Intestinal macrophages play crucial roles in both intestinal inflammation and immune homeostasis. They can adopt two distinct phenotypes, primarily determined by environmental cues. These phenotypes encompass the classically activated pro-inflammatory M1 phenotype, as well as the alternatively activated anti-inflammatory M2 phenotype. In regular conditions, intestinal macrophages serve to shield the gut from inflammatory harm. However, when a combination of genetic and environmental elements influences the polarization of these macrophages, it can result in an M1/M2 macrophage activation imbalance, subsequently leading to a loss of control over intestinal inflammation. This shift transforms normal inflammatory responses into pathological damage within the intestines. In patients with ulcerative colitis-associated colorectal cancer (UC-CRC), disorders related to intestinal inflammation are closely correlated with an imbalance in the polarization of intestinal M1/M2 macrophages. Therefore, reinstating the equilibrium in M1/M2 macrophage polarization could potentially serve as an effective approach to the prevention and treatment of UC-CRC. This paper aims to scrutinize the clinical evidence regarding chinese medicine (CM) in the treatment of UC-CRC, the pivotal role of macrophage polarization in UC-CRC pathogenesis, and the potential mechanisms through which CM regulates macrophage polarization to address UC-CRC. Our objective is to offer fresh perspectives for clinical application, fundamental research, and pharmaceutical advancement in UC-CRC.

Calycosin improves atherosclerosis by reshaping the interaction between the gut microbiome and bile acid metabolism

Calycosin， Astragali Radix most prominent ingredient， has drawn more attention as a result of its ability to treat atherosclerosis(AS). However， the mechanism of action has not been fully elucidated. We investigated the effects of calycosin on bile acid (BA) metabolism and gut microbiome in ApoE-/-mice fed a high-fat diet (HFD). The data showed that the aorta of ApoE-/-mice treated with HFD showed significant atheromatous plaque formation and lipid accumulation， and the levels of TC， TG and LDL-C were significantly increased， while the levels of HDL-C were significantly decreased. Calycosin can substantially regulate lipid levels， thereby alleviating liver lipid deposition induced by atherosclerosis. In addition， 16S rRNA sequencing showed that calycosin treatment has reshaped the gut microbiota disturbed by HFD， in particular， increasing the ratio of Bacteroidetes/Firmicutes， and improving the relative abundance of Bilophila， Desulfovibrio， Bacteroides， Lactobacillus， etc. Meanwhile， targeted metabolomics analysis showed that calycosin treatment significantly modulated CDCA， TCA， LCA， DCA， TDCA and bile acid pool composition， which were associated with atherosclerotic plaque areas. In addition， calycosin treatment also down-regulated FXR protein levels and up-regulated CYP7A1 protein levels in the hepatic. At the same time， calycosin inhibits the ileum FXR/TGR5 signaling pathway， inhibits BA reabsorption， promotes BA excretion， and reduces hepatic cholesterol accumulation by enterohepatic circulation. In addition， we found that calycosin significantly promoted the expression of hepatic ABCA1 and ABCG1 to mediate cholesterol efflux. Meanwhile， calycosin regulates gut microbiota， and Bacteroides， Alistipes， Desulfovibrio， Lactobacillus， Bilophila and Odoribacter are closely related to specific BAs. BACKGROUND Partial splenic embolization(PSE)has been suggested as an alternative to splenectomy in the treatment of ***,some patients may experience recurrence of hypersplenism after PSE and require ***,there is a lack of evidence-based medical support regarding whether preoperative PSE followed by splenectomy can reduce the incidence of *** To investigate the safety and therapeutic efficacy of preoperative PSE followed by splenectomy in patients with cirrhosis and *** Between January 2010 and December 2021,321 consecutive patients with cirrhosis and hypersplenism underwent splenectomy at our *** on whether PSE was performed prior to splenectomy,the patients were divided into two groups:PSE group(n=40)and non-PSE group(n=281).Patient characteristics,postoperative complications,and follow-up data were compared between *** score matching(PSM)was conducted,and univariable and multivariable analyses were used to establish a nomogram predictive model for intraoperative bleeding(IB).The receiver operating characteristic curve,Hosmer-Lemeshow goodness-of-fit test,and decision curve analysis(DCA)were employed to evaluate the differentiation,calibration,and clinical performance of the *** After PSM,the non-PSE group showed significant reductions in hospital stay,intraoperative blood loss,and operation time(all P=0.00).Multivariate analysis revealed that spleen length,portal vein diameter,splenic vein diameter,and history of PSE were independent predictive factors for IB.A nomogram predictive model of IB was constructed,and DCA demonstrated the clinical utility of this *** groups exhibited similar results in terms of overall survival during the follow-up *** Preoperative PSE followed by splenectomy may increase the incidence of IB and a nomogram-based prediction model can predict the occurrence of IB.
