Metabolomics signature of blood pressure salt sensitivity and its link to cardiovascular disease:A dietary salt-intervention trial

Background: Our study aimed to examine cardiovascular mortality within the working-age population, exploring epidemiological, clinical, and paraclinical features, complications, and identifying etiological factors linked to mortality. Methods: We conducted a descriptive and analytical retrospective study from September 2019 to August 2022 at the General Hospital Idrissa POUYE in Dakar, we reviewed all the medical records of patients from 15 to 60 years old who died while admitted in the cardiology department. Data collected were socioeconomic status, clinical history, type of cardiovascular disaese, length of hospitalization, circumstances and timing of death. The data were analyzed with R. Studio version 2022.12.0 + 353 and Excel 2019, with a P-value Results: The study included 73 patients, indicating a specific mortality rate of 8.8% and a proportional mortality of 39%. Predominantly male (sex ratio 1.2), the average age was 44. Key cardiovascular risk factors identified were sedentarism (76.7%), hypertension (28.8%), and smoking (21.9%). The leading cause for consultation was dyspnea (72.6%). Notable findings included a majority of patients presenting with general condition deterioration (90%) and cardiovascular collapse upon admission (23.3%). Physical exam revealed signs of heart failure in 63%. Echocardiography showed left ventricular ejection fraction impairment (81%) and pulmonary hypertension (78%). Immediate causes of death were primarily cardiogenic shock (45.2%) and septic shock (37%). The analytical study indicates that the data most closely associated with mortality were age, socio-economic level, ischemic heart disease (p = 0.034), rheumatic valvulopathies, pulmonary embolism (p = 0.034), hypertension (HTA) (p = 0.009), smoking (p = 0.011), diabetes (p = 0.011), dyslipidemias, prolonged bedrest (p = 0.001), morbid obesity (p = 0.001), and COVID-19 infection (p = 0.017). Objective To systematically summarize the published literature on the genetic variants associated with nonalcoholic fatty liver disease(NAFLD).Methods Literature from Web of Science,PubMed,and Embase between January 1980 and September 2022 was systematically ***-analyses of the genetic variants were conducted using at least five data *** epidemiologic credibility of the significant associations was graded using the Venice *** Based on literature screening,399 eligible studies were included,comprising 381 candidate gene association,16 genome-wide association,and 2 whole-exome sequencing *** identified 465 genetic variants in 173 genes in candidate gene association studies,and 25 genetic variants in 17 genes were included in the *** meta-analysis identified 11 variants in 10 genes that were significantly associated with NAFLD,with cumulative epidemiological evidence of an association graded as strong for two variants in two genes(HFE,TNF),moderate for four variants in three genes(TM6SF2,GCKR,and ADIPOQ),and weak for five variants in five genes(MBOAT7,PEMT,PNPLA3,LEPR,and MTHFR).Conclusion This study identified six variants in five genes that had moderate to strong evidence of an association with NAFLD,which may help understand the genetic architecture of NAFLD risk.

Background: Although maternal mortality is declining in most countries, it remains a significant public health problem worldwide, with high rates, particularly in developing and insecure countries like ours. Objective: To study the epidemiological factors and factors associated with the occurrence of maternal death in the Gynecology-Obstetrics department of University Hospital of Tengandogo. Method: It was a retrospective case-control study with a descriptive and analytical purpose over a period of 6 years from January 1, 2017 to December 31, 2022. Cases were women with maternal deaths during the study period. Data processing and analysis were performed using Stata version 13 software. Univariate and multivariate analyses were performed with Stata version 13 software, and logistic regression modeling was used to estimate crude and adjusted odds ratios (OR), their 95% confidence intervals (CI), and the threshold for statistical significance was set at a p value < 0.05. Results: A total of 372 patients were included in the study, including 146 cases of maternal death. The in-hospital maternal mortality rate was 1933 deaths per 100,000 live births. The average age was 28.5 years. 58.9% of patients lived in rural areas. Married patients accounted for 88.7% of cases. The average parity was 3. Direct obstetrical causes were the main causes of death, accounting for 72.6%. They were dominated by post-partum hemorrhage (24.2%), puerperal infection (18.6%), pre-eclampsia/eclampsia (16.1%) and retroplacental hematoma (8.9%). Chronic anemia (12.9%) was the main indirect obstetric cause. Risk factors associated with maternal death were primiparity (OR for paucigravida and multigravida at 0.05;P = 0.001);ambulance transport (OR for patients referred and brought in by personal vehicle = 0.3, p < 0.001) and vaginal delivery (OR for cesarean deliveries = 0.4, p < 0.001). During the COVID-19 pandemic,over one thousand papers were published on“Susceptible-Exposed-Infectious-Removed”(SEIR)epidemic computational *** English word“exposed”in its vernacular and public health usage means a state of having been in contact with an infectious individual,but not necessarily *** contrast,the term“exposed”in SEIR modeling usage typically stands for a state of already being infected but not yet being infectious to others,a state more properly termed“latently infected.”In public health language,“exposed”means possibly infected,yet in SEIR modeling language,“exposed”means already *** paper retraces the conceptual and mathematical origins of this terminological disconnect and concludes that epidemic modelers should consider using the“SLIR”notational short-hand(L for Latent)instead of SEIR.

Healthy life expectancy with cardiovascular disease among Chinese rural population based on the prospective cohort study

BACKGROUND Limited research has explored the impact of cardiovascular disease(CVD)on healthy life expectancy(HLE)especially in resource-limited *** study aimed to investigate the association between CVD and HLE in Chinese rural *** This study included 11,994 participants aged 45 years and older from the baseline and follow-up surveys of the Henan rural cohort *** status was measured via a Visual Analogue *** multistate Markov model was applied to estimate the association between CVD and transitions in health,unhealthiness and ***-specific total life expectancy,HLE and unhealthy life expectancy were calculated by the multistate life table *** During a mean follow-up time of 3.85(3.84–3.86)years,there were 588 deaths *** individuals with CVD,the risk of switching from health to unhealthiness status was increased by 71%[hazard ratio(HR)=1.71,95%CI:1.42–2.07],the chance of recovery was reduced by 30%(HR=0.70,95%CI:0.60–0.82).Men aged 45 years without CVD could gain an extra 7.08(4.15–10.01)years of HLE and lose 4.00(1.60–6.40)years of unhealthy life expectancy compared to their peers with CVD,*** corresponding estimates among women were 8.62(5.55–11.68)years and 5.82(2.59–9.04)years,*** This study indicated that CVD was significantly associated with poorer health status and lower HLE among Chinese rural *** is an important public health policy to adopt targeted measures to reduce the CVD burden and enhance the quality of life and HLE in resource-limited areas.

Counterfactual analysis of the 2023 Omicron XBB wave in China

Background:China has experienced a COVID-19 wave caused by Omicron XBB variant starting in April *** aim is to conduct a retrospective analysis exploring the dynamics of the outbreak under counterfactual scenarios that combine the use of vaccines,antiviral drugs,and nonpharmaceutical ***:We developed a mathematical model of XBB transmission in China,which has been calibrated using SARS-CoV-2 positive rates per *** age-specific infection-hospitalization risk,infection-ICU risk,and infection-fatality risk were used to estimate disease burdens,characterized as number of hospital admissions,ICU admissions,and ***:We estimated that in absence of behavioral change,the XBB outbreak in spring 2023 would have resulted in 0.86 billion infections(∼61%of the total population).Our counterfactual analysis shows that the synergetic effect of vaccination(70%vaccination coverage),antiviral treatment(20%receiving antiviral treatment),and moderate nonpharmaceutical interventions(20%isolation and L1 PHSMs)could reduce the number of deaths to levels close to seasonal influenza(1.17 vs.0.65 per 10,000 individuals and 5.85 vs.3.85 per 10,000 individuals aged 60+,respectively).The maximum peak prevalence of hospital and ICU admissions are estimated to be lower than the corresponding capacities(8.6 vs.10.4 per 10,000 individuals and 1.2 vs.2.1 per 10,000 individuals,respectively).Conclusion:Our findings suggest that the capacity of the Chinese healthcare system was adequate to face the Omicron XBB wave in spring 2023 but,at the same time,supports the importance of administering highly effective vaccine with long-lasting immune response,and the use of antiviral treatments.

Rocky Mountain spotted fever(RMSF)is a fatal tick-borne zoonotic disease that has emerged as an epidemic in western North America since the turn of the 21st *** the US south-western border and across northern Mexico,the brown dog tick,Rhipicephalus sanguineus,is responsible for spreading the disease between dogs and *** widespread nature of the disease and the ongoing epidemics contrast with historically sporadic patterns of the *** dogs are amplifying hosts for the Rickettsia rickettsii bacteria,transmission dynamics between dogs and ticks are critical for understanding the *** this paper,we developed a compartment metapopulation model and used it to explore the dynamics and drivers of RMSF in dogs and brown dog ticks in a theoretical region in western North *** discovered that there is an extended lag—as much as two years—between introduction of the pathogen to a naïve population and epidemic-level transmission,suggesting that infected ticks could disseminate extensively before disease is detected.A single large city-size population of dogs was sufficient to maintain the disease over a decade and serve as a source for disease in surrounding smaller *** model is a novel tool that can be used to identify high risk areas and key intervention points for epidemic RMSF spread by brown dog ticks.

Effectiveness of colorectal cancer screening integrating non-genetic and genetic risk: a prospective study based on UK Biobank data

Objective: Few studies have evaluated the benefits of colorectal cancer(CRC) screening integrating both non-genetic and genetic risk factors. Here, we aimed to integrate an existing non-genetic risk model(QCancer-10) and a 139-variant polygenic risk score to evaluate the effectiveness of screening on CRC incidence and ***: We applied the integrated model to calculate 10-year CRC risk for 430,908 participants in the UK Biobank, and divided the participants into low-, intermediate-, and high-risk groups. We calculated the screening-associated hazard ratios(HRs) and absolute risk reductions(ARRs) for CRC incidence and mortality according to risk ***: During a median follow-up of 11.03 years and 12.60 years, we observed 5,158 CRC cases and 1,487 CRC deaths, respectively. CRC incidence and mortality were significantly lower among screened than non-screened participants in both the intermediateand high-risk groups [incidence: HR: 0.87, 95% confidence interval(CI): 0.81±0.94;0.81, 0.73±0.90;mortality: 0.75, 0.64±0.87;0.70, 0.58±0.85], which composed approximately 60% of the study population. The ARRs(95% CI) were 0.17(0.11±0.24) and 0.43(0.24±0.61), respectively, for CRC incidence, and 0.08(0.05±0.11) and 0.24(0.15±0.33), respectively, for mortality. Screening did not significantly reduce the relative or absolute risk of CRC incidence and mortality in the low-risk group. Further analysis revealed that screening was most effective for men and individuals with distal CRC among the intermediate to high-risk ***: After integrating both genetic and non-genetic factors, our findings provided priority evidence of risk-stratified CRC screening and valuable insights for the rational allocation of health resources.
