Trauma in Pregnancy at the yalgado ouedraogo teaching hospital in Ouagadougou,Burkina Faso:Epidemiological,Clinical,Therapeutic and Prognostic Aspect

Objective: To study the epidemiological, clinical, therapeutic and prognostic aspects of trauma in pregnancy at the yalgado ouedraogo teaching hospital in Ouagadougou. Patients and Methods: A descriptive retrospective study was done over a period of three years from 1st January 2010 to 31st December 2012. We included in our sample, all pregnant women admitted for trauma in pregnancy and eligible in three clinics: obstetrics/gynecology, orthopedics, general and visceral surgery. Anonymity and confidentiality of collected data were respected. Results: We recorded 224 emergency consultations for trauma in pregnancy out of 26,735 obstetric and gynecological consultations which gave a frequency of 0.8%. The mean age of our patients was 25.9 ± 5 years and mean parity was 1. Housewives accounted for 50% of patients and referrals were the mode of admission in 74.6% of cases. The causes of injury were dominated by road traffic accidents in 82.6% of cases, falls in 9.8% and assault in 7.6%. The mean gestational age of onset of trauma was 26.3 ± 7 weeks amenorrhea. The management was multidisciplinary with a medical component in 99.1% of cases, obstetric component in 13% of cases, a visceral surgery component in 6.7% of cases and an orthopedic component in 4.2% case. Multiple traumas were found in 4 patients. The fetal prognosis showed in the 1st trimester 3 cases of abortion in the 2nd trimester 5 cases of intrauterine fetal death and in the 3rd trimester 10 cases of preterm labor. Conclusion: Traumas during pregnancy are beyond emergencies within multiple causes and requiring a multidisciplinary management. Seen the limited means of support, establishment of a national strategy to prevent trauma in pregnancy will improve the prognosis for mother and child.

Objective: To determine the epidemiological and clinical characteristics of male hypofertility attending at urology for paternity desire. Materials and methods: Cross-sectional study conducted from January 1st to December 31 2012 at urology department of teaching hospital yalgado ouedraogo. We collated men attending for hypofertility. Only subjects living regularly with their partner were retained. Results: The study concerned 95 men. The average age was 36.9 years (range: 23 years and 53 years) and 28.6 years for their wives (range: 19 years and 40 years). Patients resided in urban areas in 90.5% of cases. Gynecologists and general physicians sent respectively 77.9% (N = 74) and 17.9% (N = 17) of cases. The average consultation delay time was 3 years. The average duration of infertility was 5.8 years (Extreme: 1 year and 20 years). Infertility was primary in 68.4% of cases and secondary in 31.6% of cases. 24.2% of patients admitted for history of genitourinary infection. Varicocele was found in 22.1% of cases. The semen analysis showed that 42 men had azoospermia. The semen culture isolated a germ in 19.1% of cases. Serum FSH was high in 47.3% of cases. Conclusion: Male hypofertility affected men of all age. Urologists are the last resort of the male hypo fertile, gynecologists constituting the first contact. Azoospermia is a problem commonly encountered in urological consultation.

Objective: To describe the epidemiology, diagnosis and therapeutic outcomes of trauma to the external genitalia trauma. Materials and Methods: This is a retrospective descriptive study over a period of six and a half years from January 1st, 2006 to June 30th, 2012. About 42 cases of trauma to the male external genitalia were received at the emergency department of General surgery and Urology of yalgado ouedraogo teaching hospital of Ouagadougou. Results: The average age of the patients was 31.7 years and the age groups most affected were those between 16 - 26 years. Our study population were mainly pupils and students with 40.5% of cases. Causes of trauma to the external genitalia were mainly highway accident with 19 cases (45.2% of cases). 85.7% of patients presented on the day of the trauma. Scrotum and its contents were more exposed to trauma (30 cases) than penis (12 cases). Treatment was most often surgical (78.6%). The postoperative course was uneventful in 75% of the cases of testicular trauma and 72.7% of the cases of penile trauma. Conclusion: External genitalia traumas affect young people. Complications can be important like erectile dysfunction. However, the prognosis is good if treatment is early.

Sexual Experience of Women after Gynecological Hysterectomy in the Department of Obstetrics and Gynecology of yalgado ouedraogo teaching hospital, Burkina Faso

Objective: To study the sexual experience of women after gynecological hysterectomy in the obstetrics and gynecology department of yalgado ouedraogo University teaching hospital in Burkina Faso. Patient and Method: We conducted a cross-sectional study from October 1, 2016 to March 30, 2017. It included any patient who had a gynecological indication for a hysterectomy in the Department of Obstetrics and Gynecology of yalgado ouedraogo University teaching hospital (CHU-YO) during the study period. A total of 85 patients were included. We collected information from the consultation records, clinical records, and operating room registries and by surveying the patients themselves. Result: The frequency of hysterectomy in the gynecological period was 3.9%. The mean age of the patients was 51.71 (extremes: 35 and 66 years) and the mean parity was 4.8. Married women were the majority. Fibromyoma and genital prolapse accounted for 56.47% and 23.53% respectively of operative indications. Regarding postintervention sexuality, 29.41% of women declared no longer having sexual desire. Also, they complained of vaginal dryness (31.8%), decreased frequency of orgasm (42.3%), dyspareunia (37.65). Sexual dysfunction was present in 82.35% of women. There was a statistically significant difference between global sexual functioning and geographic origin, educational attainment, socio-economic level, surgical approach, preintervention sexual counseling, women’s psychological profile and their emotional relationship with their spouses. Conclusion: The psychosexual soundness of hysterectomy is important. A diagnosis of sexual disorders prior to the intervention and psychological follow-up throughout the therapeutic process of the pathology indicative of a hysterectomy is necessary for successful postoperative sexuality.

Objective: To study emergency cesarean section in teenagers with the goal of reducing morbidity and maternal-fetal mortality. Methods: This was a descriptive cross-sectional study with prospective collection of data on 248 cases collected from July 1st to December 31st, 2016. Results: The incidence of cesarean section in teenagers was 56.4%. The average age was 18 ± 0.4 years old. The vast majority were primiparous (92.7%). The main indications for emergency Caesarean section were: pre-eclampsia and its complications (20.2%), acute fetal distress (18.5), pre-rupture syndrome (14.1%) and bone dystocia (11.7%). The maternal mortality rate was 1.6% and perinatal mortality was 134 per 1000 live births. Conclusion: The rate of caesarean section is high at yalgado OUéDRAOGO teaching hospital of Ouagadougou. Postoperative complications are sometimes serious and compromise the maternal and fetal outcome. There is a need for increased surveillance of all pregnant and recently delivered women to reduce maternal and perinatal mortality in teenagers.

Objective: To describe diagnostic and therapeutic outcomes of infected vaginal *** and Methods: This is a retrospective descriptive study over a period of two years from June 2013 to May 2015 at the urological emergency department of yalgado Ouédraogo University hospital (CHU-YO). Results: We collected seven cases during the period of study. The average age of patients was 35 years. Two patients (2/7) were from urban and five from rural areas. Five patients (5/7) were married and two (2/7) unmarried. Six patients (6/7) were farmers and one (1/7) was civil servent. Clinical examination findings were dominated by bilateral painful scrotal swelling and fever in all cases, a scrotal necrosis in two cases. The dominant indication of the puncture-aspiration was discomfort during sexual intercourse (5/7). At the opening of the scrotum purulent fluid was aspirated in all cases, and we performed a unilateral orchidectomy in two cases. A hydrocelectomy was performed in all cases. The postoperative evolution was simple in all patients. Conclusion: The puncture-aspiration must be avoided because the risk of infection and the high recurrence rate.

Urachus Fistula about Two Cases in yalgado ouedraogo teaching hospital, Ouagadougou (Burkina-Faso)

The purpose of these serial cases was to report the diagnostic and therapeutic features of urachal fistulas at yalgado ouedraogo teaching hospital. We have reported retrospectively two cases of urachus fistula at the Surgery Department of yalgado ouedraogo teaching hospital in Ouagadougou. The parameters studied were sex, age, reason for consultation, clinical and paraclinical signs, treatment, length of hospital stay, delay of urinary catheterisation and evolution. Two serial cases of urachal fistula were reported, one 14 years old female patient and a 32 years old male patient. Clinical signs were marked by urine flow through the umbilicus. The diagnosis was made by fistulography in one case and during surgery for the second case. The treatment was surgical and consisted of laparotomy followed by removal of the urachal fistula from bladder. The urinary catheter was removed after 10 days. The postoperative course for the two patients was uneventful.

Background: Achieving and maintaining glycemic targets are a challenge for health practitioners around the world. We aimed to study the factors associated with prolonged poor diabetes control in the cohort of T2DM patients monitored and treated in the Department of Internal Medicine at the Yalgado OUEDRAOGO Teaching Hospital in Ouagadougou in order to optimize therapeutic education in these patients. Methodology: This was a descriptive and analytical cross-sectional study combining retrospective data collection from the last year of patient follow-up and prospective collection of some information. The study included all diabetic patients, aged at least 18 years old, followed and treated in the Department of Internal Medicine at the Yalgado OUEDRAOGO Teaching Hospital between January 1, 2010 and December 31, 2018 following a systematic random sampling with a sampling step of 10. The variables collected were sociodemographic, anthropometric, lifestyle, cardiovascular risk factors and diabetes-related characteristics. To determine the risk factors associated with prolonged poor glycemic control, we performed modeling using logistic regression. All variables associated with prolonged poor glycemic control, in bivariate logistic regression with a p-value less than 0.20 were included in the full model. Later, we used a stepwise descending method to obtain the final model, which was then tested by a receiver operating characteristic (ROC) curve. The significance threshold was set at 5%. Raw and fitted Odds-Ratio (OR) and 95% confidence interval were presented. Results: 270 patients were included. Prolonged poor control of diabetes mellitus was observed in 73.70%. Objective: Conduct a study about the epidemiological, clinical, therapeutic and prognostic aspects of puerperal infections within Gynecology Obstetrics department of CHU-YO during the study period. Patients and Methods: This was a cross-sectional and descriptive study. The study period was from January 1st to December 31st, 2019. Data collection was retrospective. The exhaustive sample included all female patients having a puerperal infection. Results: Our study included 65 patients. The frequency of puerperal infections represented 1% of deliveries, the average age of patients was 27 ± 7.26 years. Housewives represented 64.4% of the patients. Hyperthermia and abdominal-pelvic pain respectively occurred in 63.8% and 21.3% of patients. The patients had given birth by caesarean section in 83.1% of cases. Endometritis represented 76.9% of cases of puerperal infections. The patients were treated with antibiotics, uterotonics and symptomatic therapy. Maternal death occurred in 13.9% of cases. Introduction: A higher risk of death is associated with wasting in children if it is not treated properly. The objective of this study was to investigate the ionic disorders observed in infants suffering from severe wasting at yalgado ouedraogo teaching hospital (YO-TH) and at Charles de Gaulle Pediatric teaching hospital (CDG-PTH) in Ouagadougou (Burkina Faso). Methods: This was a retrospective study with a descriptive and analytical aim over the period from January 1, 2016 to December 31, 2020. Results: It concerned infants aged 6 to 24 months hospitalized at YO-TH and at CDG-PTH from Ouagadougou. We included 271 infants. The mean age of the infants was 14.48 ± 5.44 months with 42.07% which was in the age range of [12 - 18] months. On admission to the hospital, the children had an average weight, height and BMI of 6.22 ± 1.32 kg, 0.73 ± 0.07 m and 11.67 ± 1.53 kg/m2. In urban areas 56.46% of children and the main reasons for consultation were fever (88.19%), vomiting (52.80%) and diarrhea (50.20%). Electrolyte disturbances in emaciated infants affected all 8 parameters of the blood ionogram. However, the major disorders were 65.68% hyponatremia, 55.35% hypobicarbonatemia, 41.33% hypoprotidemia and 32.47% hypokalemia in infants aged 6 to 24 months. We found an association between diarrhea and residence with hypokalemia (p = 0.0000) and hypochloremia (p = 0.010), respectively. Conclusion: Severe acute wasting in infants 6 to 24 months of age remains a concern in the hospital setting. The frequency of biochemical disturbances is also high.
