This study was to explore the optimal threshold of thyroid-stimulating hormone(TSH)-stimulated serum thyroglobulin(s-Tg) for patients who were to receive18F-fluorodeoxyglucose(18F-FDG) PET/CT scan owing to clinical suspicion of differentiated thyroid cancer(DTC) recurrence but negative post-therapeutic 131I whole-body scan(131I-WBS). A total of 60 qualified patients underwent PET/CT scanning from October 2010 to July 2014. The receiver operating characteristic(ROC) curve analyses showed that s-Tg levels over 49 μg/L led to the highest diagnostic accuracy of PET/CT to detect recurrence, with a sensitivity of 89.5% and a specificity of 90.9%. Besides, bivariate correlation analysis showed positive correlation between s-Tg levels and the maximum standardized uptake values(SUVmax) of18F-FDG in patients with positive PET/CT scanning, suggesting a significant influence of TSH both on Tg release and uptake of18F-FDG. So, positive PET/CT imaging is expected when patients have negative 131I-WBS but s-Tg levels over 49 μg/L.

Clinical Evaluation of Hepatic Fibrosis in Chinese Patients with Nonalcoholic Fatty Liver Disease with FibroScan

Aim: The clinical value of FibroScan in evaluation of hepatic fibrosis was assessed in Chinese patients with nonalcoholic fatty liver disease (NAFLD). Methods: Liver stiffness and other NAFLD related variables were measured in NAFLD patients with either elevated or normal hyaluronic acid (HA) levels, and in healthy volunteers respectively. Moreover, hepatic fibrosis rat model was used for the correlation analysis between the measured liver elasticity and its corresponding pathological changes. Results: The severity of NAFLD in patients was positively correlated with both HA level and FibroScan values suggesting that fibrosis occured along with the progression of NAFLD. Interestingly, the FibroScan value was found to change at an earlier stage of the disease than that of HA level. The mean liver elasticity in the sampling volume of the region of interest in experimental rats, which were subject to the induction of liver fibrosis using peritoneal injection of carbon tetrachloride (CCl4) for 6 or 9 weeks respectively, was measured using Fibroscan. The examination of pathological changes of these experimental rats demonstrated the positive correlation of the measured elasticity values with the extent (severity) of the corresponding pathological changes in the rats of 6 weeks post induction of liver fibrosis with a moderate liver fibrosis, or 9 weeks with a severe liver fibrosis. The average liver stiffness for the normal liver (S0) was 4.51 ± 0.82 kPa, for moderate liver fibrosis (S2) was 6.91 ± 1.32 kPa, and for severe liver fibrosis (S3) 9.62 ± 1.82 kPa. Conclusion: FibroScan can be used to objectively and quantitatively identify the trend in the changing stiffness of the liver and noninvasively detect the development of liver fibrosis.

Background In our clinical practice we have been attracted by a group of patients with airway aspergillosis who have airway obstruction; we termed the condition as pseudomembranous necrotizing tracheobronchial aspergillosis （PNTA）. In this study we analyzed the clinical data from patients with PNTA, so as to guide the diagnosis and treatment of the disease. Methods A total of 16 PNTA patients were treated in Changhai hospital from January 2000 clinical data, including the demographic information, clinical symptoms, imaging findings treatment strategies and efficacy, and prognosis, were retrospectively analyzed. to January 2009. Their bronchoscopy findings, Results All 16 patients were found to have primary systemic immunodeficiency diseases and/or damage of the focal airways. Nine patients （9/16, 56.3%） had pulmonary and tracheobronchial tumors, 5/16 （31.3%） had tracheobronchial involvement secondary to non-pulmonary tumors, and 2/16 （12.5%） had lung transplantation. The most common causes of PNTA included local radiotherapy （10/16, 62.5%）, repeated chemotherapy （7/16, 43.8%） and recurrent intervention therapy by bronchoscope （4/16, 25.0%）. Aspergillus fumigatus was the most frequent pathogen （62.5%, 10/16）. The main clinical manifestations included progressive dyspnea （14/16, 87.5%） and irritable cough （12/16, 75.0%）. The trachea was involved in 9/16 patients （56.3%）, right main bronchus in 10/16 （62.5%）. All 16 patients were treated with systemic anti-aspergillosis agents, local anti-aspergillosis agents with amphotericin B inhalation and direct perfusion of amphotericin B by bronchoscope, and interventional treatment by bronchoscope to ensure an unobstructed airway. The total efficiency was 31.3%. Conclusions PNTA is an infectious disease caused by aspergillus and it mainly involves the trachea, primary bronchus and segmental bronchus. A. fumigatus is the most common pathogen. PNTA can pose a severe clinical threat and often occurs after systemic immunodeficiency
