Reanalysis of Lymph node metastasis as predictor of the survival of patients with follicular variant papillary thyroid carcinoma: From SEER database 2004-2014

Background: Follicular variant papillary thyroid carcinoma (FVPTC) is the most common subset of papillary carcinoma. Lymph-node metastasis (LNM) is one of the main predictors of survival in PTC patients, but we found few data related to the correlation between LNM and FVPTC. We therefore aimed to elucidate the relationship between LNM and survival in patients with FVPTC. Methods: Logistic regression models were used to determine the factors associated with LNM. The Cox proportional-hazards model was applied to assess the association between LNM and disease-specific mortality. Kaplan-Meier survival analysis and the log-rank test were used to estimate the disease-specific survival rates in patients with and without LNM. Results: In total, 1418 FVPTC patients were identified. The multivariate logistic regression model showed that age, female sex, being married, tumor size > 4cm and extrathyroidal extension and radiation were predictors of LNM. The multivariate Cox regression model showed that age, multifocality and radiation were risk factors of diseasespecific mortality. Especially, LNM (p=0.271) did not independently predict disease-specific mortality. Kaplan-Meier survival analysis and the log-rank method also showed that LNM did not impact survival in FVPTC (p=0.235). Conclusion: Using data in the SEER database from 2004 to 2014, and after multivariate model correction, we found that age, multifocality and radiation were strongly related to disease-specific mortality. It is worth noting that LNM does not impact the survival of FVPTC patients.

Immune response pattern varies with the natural history of chronic hepatitis B

BACKGROUND Chronic hepatitis B is a highly heterogeneous disease that can be divided into four phases: Immune tolerant(IT), immune active(IA), inactive carrier(IC) and hepatitis B envelope antigen(HBeAg)-negative hepatitis(ENEG).AIM To investigate the immune status of natural killer(NK) and T cells in different phases of chronic hepatitis *** The frequency, phenotype and function of circulating NK cells, as well as nonantigen-specific and hepatitis B virus(HBV)-specific T cell responses were detected by flow cytometry in healthy and HBV-infected *** The ability of NK cells to produce IFN-γ was markedly attenuated in HBVinfected patients overall but was less compromised in IC patients. Patients in the IT and IA phases also displayed significantly lower TNF-α production compared to healthy subjects. NK cells were phenotypically activated in the IA and ENEGphases, as evidenced by the upregulation of NKp44 in CD56^(bright) NK cells and CD69 in CD56^(dim) NK cells. Furthermore, global T-cells from the ENEG phase displayed a proinflammatory cytokine profile with upregulated IFN-γ and TNF-αexpression, while this profile was suppressed in IT and IA patients. Finally, core and S antigen-specific T cell responses were significantly stronger after in vitro expansion in the IC phase compared to other *** Our findings demonstrate the changes in immune response pattern during the natural history of HBV infection. Both NK and T cells are functionally impaired in the IT and IA phases. With the spontaneous clearance of HBeAg and hepatitis B surface antigen decline, NK cell cytokine production and HBV-specific T responses are partially restored in IC phase, and the ENEG phase is dominated by nonantigen-specific T cell responses.

Objective:This study aimed to compare the efficacy of anti-CD19 chimeric antigen receptor T cells(CAR-T cells)versus chemotherapy plus donor lymphocyte infusion(chemo-DLI)for treating relapsed CD 19-positive B-cell acute lymphoblastic leukemia(B-ALL)after allogeneic hematopoietic stem cell transplantation(allo-HSCT).Methods:Clinical data of 43 patients with B-ALL who relapsed after allo-HSCT were retrospectively ***-two patients were treated with CAR-T cells(CAR-T group),and 21 with chemotherapy plus DLI(chemo-DLI group).The complete remission(CR)and minimal residual disease(MRD)-negative CR rates,leukemia-free survival(LFS)rate,overall survival(OS)rate,and incidence of acute graft-versus-host disease(aGVHD),cytokine release syndrome(CRS)and immune effector cell-associated neurotoxicity syndrome(ICANS)were compared between the two ***:The CR and MRD-negative CR rates in the CAR-T group(77.3%and 61.5%)were significantly higher than those in the chemo-DLI group(38.1%and 23.8%)(P=0.008 and P=0.003).The 1-and 2-year LFS rates in the CAR-T group were superior to those in the chemo-DLI group:54.5%and 50.0%vs.9.5%and 4.8%(P=0.0001 and P=0.00004).The 1-and 2-year OS rates in the CAR-T versus chemo-DLI group were 59.1%and 54.5%vs.19%and 9.5%(P=0.011 and P=0.003).Six patients(28.6%)with grade 2-4 aGVHD were identified in the chemo-DLI *** patients(9.1%)in the CAR-T group developed grade 1-2 *** patients(86.4%)developed CRS in the CAR-T group,comprising grade 1-2 CRS in 13 patients(59.1%)and grade 3 CRS in 6 patients(27.3%).Two patients(9.1%)developed grade 1-2 ***:Donor-derived anti-CD19 CAR-T-cell therapy may be better,safer,and more effective than chemo-DLI for B-ALL patients who relapse after allo-HSCT.

Intraocular lens implantation performed first to protect the posterior capsule in Morgagnian cataracts during phacoemulsification

This study evaluated the safety of a modified method to implant an intraocular lens (IOL) into the capsular bag immediately after capsulorhexis with a whole or partial nucleus to protect the posterior capsule (PC) during phacoemulsification in a series of 12 Morgagnian cataracts. For 3 cases of hypermature cataracts with smaller and rigid nuclei, after a complete capsulorhexis, an IOL was directly inserted into the capsular bag, which protected the PC during the subsequent phacoemulsification process in the iris plate. For the other 9 cases with larger and softer nuclei, after the nucleus was partially emulsified, the IOL was inserted into the bag. Even with an obvious surge for some cases, the surgeries were uneventful in all 12 cases, with no PC rent or vitreous loss. IOL implantation into the capsular bag with a whole or partial nucleus can provide effective protection for the PC for hypermature cataract during phacoemulsification.

Screening of miRNAs associated with lymph node metastasis in Her-2-positive breast cancer and their relationship with prognosis

The aim of this study was to identify some biomarkers for predicting lymph node metastasis and prognosis of human epidermal growth factor receptor 2(Her-2)-positive breast cancer(BC). We analyzed correlations between micro RNAs(mi RNAs) and the prognosis of patients with BC based on data collected from The Cancer Genome Atlas(TCGA) database. The expression levels of mi R-455, mi R-143, and mi R-99 a were measured in clinical samples of Her-2-positive BC patients with different degrees of lymph node metastasis. We investigated the impacts of overexpressed mi R-455 on the proliferation and invasiveness of MDA-MB-453 cells and measured its effects on the expression of long non-coding RNA(lnc RNA) metastasis-associated lung adenocarcinoma transcript 1(MALAT1) by quantitative real-time polymerase chain reaction(q RT-PCR). The expression of mi R-455 was significantly and positively correlated to the prognosis and overall survival(OS) of the BC(P=0.028), according to TCGA information. The expression level of mi R-455 was positively correlated with OS and relapse-free survival(RFS) of patients with Her-2-positive BC, and was negatively correlated with the number of metastatic lymph nodes(P<0.05). Transwell assay suggested that MDA-MB-453 cells became much less invasive(P<0.01) after being transfected with mi R-455 mimics. During the q RT-PCR, the expression level of MALAT1 declined significantly after transfection(P<0.01). Overexpressed mi R-455 significantly inhibited the proliferation and migration of MDA-MB-453 cells and the expression of MALAT1. We conclude that mi R-455 may be a useful potential biomarker for forecasting lymph node metastasis and the prognosis of Her-2-positive BC patients. mi R-455 may play an important role in lymph node metastasis of BC by interacting with MALAT1.

Colorectal cancer(CRC)causes approximately 600000deaths annually and is the third leading cause of cancer mortality *** significant advancements in treatment options,CRC patient survival is still poor owing to a lack of effective tools for early diagnosis and a limited capacity for optimal therapeutic decision *** there exists a need to find new biomarkers to improve diagnosis of CRC,the research on epigenetic biomarkers for molecular diagnostics encourages the translation of this field from the bench to clinical *** alterations are thought to hold great promise as tumor *** this review,we will primarily focus on recent advances in the study of epigenetic biomarkers for colorectal cancer and discuss epigenetic biomarkers,including DNA methylation,microRNA expression and histone modification,in cancer tissue,stool,plasma,serum,cell lines and *** studies have improved the chances that epigenetic biomarkers will find a place in the clinical practices of screening,early diagnosis,prognosis,therapy choice and recurrence surveillance for CRC ***,these studies have typically been small in size,and evaluation at a larger scale of well-controlled randomized clinical trials is the next step that is necessary to increase the quality of epigenetic biomarkers and ensure their widespread clinical use.

The 7^(th) National Gastric Cancer Academic Conference:focus on translational research in gastric cancer

Introduction Gastric cancer is a heterogeneous disease with large variations across geographical regions （1）. Although the global incidence of gastric cancer is declining, it remains highly prevalent in Asia as compared to the West （2,3）. China is one of the countries with the highest incidence of gastric cancer, and accounts for over 40% of all new gastric cancer cases in the world （4,5）. Gastric cancer is the third leading cause of cancer mortality in China （6,7）. The 7th National Gastric Cancer Academic Conference （NGCAC） was held in Beijing from June 22 to June 24 with approximately 1,200 attendees, which was organized by Gastric Cancer Committee, Chinese Anti-Cancer Association （CACA）（Figure I）. The theme of this conference was specification, integration, and transformation. Dr. Jiafu Ji, MD, FACS, the chairman of the conference, the president of Gastric Cancer Committee, CACA, pointed out that the conference was aimed at standardizing the diagnosis and treatment of gastric cancer, integrating the information platform, and transferring current research achievements （Figure 1）. It has been about thirteen years since last National gastric cancer academic conference, which was held in Nanjing in 1999. In this conference, nine pioneer experts （Drs Yan-Zhen Lin, Ji-Fu Wang, Guang-Wei Xu, Jun-Qing Chen, Wen-Fan Zhang, Yin-Chang Zhang, Mao-Lin Jin, xiang-Fu Zhang, and xi-Shan Hao） were awarded Lifetime Achievement Award by Gastric Cancer Committee, CACA. There were ten sessions and more than two hundred papers or abstracts submitted, the proceeding was published in Translational Gastrointestinal Cancer Vol 1, supplement 1, June 2 012.
