In 220 consecutive patients with non-obstructive azoospermia, sperm retrieval was attempted by a combination of.conventional and microdissection testicular sperm extraction （TESE）. f.r sperm retrieval, 2-3 conventional biopsies were perf.rmed f.llowed by a microdissection TESE in cases of.negative conventional biopsies. During the surgery, the vasculature of.the testis was assessed using the operative microscope, and the location of.positive biopsies was registered in relation to the blood supply. The overall sperm retrieval rate was 58.2%. f.om the initial conventional biopsies, sperm could be retrieved in 46.8% of.the patients. With microdissection TESE, sperm could be retrieved f.om an additional 11.4% of.the patients. The f.rther use of.microdissection TESE improved the sperm retrieval rate signif.cantly （P=0.017）. No signif.cant accumulation of.positive biopsies was f.und towards the rete testis or the main testicular vessels.

There are f.w reported cases in the prostate adenocarcinoma literature with locoregional extension and normal prostate-specif.c antigen (PSA). We present the case of.a patient with prostate adenocarcinoma and metastases with PSA values within the normal range, despite evidence of.clinical and radiological progression of.the disease. This is a 72-year-old patient presenting with a dysectatic syndrome in which there is a budding ulceration of.the glans, an induration of.the corpora cavernosa and whose rectal examination has objectif.ed a malignant prostate. PSA was assayed at 3.47 ng/ml;prostatic biopsy and magnetic resonance imaging (MRI) revealed prostate cancer with extension to the urethral, cavernous and rectal areas. Hormone therapy was instituted and the course af.er two months was marked by the occurrence of.several episodes of.anemia treated by blood transf.sion.

Background: Comorbidities are additive diseases and care burdens in urological patients. Determining the epidemiologic prof.le of.comorbidities in urological patients in our setting may help us to better the management of.urological disease. Objective: To evaluate the prevalence of.comorbidities in urological patients. Patient and Method: We collected comorbidity, urological disease and demographic data in all urological patients managed at the f.rmer Military Teaching Hospital of.Cotonou f.om January 1, 2012, to December 31, 2020. We used the sof.ware R 4.2.2 to perf.rm descriptive and bi-varied data analysis. Student’s t test was used to compare means. Results: The prevalence of.comorbidities was 14.2%, i.e., 601 comorbidity-af.ected among 4242 patients. The comorbidities predominantly af.ected men: the sex ratio was 13:1. The presence of.comorbidity was correlated with patients’ age (p f.ected patients were hypertension (84.5%), diabetes (26.5%), asthma (2%), and heart f.ilure (1.2%). Hypertension (p = 0.001) and asthma (p = 0.030) were correlated with age. No comorbidity was associated with gender. The comorbidities’ prevalence was highest in patients aged 40 - 80 years who presented urological diseases such as BPH (68.9%), Erectile dysf.nction (ED) and ejaculatory disorders, overactive bladder (OAB) and neurogenic lower urinary tract dysf.nction (LUTD), renal cyst (5%), inguinal hernia (4.2%), urinary stones (2.8%), and prostate cancer (2.3%). Conclusion: The comorbidities’ prevalence was 14.2% in the urological patients. The main comorbidities were hypertension (84.5%) and diabetes mellitus (26.5%).

Proteoglycans and collagen molecules are interacting with each other thereby f.rming various connective tissues. The sulf.tion pattern of.proteoglycans dif.ers depending on the kind of.tissue and/or the degree of.maturation. Tissues f.om Cnidaria are suitable examples f.r exploration of.the ef.ects in relation to the presence and the absence of.sulf.te groups, when studying characteristic f.agments of.the long proteoglycan carbohydrate chains in silico. It has been described that a non-sulf.ted chondroitin appears as a scaf.old in early morphogenesis of.all nematocyst types in Hydra. On the other hand, sulf.ted glucosaminoglycans play an important role in various developmental processes of.Cnidaria. In order to understand this biological phenomenon on a sub-molecular level we have analysed the structures of.sulf.ted and non-sulf.ted proteoglycan carbohydrate chains as well as the structure of.diverse collagen molecules with computational methods including quantum chemical calculations. The strong interactions between the sulf.te groups of.the carbohydrates moieties in proteoglycans and positively charged regions of.collagen are essential in stabilizing various Cnidaria tissues but could hinder the nematocyst f.rmation and its proper f.nction. The results of.our quantum chemical calculations show that the sulf.tion pattern has a signif.cant ef.ect on the conf.rmation of.chondroitin structures under study.

The Impact of.Body Mass on Male f.rtility in a Cohort of.127 Patients

Background: Aromatase and leptin are two adipose tissue cytokines. The f.rmer converts androgens into estrogens and stimulates adipogenesis. The latter cannot f.lly stimulate GnRH release as its hypothalamic receptors are reduced in obese men. Thus, obesity which is associated with an adipose tissue increment can interf.re with male f.rtility. Objective: We aim to study the correlation between the body mass index (BMI) of.an individual and the quality of.semen he produces. Patients and Method: By means of.the sof.ware R 4.2.1 we perf.rmed a retrospective analysis of.the relationship between the BMI and the semen alterations in the patients managed at the f.rmer Military Teaching Hospital of.Cotonou f.om October 1, 2017, to September 30, 2022: a bi-varied analysis and f.scher’s exact test (signif.cance threshold 5%, conf.dence interval 95%) f.llowed by a logistic regression when a non-signif.cant p-value is below 0.20. Results: 127 males managed f.r inf.rtility (mean age = 36.2 years) were recorded, including 11.1% obese (BMI > 30 kg/m2) and 36.5% overweighted (25 kg/m2 2). The most f.equent semen alterations were: oligoasthenospermia (27.8%), asthenospermia (22.2%), oligoasthenoteratospermia (14.3%), azoospermia (13.5%) and asthenoteratospermia (9.5%). Bi-varied analysis showed no correlation between the BMI and the semen alterations (p-value ranged f.om 0.086 to 0.9) and no dif.erence in semen alterations between patients with BMI below and above 25 kg/m2 (p-value ranged f.om 0.12 to 0.9). Logistic regression demonstrated that asthenoteratospermia were correlated with BMI ≥ 25 kg/m2 [OR = 2.1, 95% CI (1.50 - 2.70), p = 0.021]. Conclusion: Male obesity and overweight can trigger asthenoteratospermia.

Background: Urological care has been advancing quickly over the last ten years in Benin. In order to conveniently support that trend towards better urological care standards, decision-makers need sound data on urological diseases in the country. Objective: To determine the prevalence of.urological diseases in the f.rmer Military Teaching Hospital of.Cotonou. Patients and Method: We retrospectively collected the urological diseases that the institution had managed f.om January 2012 to December 2020. We used Excel© 2010 and SPSS© to analyze the collected data. Results: 4244 patients, i.e. 3717 males (87.58%) and 527 f.males (12.42%) were managed during the study period. The main diseases diagnosed were benign prostatic hyperplasia (32.61%, n = 1384), erectile dysf.nction (10.44%, n = 443), chronic prostatitis (5.94%, n = 252), prostate cancer (4.03%, n = 171), and ejaculatory disorders (3.44%, n = 146). In patients ≤ 15 years, predominant diseases were peritoneal vaginal canal (15.9%, n = 43), circumcision (15.6%, n = 42), testicular dystopia (10.7%, n = 29), hydrocele (7.8%, n = 21), and hypospadias (5.6%, n = 15). Wilms tumor (1.9%, n = 5) was the f.rst cancer, testicular (0.4%, n = 1) and para-testicular (0.4%, n = 1) tumors were the next. In patients > 15 and ≤40 years, the main diseases were inf.rtility (14.1%, n = 189), chronic prostatitis (12.9%, n = 173), erectile dysf.nction (10.1%, n = 135), ejaculatory disorders (6.2%, n = 83), genital inf.ctions (6.2%, n = 83) and urinary stone (4.9%, n = 66). The f.rst cancer was kidney cancer (0.97%, n = 13) f.llowed by bladder cancer (0.3%, n = 4) and testicular tumor (0.3%, n = 4). In patients >40 years, the most prevalent diseases were BPH (52.0%, n = 1370), erectile dysf.nction (11.7%, n = 308), prostate cancer (6.5%, n = 171), inguinal hernia (5.1%, n = 134), and urinary stone (3.9%, n = 102). Correlation of.PSA Level and ISUP Grade Group with Scintigraphic Bone Metastases Detection in 36 Prostate Cancer Patients

Background: We need population-specif.c clinical f.atures that can predict bone metastases as an af.ordable therapeutic decision-making tool in newly diagnosed prostate cancer patients as scintigraphy or positron emission tomography are not available and as no such study had ever been perf.rmed in our country. Objective: To determine biologic and pathologic criteria that can predict the scintigraphic detection of.bone metastases in our prostate cancer patients. Patients and Method: We analyzed with student’s t test and logistic regression the PSA level, the ISUP grade and the scintigraphic data retrospectively collected in newly diagnosed prostate cancer patients. Results: In ten years, 36 prostate cancer patients were sent to the Korle Bu Teaching Hospital in Accra (Ghana) f.r bone scintigraphy (mean age = 63.9 years;55.6%, 19.4% and 25.0% ISUP grade ≤ 2, 3 or ≥4). Among 28 patients who had perf.rmed the bone scintigraphy, 6 (21.4%) presented bone metastases, 22 (78.6%) had no bone metastasis. The mean PSA level was 36.7ng/mL in the non-metastatic patients and 97.7 ng/mL in the metastatic patients. The dif.erence in PSA level between the 2 groups was signif.cative (p = 0.041). 63.6% of.the non-metastatic cancers versus 16.7% of.the metastatic cancers were ISUP grade 2 or less. Inversely, 36.4% of.the non-metastatic cancers versus 83.3% of.the metastatic cancers were ISUP grade 3 or more. The dif.erence was signif.cative in the ISUP grade 2 or less (p = 0.035), was signif.cative in the ISUP grade group 3 or more (p = 0.035). Metastasis was more likely in prostate cancer patients with PSA equal 30 ng/mL or more and ISUP grade 3 or more (83.3%) than in prostate cancer patients with PSA less than 30 ng/mL and ISUP grade less than 3 (16.7%) [OR = 13.7;CI 95% (1.59;31.0);p = 0.035]. Objective: To analyze the management of.VVf.in the Service of.Urology-Andrology Obstetrics and Gynecology of.the University Hospital in Conakry. Materials and Methods: f.om January 2012 to December 2013, 152 patients with a mean age of.30 years (14 - 80 years) were hospitalized in the Departments of.Urology-Andrology and Gynecology-Obstetrics of.the Conakry University Hospital Center. Clinically f.stulas were divided according to the classif.cation of.Benchekroun as single, complex and complicated. The f.stulas were diagnosed af.er a minimum period of.three months and the results were assessed with a mean f.llow-up of.7 months (range 3 to 10 months) according to the f.llowing criteria: complete healing, intermediate healing and f.ilure. Results: f.stulas occur mainly in young multiparous women. The obstetric etiology was dominant (98%). Clinically, there were 30% simple f.stulas, 46% complex f.stulas and 24% of.complicated f.stulas. f.om a therapeutic standpoint, the treatment consisted of.a single f.stulorraphie (Chassar Moir) in 82% of.cases and a f.stulorraphie with interposition of.healthy tissue in 18% of.cases. Af.er a mean f.llow-up of.7 months we obtained a healing in 62% of.cases, a f.ilure in 31% of.cases and the results were intermediate in 7% of.cases. Conclusion: It appears that the VVf.represents a public health concern in Guinea and surgical treatment is technical dif.icult due to the higher f.equency of.complex f.stulas.

Background: Benign Prostatic Hyperplasia (BPH) is one of.the commonest causes of.lower obstructive uropathy and usually presents with lower urinary tract symptoms. However, in developing nations, the presentation is late of.en large prostate and associated complications of.bladder outlet obstruction (diverticulae, stones, impaired renal f.nction etc.) warranting open prostatectomy. Objective: The objective of.this study is to share our experience of.perioperative outcomes of.open transvesical prostatectomy over a year involving two ref.rral centers in Monrovia Liberia. Methodology: This was a retrospective study perf.rmed at The John f. Kennedy Medical Center and the Saint Joseph Catholic Hospital involving a total of.31 patients. Results: The mean age was 64.6 years (SD = 9.03) with a range 52 - 85 years. The study showed that 54.8% (17/31) of.patients had preoperative catheter. The maximum duration of.Catheter was greater than 4 weeks with a mean duration of.3.17 weeks. The mean hemoglobin was 11.30 g/dl. The commonest presenting complication was urinary retention at 25.8% (8/31) f.llowed by hematuria 16.1% (5/31). Hypertension 41.9% (13/31) and diabetes 6.5% (/31) were the commonest comorbidity. The rate of.postoperative complication was 38.7% with the commonest being surgical site inf.ction 16.1% (5/31) and clot retentions 9.7% (3/31). There was a high rate of.transf.sion was 77.4% (24/31) patients. Up to about 13% (4/32) patients had coexisting stricture with 6.5% (2/31) patients treated with urethroplasty and 6.5% (2/31) patients treated by dilatation. The mortality rate was 6.5% (2/31) f.om renal impairment and severe sepsis. Conclusion: BPH is a common cause of.lower obstructive uropathy. Most patients present with complications of.the disease such as ref.actory symptoms, bladder stones or diverticulae necessitating surgery. However, due to the absence of.endourological equipments in most parts of.Af.ica and the large prostate at presentation, most settings
