Endoscopic ultrasound-guided fine needle aspiration in the differentiation of type 1 and type 2 autoimmune pancreatitis

AIM:To investigate the usefulness of endoscopic ultrasound-guided fine needle aspiration(EUS-FNA) in the differentiation of autoimmune pancreatitis(AIP).METHODS:We retrospectively reviewed 47 of 56 AIP patients who underwent EUS-FNA and met the Asian diagnostic *** 47 EUS-FNA specimens,we evaluated the presence of adequate material and characteristic features of lymphoplasmacytic sclerosing pancreatitis(LPSP) and idiopathic duct-centric pancreatitis(IDCP) mentioned in the International Consensus Diagnostic Criteria and examined if these findings make a contribution to the differential diagnosis of type 1 and type 2 AIP.A disposable 22-gauge needle was used for ***:Adequate specimens including pancreatic tissue for differentiating AIP from cancer were obtained from 43 of 47 patients who underwent ***-FNA was performed from the pancreatic head in 21 cases,which is known to be technically difficult when performed by core biopsy;there was no significant difference in the results compared with pancreatic *** of 47 patients met level 1 findings of LPSP and 5 patients met level 2 findings of *** one met level 1 findings of IDCP,but 3 patients met level 2 findings of *** 10 seronegative cases,2 cases were diagnosed with "definitive type 1 AIP",and 3 cases were diagnosed with "probable type 2 AIP" when considering both the level 2 histological findings and response to ***:EUS-FNA is useful in the differentiation of type 1 and type 2 AIP,particularly in seronegative cases.

Local regulator adrenomedullin contributes to the circulatory disturbance in cirrhotic rats

AIM： To investigate whether adrenomedullin, a potent vasodilator peptide, plays a role in the circulatory disturbance in cirrhosis. METHODS： Cirrhosis was induced in rats by weekly gavage of carbon tetrachloride. Hemodynamic studies were performed in vivo using radioactive microspheres and in vitro using isolated aortic rings. The adrenomedullin concentrations were measured by radioimmunoassay. RESULTS： Acute administration of adrenomedullin to the control rats reduced the systemic arterial pressure along with an increase of serum levels of the stable metabolite of nitric oxide （NOx）, in a dose-dependent manner. Chronic infusion of adrenomedullin reduced the vascular resistance and increased the blood flow in the systemic and splanchnic circulation. Intravenous administration of anti-adrenomedullin antibody did not affect any hemodynamic parameters in the cirrhotic rats, whereas this antibody ameliorated the blunted contractile response to phenylephrine, o-adrenergic receptor agonist, in the aortic rings of the cirrhotic rats. The adrenomedullin concentrations in the aorta were higher in the cirrhotic rats than in the controls, and correlated with the mean arterial pressure in the cirrhotic rats. Moreover, adrenomedullin blunted the contractile response to phenylephrine in both of the control aorta and cirrhotic aorta, but not in the presence of NG-nitro L-arginine methyl ester, an NO synthase inhibitor. CONCLUSION： Adrenomedullin overproduced in the vascular wall may contribute to the circulatory disturbance in cirrhosis as a local regulator of the vascular tonus rather than a circulating hormone.

Pollen tube guidance is controlled by multiple complex interactions with the female tissues. Here, we show that pollen tubes of Torenia fournieri are regulated by a stylar tissue in a length-dependent manner to receive and respond to attractant LURE peptides secreted from synergid cells. We developed an immunostaining method to visualize LURE peptides bound at the plasma membrane of the tip region of the pollen tube. Using this method, we found that LURE peptides bound specifically to pollen tubes growing through a cut style. The peptides also bound to pollen tubes growing through a shorter style, which were not competent to respond to these peptides. These observations suggested a possibility that acquisition of the LURE peptide reception ability and acquisition of full competency are separable pro- cesses. RNA-Seq suggested that the transcription profile of pollen tubes was affected by both the length of the style and the cultivation period, consistently with physiological changes in binding activity and LURE response ability. The data- base generated from de novo RNA-Seq of Torenia pollen tubes was shown to be useful to identify pollen tube proteins by mass spectrometry. Our studies provide insight and an effective platform for protein identification to understand pollen tube guidance.

Development of High Strength Steel Designed for Carbonitriding with High Nitrogen Content to Be Used for Automatic Transmission Gears

To downsize and lighten automatic transmission components, the gears installed must be strengthened in terms of pitting endurance and/or wear resistance. The most important metallurgical factor affecting fractures is well known to be resistance to softening when steel is tempered at approximately 573 K. Carbonitriding with a high amount of nitrogen is a very effective production technique because nitrogen increases the resistance during tempering. However, structural anomalies begin to appear in the surface layer when the nitrogen content exceeds 0.6 mass% in the chromium steel generally used. To address this, we have developed new high-strength chromium steel with an optimized chemical composition that effectively inhibits anomalies even when Carbonitriding with a nitrogen content of more than 0.6 mass%. We performed a drivetrain durability test on an automatic transmission component designed to have excellent contact fatigue strength and a tooth root bending impact and fatigue strength. We found that the developed steel that was carbonitrided with a content of about 0.9 mass%, and then shot peened hard, has a pitting life of roughly 4.5 times that of conventionally manufactured steel.

Pneumatic active suspension system for a one-wheel car model using fuzzy reasoning and a disturbance observer

This paper presents the construction of a pneumatic active suspension system for a one-wheel car model using fuzzy reasoning and a disturbance observer. The one-wheel car model can be approximately described as a nonlinear two degrees of freedom system subject to excitation from a road profile. The active control is composed of fuzzy and disturbance controls, and the active control force is constructed by actuating a pneumatic actuator. A phase lead-lag compensator is inserted to counter the performance degradation due to the delay of the pneumatic actuator. The experimental result indicates that the proposed active suspension improves much the vibration suppression of the car model.

Hepatectomy is an effective surgical treatment for multiple bilobar liver metastases from colon cancer;however,one of the primary obstacles to completing surgical resection for these cases is an insufficient volume of the future remnant liver,which may cause postoperative liver *** induce atrophy of the unilateral lobe and hypertrophy of the future remnant liver,procedures to occlude the portal vein have been conventionally used prior to major *** report a case of a 50-year-old woman in whom two-stage hepatectomy was performed in combination with intraoperative ligation of the portal vein and the bile duct of the right hepatic *** procedure was designed to promote the atrophic effect on the right hepatic lobe more effectively than the conventional technique,and to the best of our knowledge,it was used for the first time in the present *** successful induction of liver volume shift as well as the following procedure,the patient died of subsequent liver failure after developing recurrent *** discuss the first case in which simultaneous ligation of the portal vein and the biliary system was successfully applied as part of the first step of two-stage hepatectomy.

Induced Charge of Spherical Dust Particle on Plasma-facing Wall in Non-uniform Electric Field

Induced charge of a spherical dust particle on a plasma-facing wall was investigated analytically, where non-uniform electric field was applied externally. The one-dimensional nonuniform electrostatic potential was approximated by the polynomial of the normal coordinate toward the wall. The bipolar coordinate was introduced to solve the Laplace equation of the induced electrostatic potential. The boundary condition at the dust surface determines the unknown coefficients of the general solution of the Laplace equation for the induced potential. From the obtained potential the surface induced charge can be calculated. This result allows estimating the effect of the surrounding plasma, which shields the induced charge.

This paper presents our recent progress for the development of low-loss Bi2223/Ag tapes with interfilamentary oxide *** the compatibility with Bi2223 superconductors,SrZrO3was selected as barrier ***,small amount of Bi2212 was mixed with SrZrO3to improve its ductility for cold *** controlling the thickness of barrier layers among the filaments,reducing the tape width（<3 mm）and twisting the filaments tightly（<5 mm in twist pitch lengths）,coupling frequency fcexceeded 250 Hz even in an AC perpendicular field at *** Jcof the barrier tapes with tightly twisted filaments was in the range of 12-15 kA/cm2at 77 K and *** 50 mT and 50 Hz,the twisted barrier tapes showed 60%-70%lower perpendicular field losses than the conventional 4 mm-width tape with fully coupled filaments.

Effectiveness of impedance monitoring during radiofrequency ablation for predicting popping

AIM： To retrospectively evaluate the effectiveness of impedance monitoring for predicting popping during radiofrequency ablation （RFA） using internally cooled electrodes. METHODS： We reviewed 140 patients （94 males, 46 females; age range 73.0 ＋ 11.1 year） who underwent RFA between February 2006 and November 2008 with a modified protocol using a limited power delivery rather than a conventional one to avoid popping. All the patients provided their written informed consent, and the study was approved by the institutional review board. Intraprocedural impedances were measured for the study subjects, and the tumors were classified into three types according to the characteristics of their impedance curves： increasing, flat, or decreasing. The tumors were further sorted into seven subtypes （A-G） depending on the curvature of the impedance curve＇ s increase or decrease. Relative popping rates were determined for the three types and seven subtypes. A chi-square test was performed to estimate statistical ***： A total of 148 nodules treated by RFA were analyzed. The study samples included 132 nodules of hepatocellular carcinoma, 14 nodules of metastatic liver cancer, and two nodules of intrahepatic cholangio- carcinoma. The numbers of nodules with each imped- ance curve type were as follows： 37 increasing-type nodules, 43 fiat-type nodules, and 68 decreasing-type nodules. Popping occurrence rates were 24.3%, 46.5% and 64.7%, respectively. Flat-type nodules exhibited a significantly higher rate of popping compared to increasing-type nodules （P = 0.039）. Decreasing-type nodules exhibited a significantly higher rate of popping compared to increasing-type nodules （P 〈 0.0001）. No- tably, nodules that showed a sharp decrease in imped- ance in the latter ablation period （subtype E） exhibited a significantly higher rate of popping compared to other subtypes. CONCLUSION： Intraprocedural impedance monitoring can be a useful tool to predict the occurrence of pop- ping
