Isocitrate dehydrogenase(IDH)mutations frequently occur in lower-grade gliomas and secondary *** IDHs exhibit a gain-of-function activity,leading to the production of D-2-hydroxyglutarate(D-2HG)by reducingα-ketoglutarate(α-KG),a central player in metabolism and epigenetic ***,the role ofα-KG homeostasis in IDH-mutated gliomagenesis remains *** this study,we found that low expression of oxoglutarate dehydrogenase(OGDH)was a common feature in IDH-mutated gliomas,***α-***,IDH1 mutation significantly reducedα-KG levels and increased glutaminolysis and DNA/histone methylation in *** metabolic and epigenetic alterations inhibited astrocyte maturation and led to cortical dysplasia in ***,our results also indicated that reduced OGDH expression can promote the differentiation of glioma cells,while IDH1 mutations impeded the differentiation of glioma cells with low OGDH by reducing the accumulation ofα-KG and increasing ***,we found that l-glutamine increasedα-KG levels and augmented the differentiation-promoting effects of AGI5198,an IDH1-mutant inhibitor,in IDH1-mutant glioma ***,this study reveals that low OGDH expression is a crucial metabolic characteristic of IDH-mutant gliomas,providing a potential strategy for the treatment of IDH-mutant gliomas by targetingα-KG homeostasis.

Targeted therapy is crucial for advanced colorectal cancer(CRC) positive for genetic drivers. With advances in deep sequencing technology and new targeted drugs, existing standard molecular pathological detection systems and therapeutic strategies can no longer meet the requirements for careful management of patients with advanced CRC. Thus, rare genetic variations require diagnosis and targeted therapy in clinical practice. Rare gene mutations, amplifications, and rearrangements are usually associated with poor prognosis and poor response to conventional therapy. This review summarizes the clinical diagnosis and treatment of rare genetic variations, in genes including erb-b2 receptor tyrosine kinase 2(ERBB2), B-Raf proto-oncogene, serine/threonine kinase(BRAF), ALK receptor tyrosine kinase/ROS proto-oncogene 1, receptor tyrosine kinase(ALK/ROS1), neurotrophic receptor tyrosine kinases(NTRKs), ret proto-oncogene(RET), fibroblast growth factor receptor 2(FGFR2), and epidermal growth factor receptor(EGFR), to enhance understanding and identify more accurate personalized treatments for patients with rare genetic variations.

AIM:To describe the surgical procedure of fusiform penetrating keratoplasty(FPK)using multiple trephines of different sizes for treating patients with severe infectious ***:Fourteen eyes underwent FPK,and 15 eyes received conventional penetrating keratoplasty(PK)were included in the *** best-corrected visual acuity(BCVA),refractive outcomes,endothelial cell density,and postoperative complications were ***:The FPK group was followed for an average of 15.3±2.1mo,whereas the PK group was followed for 16.1±*** corneal ulcers were elliptical-shaped in all 14 eyes in the FPK *** mean BCVA(logMAR,0.26±0.13)showed no statistically significant differences from that in the PK group(logMAR,0.21±0.12,P>0.05)at 1y after *** the mean curvature,mean astigmatism,and mean spherical equivalent in the FPK group were lower than those in the PK group(Peyes in the PK group,*** graft immune rejection or elevation of intraocular pressure was observed in the two ***:For patients with elliptical-shaped corneas or corneal ulcers,FPK can avoid disrupting of corneal limbus,reduce the risk of postoperative complications,and can result in satisfactory visual quality.

AIM:To observe early clinical outcome with lens position adjustment following the implantable collamer lens(ICL)***:Sixty patients were selected for this retrospective *** eye from each patient received Toric ICL for astigmatism correction,and the other received non-astigmatic ICL surgery using horizontal *** with higher postoperative arch height were selected,and their non-astigmatic eye clinical outcome were observed after ICL surgery at 1wk,1,and *** clinical measurements included uncorrected visual acuity(UCVA),intraocular pressure(IOP),refractive state,corneal endothelium cell count,and arch *** months later,the ICL in each patient’s non-astigmatic eye was adjusted to the vertical from the horizontal *** results were compared before and 1wk,1,and 3mo after ***:UCVA and IOP were significantly reduced 1wk after position adjustment compared to 1wk after ICL implantation(P0.05).CONCLUSION:Early positioning adjustment postphakic ICL implantation can benefit patients with adjusted arch height or higher *** the good clinical effects,the doctors should pay attention to the potential for adverse effects on UCVA and corneal endothelium cells following early position adjustment after posterior chamber phakic ICL implantation.

Background:This systematic review aimed to examine whether dual-task(DT)training was superior to single-task(ST)training in improving DT walking,balance and cognitive functions for individuals with Parkinson’s disease(PD).Methods:Literature search was performed in the following electronic databases:PubMed,the Cochrane Library,Web of Science,and Metstr covering inception to May 10,*** in order to facilitate comparison across trials,we calculated the effect size(Hedges’g)of gait,balance,cognitive,and other parameters under both ST and DT conditions,using the mean change score and standard deviation(SD)of change score of the experimental and control *** controlled trials that examined the effects of DT motor and cognitive training in individuals with Parkinson’s disease were included for this systematic ***:A total of 214 participants recruited from six articles(actually five trials)were involved in this *** terms of walking ability,only double support time and stride time variability showed significant between-group difference(Hedges’g=0.34,0.18,respectively).Compared to ST training group,DT training group had a more improvement effect in laboratory balance measurement(Hedges’g=0.18,1.25),but no significant improvement in clinical balance *** significant between-group differences were observed,thus its training effect on cognitive function was ***:The DT training failed to achieve promising results better than ST training in improving DT walking and balance functions for individuals with *** firm conclusion cannot be drawn at present,due to the limited number of eligible *** sample size and high-quality studies are needed to investigate the effectiveness of DT training in individuals with PD.

●AIM:To determine the teaching effects of a real-time three dimensional(3D)visualization system in the operating room for early-stage phacoemulsification training.●METHODS:A total of 10 ophthalmology residents of the first-year postgraduate were *** the residents were novices to cataract ***-time cataract surgical observations were performed using a custom-built 3D visualization *** training lasted 4wk(32h)in all.A modified International Council of Ophthalmology’s Ophthalmology Surgical Competency Assessment Rubric(ICO-OSCAR)containing 4 specific steps of cataract surgery was *** self-assessment(self)and expert-assessment(expert)were performed through the microsurgical attempts in the wet lab for each participant.●RESULTS:Compared with pre-training assessments(self 3.2±0.8,expert 2.5±0.6),the overall mean scores of posttraining(self 5.2±0.4,expert 4.7±0.6)were significantly improved after real-time observation training of 3D visualization system(P<0.05).Scores of 4 surgical items were significantly improved both self and expert assessment after training(P<0.05).●CONCLUSION:The 3D observation training provides novice ophthalmic residents with a better understanding of intraocular microsurgical *** is a useful tool to improve teaching efficiency of surgical education.

Chronic hepatitis B virus(HBV)infection,which threatens global public health,is a major contributor to liver-related morbidity and *** for liver diseases related to chronic HBV infection-including laboratory tests,ultrasounds,computed tomography(CT),and liver biopsies-may take up medical resources,particularly since they overlap in most ***,there is an urgent need to establish an economical and effective diagnosis method in order to streamline the medical process for HBV-related disea *** complex network models constructed based on clinical blood tests,we provide such a method by defining the novel measure of functional resilience to assess patients’liver *** combining network models and dynamics,we discovered the pivotal items and their corresponding thresholds,which can guide further research on preventing disease deterioration in critical states of these *** macro-averaged precision of our method,functional resilience,is84.74%,whereas the macro-averaged precision of physicians’experience without assistance from imaging or biopsy is 55.63%.From an economic perspective,our approach could save the equivalent of at least30 USD per visit for most Chinese patients and at least 400 USD per visit for most US patients,compared with general diagnostic ***,this will add to savings of at least 10.5 billion USD *** method can comprehensively evaluate the condition of patients’livers and help avert the waste of medical resources during the diagnosis of liver disease by reducing excessive imaging exams.

AIM:To characterize the distribution of meibomian gland(MG)area loss(MGL)and its relationship with demographic characteristics,mites,and ***:This retrospective observational study included patients who visited the Dry eye Clinic of Shenzhen eye hospital between June 2020 and August *** patient characteristics,ocular symptoms,Demodex test results of the eyelid edges,and the results of a comprehensive ocular surface analysis were *** was analyzed using Image J ***:This study enrolled 1204 outpatients aged 20-80(40.70±13.44)y,including 357 males(29.65%)and 847 females(70.35%).The patients were classified into mild(n=155;12.87%),moderate(n=795;66.03%),severe(n=206;17.11%),and extremely severe(n=48;3.99%)MGL *** was significantly larger in female than in male(P=0.006).The degree of MGL also significantly differed in age(P<0.001)and the more numbers of mites with severity(P<0.001).Multivariate disordered multinomial logistic regression analysis identified that female sex,older age,secretory symptoms,and a large number of mites were risk factors for MGL(P<0.05).CONCLUSION:Patients with MGL are more likely to be older,female,more numbers of mites,and increased secretion.
