Qianjin Wenwu decoction suppresses renal interstitial fibrosis by enhancing the degradation of extracellular matrix in mice with unilateral ureteral obstruction

Diabetic kidney disease(DKD)is the most common complication of diabetes mellitus(DM).Qianjin Wenwu decoction(QWD),a well-known traditional Korean medicine,has been used for the treatment of DKD,with satisfactory therapeutic effects.This study was designed to investigate the active components and mechanisms of action of QWD in the treatment of DKD.The results demonstrated that a total of 13 active components in five types were found in QwD,including flavonoids,flavonoid glycosides,phenylpropionic acids,saponins,coumarins,and lignins.Two key proteins,TGF-β1 and TIMP-1,were identified as the target proteins through molecular docking.Furthermore,QWD significantly suppressed Scr and BUN levels which increased after unilateral ureteral obstruction(UUO).Hematoxylin&eosin(H&E)and Masson staining results demonstrated that QWD significantly alleviated renal interstitial fibrosis in UUO mice.We also found that QWD promoted ECM degradation by regulating MMP-9/TIMP-1 homeostasis to improve renal tubulointerstitial fibrosis and interfere with the expression and activity of TGF-βl in DKD treatment.These findings explain the underlying mechanism of QWD for the treatment of DKD,and also provide methodological reference for investigating the mechanism of traditional medicine in the treatment of DKD.

Median arcuate ligament syndrome with retroperitoneal haemorrhage:A case report

BACKGROUND Median arcuate ligament syndrome(MALS)is relatively rare and is due to extraluminal compression of the coeliac artery by the median arcuate ligament of the diaphragm.Here,we report a case of MALS found in a patient with abdominal pain and retroperitoneal haemorrhage for education and dissemination.CASE SUMMARY This article describes a 46-year-old female patient who was admitted to our hospital with abdominal pain as her chief complaint.She had experienced no obvious symptoms but had retroperitoneal bleeding during the course of the disease.Contrast-enhanced computed tomography(CT)and noninvasive CT angiography(CTA)led to an initial misdiagnosis of pancreaticoduodenal artery aneurysm(PDAA)causing retroperitoneal hemorrhage.After intraoperative exploration and detailed analysis of enhanced CT and CTA images,a final diagnosis of MALS was made.The cause of the haemorrhage was bleeding from a branch of the gastroduodenal artery,not rupture of a PDAA.The prognosis of MALS combined with PDAA treated by laparoscopy and interventional therapy is still acceptable.The patient was temporarily treated by gastroduodenal suture haemostasis and was referred for further treatment.CONCLUSION MALS is very rare and usually has postprandial abdominal pain,upper abdominal murmur,and weight loss.It is diagnosed by imaging or due to complications.When a patient has abdominal bleeding or PDAA,we should consider whether the patient has celiac trunk stenosis(MALS or other etiology).When abdominal bleeding is combined with an aneurysm,we generally think of aneurysm rupture and hemorrhage first,but it may also be collateral artery rupture and hemorrhage.

Carotid-cavernous fistula following mechanical thrombectomy of the tortuous internal carotid artery:A case report

BACKGROUND A carotid-cavernous fistula(CCF)is an abnormal connection between the internal carotid artery(ICA)and the cavernous sinus.Although direct CCFs typically result from trauma or as an iatrogenic complication of neuroendovascular procedures,they can occur as surgery-related complications after mechanical thrombectomy(MT).With the widespread use of MT in patients with acute ischemic stroke complicated with large vessel occlusion,it is important to document CCF following MT and how to avoid them.In this study,we present a case of a patient who developed a CCF following MT and describe in detail the characteristics of ICA tortuosity in this case.CASE SUMMARY A 60-year-old woman experienced weakness in the left upper and lower limbs as well as difficulty speaking for 4 h.The neurological examination revealed left central facial paralysis and left hemiplegia,with a National Institutes of Health Stroke Scale score of 9.Head magnetic resonance imaging revealed an acute cerebral infarction in the right basal ganglia and radial crown.Magnetic resonance angiography demonstrated an occlusion of the right ICA and middle cerebral artery.Digital subtraction angiography demonstrated distal occlusion of the cervical segment of the right ICA.We performed suction combined with stent thrombectomy.Then,postoperative angiography was performed,which showed a right CCF.One month later,CCF embolization was performed,and the patient’s clinical symptoms have significantly improved 5 mo after the operation.CONCLUSION Although a CCF is a rare complication after MT,it should be considered.Understanding the tortuosity of the internal carotid-cavernous sinus may help predict the complexity of MT and avoid this complication.

Meta-analysis of the efficacy and safety of daratumumab in the treatment of multiple myeloma

BACKGROUND The treatment of multiple myeloma has significantly progressed over the past half-century.The purpose of this study was to perform a systematic review and meta-analysis in order to explore the efficacy and safety of daratumumab in treating multiple myeloma.AIM To explore the efficacy and safety of daratumumab in treating multiple myeloma.METHODS A systematic literature search was performed using Chinese and English databases,including the China National Knowledge Infrastructure,Wanfang,China Biology Medicine,VIP,the Cochrane Library,Embase,and PubMed.The search encompassed studies in treating multiple myeloma with daratumumab,spanning from the inception of the database to June 2023.Revman 5.1 software was used for analysis.RESULTS Our analysis included eight English articles and one Chinese article of high quality.The meta-analysis results indicated that compared to other therapies,daratumumab could improve the overall response rate(ORR)[odds ratio(OR)=2.67,95%confidence interval(CI)=2.01,3.53,Z=6.85,P<0.00001],complete remission(CR)(OR=2.87,95%CI=2.16,3.83,Z=7.23,P<0.00001)and progression-free survival(PFS)time(hazard ratio=0.48,95%CI=0.38,0.60,Z=6.54,P<0.00001)in patients with multiple myeloma.These differences were statistically significant.Additionally,these results suggested that daratumumab increases the risk of neutropenia and thrombocytopenia with minimal effect on the incidences of anemia and upper respiratory tract infections.CONCLUSION Daratumumab can improve ORR,CR rate,and PFS in patients with multiple myeloma.It also increases the risk of neutropenia and thrombocytopenia,necessitating careful monitoring during its clinical application.

Background and Aims:In this study,we aimed to evaluate the diagnostic values of alpha-fetoprotein(AFP),soluble AXL(sAXL),des-γ-carboxy prothrombin(DCP),the aspartate aminotransferase-to-platelet ratio index(APRI),and the gamma-glutamyl transpeptidase-to-platelet ratio(GPR)in hepatocellular carcinoma(HCC)and the possible underlying mechanisms of the correlations between them.Methods:We collected serum samples from 190,128,and 75 patients with HCC,cirrhosis,and chronic viral hepatitis,and from 82 healthy subjects.Serum levels of AFP,sAXL,and DCP were determined,and APRI and GPR values were calculated.Receiver operating characteristic(ROC)curves were used to analyze the diagnostic value of single and combined biomarkers.Results:We detected significant differences between the HCC group and other groups regarding serum AFP,sAXL,DCP,and APRI levels.GPR significantly differed between the HCC group and other groups,except for the liver cirrhosis group.AFP,sAXL,DCP,APRI,and GPR had positive correlations with each other,and AFP showed a higher area under the curve(AUC)and Youden index values,while APRI and DCP showed the highest sensitivity and specificity.Also,when AFP was combined with sAXL,DCP,APRI,and GRP,the highest AUC(0.911)and a higher net reclassification improvement value were obtained compared with those obtained for the individual biomarkers.Conclusions:AFP,sAXL,DCP,APRI,and GPR are independent risk factors for HCC,and the diagnostic performance of AFP combined with sAXL,DCP,APRI,and GPR for HCC diagnosis was superior to that of the individual biomarkers.

BACKGROUND Mesenteric ischemia represents an uncommon complication of splanchnic vein thrombosis,and it is less infrequently seen in young women using oral contraceptives.Diagnosis is often delayed in the emergency room;thus,surgical intervention may be inevitable and the absence of thrombus regression or collateral circulation may lead to further postoperative ischemia and a fatal outcome.CASE SUMMARY We report a 28-year-old female patient on oral contraceptives who presented with acute abdominal pain.Her physical examination findings were not consistent with her symptoms of severe pain and abdominal distention.These findings and her abnormal blood tests raised suspicion of acute mesenteric ischemia(AMI)induced by splanchnic vein thrombosis.Contrast-enhanced abdominal computed tomography revealed ischemia of the small intestine with portomesenteric and splenic vein thrombosis(PMSVT).We treated the case promptly by anticoagulation after diagnosis.We then performed delayed segmental bowel resection after thrombus regression and established collateral circulation guided by collaboration with a multidisciplinary team.The patient had an uneventful postoperative course and was discharged 14 d after surgery and took rivaroxaban orally for 6 mo.In subsequent follow-up to date,the patient has not complained of any other discomfort.CONCLUSION AMI induced by PMSVT should be considered in young women who are taking oral contraceptives and have acute abdominal pain.Prompt anticoagulation followed by surgery is an effective treatment strategy.

Application effect of quality control circle in postoperative functional exercise compliance of breast neoplasms patients:a meta-analysis

Background:Good adherence to post-operative exercise is crucial for long-term exercise intervention programs aimed at improving compliance,motivation for functional exercise,and quality of life among breast cancer patients.This study aims to evaluate the application effect of quality control circles on postoperative functional exercise compliance in breast neoplasm patients.Methods:A comprehensive search was conducted in CNKI,Wanfang,VIP,CBM,PubMed,Cochrane Library,Web of Science and CINAHL databases from their establishment to May 06,2023.Manual search and backtracking of relevant literature were also conducted.The search terms included a combination of subject terms and free words.Meta-analysis was performed using RevMan 5.4 software on eligible literature.Results:Initially,517 papers were retrieved,and after removing duplicates 238 papers using NoteExpress software,279 titles and abstracts were reviewed.Subsequently,225 non-compliant papers were excluded,leaving 54 full texts to be read.After excluding 38 non-compliant papers,16 eligible papers were included for Meta-analysis.These papers comprised a total of 1,342 patients(679 in the test group and 663 in the control group).The heterogeneity test using a fixed effects model showed P=0.22 and I2=21%.The Meta-analysis results demonstrated that the implementation of quality control circles alongside usual care significantly improved compliance with postoperative functional exercise in breast cancer patients(odds ratio=6.68,95%confidence interval(4.97,8.98),P<0.00001).Conclusion:Quality control circles activities can effectively enhance compliance with post-operative functional exercise among breast cancer patients,aiding in the recovery of affected limb function,improvement of self-care ability,and early reintegration into society and normal life,thereby enhancing quality of life.It is important to continually accumulate experience in applying relevant quality control circles activities in clinical work and promptly enhance the level of care accordingly,as the f

Amyloid precursor-like protein 2 C-terminal fragments upregulate S100A9 gene and protein expression in BV2 cells

The murine microglial cell line BV2 has neuroprotective effects, but is toxic to neurons by secret-ing inlfammatory cytokines, and is an important target in the treatment of nerve inlfammation and neurodegenerative diseases. In the present study, we observed the effects of transfecting three amyloid precursor-like protein 2 （APLP2） C-terminal fragments （CTFs; C57, C50 and C31） in the pEGFP-N1 vector on S100A9 expression in BV2 cells. Reverse transcription-PCR, western blot assay and immunocytochemistry revealed that S100A9 protein and mRNA expression was greater in BV2 cells after CTF transfection than after mock transfection with an empty vector. Furthermore, transfection of full-length APLP2-751 resulted in low levels of S100A9 protein ex-pression. Our results show that APLP2-CTFs upregulate S100A9 protein and mRNA expression in BV2 cells, and identify a novel pathway involved in neuronal injury and apoptosis, and repair and protection in Alzheimer’s disease.

To the Editor:Psoriasis is a chronic immune-related inflammatory skin disease.The clinical severity of psoriasis varies greatly,and recurrence occurs frequently.[1]Many attempts have been made to identify biomarkers for psoriasis;however,there are currently no specific markers that can accurately monitor disease severity and treatment efficacy.Circular RNAs(circRNAs)are a novel class of endogenous noncoding RNAs that are highly stable and resistant to RNase R since they can form covalently closed loops without 50 caps and 30 tails.[2]It has been reported that circRNAs could exist in body fluids,such as serum and plasma,and could serve as molecular biomarkers in the diagnosis and prognosis of several diseases.[3]In this study,we detected the alteration of circRNAs and investigated the potential value of hsa_circ_0056856 as a biomarker for disease severity in serum samples from psoriasis patients and healthy controls(HCs).

A Case of Choledocholithiasis Complicated by Choledochoduodenal Fistula

Duodenal fistula refers to the pathological channel formed between the duodenum and the hollow viscera of the human body.If it is connected to a single cavity organ,it is a simple duodenal fistula.If it is connected to≥2 and the hollow viscera are connected,it is a complicated internal duodenal fistula,but simple ones are more common.Once a duodenal fistula develops,it establishes a pathological communication between duodenal contents and related organs,resulting in compromised organ function,infections,malnutrition,bleeding,and other detrimental effects.A patient with choledocholithiasis and choledochoduodenal fistula was treated in our hospital.She underwent laparoscopic left hemi-hepatectomy+laparoscopic choledochojejunostomy and was discharged 4 days after surgery.
