalation, lipid regulation, diuresis, ARB, and other routine treatments. Digitalis preparations were given for those who had poor cardiac function, and bisoprolol were orally administrated in the same time. On the above basis, the patients in the observation group were given trimetazidine dihydrochloride tablets, continuously for 3 months. The morning fasting peripheral venous blood before and after treatment in the two groups was collected. CRP, IL-6, TNF-α, BNP, NE, AngⅡ, ANP, ALD, and ET were detected. The cardiac color Doppler ultrasound diagnostic apparatus was used to detect LVPWT, PWS, PWD, IVSS, and IVMI.Results: CRP, IL-6, TNF-α, and BNP levels after treatment in the observation group were significantly lower than those in the control group. NE, AngⅡ, ANP, ALD, and ET levels after treatment in the observation group were significantly lower than those in the control group. LVPWT, PWS, PWD, IVSS, and IVMI levels after treatment in the observation group were significantly lower than those in the control group.Conclusions: Bisoprolol in combined with trimetazidine can significantly reduce the inflammatory reaction in patients with CHF, and effectively regulate the neuroendocrine stability in order to reverse or reduce VR and improve the left ventricular function.

BACKGROUND: Cirrhotic Cardiomyopathy is a relatively ill-characterized condition, which is often under-diagnosed due to absence of defined diagnostic criteria. ECG showing corrected QT Interval prolongation is the most suitable available option for diagnosis of this condition. OBJECTIVE: To determine the frequency of corrected QT interval prolongation in patients with liver cirrhosis. METHODOLOGY: Patients (n = 166) with confirmed cirrhosis, 30 years or older, presented in the outpatient and emergency department of medicine at Capital hospital Islamabad between 1 October 2011 and 30 September 2012, were enrolled in this cross-sectional study after taking consent. ECG was done using calibrated ECG machine, and the QT Interval was measured. Corrected QT was calculated using Bazett’s formula and a QTc of more than 0.44 seconds was considered as being prolonged. RESULTS: The mean age of the patients was 57.05 ± 12.03 years. The corrected QT Interval varied from 337 ms to 560 ms. The mean QTc Interval was 429.92 ms ± 45.11. QTc was prolonged in 41 out of 166 patients (24.7%). Frequency of QTc prolongation was 4.5% in Child Pugh Grade A, 23.2% in Child Pugh Grade B, and 32.0% in Child Pugh Grade C. Association of Child Pugh Scoring with QTc prolongation was determined and found to be statistically significant (P < 0.05). CONCLUSION: QTc interval was prolonged in 24.7% of cirrhotic patients in our study. There was a significant increase in frequency with worsening of Child Pugh Grade, thereby indicating an association between QTc prolongation and the severity of cirrhosis.

Clinical and Pathological Comparison of Pyogenic and Amoebic Liver Abscesses

BACKGROUND: Pyogenic and amoebic liver abscesses are rare, potentially lethal conditions. In this study, we aimed to examine the clinical and pathological differences between them. METHODS: Patients with confirmed liver abscesses were divided into two groups: the pyogenic (n = 47) and amoebic group (n = 21), which were analyzed for differences in clinical and laboratory findings. RESULTS: Amoebic liver abscesses presented most frequently in young adults (14 - 30 years;71%), whereas pyogenic liver abscesses were most commonly observed in adults 41 - 50 years (49%). Indirect hemagglutination test revealed a 100% positive response in the amoebic group, whereas 68% of the pyogenic group presented with blood/pus culture. Multiple abscesses were observed in 66% and 24% of patients in the pyogenic and amoebic group, respectively. CONCLUSIONS: Pyogenic abscesses were commonly observed in older patients, and were associated with features such as markedly deranged liver function test, higher prothrombin time, and multiple abscesses, compared to amoebic abscess. Early and improved diagnoses and differentiation between the two conditions, followed by the correct treatment, can help prevent serious complications and lead to an overall improved mortality rate.
